국내 교대근무자의 수면연구에 관한 체계적 문헌고찰 (A Systematic Literature Review of Shift Workers' Sleep in Korea)

  • Purpose: The aim of this systematic literature review was to synthesize and investigate the effects of shift workers' sleep in Korea. Methods: A search was conducted through three electronic databases using keywords such as "shift work" or "rotation work" and "sleep", with sleep as the independent variable, and 17 papers were reviewed. Results: The design of those 17 studies was analyzed in a cross-sectional analysis. The most commonly measured characteristic was quality of sleep, whereas the others were sleep disturbance, sleep efficiency, and sleepiness. The study outcome variables were job-related factors, mental health, wellness, stress, fatigue, and metabolic syndrome. Conclusion: The results inform sleep and health-promoting behaviors and improvement in shift workers' working condition.

디지털 수면다원기록법:현재와 미래 (Digital Polysomnography:The Present and Future)

  • Digital polysomnography was developed to overcome the limitations of Rechtschaffen and Kales rule and to compensate the shortcomings of paper polysomnography. It enables easy access to and secure preservation of sleep records, and provides various displays of sleep data to enhance efficiency of visual scoring of sleep records. Rechtschaffen and Kales rule had been criticized for its ambiguity and lack of considerations in spatial information of EEG. As sleep records are acquired and processed in digital mode, they can be analyzed at microscopic and macroscopic levels. Digital analysis of sleep records provides the basis for development of new sleep measures. Sleep staging in digital polysomnography is based on the various analyses of EEG. Sleep apnea, hypopnea and periodic limb movement are detected automatically by digital analysis of respiratory signals and leg EMG. Digital polysomnography plays a complementary role to visual scoring and compensates the limitations of paper polysomnography. Digital polysomnography, including acquisition, processing and analysis of sleep records in digital mode, can be a great help in the development of sleep medicine, enabling the development of new sleep measures and the exchange of sleep records between sleep laboratories.

어싱매트가 수면의 질에 미치는 영향 (Effect of Earthing Mat on the Quality of Sleep)

  • Objectives: Using as a sleep evaluation tool the Pittsburgh Sleep Quality Index (PSQI), this paper studied the effects of earthing on the amount and quality of sleep by examining a patient population of people who used and did not use earthing mats and a group of general healthy people. Methods: From September to November 2019, 30 patients who voluntarily used an earthing mat, 40 patients who did not use an earthing mat, and 40 healthy people who did not use an earthing mat were selected. A questionnaire applying the PSQI (Pittsburgh Sleep Quality Index) after being translated into Korean was distributed starting November 29, 2019 to 110 subjects. Out of them, 101 subjects who completed the questionnaire were examined. All statistics used SPSS 25.0. Results: The first overall score showed that all three groups had problems sleeping, with five points or more, but the general population had better sleep quality than the patient group. It also showed that the using matgroup had better quality of sleep than the ones that did not use the earthing mat In addition, the analysis of the third group through the variance analysis showed significance at >0.05 for the general population and the patient group. Based on this, a multi-comparison analysis of the third group showed significantly less than 0.05 in patients who do not use an earthing mat compared to the public. First of all, there are no statistically significant differences between the three groups in subjective sleep quality, sleep latency, and sleep time, but an analysis of multiple comparison analysis with the general population group showed that the quality of sleep was worse compared to those that used the mat. In addition, sleep efficiency, sleep disorder, sleeping pills, and daytime dysfunction were found to be less than >0.05 in the three groups. Conclusions: These results suggest that if you ground your body to Earth during sleep, the secretion of cortisol will decrease night levels, sync more with the natural 24-hour circulation rhythm profile, and that when you sleep, sleep is better and pain and stress are associated with the study. While the preceding study cannot confirm that contact with the ground affects the quality of sleep, it suggests that it is relevant as shown in this paper.

IEEE 802.16e 단말의 저전력 모드 성능 평가에 관한 연구 (A Performance Analysis of Power Saving Modes on IEEE 802.16e Mobile Terminal)

  • IEEE 802.16e 표준은 sleep 모드와 idle 모드의 두 저전력 모드를(power saving mode: PSM) 정의하고 있다. 이들은 모드 천이시 단말과 망간 상태 정보 유지 여부에 따라 소모 전력 측면에서 성능이 달라진다. 따라서 802.16e 단말의 전력 소모 최적화를 위해서는 각 기법의 성능에 영향을 주는 요소들을 모두 고려한 상세한 성능분석이 필요하다. 본 논문에서는 단말의 이동성과 호 사용 빈도를 고려하여 소모 전력 측면에서 두 PSM의 성능분석 모델을 제안하고, idle 모드가 전력 소모 측면에서 sleep 모드보다 우수하다는 것을 보인다. 또한 분석 결과와 모의실험과의 비교를 통해 제안한 분석 모델의 타당성을 검증하였다.

수면다원기록법으로 확진된 폐쇄성 수면무호흡증 환자의 임상특성, 그리고 호흡장애지수와 수면 구조간의 상관관계 (Clinical Characteristic and Respiratory Disturbance Index as Correlates of Sleep Architecture in Obstructive Sleep Apnea Syndromes Diagnosed with Polysomnography)

  • 배 경 : 폐쇄성 수면무호흡증은 다양하고 심각한 임상 증상과 합병증을 유발하는 질환으로 국내에서도 흔히 진단되고 있다. 그러나 현재까지 수면다원기록법을 사용해 폐쇄성 수면무호흡증을 확진한 환자들의 대규모 국내 연구를 찾아 볼 수 없었다. 본 연구에서는 종래의 연구에서보다 더 큰 집단을 대상으로 수면다원기록법을 사용해 폐쇄성 수면무호흡증으로 확진된 한국인 환자의 임상적 특성과 수면 양상에 관해 조사하였다. 방 법 : 서울대학교병원 수면다원검사실에 의뢰된 환자들 중 수면다원검사 결과 폐쇄성 수면무호흡증으로 진단된 환자 801명을 대상으로 하였다. 중추성 무호흡증, 주기성 사지운동증, 기면병, 렘수면관련행동장애, 그리고 외국인 환자는 제외하였다. 연구 대상 군의 임상적 특징을 조사하였으며 수면 변인들을 연령, 성별에 따른 정상치와 비교하였다. 그리고 호흡장애지수가 수면변인에 미치는 영향을 분석하였다. 결 과 : 전체 연구대상자 801명 중 남자가 83.4%, 여자가 16.4%이었고 평균 연령은 46.6세였다. 평균 비만정도 지수는 25.8이었고 비만한 환자가 22.8%를 차지하였다. 전체 대상 중 6.2%가 벤조디아제핀계 약물을 복용 중이었다. 연구 대상군에서 정상치에 비해 입면잠복시간은 증가되었고 수면효율, 렘수면 분율, 그리고 서파수면 분율은 감소하였다. 서파수면 분율과 렘수면 분율은 각각 호흡장애지수와 음의 상관성을 보였으나 수면효율과 입면잠복시간은 모두 호흡장애지수와 유의한 상관관계가 없었다. 결 론 : 본 연구에서 다수의 폐쇄성 수면무호흡증 환자가 비만과 무관하였다. 또한 6.2%의 폐쇄성 수면무호흡증 환자가 벤조디아제핀계 약물을 복용하고 있었다. 폐쇄성 수면무호흡증의 발병은 수면효율도 낮추고 서파수면과 렘수면 분율을 감소시키나 호흡장애지수가 커져도 서파수면과 렘수면 분율이 감소할 뿐 수면효율은 별 다른 변화가 없었다. 즉, 수면효율보다는 수면 구조의 변화가 수면 중 호흡장애의 정도에 더 관련되었음을 알 수 있었다.

렘수면 의존성 수면무호흡증과 비렘수면 의존성 수면무호흡증의 특징 (The Characteristics of REM Sleep-Dependent Obstructive Sleep Apnea and NREM Sleep-Dependent Obstructive Sleep Apnea)

  • 목 적 : 폐쇄성 수면무호흡증(obstructive sleep apnea)은 수면 시 호흡을 하려고 하는 노력에도 불구하고 기도의 부분적/완전 폐색이 반복되는 수면 관련 호흡 질환이다. 일반적으로 렘수면에서 근긴장도의 저하가 나타나게 되므로 렘수면에서 수면무호흡증의 악화가 예상되지만 많은 경우에 있어 비렘수면에서의 무호흡/저호흡 지수(apnea-hypopnea index, AHI)가 렘수면에서의 AHI보다 높은 경우가 관찰되었다. 그러나 그런 현상의 분명한 이유는 밝혀져 있지 않았기에 렘/비렘수면 의존성 수면무호흡증에 대한 특징을 종합적으로 알아보고자 하였다. 방 법 : 560명의 성인 수면무호흡증 환자를 대상으로 후향적 연구를 진행하였다. 모든 환자는 렘 수면시의 AHI (REM-AHI), 비렘수면시의 AHI (NREM-AHI)의 비율에 따라 세군으로 분류하였다. 렘수면 의존성 수면무호흡증(REM sleep dependent OSA, REM-OSA)는 REM-AHI/NREMAHI > 2, 비렘수면 의존성 수면무호흡증(NREM sleep dependent OSA, NREM-OSA)는 NREM-AHI/REM-AHI > 2, 수면 단계 비의존성 수면무호흡증(Sleep stage independent OSA, IND-OSA)로 분류하였다. 수면다원검사 이외에도 주관적인 수면의 질, 낮시간 졸림, 정서와 연관된 설문지를 함께 시행하였다. 카이제곱 검정, 일원배치분산분석, 공분산분석을 시행하였다. 결 과 : 세 군 사이에서 연령의 차이는 보이지 않았다. REM-OSA는 대체로 경도의 수면무호흡증 환자가 많았고 여성의 비율이 가장 높았다. 또한 주관적인 수면의 질이 가장 저하되어 있다고 느꼈으며 기분 상태 평가 척도인 K-POMS의 점수는 가장 높았다. IND-OSA는 BMI, 목둘레(neck circumference, NC), 복부 둘레(abdominal circumfefence)가 REM-OSA에 비해 높았고 주간 졸림의 호소는 세군 중 가장 컸고, 무호흡 관련 변인의 값이 가장 높아 중증도가 심한 군이었다. 공변량을 통제하고 시행한 공분산 분석의 결과에서 NREM-OSA는 앙와위에서의 AHI가 가장 높았고 수면 중 측와위의 비율이 가장 컸다. 또한 수면 효율이 가장 떨어졌고 렘수면 잠복기가 가장 길었으며 입면 후 각성 시간의 비율이 가장 컸다. 결 론 : REM-OSA는 여성이 많고 부정적인 정서를 가장 많이 호소하는 군으로, 경도의 수면무호흡증을 가장 많이 포함하였고 IND-OSA는 중증의 수면무호흡증을 반영하였다. NREM-OSA는 자세 변화와 가장 밀접한 관련은 지니고 있었으며 수면 효율이 가장 떨어졌다. 세 군의 수면무호흡증이 일련의 중중도 차이만이 아닌 또 다른 특성을 반영하는 것으로 보이고 이를 통해 폐쇄성 수면무호흡증의 이해를 넓힐 수 있을 것이다.

Effects of sleep-inducing juice on sleep quality and heart rate variability in adults with disturbed sleep

  • BACKGROUND/OBJECTIVES: Although some juices affect subjective sleep quality, there is a lack of information on the effect of a specific juice on objective sleep quality and heart rate variability (HRV) during sleep; thus the present study investigated whether a blended juice made from natural extracts influenced sleep quality and HRV during sleep in adults with disturbed sleep. SUBJECTS/METHODS: A randomized, crossover study was conducted on twenty-five adults (15/10, female/male) complaining of difficulty initiating or maintaining nighttime sleep (Pittsburgh sleep quality index [PSQI] ≥ 5). During feeding sessions (FS), subjects received sleep-inducing juice made of natural ingredients (250 mL/trial) twice a day for 8 weeks or non-FS (N-FS) for 8 weeks while maintaining normal activities. Sleep quality and parameters were recorded via wearable actigraph for 7 consecutive days, and PSQI scores were assessed before and after the intervention. HRV was also monitored at rest and during sleep. RESULTS: After receiving the sleep-inducing juice intervention (FS), PSQI scores were significantly decreased (P < 0.001) and correlated with a significant decline in fatigue severity scale and visual analogue scale levels (P < 0.05; both). HRV indices of vagal activity were significantly improved during FS (P < 0.05), and no significant differences in N-FS were observed. Sleep efficiency and total sleep time increased significantly (P < 0.05) and sleep latency, total counts, sleep fragmentation index, and movement index, decreased significantly (P < 0.05, all 4) during FS, with no significant differences-observed during N-FS. CONCLUSIONS: This study results demonstrated that an 8-week course of sleep-inducing juice has led to improve sleep quality, suggesting an enhanced cardiac vagal tone during sleep. Thus, it could be a well-tolerated option for adults with disturbed sleep.

수면과 회복력 (Sleep and Resilience)

  • Good sleepers tend to more resilient than poor sleepers. As sleep and resilience may have a bidirectional relationship, it is important to explore the relationship between healthy sleep and resilience. Objectively and subjectively measured sleep quality showed positive association with resiliency. In one study, more resilient adolescents had higher sleep efficiency, less light sleep, more slow-wave sleep, and a smaller number of awakenings after sleep onset. Circadian typology, jetlag and circadian misalignment may be associated with the capacity to deal with adversity. Eveningness, exposure to chronic jetlag and circadian misalignment might be risk factors for development of psychological problems and mental disorders. Additional studies are needed to evaluate whether promoting healthy sleep behaviors results in increased resilience.

A Preliminary Study on the Effectiveness of Far-Infrared Emitting Ceramic Mattresses in Improving Sleep Quality

  • Objectives : Far-infrared (FIR) lights have been investigated for sleep quality intervention. We sought to measure the advantageous effects of FIR in sleep using polysomnographic data as the objective outcomes. Methods : The ten healthy volunteers were enrolled in a single-center, prospective, patient-blind, single-arm trial. Individuals slept on a sham mattress and a FIR emitting mattress with polysomnography for one night each. Results : Sleep efficiency showed an increasing trend but was not statistically significant. PSQI-K significantly decreased (p=0.013). The latency to REM of the baseline was shorter than that of the intervention (p=0.008). Though there was no statistical significance, Stage N1 and N2 were shortened, and Stage N3 was prolonged after the intervention compared to the baseline. Conclusions : The FIR-emitting mattresses improved sleep quality on self-reported insomnia. We suggested the candidate for the markers altered by the FIR therapy, such as the normalization of REM latency and increased N3 sleep.

전립선암환자의 수면장애 (Sleep Disturbance in Prostate Cancer Patients)

  • Purpose: This study was done to investigate clinical characteristics and risk factors for sleep disturbance in patients with prostate cancer. Method: Participants were recruited from P hospital outpatient clinic from March 23 to April 20, 2006, and 101 participants completed a questionnaire assessing general and clinical characteristics, sleep quality, physical symptoms and psychological symptoms such as anxiety and depression. The data was analyzed using the SPSS 12.0 program. Results: In this study, 29.7% of the patients reported sleep disturbance. Cancer diagnosis related factors which affected sleep disturbance were onset (55.3%) and aggravation (83.3%). Habitual sleep efficiency of patients with sleep disturbance was as follows: bedtime was 10 PM, wake-up time was 6AM, sleep duration was six hours and twenty minutes. Risk factors for the presence of sleep disturbance included metastasis, the presence of intestinal symptoms, depression and anxiety. Conclusion: Sleep disturbance is a frequent problem associated with prostate cancer and seems to be influenced by aggravation of illness and the presence of physical and psychological symptoms.
