여자 대학생의 수면습관이 감각운동리듬 뇌파에 미치는 영향 (Effects of Sleep Habits on EEG Sensory Motor Rhythm in Female College Students)

  • 본 연구의 목적은 수면장애가 뇌파(EEG)의 beta파와 감각운동리듬(sensory motor rhythm)에 미치는 영향을 규명하는 것이다. 여자 대학생을 대상으로 하여 수면습관에 따라서 다음과 같이 네 군(n=6)으로 나누었다. 즉, 좋은 수면습관을 가진 GSHG (Good sleep habit group), 일반적으로 대학생들이 늦게 취침하는 수면 습관을 갖고 있기 때문에 오전 0~2시에 취침하는 CSHG (Common sleep habit group), 수면위상지연증후군으로 자신의 수면 장애를 인식하고 있는 CSDG (Cognitive sleep disorder with delayed sleep phase syndrome group), 수면장애를 인식하지 못하는 NSDG (Non-cognitive sleep disorder with delayed sleep phase syndrome group)으로 나누었다. 뇌파는 QEEG 8-System을 사용하여 오전 9~12시 사이에 측정하였다. 책읽기 자극시에 뇌파를 측정하여 안정시 기준치와 비교 분석하였다. 연구 결과, 책읽기 자극시 beta파와 감각운동리듬 상대파워는 GSHG가 다른 세군에 비하여 유의적으로 높았다($p$ <0.05). 책읽기 자극시 수치와 안정시 기준치를 대응 비교하여 볼 때에도, GSHG에서 beta 파워가 대뇌피질의 모든 부위에서 유의적으로 증가하였다($p$ <0.05). 그러나 CSHG에서는 전두부와 측두부에서 beta 파워가 감소하였다($p$ <0.05). 한편 CSDG와 NSDG에서는 책읽기 자극에 의하여 beta 파워의 유의적인 변화가 발생하지 않았다. 감각운동리듬 상대파워는 GSHG에서만 책읽기 자극에 의하여 향상되었다. 결론적으로 여자 대학생에게 있어서 오전 0~2시경에 늦게 취침을 하면 다음날 오전에 집중력 뇌파인 beta파 기능이 현저히 감소하고, 수면습관이 나쁘면 자신의 수면장애에 대한 자가인식 유무와 관련 없이 beta파와 운동감각리듬에 대한 뇌기능이 활성화되지 않아서 집중력에 부정적인 영향을 준다.

  • Objective : To explore the meaning of Eumyangyeokchahunobok-byung (陰陽易差後勞復病) through two cases and finding the relationship with the DSM-5 sleep-wake disorder. Methods : Following a disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS), we analyzed cases with chronic fatigue and diplopia caused after sleep disorders. The patients were treated with the Soshiho-tang and Yijoong-tang. Results : The two herbal medicines showed remarkable improvement in the patients. The sleep disorders and the secondary pathologies were rapidly relieved within a month. Conclusions : The patterns of sleep disorders of the two patients were found to be very similar to the circadian rhythm sleep-wake disorders, as described in DSM-5. Although the etymology of the Chinese characters has not been clearly known yet, it was possible to present the original hypotheses on 更and 了due to clinical inference.

간호대학생의 사회적 고립감, 심리적 수용, 수면장애가 우울에 미치는 영향 (The Impacts of Social Isolation, Psychological Acceptance, and Sleep Disorder of Nursing College Students on Depression)

  • 본 연구의 목적은 간호대학생의 사회적 고립감, 심리적 수용, 수면장애가 우울에 미치는 영향을 파악하기 위함이다. 연구대상은 경기도 소재의 일개 간호대학생 383명으로 구조화된 설문지를 이용하여 2015년 6월부터 7월까지 자료수집하였다. 수집된 자료는 t-test, ANOVA, Pearson's correlation coefficients, 다중회귀분석을 통해 분석하였다. 연구결과, 학년, 학업스트레스, 간호학 전공만족도에 따라 우울 정도에 차이가 있었다. 우울 정도는 사회적 고립감(r=.47, p<.001), 수면장애(r=.43, p<.001)와 정상관관계가 있었고, 심리적 수용(r=-.41, p<.001)과는 역상관관계가 있었다. 우울에 영향을 미치는 요인으로 학업스트레스, 전공만족도, 수면장애. 사회적 고립감, 심리적 수용이 확인되었다. 본 연구결과를 바탕으로 간호대학생의 우울을 감소시키는 스트레스 증재 프로그램을 계획할 때는 학업스트레스, 전공만족도, 사회적 고립감, 심리적 수용과 수면장애의 변수를 고려한 복합적 중재 프로그램 개발과 적용이 필요하다고 생각된다.

불면증의 생리학적 모델 (Human Physiological Models of Insomnia)

  • Relatively little is known about the neurobiology of insomnia, despite its wide prevalence and broad medical impact. Although much is still to be learned about the pathophysiology of the disorder, identification, systematic assessment, and appropriate treatment are clearly beneficial to patients. Recent research, using quantitative EEG, polysomnography (PSG), multiple sleep latency test (MSLT) and neuroimaging techniques, suggests that some broad areas can be identified as possible pathophysiological models. Sleep-wake homeostat model hypothesizes a failure in homeostatic regulation of sleep, an attenuated increase in sleep drive with time awake, and/or defective sensing of sleep need. Circadian clock model hypothesizes a dysfunctional circadian clock, resulting in changes in the timing of sleep-wake propensity that are incompatible with normal sleep. Intrinsic sleep-wake state mechanism model suggests that abnormal function of insomnia comprises the systems responsible for expression of the sleep states themselves. Extrinsic over-ride mechanism (stress-response) model suggests that insomnia reflects the consequences of overactivity of one of the systems considered "extrinsic" to normal sleep-wake control. Many current therapies for insomnia are based on these physiological models. Several attempts have been made to create a physiological model that would explain this disorder and could be used as a foundation for treatment. However, it appeared that no model can fully explain and clarify all aspects of insomnia. Future research should be necessary to expand our knowledge on the biological dimensions of insomnia.

아로마요법이 수면의 질에 미치는 영향 (Effects of Aromatherapy on Sleep Quality)

  • 본 연구에서는 헬스케어의 주력 디바이스로 이용되고 있는 스마트밴드를 이용하여 아로마 요법에 따른 수면의 패턴을 분석하고자 한다. 우선, 수면장애를 겪고 있는 학생들을 선별한 후, 수면의 질을 높여줄 에센셜 오일을 블랜딩하여 적용하였다. 스마트밴드와 연동된 앱을 활용하여 수면 패턴을 객관적으로 분석한 후 아로마요법이 수면의 질에 어떤 영향을 주는지 확인한다.

주요 정신장애에서의 주의력 장애 (Attentional Dysfunction in Major Psychiatric Disorders)

  • Attentional dysfunction is considered as one of the core deficits in schizophrenic process. The findings, pathophysiological mechanisms, and their clinical implications of clinical and experimental neurocognitive tests for the attentional impairment in schizophrenics are reviewed. The influences of psychopathology, antipsychotic treatment, and chronic institutionalization are also included in the review. In contrast, there are only a few evidences that attentional dysfunction would be a core deficit of depressive, manic, and anxiety disorders. Some recent findings of attentional impairment in these disorders are reviewed.

졸린 소아에 대한 평가와 치료 (The Sleepy Child)

  • Excessive daytime sleepiness in childhood might be abnormal phenomenon and often related to the sleep disorders or insufficient sleep duration. The most common cause of excessive daytime sleepiness would be insufficient sleep. However, narcolepsy, idiopathic hypersomnia, circadian rhythm sleep disorders, medication, medical illness and other sleep disorders that could cause insomnia and poor quality of sleep also result in excessive daytime sleepiness. The misdiagnosed and untreated excessive daytime sleepiness in childhood can lead to serious developmental and educational problem.

Krabbe Disease 환아의 한방치료 증례 1례 보고 (A Case Report on Krabbe Disease Patient with Oriental Medicine)

  • Objectives: This study is to report a treatment case of the Krabbe Disease with herbal medicine and acupuncture. The purpose of this study is to report clinical effects of an oriental medical treatment on the Krabbe Disease patient who also has stool problem, sleep disorder, and anorexia. Methods: Herbal medicine and acupuncture are performed on the patient with the general symptoms such as constipation, sleep disorder, anorexia, and coldness of feet. Results: Only treated by oriental medicine, the patient's constipation, sleep disorder, anorexia, and coldness of feet vanish, and the patient maintains her condition for a month. Conclusions: This study shows that oriental medical treatments are effective in treating the general symptoms that a Krabbe Disease patients has. To be more accurate, further studies are necessary.

Relationship between Comorbid Health Problems and Musculoskeletal Disorders Resulting in Musculoskeletal Complaints and Musculoskeletal Sickness Absence among Employees in Korea

  • Background: To investigate the relationship between musculoskeletal disorders and comorbid health problems, including depression/anxiety disorder, insomnia/sleep disorder, fatigue, and injury by accident, and to determine whether certain physical and psychological factors reduce comorbid health problems. Methods: In total, 29,711 employees were selected from respondents of the Third Korean Working Conditions Survey and categorized into two groups: Musculoskeletal Complaints or Musculoskeletal Sickness Absence. Four self-reported health indicators (overall fatigue, depression/anxiety, insomnia/sleep disorder, and injury by accident) were selected as outcomes, based on their high prevalence in Korea. We used multiple logistic regression analysis to determine the relationship between comorbid health problems, musculoskeletal complaints, and sickness absence. Results: The prevalence of musculoskeletal complaints and musculoskeletal sickness absence due to muscular pain was 32.26% and 0.59%, respectively. Compared to the reference group, depression/anxiety disorder and overall fatigue were 5.2-6.1 times more prevalent in the Musculoskeletal Complaints Group and insomnia/sleep disorder and injury by accident were 7.6-11.0 times more prevalent in the Sickness Absence Group. When adjusted for individual and work-related physical factors, prevalence of all four comorbid health problems were slightly decreased in both groups. Conclusion: Increases in overall fatigue and depression/anxiety disorder were observed in the Musculoskeletal Complaints Group, while increases in insomnia/sleep disorder and injury by accident were observed in the Sickness Absence Group. For management of musculoskeletal complaints and sickness absence in the workplace, differences in health problems between employees with musculoskeletal complaints and those with sickness absence as well as the physical and psychological risk factors should be considered.

수면장애가 있는 성인의 통증, 목과 어깨의 기능장애수준, 두통 및 스마트폰 중독 간 상관성 연구 (Correlation between Neck and Shoulder Pain, Neck and Shoulder Disability, Headache and Smartphone Addiction in Adults with Sleep Disorders)

  • PURPOSE: To investigate the correlation between sleep quality, neck and shoulder pain, neck and shoulder disability, headache, and smartphone addiction in adults, as well as factors affecting sleep disorders in adults. METHODS: A total of 112 adults in their 20s were recruited for participation in this study. This study measured Pearson's correlation of sleep quality, neck and shoulder pain, neck and shoulder disability, headache and smartphone addiction. Logistic regression analysis (sleep disorder, without sleep disorder adults), was performed to analyze factors affecting sleep disorders. RESULTS: Sleep quality index was positively correlated with shoulder pain and disability index(SPADI)-pain score, SPADI-global score and smartphone addiction score. There was a significant difference in SPADI-pain score between the group with sleep disorders and the group without sleep disorders. Shoulder pain and disability were identified as factors affecting sleep disorders. The group with sleep disorders showed 1.088 times more shoulder pain than the group without sleep disorders, and the group with sleep disorders showed 1.006 times higher frequency of shoulder disorders than the group without sleep disorders. CONCLUSION: The results of the present study indicate that as sleep quality worsens, shoulder pain, disability, and smartphone addiction get worse. The group with sleep disorders had significantly increased shoulder pain compared to the group without sleep disorders. We also found that shoulder pain and disability were factors affecting sleep quality. Comparison between intervention method is warranted to better understand adults with sleep disorders.
