두개골에 발생한 결합조직형성 섬유종 ; 증례보고 및 문헌 검토 (Desmoplastic Fibroma of the Skull;A Case Report, Review of the Literature, and Therapeutic Implications)

  • 결합조직형성 섬유종은 드문 골종양으로 조직학적으로는 양성이나 국소적으로 빨리 자라는 질환이다. 두개골에 발생한 결합조직형성 섬유종은 지금까지 9례가 보고되고 있으며, 이 중 8명은 여자에서 발생하다. 이 논문은 남자에서 발생한 두개골 결합조직형성 섬유종을 보고하며, 문헌을 검토하여, 이 질환의 임상양상 및 조직학적소견, 그리고 치료방법등을 보고하고자 한다. 환자는 21세 남자로 두통을 호소하으며, 두부 전산화 단층 촬상 뇌을 침습한 소견이 없는 단일 골용해성 부위가 관찰되었으며, 조직학적 검사상 결합조직형성 섬유종으로 밝혀졌다. 면역조직화학적 염색상, 종양의 에스트로겐과 프로게스테론 수용기는 음성이었다. 정상골을 포함한 종양을 전적출하으며, 35개월간 추적관찰결과 재발소견은 없었다. 다른 부위의 결합조직형성 섬유종과 비교했을 때, 종양의 정상골을 포함한 전적출이 두개골에서 가능하므로, 재발이 없는 것으로 생각되며, 남자에서 발생한 결합조직형성 섬유종을 치험하기에 문헌검토와 함께 보고하는 바이다.

Successful Management of a Comatose Patient with Traumatic Brain Exposure with a Fronto-Parieto-Occipital Flap

  • Composite skull defects in patients with severe head injuries are very challenging to manage. The dilemma when deciding whether to perform a definitive reconstruction is how long to wait for physiological recovery before an intervention complicates the situation. The inability of such patients to tolerate prolonged anesthetic exposure is a driving factor for performing the minimal intervention necessary to facilitate recovery. Herein, we present a case involving the successful immediate reconstructive treatment of a severely head-injured adolescent with a composite scalp defect secondary to trauma. A 14-year-old boy sustained a severe head injury from a motor vehicle accident with a composite scalp defect in the right fronto-parietal region. The frontal lobe was exposed, and the right eye was crushed and devitalized. The patient was deeply unconscious for 3 days, without any significant improvements before reconstructive surgery was proposed due to fear of possible meningitis resulting from the exposure of brain structures. We successfully managed the patient with a fronto-parieto-occipital flap, after which the patient promptly recovered consciousness.

Demonstration of Traumatic Subarachnoid Hemorrhage from the Anterior Choroidal Artery

  • We present a case of angiographically confirmed transection of the cisternal segment of the anterior choroidal artery (AChA) associated with a severe head trauma in a 15-year old boy. The initial brain computed tomography scan revealed a diffuse subarachnoid hemorrhage (SAH) and pneumocephalus with multiple skull fractures. Subsequent cerebral angiography clearly demonstrated a complete transection of the AChA at its origin with a massive extravasation of contrast medium as a jet trajectory creating a plume. We speculate that severe blunt traumatic force stretched and tore the left AChA between the internal carotid artery and the optic tract. In a simulation of the patient's brain using a fresh-frozen male cadaver, the AChA is shown to be vulnerable to stretching injury as the ipsilateral optic tract is retracted. We conclude that the arterial injury like an AChA rupture should be considered in the differential diagnosis of severe traumatic SAH.

Experience with 7.0 T MRI in Patients with Supratentorial Meningiomas

  • Meningiomas are typically diagnosed by their characteristic appearance on conventional magnetic resonance imaging (MRI). However, detailed image findings regarding peri- and intra-tumoral anatomical structures, tumor consistency and vascularity are very important in pre-surgical planning and surgical outcomes. At the 7.0 T MRI achieving ultra-high resolution, it could be possible to obtain more useful information in surgical strategy. Four patients who were radiologically diagnosed with intracranial meningioma in 1.5 T MRI underwent a 7.0 T MRI. Three of them underwent surgery afterwards, and one received gamma knife radiosurgery. In our study, the advantages of 7.0 T MRI over 1.5 T MRI were a more detailed depiction of the peri- and intra-tumoral vasculature and a clear delineation of tumor-brain interface. In the safety issues, all patients received 7.0 T MRI without any adverse event. One disadvantage of 7.0 T MRI was the reduced image quality of skull base lesions. 7.0 T MRI in patients with meningiomas could provide useful information in surgical strategy, such as the peri-tumoral vasculature and the tumor-brain interface.

Matrix Metalloproteinase Inhibitors Attenuate Neuroinflammation Following Focal Cerebral Ischemia in Mice

  • The aim of this study was to investigate whether matrix metalloproteinase (MMP) inhibitors attenuate neuroinflammation in an ischemic brain following photothrombotic cortical ischemia in mice. Male C57BL/6 mice were anesthetized, and Rose Bengal was systemically administered. Permanent focal ischemia was induced in the medial frontal and somatosensory cortices by irradiating the skull with cold white light. MMP inhibitors, such as doxycycline, minocycline, and batimastat, significantly reduced the cerebral infarct size, and the expressions of monocyte chemotactic protein-1 (MCP-1), tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), and indoleamine 2,3-dioxygenase (IDO). However, they had no effect on the expressions of heme oxygenase-1 and neuroglobin in the ischemic cortex. These results suggest that MMP inhibitors attenuate ischemic brain injury by decreasing the expression levels of MCP-1, TNF-${\alpha}$, and IDO, thereby providing a therapeutic benefit against cerebral ischemia.

The effects of scanning position on evaluation of cerebral atrophy level: assessed by item response theory

  • Cerebral atrophy affects the brain and is a common feature of patients with mild cognitive impairment or Alzheimer's diseases. It is evaluated by the radiologist or reader based on patient's history, age and the space between the brain and the skull as indicated by magnetic resonance (MR) images. A total of 70 patients were scanned in the supine and prone positions before three radiologist assessed their atrophy level. This study examined the radiologist's assessment of the cerebral atrophy level using a graded response model of item response theory (IRT). A graded response model (GRM) is fitted to our data and then item-fit and person-fit statistics are evaluated to assess the fitted model. Our analysis found that the cerebral atrophy level is better discriminated by readers in the prone position because all item slopes were greater than 2 at this position, versus the supine position where all the slope parameters were less than 1. However, the thresholds are very similar for the first reader and are quite different for the second and third readers because the scanning position affects readers differently as the category threshold estimates vary considerably between the readers..

내경동맥에 발생한 외상성 가성동맥류에 대한 혈관내 스텐트 치료 (Endovascular Stenting of a Traumatic Pseudoaneurysm on C5 Portion of the Internal Carotic Artery - A Case Report -)

  • This 18-year-old male patient had a massive epistaxis after motorcycle traffic accident on 1st day of admission. There were no other significant brain parenchymal lesion on initial brain CT exam, except multiple pneumocephalus and basal skull fracture lines. We treated epistaxis conservatively till vital signs were corrected, and then conventional cerebral angiogram was followed. On angiogram, there was traumatic pseudoaneurysm on C5 portion(by Fischer) of ICA, so we treated it only by endovascular stenting. For the purpose of sparing parent arterial patency, endovascular stenting on pseudoaneurysm may play a role with safety and good results.

뇌종양 및 그 주변 신경다발의 통합적 가시화를 위한 소프트웨어의 개발 (Software Development for the Integrated Visualization of Brain Tumor and its Surrounding Fiber Tracts)

  • 목적 : 뇌종양 및 확산텐서 영상으로 얻어진 그 주변 신경 다발을 동시에 가시화하는 소프트웨어를 구현하고 그것을 통해 뇌종양이 그 주변 신경다발에 미치는 영향에 대한 조사에의 적용 가능성을 시험해 보고자하였다. 대상 및 방법 : IDL을 기반으로 뇌종양과 그 주변 신경다발의 통합적 가시화를 구현하였다. 뇌종양을 가진 한 환자에 대한 T1 강조영상 및 확산텐서 영상을 포함하는 자기공명영상이 3.0T자기공명장치에서 획득되었다. 우리는 해부학적 정보를 위해 두개골을 제거한 뇌 영상과 구획화된 뇌종양을 위한 대조강화 T1 강조 영상을 이용하여 서피스 및 볼륨렌더링을 사용하였다. 대뇌 백질 신경 다발추적을 위해 사용되는 확산텐서영상을 위해서는 25개 방향의 확산경사 자계를 이용하는 SE-EPI방법을 사용하였다. 신경 다발추적 방법으로는 streamline과 tensorline 방법을 사용하였다. T1 강조 영상 및 확산텐서 영상의 공간적 불일치를 보정하기 위해 SPM을 이용한 정합을 수행하였다. 우리의 소프트웨어는 PC 윈도우 환경에서 작동할 수 있도록 구현되었다. 결과 : 한 명의 뇌종양 환자에 대하여 튜브 모양의 신경다발, 대뇌 백질 서피스 렌더링 , 뇌종양의 볼륨/서피스렌더링의 통합적 가시화를 성공적으로 구현하였다. 결론 : 우리의 결과는 뇌종양 및 그 주변 신경다발의 통합적 가시화의 실현 가능성을 보여주었다. 더불어 우리의 구현된 통합적 가시화는 뇌종양 부위 및 그 주변 부의의 대뇌 백질 확산 비등방성의 정량적인 분석에 사용될 수 있을 것으로 기대된다.

비침습적 뇌내압 측정 시스템의 개발을 위한 청각유발전위와 경막혈종간의 상관관계 분석에 관한 연구 (A Correlational Study between Auditory Evoked Potential and Subdural Hematoma for the Diveloprnent of a Noninvasive ICP Monitoring System)

  • Development of a noninvasive intensive care system calls for the use of evoked potentials (EPs) as a means of diagnosing traumatic head-injured patients. The experiment entails surgically placing two subarachnoid bolts and a subdural balloon through the skull to simulate a subdural hematoma. Using various levels of intracranial pressure (ICP) and/or different sizes of balloons, auditory evoked potentials (AEPs) were recorded from a rabbit. Six positive peak latencies ($P_1 - P_6$) and five negative peak latencies ($N_l- N_5$) were extracted from an averaged AEP waveform. Multiple regression analyses were performed for determining. a relationship between the ICP and AEP peak latencies. The results indicate that a major correlation of ch, mges on AEP peak latencies is due to mechanical forces of a mass (inflated balloon simulating a hematoma) in the distortion of the brain matter rather than increased ICP itself.

소아에서 발생한 연필에 의한 관통 뇌손상 (Penetrating Orbitocranial Injury of a Pencil in a Pediatric Patient)

  • Pencils are common instruments for children to use and play with. This report describes an unusual penetrating orbitocranial injury in a 5-year-old girl who was struck in her facewith a pencil. She was holding it at a desk, and her friend pushed her back. The pencil penetrated the left lower eyelid and went deep into the right frontal lobe through the base of the skull. It was removed at the emergency room, after which brain CT was performed to detect the development of an intracranial hematoma. No complications occurred after conservative management with antibiotics and an antiepileptic drug. Pencils can be hazardous to children, and a penetrating head injury with a pencil may be managed without cranial surgery.
