Testing and finite element modeling of stressed skin diaphragms

  • The cold formed light-gauge profiled steel sheeting can offer considerable shear resistance acting in the steel building frame. This paper conducted the full-scale test on the shear behavior of stressed skin diaphragm using profiled sheeting connected by the self-tapping screws. A three-dimensional finite element model that simulates the stressed skin diaphragm was developed. The sheet was modeled using thin element model while the supporting members were simulated using beam elements. Fasteners were represented in the numerical model as equivalent springs. A joint test program was conducted to characterize the properties of these springs and results were reported in this study. Finite element model of the full-scale test was analyzed by use of the ANSYS package, considering nonlinearity caused by the large deflection and slip of fasteners. The experimental data was compared with the results acquired by the EUR formulas and finite element analysis.

  • Melanoma is originated from the melanocyte producing the melanin which determines the complexion, and it has the highest mortality among skin cancers. Acral lentiginous melanoma(ALM) arises from extremities such as hands, feet or fingernails. Since the appearance of ALM is different from melanoma on the body, conventional auto diagnosis systems for melanoma is inappropriate to detect ALM. Therefore, ALM is typically difficult to distinguish from general nevus, resulting in delayed diagnosis and bad prognosis. In this paper, we firstly introduce a determination method for ALM by dermatologists and propose a method to rotate dermoscopic images automatically as a pre-processing for facilitating the easy determination of ALM and to select the optimal value of the Gaussian differentiation filter parameter which is significant for precise pattern extraction using the scale space analysis. From experimental results, it is shown that there exists the consistency between empirical values of the Gaussian differential filter parameter and optimal values derived from the scale space analysis to distinguish nevus and ALM.

The Effects of Simultaneous Hand Washing by Nurse and Child Before IV Injection (아동과 간호사의 손씻기 프로그램이 정맥주사와 관련한 아동의 불안, 동통과 피부손상에 미치는 효과)

  • Purpose: To examine the effects on skin injury, pain, and anxiety when nurses and children simultaneous wash their hands before IV injections. Method: A nonequivalent control group, non-synchronized design study was used with 61 children. For a month and a half, children in the experimental group(30) following hand-washing guidelines, washed their hands with nurses before IV insertion. Anxiety and pain were observed and recorded during IV insertion using the Anxiety Reaction Scale and FACES pain scale. After 48-72 hours on IV therapy with an arm-board, skin condition was checked by a research assistant according to guidelines for assessing skin injury developed by the researcher. t-test, ${\chi}^2$-test, Pearson correlation, and Kendall's tau b were used to analyze data with the SPSS program. Results: Children who washed their hands with the nurse before IV insertion showed less pain (p =.021) and skin injury (p <.001) compared to the control group. Conclusion: This finding suggests that simultaneous hand washing by nurse and child before IV injection has a strong effect on skin injury caused by arm-boards used to maintain IV therapy as well as on pain during IV insertion. This program demonstrates an effective intervention to prevent skin injuries in children on IV therapy.

Morphology and Histochemistry of the Skin of the Mud Loach, Misgurnus mizolepis, in Relation to Cutaeneous Respiration

  • The skin structure of Misgurnus mizolepis was studied based on the microanatomical investigation of skin fragments taken from four regions. The epidermis was distinguished by two types of skin glands, a small mucous cell and a large club cell. The mucous cell was acid sulfomucins (some sialomucins) but the club cell did not give any histochemical tests for mucosubstances. The presence of a well defined lymphatic system with small lymphocytes was established in the stratum germinativum layer of the epidermis. A large number of blood capillaries run very close to each other just below the basement membrane, and a definite area giving AB and PAS positive was present between the basement membrane and scale. These structural features of skin in M. mizolepis seem to be closely related with cutaneous respiration.

A Study on the Subjective Symptoms of the Printers in Chunbuk Area (전북지역 인쇄공의 심신 자각증상 조사연구)

  • The purpose of this study was to investigate the psychosomatic health status of printers. The 77 printers and 24 control group were analysed about salary, drinking, smoking, education, sex, marriage, age and working age by the THI (Todai Health Index) questionaire. THI was modified from CMI(Cornell Medical Index) and developed by Tokyo University Research Team in Japan. The resuts obtained were summarized as follows. 1. The printers, who get more salary showed high score about mental conplaints, especially, mental irritability(j), nervusness (E), lie Scale(L), aggressiveness(F) and irregualr life(G) and lower salary showed generally high score about physical complaints, especially, mouth and anus (D), digestive symptom(C) multiple subjective symptom(I). 2. According to the printers drinking amount shows the difference, eg nondrinker scored higher on mouth and anus(D), 90mg/week drinker scored higher on multiple subjective symptom(I), digestive symptom(C), depression(K), nervousness(E), and irregular life(G), 91~179mg/week drinker scored higher on impulsiveness(H), mental irritability(J), 270~359mg/week drinker scored higher on respiratory(A), lie scale (L) and aggressiveness (F). 3. The nonsmoker scored high level on mouth and anus(D), mental irritability(J). The previous smoker scored on multiple symptom(I), eyes and skin(B), digestive(C), lie scale(L), and depression(K). The present smoker scored on respiratory(A), impulsivehess(H), aggressiveness(F), nervousness(E), and irregular life(G). 4. According to the printers working age showed almost high score about subjective symptoms on 1~3 year. 5. Men printers high scored on respiratory(A). lie scale(L), aggressiveness(F), women printers scored about mental complaints, especially, impulsiveness(H), mental irritability(J), depression (K), nervousness (E). 6. According to the printers age showed high scored about, below 20 years were lie scale(L). aggressiveness(F), irregular life(G) 21~30years were multiple subjective symptom(I) respiratory (A), eyes and skin(B), mouth and anus(D), impulsiveness(H), mental irritability(J), depression (K), nervousness(E), and over 41 years were digestive(C). 7. Married printers scored high level on eyes and skin(B), digestive(C) and impulsivehess(H), and single printers on respiratory(A), mouth and anus(D), lie scale(L), mental irritability(J). 8. According to education shows the difference, eg high school scored higher on eyes and skin (B), mental irritability(J), depression(K), nervousness(E), collage and over scored higher on multiple subjective symptom(I ), respiratory (A), mouth and anus (D), lie scale (L), aggressiveness (F), irregular life (G), and middle school scored high on digestive (C), impulsiveness (H).

The Clinical Evaluation of East-West Medical Management for Central Poststroke Pain (뇌졸중후 중추성 통증에 대한 동서협진의 임상적 효율성 평가)

  • Background : Central poststroke pain(CPSP) can occur as a result of lesion or dysfunction of the brain from stroke and may cause many difficulty in the social activities and daily life. In this study, we evaluate the clinical effectiveness of east-west medical management for CPSP through VAS(visual analogue scale), infrared themography, MBI(Moderfied Barthel Index) and Rankin scale. Methods : We treated thirty patients with oriental medical treatment method and western & oriental medical treatment method. Each group has fifteen patients of the CPSP. We evaluated their pain(characterizes tingling and burning sensation, aching, hyperalgesia, and allodynia) through VAS(visual analog scale) pain score, the skin temperature of pain site by infrared thermography and assessed their mobility & rehabilitation ability through MBI(Moderfied Barthel Index), Rankin scale before and after pain treatment. Results : The skin temperature of pain site was lower than non-pain site. The difference of skin temperature improved from $0.65{\pm}0.45^{\circ}C$ to $0.39{\pm}0.25^{\circ}C$ after oriental medical treatment and $0.68{\pm}0.54^{\circ}C$ to $0.27{\pm}0.24^{\circ}C$ after western & oriental medical treatment VAS scores improved from $7.9{\pm}1.4$ to $4.7{\pm}1.6$ after oriental medical treatment and $8.1{\pm}1.3$ to $4.6{\pm}1.2$ after western & oriental medical treatment. MBI scores improved from $61.40{\pm}13.58$ to $85.00{\pm}13.85$ after oriental medical treatment and $52.26{\pm}13.52$ to $77.13{\pm}12.04$ after western & oriental medical treatment. And Rankin scale scores improved from $3.33{\pm}0.72$ to $2.46{\pm}0.74$ after oriental medical treatment and $3.60{\pm}0.82$ to $2.66{\pm}0.81$ after western & oriental medical treatment Conclusion : The difference of skin temperature and Rankin scale scores more significantly improved after western & oriental medical treatment than oriental medical treatment. According to the results, we thought east-west medical management is very useful treatment for CPSP and rehabilitation of the patients with stroke.

A Study on Allergen Skin Test Prevalence Among the Residents of Chungju City as a Non-industrial Area (충주지역 주민 피부 알레르기 반응율에 관한 연구)

  • Objectives: The main purpose of this study is to produce background data which can be compared with several large-scale industrial complex regions. Methods: The framework of this study was designed to evaluate and monitor health effects associated with low-level and long-term exposure to environmental pollutants. In this study, 1,009 local residents were recruited in the city of Chungju using personal questionnaires, medical check-ups, and allergen skin-prick tests for 12 common allergens. The prevalence rate of skin allergens was reviewed together with that from large-scale industrial areas. Results: The diagnosis prevalence and medical treatment prevalence of asthma was 5.6% and 1.7%, respectively. Allergic rhinitis was 25.4% and 16.7%, allergic dermatitis was 16.6% and 9.9% and allergic conjunctivitis was 18.7% and 7.9%, respectively. These results were similar with those of industrial complexes. The prevalence of allergen skin-prick test was 16.4% and the most common allergen was dust mites (27.9-31.6%). This result was lower than those of the industrial complexes (22.5-39.9%) and Gangneung (18.2-24.7%). Conclusion: Further study is required to find the cause of the regional differences in skin prick prevalence.

Development and Evaluation of Non-Hydrous Skin Analogue Liquid Crystal using Thermo-Sensitivity Smart Sensor

  • In this study, skin permeation enhancement was confirmed by designing it to have a structure and composition similarity to the intercellular lipids that improve miscibility with skin by cross-linked lipids poloxamer. The cross-linked lipids poloxamer was synthesized and analyzed by 1H NMR that structure dose had conjugated pluronic with ceramide3. Active component is released by modification of liquid crystal structure because PPO part, large-scale molecule block of pluronic, has hydrophobic nature at skin temperature of $35^{\circ}C$. Conjugated pluronic with ceramide3 was synthesized using Pluronic F127 and p-NPC (4-nitrophenyl chloroformate) at room temperature yielded 89%. Pluronic(Ceramide 3-conjugated Pluronic) was synthesized by reaction of p-NP-Pluronic with Ceramide3 and DMAP. The yield was 51%. This cross-linked lipids poloxamer was blended and dissolved at isotropic state with skin surface lipids, phospholipid, ceramide, cholesterol and anhydrous additive solvent. Next step was preceded by ${\alpha}$-Transition at low temperature for making the structure of Meso-Phase Lamella, and non-hydrous skin analogue liquid crystal using thermo-sensitivity smart sensor, lamellar liquid crystal structure through aging time. For confirmation of conjugation thermo-sensitivity smart sensor and non-hydrous skin analogue liquid crystal, structural observation and stability test were performed using XRD(Xray Diffraction), DSC(Differential Scanning Calorimetry), PM (Polarized Microscope) And C-SEM (Cryo-Scanning Electron Microscope). Thermo-sensitivity observation by Franz cell revealed that synthesized smart sensor shown skin permeation effect over 75% than normal liquid crystal. Furthermore, normal non-hydrous skin analogue liquid crystal that not applied smart sensor shown similar results below $35^{\circ}C$ of skin temperature, but its effects has increased more than 30% above $35^{\circ}C$.

The Change of the Digital Skin Temperature in Dental Outpatients by Biotrainer (Biotrainer를 이용한 치과외래환자의 수지피부온도변화에 관한 연구)

  • In order to evaluate objectively the anxiety level in dental treatment, the author used Biotrainer(BF-120R), one of the skin temperature biofeedback apparatus, to examine 68 dental outpatients on their digital skin temperature change due to routine consecutive dental procedures(oral examination, anesthetic injection, cavity preparation, fissure sealing, polishing). The subjective anxiety level change was also evaluated by visual analog scale. The obtained results were as follows : 1. The skin temperature decreased through consecutive procedures and the temperature in each procedure decreased until 60sec, and then increased on 120sec. 2. The temperature changes in Preparation and Injection were greater than those in other procedures. 3. Generally, male exhibited more change of skin temperature in all procedures than female. 4. Type II, continuously decreasing after procedure, occupied the most in all patients and yhad the lowest beseline temperature. 5. The anxiety level of before-procedure was higher than that of after-procedure and the levels in Injection and Preparation were higher than in other procedures.

Split-thickness Skin Graft on the Face from the Medial Arm Skin (상완내측 피부를 이용한 안면부의 부분층 식피술)

  • Purpose: Full-thickness skin grafts are usually used in facial reconstruction, but on occasion, split-thickness skin graft is also used from the scalp due to the limitation of donor site. However, there were complications, such as alopecia, folliculitis and blood loss. In addition, it can not be used in patients with baldness. Under the circumstances, we used medial arm skin as split-thickness skin graft donor site in lieu of scalp. We investigated the efficacy of the medial arm skin as a donor site of facial skin graft in comparison with scalp. Methods: From 2000 to 2005, the split-thicknesss skin grafts were performed using the medial arm skin in 10 patients and the scalp in 10 patients. We inspected the skin color match, texture match by the visual analogue scale. Scar contracture was estimated by the Visitrak $grade^{(R)}$(Smith & Nephew). The statistical analysis was performed by SPSS 12.0. Results: There was a more satisfaction in color match, texture, and scar contracture in medial arm skin than in scalp. Conclusion: According to these results, medial arm skin may be used efficiently as an alternative donor site of scalp in the facial reconstruction.
