Iatrogenic Spark Burn Injury to the Chest From a Transcutaneous Pacing Patch (경피적 심장 충격 장치의 사용으로 발생한 의인성 전기 화상의 증례 보고)

  • Iatrogenic electrical burns that occur from the use of a defibrillator, a paddle-type cardiac shock device, have been reported in various forms. Electrical burns are usually conducted directly through the skin and are more damaging than scald burns or contact burns. A transcutaneous cardiac pacing device is a patch-type cardiac shock device that automatically delivers a shock when an abnormal heart rhythm is detected. We introduce a unique case of iatrogenic electrical burns caused by the transcutaneous pacing patch of a cardiac shock device. Electrical energy was converted into a spark due to foreign bodies deposited around the patch, resulting in damage to the peripheral area of the skin.

STUDIES ON THE SKIN TROUBLE AND THE FACIAL COLOR CHANGE DUE TO HORMONAL CYCLE IN FEMALE

  • Many eastern females concern themselves about the condition and the color of their skin. The purpose of the present study is to classify the skin trouble and the change of the facial color due to hormonal cycle in female. We examined the actual cricumstances by questionnaires, and made patch tests of methyl nicotinate, representing rubefacient, to estimate the epidermal penetratin rate, and measured the facial color change during the menstrual cycle period to invest the correlation factors between skin trouble due to cosmetics and facial color change. Fifty-two percent of subjects had skin trouble relating to cosmetics. One second of subjects with skin trouble due to cosmetic complained the change of sysptom by menstrual cycle. The changes of systptom were related on premenstrual period. The skin trouble developed mainly on the first trimester of the pregnancy. In patch test of methyl nicotinate, most cases showed decreased threshold of the reaction on minstruation, and other cases showed increased reactivity of the skin of menstruation. In facial color measurements, it proves in the appearance of red spot, darkness, increasing the value and turning the hue to yellowish, Also it demonstrates that premenstruation hue turn red and value level decrease. During the period, facial color turns pale and hue progress to yellow. This fact coincides with the questionnaire. We have quqntified through questionnaire, which demonstrates it has good correlation with done to the subjects among the internal environment factors hormonal cycle influences the facial color change and the skin trouble due to cosmetics. Through this paper the development of an more fragmented make-up and skin care products is required to increase the interest to females, to make true the creation of beauty.

Dental materials in patients with oral mucosal disease based on the results of patch test study (구강점막질환환자에서 치과재료를 이용한 첩포시험 결과에 대한 고찰)

  • The aim of this study was to investigate the frequency of positive patch test reaction to dental materials in patients with oral mucosal diseases. Epicutaneous patch test was performed in 110 patients with oral mucosal diseases; 41 patients with oral lichen planus(OLP), 44 patients with burning mouth syndrome(BMS), 25 patients with other oral mucosal diseases including recurrent aphthous ulcer and mucous membrane pemphigoid. The obtained results were as follows: Oral gold restorations were most common in patents with oral mucosal diseases and porcelain fused metal crown, implant appeared in the order. 33 of 110 patients did not appear skin reactions (negative, 30%) and 77 patients (positive, 70%) had skin reactions including redness, rash, blisters. Dental materials causing positive reaction to patch test were mainly as gold-sodium-thiosulfate (26.7%), nickel sulfate(Ni) (22.7%), cobalt chloride(Co) (14.7%), palladium chloride(Pd) (11.9%), potassium dichromate (10.7%) in order, respectively. In conclusion, old metal restorations could be the cause of oral mucosal diseases and epicutaneous patch test could be used as a tool to improve the oral conditions.

  • We have studied the transdermal flux of prostaglandin $E_1$ $(PGE_1)$ from a hydrogel patch through hairless mouse skin, to test the possibility of developing a transdermal delivery system. Karaya gum patch containing $PGE_1$ was prepared by casting method. $PGE_1$ was stable in the patch for 10 weeks. The effect of current application, enhancer (propylene glycol monolaurate : PGML), adhesive and patch thickness on the flux was studied using side-by-side diffusion cell. Passive flux of $PGE_1$ was negligible. Cathodal delivery increased the flux about 20 fold. As the concentrations of PGML increased, flux increased. When 5% PGML was used as the enhancer, maximum flux by cathodal iontophoresis was $55\;{\mu}g/cm^2\;hr$. It increased about 2 folds to $100\;{\mu}g/cm^2\;hr$, when the amount of PGML used was 9%. Large increase in flux and the decrease in time to reach maximum flux were observed when the skin was pretreated with neat PGML (maximum flux obtained was about $200\;{\mu}g/cm^2\;hr$). Use of adhesive decreased the flux significantly. To the contrary of our expectation, increase in current density decreased the flux. These flux data together with the stability data indicate that, though the onset of sufficient delivery occur after 1-2 hours of application, therapeutic amount of $PGE_1$ can be delivered through skin using iontophoresis and penetration enhancer.

Treatment of Peripheral Facial Palsy with Skin Damage Caused by Folk Remedies Using Korean Medicine: A Case Report (민간요법으로 피부손상을 동반한 말초성 안면마비에 대한 한의치료 증례 보고)

  • Peripheral facial palsy generally results from damage to the facial nerve. As facial asymmetry is observed, it accompanies not only functional problems but also psychological and social difficulties. Medical institutions treat most patients, however, there are still patients who rely on invasive methods by unqualified practitioners for fear of sequelae. This case describes a 61-year-old woman who experienced facial palsy twice. She visited our hospital with skin damage after folk patch therapy for her facial palsy. Combined Korean medicine treatment was administered during treatment period. After treatments, the symptoms of facial palsy and skin lesions improved. This case showed that Korean Medicine was efficacious in improving symptoms of facial palsy with damaged skin caused by folk patch therapy. Education and awareness of uncontrolled invasive treatments for facial palsy are needed.

Investigation on Adhesion Control Standards and Skin Adverse Effects of Skin Attached Formulations (파스제의 점착력 관리기준 및 피부 부작용 조사연구)

  • Objectives: This paper was aimed to investigate the adhesion control standards of pain relieving patch (PRP) drugs and to survey it's adverse effects on the skin of patients for safe use of PRP drugs. Methods: In this study, the related documents of PRP drugs of Korea pharmacopoeia (KP), United States pharmacopoeia (USP), Japanese Pharmacopoeia (JP), European pharmacopoeia (EP), and information web sites of the Ministry of Food and Drug Safety (MFDS) were surveyed. Also, the past and current labeling of PRP drugs marketed in the pharmacy was investigated and compared. Results: In KP and JP, the lower limit standard for PRP's adhesion control is established, but the upper limit standard is not designated. In USP and EP, neither the lower nor upper limit standard is established. The main reasons of skin adverse effects are considered as inherent adverse reactions of the applied drugs for PRP. Another reason is involved in patient's medication mistakes related to PRP's adhesion control, respiratory depression of skin according to physical skin closure, and microbial growth, etc. Conclusion: For safe use of PRP drugs, we proposed ensured guidelines like additional instructions of pharmacist's prescription and detailed labeling systems for usage of PRP drugs applied on skin.

  • In every large hospital, nurses must perform simple repetitive tasks such as measuring body temperature. Such tedious work reduces nurses' motivation to provide quality medical care, which is an important element of the medical services provided by a hospital. If a device were available to measure body temperature, nurses could focus on the more important aspects of providing quality medical care to the patients. However, body temperature is generally measured from the throat, anus, tympanum or armpit, where it is difficult to affix a patch type device. In addition, general body temperature measuring points shows moving artifact error; therefore, it is not good point to continually measure the temperature. In this paper, a patch type skin temperature measuring system was developed. To appropriately measure the skin temperature, a thermal transducer was implemented with a thin (0.5 mm) temperature sensor. The system is small and thin ($H6.6{\sim}5.3{\times}L35{\times}W24\;mm$), and weighs only 5 g including a battery, case and circuit; therefore, it is small and light enough to function as a patch type device. Moreover, the system worked for 5 days. To investigate differences between the experimental and conventional thermometer, simple clinical experiments were performed with 17 volunteers, and the result showed some correlation between the implemented system and conventional thermometer (Correlation coefficient = 0.647, P<0.1).

Adhesive Polyurethane-based Capacitive Electrode for Patch-type Wearable Electrocardiogram Measurement System (패치형 웨어러블 심전도 측정 시스템을 위한 접착성 폴리우레탄 기반의 용량성 전극)

  • Wearable medical device has been a resurgence of interest thanks to the development of technology and propagation of smart phone in recent years. Various types of wearable devices have been introduced and available in market. Capacitive coupled electrode which measures electrocardiogram over cloth is able to be applied wearable device. In previous approaches of capacitive electrode, they need proper pressure for stable contact of the electrode to body surface. However, wearable device that gives pressure on body surface is not suitable for long-term monitoring. In this study, we proposed adhesive polyurethane-based capacitive electrode for patch-type wearable electrocardiogram (ECG) monitoring device. Self-adhesive polyurethane make the electrode and whole system be adhered to the surface of skin without any pressure. The patch-type system is consisted of analog filter, analog-to-digital converter and wireless transmission module and designed to be attached on the body as a patch. To validate the feasibility of the developed system, we measured ECG signal in stable and active state and extracted heart rate. Therefore, we observed skin response after long-term attachment for biocompatibility of the adhesive polyurethane and adhesive strength of it. The result shows the possibility of applying the developed system for ECG monitoring in real-life.

In Vitro and In Vivo Studies of Topical Delivery System of Gentisic Acid in Hairless Mice

  • Gentisic acid is a skin-whitening agent which inhibits the tyrosinase activity, an essential enzyme in the process of biological synthesis of melanin. Since melanin is synthesized in melanocytes located between the viable epidermis and dermis layer, drug amount delivered into the epidermis/dermis layer can provide valuable information for the biological effect of skin-whitening agents. The purpose of this study was to prepare the gentisic acid patches with 2% dodecylamine as enhancer, and to observe the in vitro skin permeation and in vivo skin deposition of gentisic acid. Gentisic acid in DuroTak 87-2510 patch formulation permeated across hairless mouse skin at the rate of $40.79\;{\mu}g/cm^2/hr$. In vivo study showed that the gentisic acid amount in both the stratum corneum and the viable epidermis/dermis increased with the increase of application time. The amount of gentisic acid in the stratum corneum was higher than that in the epidermis/dermis layer, and was expected to provide a reservoir effect even after removing the patches. Thus, the patch formulation seems to be useful for the topical delivery of skin-whitening agent into the epidermis/dermis layer, the target site.

Analysis of Color Constancy Methods for Recovering Skin Color Independent of Illuminants (광원에 독립적인 피부색 복원을 위한 색 항등성 기법 분석)

  • The skin color has been used as important cues in the systems for detecting or recognizmg the face. However, the color difference in images under different illuminants makes it difficult to find out the skin in these systems. For solving the problem, this paper proposes a method of recovering skin colors based on well-known color constancy approaches, such as Retinex, Gray World, White Patch, and Simplified Horn. To acquire experimental images under the colored scene illumination, the effects of colored illuminants were added to source images. Next, result images, having the corrected skin color by the constancy methods, were derived from the source images. The experiment results showed that most of the skin colors in our experiments were recovered into some steady range in the color space, and that Gray World had higher performance than the other methods compared.
