자외선에 의한 피부반응 (Effects of Ultraviolet Radiation on the Skin)

  • 자외선이 피부에 조사되면 다양한 피부반응을 초래한다. 이러한 반응은 크게 1) 급성 반응과 2) 만성반응으로 나눌 수 있다. 급성반응으로서는 피부가 붉어지는 홍반반응, 피부색이 검어지는 색소반응, 그리고 피부두께의 변화 등을 들 수 있다. 홍반반응은 피부의 주된 세포인 각질형 세포를 비롯하여 진피세포들이 광여하여 진피혈관을 확장시키는 반응을 말한다. 홍반반응은 자외선의 파장, 광량, 피부의 조건, 환경조건 등에 따라 달라지며 피부에는 각질형성세포가 변형된 일광화상세포가 출형한다. 홍반반응은 가장 흔한 주된 반응이나 그 기전에는 명확하지 않은 점이 아직도 많이 있다. 색소반응은 멜라닌세포가 관여하는 반응으로서 자외선 조사 후 즉시 피부가 검어지는 즉시세포침착과 수일 후 검어지는 지연색소침착으로 나눌 수 있다. 색소반응과 피부두께의 변화 등은 자외선 조사로부터 몸을 보호하기 위한 방어작용의 일종이다. 자외선 조사에 의한 만성반응으로는 광노화현상과 광 발암현상을 들 수 있다. 광노화란 장기간에 걸친 광노출로 인한 피부의 노화를 말하며 생리적인 노화와는 임상적, 조직학적으로 다른 점이 많다. 자외선의 장기노출은 피부암을 발생시키는 중요한 원인의 하나로서 우리나라도 피부암 발생이 점차 증가하고 있다. 서울대학교병원 피부과에서의 피부암 발생빈도는 70년대에는 외래환자의 0.2% 정도였으나 80년대에는 0.4-0.8%를 차지하고 있다. 앞으로 우리나라의 경제성장과 함께 수명의 증가, 레져나 스포츠의 증가, 피부노출의 증가와 함께 오존층 파괴로 인하여 피부에 조사되는 자외선의 절대량의 증가는 자외선에 의한 피부의 급 만성반응을 더욱 증가시킬 것으로 예상되며 이에 대한 적절한 대책이 요구된다.

Quetiapine Fumarate의 제형 변경 후 발생한 피부 발진 1예 (A Case of Skin Eruption Occurred after Switching Formulation of Quetiapine Fumarate)

  • Adverse drug reactions are very common in clinical practice, and skin is one of the most frequent organs for adverse drug reactions. We report a case of a 71-year-old male patient who developed skin eruptions after switching formulation of quetiapine immediate release(IR) to quetiapine extended release(XR). He had been taking quetiapine IR(400mg/day) for treatment of manic episode which was developed one year ago. The patient showed great improvement of symptoms after taking quetiapine IR for about one year, thus dosage of medication was reduced to 50mg/day on the average. Unfortunately dose reduction has tended to worsen symptoms, so dose of quetiapine was increased again to 200mg/day with formulation changes to XR. Two days after he took new formulation, erythematous papules were occurred over his anterior neck and ventral side of left wrist. As he stopped quetiapine XR, the skin lesions gradually subsided. And he was successfully treated with readministration of quetiapine IR without any skin lesions.

록소프로펜 플라스터의 제제설계 및 평가 (Formulation and Evaluation of Loxoprofen Plasters)

  • To develop a novel transdermal delivery system of loxoprofen (LP), a potent antiinflammatory and analgesic agent, the effects of vehicle composition and drug loading dose on the skin permeation property were investigated. And in vivo skin absorption property studied by analysing the $C_{max}$ and AUC was investigated after applying the developed plaster systems on rabbit back skin. Addition of isopropyl myristate (IPM) and IPM-diethylene glycol monoethyl ether (DGME) cosolvent in the plaster showed higher permeation rates than those from propylene glycol laurate-DGME cosolvent systems. As the concentration of LP in the plaster increased from 0.56 mg/$\textrm{cm}^2$ to 1.19 mg/$\textrm{cm}^2$, the drug release and skin permeation rates increased linearly. At loading dose of 1.19 mg/$\textrm{cm}^2$, the flux reached 35.6 $\mu$g/$\textrm{cm}^2$/hr. New LP plasters showed a good adhesive property onto skin, and showed no crystal formation. The AU $C_{0-24hr}$ and $C_{max}$ after dermal application of LP plaster (60 mg/70 $\textrm{cm}^2$) were found to be 6951$\pm$230 ng.hr/ml and 400$\pm$44 ng/ml, respectively. And the plasma concentration maintained above 300 ng/ml up to 24 hr period. In the carrageenan-induced rat paw edema test, LP plaster showed similar inhibition rate with marketed ketoprofen (Ketoto $p^{R}$) plaster.aster.r.

탄소입자 치료 시 열가소성 고정기구의 공기층에 따른 선량 변화 평가 (Evaluation of Dose Variation according to Air Gap in Thermoplastic Immobilization Device in Carbon Ion)

  • 목 적: 환자 체표면과 고정기구 사이에 발생하는 공기층 두께에 따른 선량 변화를 치료 계획을 통해 알아보고자 한다. 대상 및 방법: 팬텀과 열가소성 고정기구 사이에 5 mm 두께의 Bolus를 0, 1, 2, 3장을 놓아 공기층의 두께를 조절하였고 고정기구를 씌워 총 4가지 조건으로 전산화 모의단층촬영을 시행하였다. 430 cGy (Relative Biological Effectiveness,RBE)씩 6번이 조사 되도록 계획하였으며, 임상표적체적의 95% 부피에 전달된 선량이 2580 cGy (RBE)가 되도록 치료 계획을 수립하였다. 임상표적체적의 선량은 Lateral dose profile의 반치폭값으로 평가하였고 피부 선량은 선량 체적 곡선으로 평가하였다. 결 과: 임상표적체적에서 Lateral dose profile 반치폭 값은 4.89, 4.86, 5.10, 5.10 cm로 나타났다. 피부에서 4가지 조건의 선량의 평균값은 D95%3.25±1.7 cGy (RBE), D30%1193.5±10.2 cGy (RBE)의 차이를 보였으며 처방 선량 1%에서의 피부 부피 값 평균은 83.22±4.8% 이내의 차이를 확인하였다. 공기층 두께 변화에 따른 임상표적체적과 피부에서의 선량 모두 큰 변화를 보이지는 않았다. 결론 : 탄소입자 치료를 위해 Solid 형태의 고정기구 제작 시 약간의 공기층은 CTV의 선량 적용 범위를 벗어나지 않는다.

수종 용제와 투과 촉진제를 이용한 로바스타틴의 용해성 및 피부 투과 증진 (Enhanced Solubility and In vitro Skin Permeation of Lovastatin Using Some Vehicles and Penetration Enhancers)

  • To enhance the in vitro permeation of lovastatin through excised hairless mouse and human cadaver skins, solubility was determined in various hydrophilic and lipophilic vehicles, and the effects of vehicles and penetration enhancers on the skin permeation from solution formulations were investigated. Solubility of lovastatin was highest in N-methyl-2-pyrrolidone (NMP) ($278.2{\pm}10.1$ mg/ml) and dimethyl sulfoxide (DMSO) ($162.2{\pm}9.7$ mg/ml). Among different pure vehicles used, NMP, DMSO, propylene glycol and isopropyl myristate provided some drug permeation ($6.9{\pm}1.1$, $5.9{\pm}1.6$, $3.0{\pm}0.5$ and $2.2{\pm}0.3{\mu}g/cm^2$ at 24 hr, respectively) through hairless mouse skin. The addition of oleic acid, linoleic acid and oleyl alcohol to DMSO showed the maximum permeation at around 5 v/v%, however, capric acid and caprylic acid had no enhancing effect. The increase of enhancer concentrations showed bell-shaped permeation rate, suggesting the presence of optimal concentration in lovastatin penetration. Increasing donor concentration from 10 mg/ml to 80 mg/ml in DMSO and a cosolvent of DMSO, NMP and DGME (3 : 3 : 4 v/v) did not show significant dose dependent permeation in both hairless mouse and human cadaver skins. The maximum lovastatin flux through human cadaver skin was found to be $0.87{\pm}0.46{\mu}g/cm^2$/hr with 5 v/v% linoleic acid and donor dose of 4 mg/0.64 $cm^2$ in the cosolvent. These results suggest that transdermal delivery of lovastatin would be feasible by establishing the optimal concentrations of donor dose and unsaturated fatty acids in appropriate vehicles.

진단방사선촬영에서 광자극형광선량계의 에너지의존성에 대한 보정인자 (Correction Factor for the Eenergy Dependence of a Optically Stimulated Luminescent Dosimeter in Diagnostic Radiography)

  • 이 연구의 목적은 진단방사선촬영에서 환자의 피부선량을 측정하는 나노도트선량계의 에너지의존성에 대한 보정인자들을 구하는 것이다. 보정인자들은 랜다우어사에서 제공한 팬텀 정에 관한 X-선에 상대적인 선량계의 에너지반응그래프와 로사도 등이 발표한 IEC의 RQR 표준방사선 품질들에 대한 평균에너지 값들을 사용하여 구하였다. 결과들은 관전압 40-150 kVp에서 1-1.33의 보정인자들을 나타냈다. 얻어진 보정인자들은 각 관전압에서 정확한 피부선량 측정을 위하여 임상에 사용하는데 유용할 것으로 생각된다.

Evaluation of the in vivo skin moisturizing effects and underlying mechanisms of pomegranate concentrate solution and dried pomegranate concentrate powder

  • Objectives: The aim of this study was to confirm the dose-dependent skin moisturizing effects of dried pomegranate concentrate powder (PCP) and pomegranate concentrate solution (PCS) in ICR mice. Materials and methods: To observe the in vivo skin moisturizing effects and possible underlying mechanisms of PCP and PCS, oral PCP (100, 200, and 400 mg/kg) and PCS (1, 2, and 4 mL/kg) were administered to normal ICR mice. Changes in body weight, skin water content, and skin type I collagen and hyaluronan contents were measured. Additionally, the mRNA expression levels of hyaluronan synthase (Has) 1, 2, and 3, and collagen type I alpha (COL1A) 1 and 2 were determined in the dorsal skin of mice by real-time reverse transcription polymerase chain reaction (RT-PCR). Results: Significant and dose-dependent increases in dorsal skin water content and type I collagen and hyaluronan contents were seen in PCP and PCS-treated mice. Moreover, the mRNA levels of Has 1, 2, and 3, involved in hyaluronan synthesis, and of COL1A1 and COL1A2, involved in collagen synthesis, were significantly and dose-dependently upregulated in PCS- and PCP-treated mice. Conclusions: In this study, PCP and PCS led to favorable skin moisturizing effects as indicated by increased skin water content and the upregulation of hyaluronan and collagen synthesis enzymes in mice treated with PCS (4 mL/kg) and PCP (200 mg/kg).

CJ-50001 (rG-CSF)의 Rabbit에서의 국소자극성 (Study on Local Irritation of CJ-50001 (rG-CSF) in Rabbits)

  • The local irritation study (skin & occular irritation tests) of CJ-50001, a rG-CSF (recombinant granulocyte-colony stimulating factor) was performed in Japanese White rabbits. CJ-50001 was administered at a dose of 150 $\mu\textrm{g}$/rabbit (300$\mu\textrm{g}$ /ml, 0.5 ml) to the bare skin and at a dose of 30 $\mu\textrm{g}$/rabbit (300 $\mu\textrm{g}$/ml, 0.1 ml) to the conjunctival sac of the eye, respectively. In these experiments, there were no clinical signs which were related to CJ-50001 compared with control group. In conclusion, CJ-50001 doesn't have any irritating activity to skin and eye as 0.03% solution.

  • The local irritation study (skin and occular irritation tests) of HM10411, a rhG-CSF (recombinant human granulocyte-colony stimulating factor) was carried out in New Zealand White rabbits. HM10411 was applied to the bare skin at a dose of 2.5 mg/rabbit (5.0 mg/ml, 0.5 ml) and to the conjunctival sac of eye at a dose of 0.5 mg/rabbit (5.0 mg/ml, 0.1 ml) , respectively. In this study, there were no clinical signs which were related to HM10411 compared with those of control group. From above results, HM10411 has not any irritating activity to skin and eye in rabbits.

흉부 X선촬영 조건에 따르는 의료피폭에 관한 조사연구 (A Survey on Medical Radiation Dose by the Radiographic Conditions of Chest)

  • It is a matter of common knowledge that madical radiation is most accented for of radiation is doses applied to the whole of people, and of them the radation dose by radiography diagnosis is mainly prevalent. In applying X-rays to a certain man for radiography diagnosis a radiologyist will have to have an absolute sense of mission concerning the reduction and prevention of the patient's radiation dose as the radiologyist obligation. Accordingly, the radiography conditions of the patient's chest employed 197 medical facilites were surveyed and skin dose was computated by the IPH Bit system and examined. As a result, it was shown that the average skin dose was $288\;{\mu}Sv$, its minimum value was $1600\;{\mu}Sv$, which was over 32 times its minimum value. This shows that the appropriate radiography method has not been applied at applying X-ray to the patient. It comes from the performance of X-ray equipment, the choice of auxiliary equipment materials etc. But the most important thing is to master the appropriate radiography condition, and therefore this point will have to be kept in mind.
