Concurrent presentation of porocarcinoma and basal cell carcinoma arising on a capillary malformation: a case report

  • Porocarcinoma (PC) and basal cell carcinoma (BCC) are distinct skin cancers. Few studies have documented the occurrence of two concurrent types of skin cancers, and to the best of our knowledge, this represents the inaugural report of such a coexisting lesion arising from a capillary malformation. Herein, we report a case of concurrent PC and BCC presenting with capillary malformation. A 93-year-old woman visited our hospital with a protruding mass in her right nasal ala that appeared as a capillary malformation. A biopsy was performed on the skin lesion, and BCC was diagnosed. A wide excision was performed. Permanent biopsy revealed that the skin lesion was a PC with basal cells and squamous differentiation. The safety margin of the deep tissue margin was < 0.1 cm; however, considering the advanced age of the patient, further excision was deemed to not possess any benefits. This case illustrates the importance of recognizing the possibility of multiple skin cancers, even in patients with benign lesions such as capillary malformations. The rarity of this presentation highlights the importance of thorough investigation and histopathological examination of skin lesions in guiding appropriate surgical excision.

개에 발생한 아연부족성 피부병의 임상병리학적 관찰 (A clinico-pathological observation in dogs associated with zinc-deficient dermatosis)

  • Canine dermatosis associated with zinc deficiency naturally occurred in three districts of Kyungpook. A diagnosis was made by clinical findings, clinico-pathological examinations, skin biopsy, and therapeutic tests. The result was diagnosed as a zinc-deficient dermatosis. The disease occurred in rapidly growing Tosa-dog hybrids 5 to 7 months old. Skin lesions included erytherma, alopecia, crusting, scaling around the mouth, eyes, legs and tail. Clinico-pathological findings were decreased serum zinc levels, slightly increased white blood cells, and secondary Staphylococcus intermedius infection of the crusts. The skin biopsies for histopathological examination revealed marked parakeratotic hyperkeratosis. The patients with skin lesions were successfully treated by zinc methonine(Zinpro) medication In conclusion, serum zinc level and skin biopsy are very helpful in diagnosis of canine zinc-deficient dermatosis.

  • Diabetic foot wounds have a significant effect on the health-related quality of life of patients. As diabetic foot wounds are usually chronic and recurrent, it is possible that they can lead to skin malignancies. Several factors can make it difficult to make an early and accurate diagnosis of skin malignancies of the foot in patients with diabetes mellitus. Even though the prevalence of diabetes mellitus and diabetic foot wounds is increasing, currently there are no guidelines for the biopsy of diabetic foot wounds. We have evolved a criterion for the above based on six cases of diabetic foot wounds with concomitant skin malignancies. We recommend that clinicians should broadly consider implementing this criterion when managing patients with diabetic foot wounds.

악하부 농양으로 발병한 육아종증 다발 혈관염 1예 (A Case of Granulomatosis with Polyangiitis with Submandibular Abscess as Primary Manifestation)

  • Granulomatosis with polyangiitis (GPA), formerly known as Wegener's granulomatosis, is a rare immunological small vessel disease which usually affects respiratory tract and kidneys. However, salivary gland involvement in GPA is rare as a primary manifestation. We report a case of GPA with the primary presentation of submandibular gland involvement. A 48-year-old female patient presented submandibular swelling with a skin defect that lasted for 1 month. Although the biopsy result was chronic inflammation, the skin defect did not heal for a month. Further imaging study revealed multiple lung and renal masses. More clinical manifestations such as gingivitis, ischemic change of finger joint and nasal tip skin, and positive c-ANCA test was presented. Additional biopsy was made at the submandibular gland, lung, and finger skin. The patient was finally diagnosed with GPA and treated with steroid pulse therapy and cyclophosphamide. The patient showed improvement of prior clinical symptoms.

  • 최근에 우연히 발견되는 신장 종괴의 증가와 국소 치료, 표적 치료와 같은 신장암 치료 선택지의 다양화, 그리고 신장 이식의 보편화에 따라서 신장의 조직 검사가 증가하고 있는 추세이다. 그러나 신장은 피부부터 장기까지의 거리가 멀어 조직 검사의 난이도가 높은 편으로 특히 초심자들은 신장의 조직 검사를 부담스럽게 여길 수 있다. 이 종설에서 우리는 신장 조직 검사의 적응증과 기술적인 고려 사항들, 진단 정확도, 그리고 합병증에 대해서 알아보겠다. 특히 실전적인 팁들을 다양한 증례들과 함께 알아보며 다양한 상황에서도 안전하고 효과적으로 조직 검사를 시행할 수 있게 도움이 되고자 한다.

Ehlers-Danlos Syndrome with Classical Subtype in a Cat

  • Ehlers-Danlos syndrome (EDS) is a rare genetic disorder in dogs and cats and has been mostly reported in purebred cats. In this study, we report a case of a 1-year-old castrated male Korean shorthair cat, who presented with multiple small skin tears and bruises distributed over the entire trunk area. The cat's skin was hyperextensible and easily torn with gentle touch. The skin extensibility index of the cat was 25%, indicating the possibility of EDS. The cat exhibited no signs of pruritus or inflammation, and no underlying disease was found. However, radiography revealed hip joint subluxation and arthritis. Following this, biopsy of the lacerated skin was performed. Histopathological examination of the skin revealed that in the dermis adjacent to the lesions, the collagen fibers were irregular in size and width, with a slightly thinner epidermis, and increased interfibrillar spaces containing low numbers of scattered well-differentiated fibroblasts and mast cells. Histopathological examination of the skin confirmed EDS. The symptoms observed in the cat, including skin hyperextensibility, multiple bruising, hip joint subluxation, and arthritis, corresponded to the classical subtype of EDS in humans. Thus, this study is a rare report of a classical EDS case in a Korean shorthair cat. This study suggests that skin extensibility index and biopsy are useful diagnostic procedures for confirming EDS in animals until a more definitive genetic test is established.

A Rare Case of Pulmonary Epithelioid Hemangioendothelioma Presenting with Skin Metastasis

  • Epithelioid hemangioendothelioma (EHE) is a well-differentiated and rare vascular tumor. Systemic metastases are uncommon. Herein, we present a patient with skin metastasis of pulmonary EHE (PEH) that was treated by wide excision. A 76-year-old male was evaluated due to pulmonary thromboembolism and a solitary pulmonary nodule. A biopsy was performed and pathological examination of the mass confirmed EHE. No metastasis was observed. The patient returned to care approximately two years later due to a painful nodule in the right lower leg. A skin biopsy showed metastatic EHE from the lung. We used a safety margin of 1 cm based on clinical experience, because no prior case had been reported regarding the resection margin appropriate for primary cutaneous EHE and skin metastases of PEH. At four months after surgery, the patient recovered without complications or recurrence. Skin metastasis of PEH is extremely rare, and only two cases have been reported in the literature. In this case, we report a rare case of PEH with histologically diagnosed skin metastasis that was successfully treated by curative resection. It is expected that this case report will provide a helpful contribution to the extant data regarding PEH metastases.

Quantitative sudomotor axon reflex test (QSART) as a diagnostic tool of small fiber neuropathy

  • Small fiber neuropathy is a painful neuropathy that cannot be assessed using nerve conduction studies. A skin biopsy and quantitative sensory testing (QST) are the gold standards for small fiber neuropathy diagnosis. However, a skin biopsy is invasive and commercially unavailable in Korea. QST is a method involving a thermal threshold, but its results can be affected by cognition as well as lesions of the central nervous system. Quantitative sudomotor axon reflex test (QSART) is a quantitative method of assessing sweat glands innervated by small fibers. In this review, we assessed the utility of QSART in evaluating small fiber neuropathy.

The Effectiveness of Mapping Biopsy in Patients with Extramammary Paget's Disease

  • Background Extramammary Paget's disease (EMPD) is an intraepithelial carcinoma usually occurring on the skin or mucosa of the perineum. Clinically, it resembles eczema or dermatitis, and misdiagnosis and treatment delays are common. The treatment of choice for EMPD is a wide excision with adequate margins. Wide excision with intraoperative frozen biopsy and Mohs micrographic surgery are common methods; however, these are associated with a high recurrence rate and long operation time, respectively. Methods Between January 2010 and June 2013, 21 patients diagnosed with EMPD underwent mapping biopsy. Biopsy specimens were collected from at least 10 areas, 2 cm from the tumor margin. When the specimens were positive for malignancy, additional mapping biopsy was performed around the biopsy site of the positive result, and continued until no cancer cells were found. Based on the results, excision margins and reconstruction plans were established preoperatively. Results The patients (18 male, 3 female) had a mean age of 66.5 years (range, 50-82 years). Almost all cases involved in the perineal area, except one case of axillary involvement. Permanent biopsy revealed one case (4.8%) of positive cancer cells on the resection margin, in which additional mapping biopsy and re-operation was performed. At the latest follow-up (mean, 27.4 months; range, 12-53 months), recurrence had not occurred. Conclusions Preoperative mapping biopsy enables accurate resection margins and a preoperative reconstructing plan. Additionally, it reduces the operation time and risk of recurrence. Accordingly, it represents an effective alternative to Mohs micrographic surgery and wide excision with intraoperative frozen biopsy.

Cutaneous asthenia associated with Ehlers-Danlos syndrome in a Yorkshire terrier

  • tisue disease of dog and cat, resembling Ehlers-Danlos syndrome (EDS) in man. EDS is characterizedby loose, hyperextensible and, fragile skin, it is attributed to mutations in connective tisue gene. Thesemutations cause defects in type I or II collagen synthesis and as a result lack of strength or elasticityto skin, joint, ligament and vessels. EDS-affected animals often experience subcutaneous hematomas thathave long bleeding times. The 4-years old male Yorkshire terrier was evaluated because of subcutaneoushematoma after stifle surgery. Clinical examination revealed a thin and hyperextensible skin and jointlaxity. The degree of skin extensibility index was 23.4%, marked skin stretchy. Clinical diagnosis wasconfirmed by histophathological examination of a skin biopsy revealing reduced packing density ofcollagen fiber of skin.
