수술 전 상악 교합평면각이 상하악 동시 수술 후 골격 안정성에 미치는 영향 (THE EFFECT OF PRE-OPERATIVE MAXILLARY OCCLUSAL PLANE ANGLE TO POST-OPERATIVE SKELETAL STABILITY AFTER TWO-JAW SURGERY)

  • The purpose of this study was to evaluate the effects of maxillary occlusal plane angle to postoperative skeletal stability by comparative analysis after two-jaw surgery of patients with skeletal CIII malocclusion. This study was made with lateral cephalometric radiography of 52 patients with skeletal class III malocclusion that were performed to Le Fort I osteotomy and BSSRO. And 52 patients were divided to Group A(n=30) and B(n=22). Maxillary posterior impaction was not conducted in Group A, which the pre-operative maxillary occlusal plane angle was in a normal range, and for Group B, which the pre-operative maxillary occlusal plane was low, the maxillary posterior impaction was conducted. The results were obtained as follows : 1. The relapse rate of Group A, which the pre-operative maxillary occlusal plane angle was in a normal range, was relatively stable compared to Group B, which the pre-operative maxillary occlusal plane was low. 2. The relapse rate of each measurement of Group B, which had the maxillary occlusal plane altered during the operation, was somewhat high, and of those, the post-operative relapse rate of overjet, overbite, mandibular plane angle appeared to be significantly high in the statistics. The analyzed results above, was thought to be indicating that the pre-operative maxillary occlusal plane angle was closely related to the post-operative skeletal stability, and that obtaining post-operative skeletal stability only through operative normalization of occlusal plane angle may meet limitations.

The relationship between malocclusion and menarcheal age, and its secular trend for Korean women

  • Objective: This study aimed to evaluate the mean age of menarche, its secular trend in Korean women, and the relationship between malocclusion and the rate of skeletal maturation, as defined by menarcheal age. Methods: We retrospectively collected data on menarcheal age from 931 Korean women born during 1961 - 1997. Subjects were divided by the malocclusion type and birth-year decade into 3 and 4 groups, respectively. The mean menarcheal age for each group was determined, and one-way ANOVA was performed for intergroup comparison (p = 0.05). Two-way ANOVA was also performed to compare all the 12 subgroups (p = 0.05). Results: The mean age of menarche was 12.82 years for Korean women born during 1961 - 1997. A distinct downward secular trend of menarcheal age was noticed (p < 0.05). For the birth-year decade 1961 - 1970, the Class III malocclusion group showed earlier onset of menarche than the other malocclusion groups (p < 0.05), but the other birth-year groups did not show any significant difference in the type of malocclusion (p > 0.05). Conclusions: A positive secular trend towards earlier menarche exists among Korean women. Malocclusion does not show any significant relationship with the rate of skeletal maturation, as defined by menarcheal age.

성장기 아동에서 가철식 교정장치와 MEAW를 이용한 부정교합의 치험례 (TREATMENT OF MALOCCLUSION USING REMOVABLE ORTHODONTIC APPLIANCE AND MEAW IN GROWING CHILDREN)

  • Early orthodontic treatment in growing children requires the removable orthodontic appliances. The removable orthodontic appliance can be used in the primary dentition and mixed dentition. The purposes of use of removable orthodontic appliance in prmary dentition or mixed dentition are the interception of skeletal or dental malocclusion, guiding the normal dentition, and retention after comprehensive orthodontic treatment. Therefore, it is needed to use the removable orthodontic appliance in children with malocclusion. This report presents cases of growing children with skeletal class II and III malocclusion treated with removable orthodontic appliance during mixed dentition and Multiloop Edgewise Arch Wire(MEAW) during permanent dentition. The results obtained through these cases were summarized as follows : 1. Removable orthodontic appliances guide normal dentition and skeletal growth in growing children. 2. Removable orthodontic appliances play an important role in intercepting malocclusion in mixed dentition before use of fixed orthodontic appliance. 3. MEAW can be applied to finishing stage of all cases and is effective in correction of occlusal plane, achievement of interdigitation, and control of dental inclination. 4. It is needed that removable appliances are used during mixed dentition through correct diagnosis and fixed appliance are used in permanent dentition.

골격성 III급 부정교합자의 양악수술후 연조직 변화의 평가 (SOFT TISSUE CHANGES AFTER DOUBLE JAW SURGERY IN SKELETAL CLASS III MALOCCLUSION)

  • 본 연구는 골격성 III 급 부정교합 환자에서 상악골 전방이동수술과 하악골 후방이동수술을 동시에 시행하였을 때 경,연조직 측모 및 연조직 후경의 변화를 관찰하고 경,연조직 변화의 상관성과 그 비율을 산출하여 교정-악교정 수술복합 치료 계획의 수립과 결과의 예측에 이용하고자 하였다. 서울대학교병원 치과진료부 교정과에 내원하여 상, 하악골의 수직적 골격 부조화는 경미하고 전후방적 골격 부조화가 심한 골격성 III급 부정교합으로 진단되어 술전 교정치료를 받고 1990년 7월부터 1995년 4월 중에 Le Fort I 골절단술 또는 Le Fort II 골절단술로 상악골을 전방이동시키는 동시에 시상분할 골절단술로 하악골을 후방이동시킨 성인 환자 25명(남자 13명, 여자 12명) 을 대상으로 수술전,후 측모두부방사선사진을 계측, 분석하여 다음과 같은 결과를 얻었다. 1. 상악골의 전방이동에 따른 상순부 연조직의 수평적 변화는 Stms를 제외하면 상관성이 높았으며 A point의 전방이동에 따라 Sn, SLS, LS 는 각각 $71\%,\;67\%,\;37\%$ 의 비율로 전방이동하였다. 2. 하악골의 후방이동에 따른 하순부 연조직의 수평적 변화는 상관성이 상당히 높았으며 ID, B point, Pog, Gn의 후방이동에 따라 LI, ILS, Pog, Gn 은 각각 $84\%,\;107\%,\;96\%,\;97\%$ 의 비율로 후방이동하였다. 3. 하악골의 후방이동에 따라 SLS, LS, Stm, LI 는 중등도의 상관성을 가지며 하방이동하였다. 4. 경조직의 전후안면고경비율과 연조직의 상,하안면고경 은 수술전후 유의한 차이가 없었으나 수술후 Stm 의 하방이동으로 하순고경에 대한 상순고경의 비율은 유의성있게 증가하였다(p<;0.001). 5. 연조직 후경은 수술후 LI-LIH 에서는 증가하고 LS-LSH 에서는 감소하였으며 LS-LSH 의 수술후 변화량과 수술전 후경은 역상관관계를 나타내었다(p<0.001).

Three-dimensional Assessment of Facial Soft Tissue after Orthognathic Surgery in Patients with Skeletal Class III and Asymmetry

  • Purpose: The purpose of this study was to perform three-dimensional (3D) assessment of facial soft tissue in patients with skeletal Class III and mandibular asymmetry after orthognathic surgery. Methods: Samples consisted of 3D facial images obtained from five patients with A point-nasion-B point angle less than 2 degrees, and more than 5 mm of menton deviation. All patients had been treated at Gangneung-Wonju National University Dental Hospital from 2009 to 2012. They had undergone orthognathic surgery of Lefort I, and sagittal split osteotomy for correction of skeletal deformity, and orthodontic treatment. Facial scanning was performed before treatment (T1) and post-surgical orthodontic treatment (T2). Linear and angle variables of soft tissue landmarks, antero-posterior facial depth, and facial volume were measured. Results: No significant differences in width of the alar base, mouth width, and nasal canting were observed between T1 and T2. However, lip deviation, menton deviation, alar canting, lip canting, and menton deviation angle were significantly reduced at T2. Antero-posterior facial depth on the axial plane parallel to the left cheilion was significantly reduced on the deviated side and significantly increased on the non-deviated side at T2. Volume of the lower lateral and lower medial parts of the face was reduced on the deviated side, and volume of upper lateral and lower lateral parts on the non-deviated side was significantly increased at T2. Conclusion: After orthognathic surgery, facial asymmetry of soft tissue was improved following skeletal changes, especially the mandibular region. Although the length of the alar base and mouth width did not change, lip and soft tissue menton were displaced to the medial side after treatment. Facial depth also became symmetric after treatment. Facial volume showed a decrease on the lower part of the deviated side and that on lateral parts of the non-deviated side showed an increase after treatment.

Comparative study of postoperative stability between conventional orthognathic surgery and a surgery-first orthognathic approach after bilateral sagittal split ramus osteotomy for skeletal class III correction

  • Objectives: The purpose of this study is to compare the postoperative stability of conventional orthognathic surgery to a surgery-first orthognathic approach after bilateral sagittal split ramus osteotomy (BSSRO). Materials and Methods: The study included 20 patients who underwent BSSRO for skeletal class III conventional orthognathic surgery and 20 patients who underwent a surgery-first orthognathic approach. Serial lateral cephalograms were analyzed to identify skeletal changes before surgery (T0), immediately after surgery (T1), and after surgery (T2, after 1 year or at debonding). Results: The amount of relapse of the mandible in the conventional orthognathic surgery group from T1 to T2 was $2.23{\pm}0.92mm$ (P<0.01) forward movement and $-0.87{\pm}0.57mm$ (non-significant, NS) upward movement on the basis of point B and $2.54{\pm}1.37mm$ (P<0.01) forward movement and $-1.18{\pm}0.79mm$ (NS) upward movement on the basis of the pogonion (Pog) point. The relapse amount of the mandible in the surgery-first orthognathic approach group from T1 to T2 was $3.49{\pm}1.71mm$ (P<0.01) forward movement and $-1.78{\pm}0.81mm$ (P<0.01) upward movement on the basis of the point B and $4.11{\pm}1.93mm$ (P<0.01) forward movement and $-2.40{\pm}0.98mm$ (P<0.01) upward movement on the basis of the Pog. Conclusion: The greater horizontal and vertical relapse may appear because of counter-clockwise rotation of the mandible in surgery-first orthognathic approach. Therefore, careful planning and skeletal stability should be considered in orthognathic surgery.

  • 두개악안면 영역은 많은 해부학적 구조를 포함하는 근골격구조(musculodentos keletal system)로 두부골격의 구조, 악궁상태, 저작근의 형성 및 기능은 서로 밀접한 연관성을 가지고 있다. 두개악안면 영역의 성장과 발육은 유전적인 요인이외에 환경적인 요인들 즉, 두개안면부의 근육을 포함한 주위조직의 영향을 받는 것으로 이미 알려져 있다. 그러나 반대로 두개안면부의 골격구조가 변화되는 경우 이에 대응하여 두개안면근육의 기능 또한 변화, 적응되는지에 대해서는 체계적인 연구결과가 미흡한 상태이다. 저자는 골격형 제 III급 부정교합자의 악교정수술 전, 후의 근활성도의 변화에 대한 비교연구를 통해 두부 골격구조와 교합상태에 따른 저작근 기능의 상호관계 및 변화 양상에 대해 알아보고자 정상교합을 가진 성인남자 15명과 술전 및 술후 골격형 제 III급 부정교합자 각각 15명을 대상으로 측모두부 방사선사진과 저작효율검사 및 rest, clenching, chewing, swallowing시의 전측두근, 교근, 상순의 근활성도 검사를 시행하여 다음과 같은 결과를 얻었다. 1. 하악의 안정위(rest)에서 술전 부정교합군에서 술후군보다 전측두근, 교근, 상순에서 높은 활성도를 보였으며, 술후군에서는 상순에서만 정상대조군보다 의미있는 높은 근활성도를 보였다. 2. 최대교합(clenching)시는 술후군에서 술전 부정교합군보다 전측두근, 교근, 상순에서 높은 근활성도를 보였다. 3. 저작(chewing)시는 술후군이 술전 부정교합군보다 전측두근 및 교근에서 높은 활성을 보인 반면 상순에서는 감소된 근활성도를 보였다. 4. 연하(swallowing)시 술후군에서 상순은 술전 부정교합군보다 감소되었으나 정상대조군보다 증가된 근활성도를 나타냈으며, 각 군간에 전측두근 및 교근에서는 유의차가 없었다. 5. 저작효율은 술전부정교합군에서 정상대조군보다 낮았으며, 술후군에서 술전 부정교합군보다 증가되었으나 두군 모두 정상대조군과 유의차를 보였다.

Surgery-first approach using a three-dimensional virtual setup and surgical simulation for skeletal Class III correction

  • A 19-year-old woman presented to our dental clinic with anterior crossbite and mandibular prognathism. She had a concave profile, long face, and Angle Class III molar relationship. She showed disharmony in the crowding of the maxillomandibular dentition and midline deviation. The diagnosis and treatment plan were established by a three-dimensional (3D) virtual setup and 3D surgical simulation, and a surgical wafer was produced using the stereolithography technique. No presurgical orthodontic treatment was performed. Using the surgery-first approach, Le Fort I maxillary osteotomy and mandibular bilateral intraoral vertical ramus osteotomy setback were carried out. Treatment was completed with postorthodontic treatment. Thus, symmetrical and balanced facial soft tissue and facial form as well as stabilized and well-balanced occlusion were achieved.

Three-dimensional assessment of nasal changes after maxillary advancement with impaction using stereophotogrammetry

  • Objective: To evaluate the changes in the nose in three dimensions after Le Fort I osteotomy in patients with skeletal Class III malocclusion. Methods: The subjects were 40 adult patients (20 females and 20 males; mean age, 20.3 ± 3.0 years; range, 17.0 to 31.1 years) who underwent one-piece Le Fort I osteotomy with maxillary advancement and impaction treatment for maxillary hypoplasia. The mean maxillary advancement was 4.56 ± 1.34 mm, and the mean maxillary impaction was 2.03 ± 1.04 mm. Stereophotogrammetry was used to acquire three-dimensional images before and at least 6 months after surgery. Results: Alare (Al) and alare curvature (Ac) points had moved vertically and anterolaterally postoperatively. A significant increase was observed in the nasal ala width and alar base width, and no changes were noted in the columellar length, nasolabial angle, and nasal area. There was a significant relationship between maxillary impaction and nasal ala width and horizontal and sagittal positions of the bilateral Al and Ac. The only relationship found was between maxillary advancement and postoperative sagittal location of the subnasale and pronasale. Conclusions: Nasal soft tissues were highly affected by the vertical movement of the maxilla; however, the soft tissue responses were individual-dependent.

  • The purpose of this report is to review vertical dysplasia such as openbite or deep bite in skeletal Class III malocclusion and their treatment modality and to present two cases treated with MEAW. The results obtained were as follows A. Open bite case 1. The treatment time was 3 year 8 months. 2. Upper and lower incisors showed extrusion and especially lower anterior alveolar process showed remodelling. 3. The mesially inclined upper and lower molars were uprighted and especially lower first molars showed extrusion that means remodelling of alveolar bone. 4. Normal overbite and overjet were established. 5. Mandible showed slight clockwise rotation. 6. Maxilla showed slight downward bending of ANS part. 7. Upper lip showed downward drop and lower lip showed retraction and touch between upper and lower lip was established. 8. Tongue posture of post-treatment was more raised than pretreatment. B. Deep bite case 1. The treatment time was 1 year 8 months. 2. Upper incisors showed intrusion and labioversion and lower incisors showed slight intrusion and linguoversion. 3. The lower molars showed distal uprighting and intrusion and upper molars showed mesial movement and extrusion. 4. Normal overbite and overjet were established. 5. Maxilla did not show downward movement. 6. Mandible showed slight clockwise rotation. 7. Lower lip showed retraction and downward drop and upper lip showed downward drop.
