Effects of Spinal Stabilization Exercises Using Visual Feedback on Gross Motor Function and Balance in Children with Cerebral Palsy

  • Purpose: This study examined the effects of spinal stabilization exercises using visual feedback on the gross motor function and balance of the sitting posture in children with cerebral palsy. Methods: The subjects were 18 children with cerebral palsy aged 8-15 years in the I-III stages of the Gross Motor Function Classification System. The subjects were divided into an experimental group (n=9) and control group (n=9). The experimental group was treated with 30 minutes of neurodevelopmental treatment and 20 minutes of spinal stabilization exercises using visual feedback. The control group was treated with 30 minutes of neurodevelopmental treatment and 20 minutes of spinal stabilization exercises without visual feedback. Both groups participated in the experiment twice a week for eight weeks. The Gross Motor Function Measurement was performed to evaluate the changes between pre- and the post-intervention in gross motor function. The Seated Limit of Stability Surface Area was measured to evaluate the changes in trunk balance. Results: Both experimental and control groups showed a significant increase in the gross motor function and trunk balance (p<0.05). The experimental group showed a significant increase in gross motor function compared to the control group (p<0.05). The experimental group showed a significant increase in the dynamic trunk balance in all directions when measuring the Seated Limit of Stability Surface Area (p<0.05). Conclusion: Spinal stabilization exercises using visual feedback for the neurodevelopmental treatment of children with cerebral palsy can improve their gross motor function and trunk balance when in a sitting posture more effectively.

오버롤(Overall)형 방진복의 패턴설계에 관한 연구 (A Study on the Pattern for the Clean Room Garment of the Overall Type)

  • This study was to design the clean room garment which fits for a body and adjusts well to the action. We examined the body's changes in action through a body measurment and made a sample of the clean room garment to which had adapted the body's changes in action and evaluated the function and the appearance compared with the existing the clean room garment. The followings were summery of the results : 1. Back intersyce breadth decreased, trunk length increased and the other parts were not change. At a posture of raising hands($M_3$) in comparison with an upright posture($M_0$). Back intersyce breadth, hip length and knee length increased and the other parts were a little change. At a bowing posture($M_2$) in comparison with an upright posture($M_0$). Back hip girth, knee length, hip length and leg length increased and the other parts were not change. At a working posture on a chair($M_1$) in comparison with an upright posture($M_0$). 2. The overall clean room garment was made increasing on the changes of part of a body through measuring and body measurement. 3. From the test of the function, it became known that the devised clean room garment was superior to the existing one in the parts of neck, shoulder, armpit and elbow at a working posture on a chair($M_1$), in the parts of neck, shoulder, and armpit at a bowing posture($M_2$), and the armpit at a posture of raising hands($M_3$). The appearance was better on the whole. 4. From the examination of photographs, the waist line of the devised clean room garment was not so much pulled up as that of the existing one on moving. And when sitting down on a chair, the waist line of the existing one was pulled down while that of the devised one had no change. As above, we investigated the actual conditions of wearing the clean room garment and measured the body's changes according the various actions. So we applied these results to the devised clean room garment but we couldn't research the clean mom effect of it owing to not obtaining the measuring instrument. The aspect of ergonomics liked the feeling of wearing and fitted to the action must be given consideration to the clean room garment, and simultaneously the dust-resistant effect must be taken into consideration in order to improve the quality of the goods. Therefore, we think it should be continued the research on the dust-resistant effect, material, washing method, quality control, and size along with the aspect of ergonomics.

한국 고령자의 일상생활 중 다양한 높이에서의 STS(sit-to-stand) 시 관절운동 특성 및 근길이 변화 분석 (Analysis of Joint Movements and Changes of Muscle Length During STS(sit-to-stand) at Various Sitting Heights in the Korean Elderly's daily life)

  • Sit to stand(STS) movement is one of the most common activity in daily life. In addition, Korean traditionally stand up from various sitting heights in one's daily life compared to other foreigners. As Korea enter rapidly to the aging society, needs of the elderly's independent life are increasing. Therefore the importance of research about the analysis of elderly's activity in daily life is rapidly increasing. In this study, we analyzed joint movements and changes of muscle length during STS(sit-to-stand) at various sitting heights(table seat, bath seat, bottom) in the Korean elderly's daily life by using the motion analysis and musculoskeletal modeling. Ten Korean elderly and young were participated in this experiment. Three heights of sitting posture which could represent typical sitting in Korean daily life were chosen as table seat(42cm), bath seat(21cm) and bottom(0cm). As the results, the elderly showed both smaller knee/hip flexion and larger trunk flexion relatively in comparison to the young during table seat STS. The elderly also showed larger dorsiflexion and smaller ROM of knee, hip, trunk compared to the young during bath seat STS. Additionally, the elderly showed larger plantarflexion, hip flexion, smaller knee flexion and trunk flexion during the first half of bottom STS and larger knee flexion, hip flexion and trunk flexion during the second half of bottom STS. In addition, we could know contraction and relaxation characters of major muscles in lower limb during various STS through the analysis of changes in muscle length by musculoskeltal modeling.

  • Purpose : The purpose of this study was to confirm changes in the muscle activity of the quadriceps femoris with changes in the ankle and hip joint angles during the transition from sitting to standing. Method : Twenty-five healthy 20-30-year-old women participated in the study. The subjects performed standing-up movements in four positions: standard posture (hip = $90^{\circ}$/ankle = $0^{\circ}$); posture on a decline board (hip > $90^{\circ}$/ankle < $0^{\circ}$), posture on a footboard (hip > $90^{\circ}$/ankle = $0^{\circ}$); and posture on a decline board with a higher chair (hip = $90^{\circ}$/ankle < $0^{\circ}$). Then, the muscle activities of the rectus femoris, vastus medialis and vastus lateralis were measured using surface electromyography. Result : The muscle activities of the rectus femoris, vastus medialis and vastus lateralis in the footboard application were statistically significantly higher than in the application of the decline board with a higher chair. Conclusion : This study confirms that the flexion of the hip joint has a greater effect than the plantar flexion of the ankle joint on the muscle activity of the quadriceps during a sit-to-stand movement.

Comparison of Upper and Lower Cervical Muscle Strengths Between Subjects With and Without Forward Head Posture

  • Background: Forward head posture (FHP) is common postural malalignment. FHP is described relatively extension to upper cervical and lower cervical is relatively flexion. Although several researchers mentioned the lower cervical flexion posture in FHP, most of the studies related to FHP is focused on the deep cervical flexor function. Objects: The purposes of present study is to compare the cervical strength (upper cervical extension [UCE], lower cervical extension [LCE], upper cervical flexion [UCF], lower cervical flexion [LCF]) between individuals with and without FHP. Methods: Fifty-one participants are recruited. Participants who have the craniovertebral angle (CVA) less than 48 degree were classified to the FHP group (n = 24) and the others were included in without FHP group (n = 27). The cervical strength (UCE, LCE, UCF, LCF) were measured using Smart KEMA strength sensor and the strength data was normalized by body weight. All strength measurement conducted at head and neck neutral position in sitting. Independent t-test was used to compare the cervical strength between individuals with and without FHP. Results: The mean value of CVA was greater in without FHP group than with FHP group (p < 0.000). The strength value of UCF (p < 0.002) and LCE (p < 0.001) was significant less in FHP group than without FHP group. But no significant differences were seen in the LCF and UCE strength between two groups. Conclusion: UCF and LCE weakness in FHP group should be considered to evaluate and manage the individuals with FHP.

Effect of Tactile Feedback on Trunk Posture and EMG Activity in People With Postural Kyphosis During VDT Work

  • Background: Recently, there has been an emphasis on the use of interventions with biofeedback information for the maintenance or correction of posture. Objects: This study assessed the change of trunk posture and trunk muscle activation when people exhibiting postural kyphosis performed visual display terminal work with or without a contact feedback device (CFD). Methods: Eighteen right-handed individuals were recruited. Thoracic angle and right thoracic erector spinae (TES) muscle amplitude were analyzed. There were two sessions in these experiments. The control session involved 16 minutes of typing without a CFD, and the CFD session involved 16 minutes of typing with a CFD. The visual analog scale score was analyzed with a paired t-test, and the kinematic and electromyography data were analyzed through two-way repeated analysis of variance. Results: The paired t-tests revealed that subjects had significantly less pain after the CFD sessions than after the control sessions (p<.05). Significant main effects by session and by time were observed in the thoracic kyphosis angle (p<.05). There was a significant session${\times}$time interaction for TES amplitude (p<.05), along with significant main effects by session and by time (p<.05). Conclusion: The CFD caused people with postural kyphosis to straighten and to activate their TES continuously, even though they were habituated to bend their bodies forward. Therefore, the CFD was a beneficial treatment tool.

근육 이완 접근과 관절 가동 접근이 긴장성 두통을 가진 두부 전방 전위 자세 환자의 목의 움직임 및 일상생활 편안함에 미치는 영향 (Effects of Muscle Relaxation Approach and Joint Movement Approach on Neck Movement and Comfort of Daily Living in Patients with Tension-type Headache of Forward Head Posture)

  • Objectives The purpose of this study was to improve the comfort of daily life such as reduction of headache and increase of movement of neck by using muscle relaxation approach and joint movement approach for office worker with tension type headache of foward head posture sitting over 5 hours. Methods For this, 9 male and 15 female participated in the foward head posture with tension type headache. Each group consisted of 3 male and 5 female. Groups are divided into groups, such as muscle relaxation therapy, joint movement therapy, muscle relaxation and joint movement therapy. After intervention for each group for a month, we measured neck movement and head disability index and neck disability index 2 week. SPSS 23.0 (IBM Corp., Armonk, NY, USA) was used for data analysis. The one-way repeated analysis of variance (ANOVA), one-way ANOVA, compared t-test was used for statistical analysis. Results Three intervention groups have brought improvements in neck movement and daily life comfort. There is significant difference in the improvement of neck extension and change in neck disability index between 2 and 4 weeks in the joint movement approach compared to muscle relaxation approach, muscle relaxation and joint movement approach. Conclusions Office workers are exposed to tension type headache. However, muscle relaxation approach and joint movement approach can improve neck movement and daily life comfort.

Comparison of Craniovertebral Angle and Muscle Properties after Smartphone Use in Healthy Individuals with and without Forward Head Posture

  • Purpose : Forward head posture (FHP) is one of the most common postural malalignment of the cranio-cervical region. Previous studies have reported that FHP might affect both temporomandibular joint (TMJ) and cervical muscles, but still remains unclear. The purpose of this study was to compare the changes of craniovertebral angle (CVA) and muscle properties after smartphone use in healthy individuals with and without FHP. Methods : Fifteen healthy individuals aged 18 to 22 years were included. CVA was evaluated using Dartfish motion analysis, and the subjects were divided into two groups according to their CVA: a FHP group (n = 7, CVA less than 48 °) and a control group (n = 8, CVA more than 48 °). MyotonPro was used to measure muscle properties of masseter, digastric and sternocleidomastoid muscles (SCM). Each subject underwent 15-minutes of smartphone task (web browsing or video watching) in relaxed sitting posture. CVA and muscles properties were assessed both before and after the smartphone task. Results : There were significant changes in post measurements of CVA between the groups. Masseter muscle showed significant differences in pre and post measurements of all muscle properties, and digastric muscle showed significance only in muscle tone. Amount of changes (post-pre), however, showed no significant difference in this study. Conclusion : 15-minutes of smartphone task did not affect CVA and muscle properties of masseter, digastric and SCM in both groups, however, there were significant changes in pre and post measurements of CVA and some muscle properties of masseter and digastric muscles. Therefore, CVA, masseter and digastric muscles might be significantly changed in a heavy duration of smartphone usage more than 15-minutes. Further studies are needed regarding duration of smartphone task, assessments in other various TMJ muscle groups, and participants with pathological FHP conditions.

Comparing the Effects of Manual and Self-exercise Therapy for Improving Forward Head Posture

  • Background: Studies investigating the immediate effects of a single intervention to correct forward head posture are rare. Objects: This study aimed to compare the changes in treatment effects in patients with forward head posture and neck pain after manual and self-exercise therapy over a 1-hour period. Methods: Twenty-eight participants were randomly divided into manual and self-exercise therapy groups. Following the initial evaluation, manual or self-exercise therapy was applied to each group for 30 minutes each in the prone, supine, and sitting positions. The variables measured were the craniovertebral angle (CVA), stress level, pain level, and sternocleidomastoid (SCM) stiffness. After the intervention, re-evaluation was conducted immediately, 30 minutes later, and 1 hour later. Two-way analysis of variance (ANOVA) was used to compare the maintenance of treatment effects between the two groups. Results: Based on the two-way mixed ANOVA variance, there was no interaction between the groups and time for all variables, and no main effects were found between the groups. However, a significant effect of time was observed (p < 0.05). Post hoc tests using Bonferroni's correction revealed that in both groups, the CVA, pain, and stress showed significant improvements immediately after the intervention compared with before the intervention, and these treatment effects were maintained for up to 1 hour after the treatment (p < 0.0083) in the manual therapy group. However, the stress level was maintained until 30 minutes later (p < 0.0083) in the self-exercise group. There was no significant decrease in right SCM stiffness before and after the intervention; however, left SCM stiffness significantly decreased after the self-exercise intervention (p < 0.0083). Conclusion: Both manual and self-exercise therapy for 30 minutes were effective in reducing forward head posture related to the CVA, pain, and stress levels. These effects persisted for at least 30 minutes.

한복바지 구성의 제도법에 대한 고찰 (A Study on the Drafting Method of Korean Men's Traditional Trousers)

  • According to the design method for constructing the crotch angle of traditional Korean men's trousers there are differences in from after its completion. Since there is no standardization of visual & aesthetic pattern for the construction method which is stablished by the calculation formular of waist girth hip girth pantleg end and upper outer leg length it is difficult to be used for educational use or for teaching materials. Therefore the purpose of this project according to five models in proportion to the physical standard of Korea in ages from 24 to 29 years old is to establish a design criterion and the standardization of construction methods by introduction the pant construction method of the crotch angle by converting the sitting posture length to seaming crotch center point both knees width in the Korean way of sitting.
