Purpose: Anastomotic leakage (AL) is associated with high morbidity and mortality, high reoperation rates, and increased hospital length of stay. Here we investigated the risk factors for AL after anterior resection for rectal cancer with a double stapling technique. Patients and Methods: Data for 460 patients who underwent primary anterior resection with a double stapling technique for rectal carcinoma at a single institution from 2003 to 2007 were prospectively collected. All patients experienced a total mesorectal excision (TME) operation. Clinical AL was defined as the presence of leakage signs and confirmed by diagnostic work-up according to ICD-9 codes 997.4, 567.22 (abdominopelvic abscess), and 569.81 (fistula of the intestine). Univariate and logistic regression analyses of 20 variables were undertaken to determine risk factors for AL. Survival was analysed using the Cox regression method. Results: AL was noted in 35 (7.6%) of 460 patients with rectal cancer. :Median age of the patients was 65 (50-74) and 161 (35%) were male. The diagnosis of AL was made between the 6th and 12th postoperative day (POD; mean 8th POD). After univariate and multivariate analysis, age (p=0.004), gender (p=0.007), tumor site (p<0.001), preoperative body mass index (EMI) (p<0.001), the reduction of TSGF on 5th POD less than 10U/ml (p=0.044) and the pH value of pelvic dranage less than or equal to 6.978 on 3rd POD (p<0.001) were selected as 6 independent risk factors for AL. This study was undertaken to provide a basis for planning trail standards, providing rights of movement, improvement of health and the experience of nature for at-risk walkers for application to the Umyen Park site. This study researched related laws, manuals, scholastic writing and the established site, a raku-raku mountain trail in Osata, Japan. By referencing these results and similar standards, a conceptual trail plan for at-risk walkers was created. The plan suggests five points -- accessibility, convenience, social aspects, economic efficiency, and environmentally-friendliness -- and referenced an evaluation of outdoor living environments for the elderly. The planning standards are comprised of four steps that are ranked in order and are related to the selection of location, trail structure, route patterns, and equipment. Plans for trails catering to at-risk walkers in Umyen Park were then based on these standards. As a result, it was found that the mountain has both a high elevation and abundant natural resource, both of which must be taken into account during planning. This study is valuable because it is one of the first studies of trail plans for at-risk walkers made in mountainous urban parte. Subsequent research can use the standards of this study for further evaluation while future modifications must reflect changing needs and details.
The regulation of neurotoxicants has usually been based upon setting reference doses by dividing a no observed adverse effect level (NOAEL) by uncertainty factors that theoretically account for interspecies and intraspecies extraploation of experimental results in animals to humans. Recently, we have proposed a four-step alternative procedure which provides quantitative estimates of risk as a function of dose. The first step is to establish a mathematical relationship between a biological effect or biomarker and the dose of chemical administered. The second step is to determine the distribution (variability) of individual measurements of biological effects or their biomarkers about the dose response curve. The third step is to define an adverse or abnormal level of a biological effect or biomarker in an untreated population. The fourth and final step is to combine the information from the first three steps to estimate the risk (proportion of individuals exceeding on adverse or abnormal level of a biological effect or biomarker) as a function of dose. The primary purpose of this report is to enhance the certainty of the first step of this procedure by improving our understanding of the relationship between a biomarker and dose of administered chemical. Several factors which need to be considered include: 1) the pharmacokinetics of the parent chemical, 2) the target tissue concentrations of the parent chemical or its bioactivated proximate toxicant, 3) the uptake kinetics of the parent chemical or metabolite into the target cell(s) and/or membrane interactions, and 4) the interaction of the chemical or metabolite with presumed receptor site(s). Because these theoretical factors each contain a saturable step due to definitive amounts of required enzyme, reuptake or receptor site(s), a nonlinear, saturable dose-response curve would be predicted. In order to exemplify this process, effects of the neurotoxicant, methlenedioxymethamphetamine (MDMA), were reviewed and analyzed. Our results and those of others indicate that: 1) peak concentrations of MDMA and metabolites are ochieved in rat brain by 30 min and are negligible by 24 hr, 2) a metabolite of MDMA is probably responsible for its neurotoxic effects, and 3) pretreatment with monoamine uptake blockers prevents MDMA neurotoxicity. When data generated from rats administerde MDMA were plotted as bilolgical effect (decreases in hippocampal serotonin concentrations) versus dose, a saturation curve best described the observed relationship. These results support the hypothesis that at least one saturable step is involved in MDMA neurotoxicity. We conclude that the mathematical relationship between biological effect and dose of MDMA, the first step of our quantitative neurotoxicity risk assessment procedure, should reflect this biological model information generated from the whole of the dose-response curve.

A conventional Design for Safety (DfS), introduced to eliminate potential hazards in the design phase proactively, has encountered persistent challenges, such as perfunctory risk assessments and hazard identifications based on 2D drawings and inefficient workflow processes. This study proposes a BIM-based approach to Design for Safety (DfS) to address the limitations of conventional methods, aiming to enhance efficiency and achieve practical safety management benefits. The proposed workflow process for BIM-based DfS has been refined and validated for on-site applicability through various case studies, including risk assessments during the design phase and field applications for safety management activities during the construction phase. Specifically, the critical process of risk assessment within the DfS methodology has also been transitioned to a BIM-based approach. This BIM-based risk assessment process has been evaluated through case studies, encompassing safety reviews for structural design, construction equipment operation, and construction methodology with sequence in design projects. Additionally, the proposed BIM-based DfS has demonstrated exceptional on-site applicability and efficiency, as validated by the application of a BIM deliverable embedded in DfS information for CDE-based daily activity briefing, VR-based safety training, AR-based mitigation measures inspections, and other safety management activities in the construction phase.

According to a recent study, most construction companies in Korea do not manage risk effectively, and it is judged that the risk management system needs to be improved. In addition, most risk-related studies deal with risks from a macroscopic perspective, and there are few studies dealing with process risks at the project construction stage. Therefore, this study tried to suggest a risk response plan through analysis and classification of risk factors that may occur in retaining work among process risks. To this end, a workshop was held for risk experts to identify and analyze risks that may occur during the construction of retaining work for apartments. As a result of the study, it was expected that savings of KRW 4.97 billion would be possible in the 95 % confidence interval, and the maximum possible cost was reduced from KRW 15 billion to about KRW 10 billion. Based on the risk reduction ratio, it was found that risks that can be reduced without any special input cost, risks with large effects in response to risks, and risks with insignificant effects were found. Therefore, using the types and risk factors presented in this study as guides, it is expected that it will be helpful in successfully operating the project if an appropriate response strategy is prepared and systematically responded to the site conditions.

As the incidence and mortality of serious disasters in the construction industry are the highest, various efforts are being made in Korea to reduce them. Among them, risk assessment is used as data for disaster reduction measures and evaluation of risk factors at the construction stage. However, the existing risk assessment involves the subjectivity of the performer and is vulnerable to the domestic construction site. This study established a DB classification system for risk assessment with the aim of early identification and pre-removal of risks by quantitatively deriving risk factors using BIM in the risk assessment field and presents a methodology for risk assessment using BIM. Through this, prior removal of risks increases the safety of construction workers and reduces additional costs in the field of safety management. In addition, since it can be applied to new construction methods, it improves the understanding of project participants and becomes a tool for communication. This study proposes a framework for deriving quantitative risks based on BIM, and will be used as a base technology in the field of risk assessment using BIM in the future.
Purpose: Fires that occur during construction are infrequent, but cause great damage. Recently, with the growth of the logistics and distribution industry, the number of construction sites for new logistics warehouses is increasing, so it was selected as a research subject and research was conducted to reduce accidents at construction sites through the development of a fire risk assessment tool to quantitatively approach fire prevention. Method: A comprehensive fire risk assessment tool was accumulated by classifying the work in progress, classifying combustibles and ignition sources by grade, excluding air (oxygen), which is difficult to control, and additionally substituting evacuation safety. Result: Using the developed and proposed fire risk evaluation tool, excavation work with low fire risk, facility construction with medium fire risk, and finishing work with high fire risk were sampled to derive the result (CGI). Conclusion: In this study, it was possible to establish specific preventive measures and evaluate evacuation safety by controlling physical conditions (combustibles) and energy conditions (ignition sources) according to the risk assessment by developing a tool that can evaluate the risk of 14fire occurrence at construction sites. It is expected that in the future, through the application of the fire risk evaluation tool at construction sites, it will be provided as a criterion for establishing a process plan that can reduce risk and evaluating the adaptability of firefighting equipment.14
Objectives: The aim of this study is to determine prevalence and related factors of musculoskeletal symptoms in electronics assembly line workers. Method: A questionnaire survey was performed on 350 workers in In-chon city. The questionnaire consisted of questions on the general characteristics, work duration, working hours, hobby activity hours, housekeeping activity hours and standardized descriptions of NOISH on musculoskeletal symptoms. Result: Complaint rates of musculoskeletal symptoms by anatomical site were as follows: shoulder, 67% : neck, 40.4% : waist, 28.7% : wrist and hand, 27.1% : foot and leg, 14.4% : arm and elbow, 11.2%. Significantly higher prevalence of shoulder musculoskeletal symptom was found women than men, married than unmarried, less than 1hours housekeeping activity than over 1hours housekeeping activity. Multiple logistic regression analysis identified gender, marital status and housekeeping activity as the main affecting factors for shoulder musculoskeletal symptoms. Conclusions: The most prevalence of musculoskeletal symptoms was shoulder site. It was higher in married women and over 1hours housekeeping activity in a day. It is recommended that married women should be reduce housekeeping activity time.
The Semiconductor industry in Korea has achieved a great contribution to the growth of its economy for the last 20 years with its product export ranked to #1 nowadays. However, the working environment in semiconductor industry is always exposed to a potential risk of critical safety issue for workers with many kinds of utilities used such as toxic chemicals, gases, high vacuum and high voltages of electricity. This study is focused on empirical research to find out the factors on safety climate and to examine the relationships of safety climate and safe work behaviour in the work site of Korea semiconductor industry. As a result of simulation, a strong positive relationships and safe work behaviour have been identified in this study. Its result and implications has been discussed and suggested further studies on its limitation from this study.
Recently, the domestic construction companies are increasingly using the scaffolding system in order to secure safety although it may increase the construction cost. However since the actual situation of scaffolding system has not been checked yet, no safety standards and work method were made. Hence, various risk factors are involved in the scaffolding system work. Accordingly, this study is intended not only to check the installation/use status of scaffolding system in the domestic construction sites through the in-depth survey and investigation but also to suggest the measures of using the scaffolding system more effectively and systematically while basing the study results.
