An Unusual Cause of Acute Maxillary Sinusitis in a 9-year-old Child: Odontogenic Origin of Infected Dentigerous Cyst with Supernumerary Teeth (9세 남자 환아에서 급성 부비동염의 드문 원인 : 과잉치가 동반된 감염된 함기성 낭종)

  • Acute maxillary sinusitis is a common disorder affecting children. Untreated acute sinusitis can develop into chronic sinusitis, and complications, such as orbital cellulitis or abscess, can occur. Maxillary sinusitis of odontogenic origin is not a well-recognized condition and is frequently missed in children. As an odontogenic source of sinusitis, the dentigerous cyst is one of the most prevalent types of odontogenic cysts, and it is associated with the crown of an unerupted or developing tooth. This report concerns a nine-year-old boy who was diagnosed with maxillary sinusitis originating from a dentigerous cyst with supernumerary teeth. The boy visited our pediatric clinic presenting with rhinorrhea and nasal obstruction and was initially diagnosed with maxillary sinusitis only. With antibiotic treatment, his symptoms seemed to improve, but after 2 months, he came to our clinic with left facial swelling with persistent rhinorrhea and nasal obstruction. Radiographic examinations of the sinuses were performed, and he was diagnosed with maxillary sinusitis originating from a dentigerous cyst with supernumerary teeth. After a surgical procedure involving the removal of the dentigerous cyst with supernumerary teeth, the symptoms of sinusitis gradually diminished. There are only very few cases in the pediatric medical literature that remind us that odontogenic origin can cause maxillary sinusitis in children. Our patient can act as a reminder to general pediatricians to include dentigerous cysts in the differential diagnosis of maxillary sinusitis.

Two Cases of Chronic Sinusitis with Asthma Improved by Socheongryong-tang (소청용탕으로 호전된 천식을 동반하는 만성부비동염 2례)

  • Two cases of chronic sinusitis with asthma were treated by Socheongryong-tang for 8 weeks in Respiratory System Division, Department of Internal Medicine of Kyunghee Oriental Medical Center. Socheongryong-tang is herbal decoction that has been used as the traditional therapeutic agent for asthma. The author has observed two cases of chronic sinusitis with asthma. Treatment with Socheongryong-tang for 8 weeks didn't result in significant change in the symptoms of asthma, but chronic sinusitis improved without any side effects. This study suggests that Socheongryong-tang is an effective herbal decoction in the treatment of chronic sinusitis. Obviously, further research concerning this is still necessary.

  • Obective : This study was designed to investigated clinical features and frequent parts of chronic paranasal sinusitis Method : The ratio of male and female, the distribution of symptoms, the frequent parts of paranasal sinusitis on PNS images were investigated in 26 patients who visited Department of otorhinolaryngology Semyung university Oriental medical hostipal during from march, 2004 to october, 2004. Results: 1. The ratio of male and female was 1:1.5 and the teens were high rate of age group 2. The results reveal1he most common symptoms as follows ; nasal stuffiness(28.1$\%$), rhinorrhea26.5$\%$), laryngopharyngial disorder(14$\%$). 3. The frequent parts of paranasal sinusitis on PNS images were ethmodial sinus, maxillary sinus, fronal sinus in good order. 4. 87.5$\%$ of patients show effective result in medical treatment Conclusion: We suggest that this results form the foundation of objectification of medical treatment of paranasal sinusitis

The Correlation between Fine Dust(PM10, PM2.5) and The Number of Acute/Chronic Sinusitis Patients (미세먼지(PM10, PM2.5) 농도가 급성/만성 부비동염의 환자 수에 미치는 영향)

  • Objectives : The purpose of this study is to analyze the correlation between fine dust(PM10.5, PM2.5) and the number of acute/chronic sinusitis patients. Methods : A simple regression analysis was performed based on the concentration of PM10 and PM2.5 as independent variables and the number of acute/chronic sinusitis patients as dependent variables. Results : As a result of simple regression analysis, if PM10 increases by $1{\mu}g/m^3$, the number of acute sinusitis patients increases by 7,000.291(P<.001, 95%CI :4,951.983-9,048.600). If PM2.5 increases by $1{\mu}g/m^3$, the number of acute sinusitis patients increases by 17,524.476.(P<.001, 95%CI:9,728.725-25,320.228) In addition, PM10 increases by $1{\mu}g/m^3$, the number of acute sinusitis patients increases by 3,163.471 (P<.001, 95% CI:2,268.642-4,058.301). If PM2.5 increases by $1{\mu}g/m^3$, the number of chronic sinusitis patients increases by 8,651.644.(P<.001, 95%CI:5,115.697-12,187.592) Conclusions : Both PM10 and PM2.5 are correlated with changes in the number of sinusitis patients. PM2.5 has effect on the number of patients than PM10. PM10 is the highest correlation in their 50s, PM2.5 in their 60s and 70s.

The Effects of Taklisodok-eum Extract on Acute Sinusitis-induced mouse (탁리소독음(托裏消毒飮)이 급성 부비동염을 유발시킨 마우스에 미치는 영향)

  • Background and Objectives: Sinusitis is a common disease in the otorhinolaryngology field. It is inflammatory change of the mucous membrane which surrounds the sinus. The aim of this study was to investigate the anti-inflammatory effects of the Taklisodok-eum (TSE) extract on the mouse model with acute sinusitis induced by S. pneumoniae. Methods: Thirty six-week-old male BALB/c mice were divided into three groups: the normal group, the group inoculated with S. pneumoniae which caused allergic rhinitis (control group), and the group treated with the TSE extract after it was treated the same as the control group (sample group). Results: $NF-{\kappa}B$ activation was suppressed, and iNOS & COX-2 production were inhibited by TSE in acute sinusitis. Apoptosis was increased by TSE in acute sinusitis. The number of eosinophils in the sample group noticeably decreased when compared to the control group. In the general morphologic change, the increase of damaged respiratory ciliated epithelia & eosinophils' infiltration was lessin the sample group. Goblet cells were maintained in the sample group. MMP-9, HSP-70 and BrdU decreased in the sample group. Apocrine secretion decreased in the sample group. Conclusion: The findings in this study show that TSE reduces acute sinusitis through its anti-inflammatory effects, such as the inhibition of $NF-{\kappa}B$ activity.

Clinical Study for Mucociliary clearance of Pediatric Chronic Sinusitis after KamiHyunggyeyungotang Therapy (가미형개연교탕(加味荊芥蓮翹湯)이 만성부비동염(慢性副鼻洞炎)의 비강섬모운동(鼻腔纖毛運動)에 미치는 영향(影響))

  • Background: Delayed mucociliary clearance develops the pooling of nasal secretion in the nasal cavity and is a common cause of vicious cycle of symptoms in chronic sinusitis. mucociliary clearance depends on the ciliary activity, the rheological property of the secretion and on the interaction between cilia and the mucous blanket. Objective: To demonstrate the therapeutic effect of herbal medicine (Hyunggyeyungotang) on the mucociliary transport in the chronic sinusitis using saccharine Materials and methods: Forty patients treated in our hospital between september 2000 and september 2001 were studied. Before and after therapy group are composed of twenty patients. Kamihyunggyeyungyotang was administered mainly. We analyzed the saccharine mucociliary transport time of patients. Results: 1) In the control group, the transport time is about 13.4 minutes. 2) In chronic sinusitis before Kamihyunggyeyungotang therapy, the transport time is about 27.7 minutes. 3) In chronic sinusitis after Kamihyunggyeyungotang therapy, the transport time is about 13.8 minutes. Conclusion: Delayed mucociliary transport time in chronic sinusitis is earlier after Kamihyunggyeyungotang therapy.

Sinusitis and Adenoid size is related to Snoring in children (코골이를 주소로 내원한 환자의 부비동염과 아데노이드 비후와의 관계)

  • Background: Snoring in children, is associated Kith adenotonsillar hypertrophy and sinusitis which contribute to upper airway obstruction, so we studied the sinusitis and adenoid size of snoring children with plain radiography Materials and method: Fifty patients having snoring, nasal obstruction in our hospital between November 2001 and November 2002 were studied using plain radiology PNS(water's view) and Neck lateral view(adenoid size with A/N ratio) Results: 1. In oriental medicine, Hu-Bi means laryngopharyngeal edema and obstruction, laryngopharyngeal disease are due to Hwa-Yeoul( fire and hot), sinus disease are due to Fung-Han(wind and cold) and Fung-Yeoul(wind and hot). 2. Age ranged from 2 to 17 year-old ( mean age: 6 years), 5-7 year-old were 18 patients (36%). Age of on set, 25 patients were 1-4 year-old (50%). 3. Of the 50 snoring patients, 37 patients were sinusitis(74%), 20 patients had enlarged adenoid(40%). Of the 20 large adenoid patients, 19 patients were sinusitis(95%). 4. Of the 50 snoring patients, size of tonsil were flowed. Fifteen were severe(3+), 17 patients were moderate(2+) and 15 patients were mild(1+). Of the 20 enlarged adenoid patients, size of tonsil flowed. Five were severe(3+), 11 patients were moderate(2+) and 4 patients were mild(1+). Conclusion : Of the 50 Snoring patients, 37 patients showed sinusitis(74%), 32 patients showed large tonsil(64%), 20 patients showed large adenoid(40%).

In oriental medicine, the case of sinusitis can be explained, not only external invasion such as PUNG HAN(wind-cold), PUNG YUL(wind-hot), SHUP YUL(damp-heat), but also functional disorder of internal organ such as spleen(脾), lung(肺), kidney(腎) The western medicine classified the cause of sinusitis as two factors. That is bacterial infection factor-Haemophilus influenza, streptococcus pyogeues, streptococcus pneumonia- and viral infection factor-Rhinovirus, parainfluenza, Echo28, Coxsacki21, Sinusitis is complicated to allergic rhinitis, chronic otitis media purulent, chronic tonsilitis, sinubronchitis. On the condition of nasal septum deformity, turbinates deformity, nasal septum deviation, sinusitis can be developed. the predisposing factors of sinusitis is swimming, air pollution, malnutrition, shortage of immunity.2. According to survey, sinusitis occurred that children from 4 to 12 years old and from 5 to 7 years old occupied 70% 3. From the past history data, they experienced chronic tonsillar hypertropy(20%), otitis media, atopic dermatitis, allergic rhinitis, bronchial asthma, pneumonia, bronchiolitis, chronic sore throat, urticaria, milk allergy in sequence. 4. the symptoms of sinusitis is nasal obstruction, postnasal dripping, purulunt(yellow)or white discharge, cough, nose bleeding in sequence. nasal obstruction take the portion of 95%, postnasal dripping 65%, night time or early morning cough 60%. 5. The suffering period of sinusitis is 6 month minimum, 4 years maximum, most cases are included in a year. The suffering period of children was shorter than adult. 6. Diagnosis depend on inspection of nasal cavity, postnasal dripping, X-ray finding. 75% of patient(15case) showed both maxillary sinusitis, 25%(5cases) showed left or right maxillary sinusitis. 7. Treatment of oriental medicine, consist of Herb-medicine, acupuncture and exposing of Lazer beam. Kamihyunggyeyungyotang(加味荊芥蓮翹湯) is administered mainly as the medical therapy, Kamigwaghyangjeungkisan(加味藿香正氣散) Kimizwakwieum(加味左歸飮), Kamihyangsosan(加味香蘇散) is administered for a additional symptoms which occurred by influenza recurrence. Kamijeonxibackchulsan(加味錢氏白朮散) is administered to treat gastro-intestine trouble patients who have sinusitis. 8. The period of treatment is varied with patient conditions and X-ray finding. The minimal period is 35days, maximal period is 202days. So it took about 86days in average and about 50% of patient(10cases) is recovered in one or two month.

Clinical study for recurrence rate of the pediatric chronic sinusitis after herbal medicine therapy (소아 만성 부비동염(副鼻洞炎)(축농증)의 한방치료효과 후 재발율에 대한 임상적 연구)

  • Background: In recent years, pediatric chronic sinusitis patients who didn't respond with antibiotics were increasing, but there were a few reports on recurrence rate of the herbal medicine therapy treating pediatric chronic sinusitis, so this report was studied. Objective: To know the recurrence rate of herbal medicine therapy in the chronic sinusitis using follow-up investigation. Materials and methods: Thirty four patients( 16 male and 18 female) treated in our hospital between December 1998 and August 1999 were studied. Ages ranged from 4 to 13 years (mean age :7.7 years). Twenty five patients(66%) had a underlying family history (allergy or sinusitis of family). In the past history, 32 patients (86%) had asthma, allergy of milk, atopic dermatitis, bronchiolitis and irritable bowel syndrome. Duration of treatment were from eleven to eighteen month (mean:14.3 month). Results: Of the 34 patients, 31 patients were improvement, 3 patients not. Of the 31 patients, thirty patients showed not recurrent (91%). Two patients showed no significant interval change(6%) and one patient was aggrevated (3%). The degree of the Health , thinking of protector for the herb medicine therapy and improvement of sinusitis were significant statistically. (significant <0.005) Conclusion: We know that herbal medicine therapy is the effective treatment of pediatric chronic sinusitis.

  • Acute bacterial sinusitis is one of the most common diseases in children, and 5-13% of patients with viral upper respiratory infection experience secondary bacterial sinusitis as a complication. Isolated sphenoid sinusitis is rarer than frontal, maxillary, and ethmoid sinusitis. However, it presents potentially devastating complications, such as cranial nerve involvement, brain abscess, and meningitis, owing to its anatomical location. Nontypeable Haemophilus influenzae (NTHi) commonly causes acute bacterial sinusitis but rarely causes bacteremia, particularly in immunocompetent patients. We present a rare case of a 14-year-old boy with bacteremia due to isolated sphenoid sinusitis caused by NTHi.
