Clinical outcomes of a low-cost single-channel myoelectric-interface three-dimensional hand prosthesis

  • Background Prosthetic hands with a myoelectric interface have recently received interest within the broader category of hand prostheses, but their high cost is a major barrier to use. Modern three-dimensional (3D) printing technology has enabled more widespread development and cost-effectiveness in the field of prostheses. The objective of the present study was to evaluate the clinical impact of a low-cost 3D-printed myoelectric-interface prosthetic hand on patients' daily life. Methods A prospective review of all upper-arm transradial amputation amputees who used 3D-printed myoelectric interface prostheses (Mark V) between January 2016 and August 2017 was conducted. The functional outcomes of prosthesis usage over a 3-month follow-up period were measured using a validated method (Orthotics Prosthetics User Survey-Upper Extremity Functional Status [OPUS-UEFS]). In addition, the correlation between the length of the amputated radius and changes in OPUS-UEFS scores was analyzed. Results Ten patients were included in the study. After use of the 3D-printed myoelectric single electromyography channel prosthesis for 3 months, the average OPUS-UEFS score significantly increased from 45.50 to 60.10. The Spearman correlation coefficient (r) of the correlation between radius length and OPUS-UEFS at the 3rd month of prosthetic use was 0.815. Conclusions This low-cost 3D-printed myoelectric-interface prosthetic hand with a single reliable myoelectrical signal shows the potential to positively impact amputees' quality of life through daily usage. The emergence of a low-cost 3D-printed myoelectric prosthesis could lead to new market trends, with such a device gaining popularity via reduced production costs and increased market demand.

  • This paper proposes a compact-sized surface myoelectric sensor for myoelectric hand prosthesis. To fit the surface myoelectric sensor in the socket of the myoelectric hand prosthesis, the sensor should be a compact size. The surface myoelectric sensor is composed of a skin interface and a single processing circuit that are mounted on a single package. Since the skin interface has one reference and two input electrodes, and the reference electrode is located in middle of two input electrodes, we propose two types of sensors with the circle- and bar-shaped reference electrode, but all input electrodes are the bar-shaped. The metal material used for the electrodes is the stainless steel (SUS440) that endures sweat and wet conditions. Considering conduction velocity and median frequency of the myoelectric signal, we select the inter-electrode distance (IED) between two input electrodes as 18mm, 20mm, and 22 mm. The signal processing circuit consists of a differential amplifier with band pass filter, a band rejection filter for rejecting 60Hz power-line noise, amplifiers, and a mean absolute value circuit. We evaluate the proposed sensor from the output characteristics according to the IED and the shape of the reference electrode. From the experimental results we show the surface myoelectric sensor with the 18mm IED and the bar-shaped reference electrode is suitable for the myoelectric hand prosthesis.

Comparative clinical study of the marginal discrepancy of fixed dental prosthesis fabricated by the milling-sintering method using a presintered alloy

  • PURPOSE. The present study was designed to examine the clinical fit of fixed dental prosthesis fabricated by the milling-sintering method using a presintered cobalt-chromium alloy. MATERIALS AND METHODS. Two single metal-ceramic crowns were fabricated via milling-sintering method and casting method in each of the twelve consecutive patients who required an implant-supported fixed prosthesis. In the milling-sintering method, the prosthetic coping was designed in computer software, and the design was converted to a non-precious alloy coping using milling and post-sintering process. In the casting method, the conventional manual fabrication process was applied. The absolute marginal discrepancy of the prostheses was evaluated intraorally using the triple-scan technique. Statistical analysis was conducted using Mann-Whitney U test (${\alpha}=.05$). RESULTS. Eight patients (66.7%) showed a lower marginal discrepancy of the prostheses made using the milling-sintering method than that of the prosthesis made by the casting method. Statistically, the misfit of the prosthesis fabricated using the milling-sintering method was not significantly different from that fabricated using the casting method (P=.782). There was no tendency between the amount of marginal discrepancy and the measurement point. CONCLUSION. The overall marginal fit of prosthesis fabricated by milling-sintering using a presintered alloy was comparable to that of the prosthesis fabricated by the conventional casting method in clinical use.

하지 절단환자의 수술 전후 관리와 의지의 실용성에 대한 연구 (Pre and Post Surgery Management and Practicality of Prosthesis for Patients with Lower Limb Amputation)

  • Objective: The purpose of this study was to access quality of life and evaluate pre and post surgery management for persons with lower limb amputations. Method: This study was designed as a telephone survey. The subjects were patients who underwent their lower limb amputation between January 1994, and February 2005 at Asan Medical Center in seoul. sixty one of 203 subjects had granted consent and were studied. Results: The major cause of below knee amputations was vascular disease. Traumatic injuries were more common among subjects with above knee amputations. 93.4% of subjects were in use of prosthesis and 70.1 days were required to fit the prosthesis. 68.4% of subjects were able to ambulate with single point cane or without assistive devices. 80.3% of subjects complained phantom pain. Most subjects expressed the unsatisfactory result regarding the functional usage of prosthesis and education from hospital. Conclusion: Pre and post surgery rehabilitation program will benefit to promote better functional status and quality of life for persons with lower limb amputations.

쎈트쥬드 중복판막치환의 장기 임상성적 (Clinical Results of Double Mitral and Aortic Valve Replacement with the St. Jude Medical Prosthesis)

  • A total of and consecutive 87 patients underwent concomitant double mitral and aortic valve replacement with the St. Jude Medical prosthesis between January 1985 and December 1993. They were 44 males and 43 females with the ages ranging from 18 to 59 years[mean$\pm$SD: 40.9$\pm$9.5 years . Fifteen patients[17.2% had a history of previous cardiac valve replacement. There were 2 early deaths[2.3% , and 85 early survivors were followed up for a total of 352.6 patient-years[mean$\pm$SD: 4.1 $\pm$2.6 years . All were anticoagulated with coumadin keeping the target international normalized ratio within the range of 1.5 and 2.5. There was a single late death[late mortality of 0.284%/patient-year . Thromboembolism was the most frequent complication[1.985%/patient-year , and bleeding related to anticoagulation was experienced in one patient [0.284%/patient-year . The incidences of prosthetic valve endocarditis and of paravalvular leak were also low[0.284%/patient-year, respectively . The survival including operative mortality was 96.1%$\pm$2.2% at 10 years. The actuarial probabilities of freedom from thromboembolism and from all events were 77.9%$\pm$11.1% and 72.4%$\pm$10.7%, respectively, at 10 years. There was no structural failure of the prosthesis. Results from a series of clinical studies suggest strongly that the use of lower intensity of anticoagulation therapy lowers the thromboembolic as well as bleeding rates in patients with the ST. 인공 종양대치물을 이용한 사지구제술후의 보행 분석 (Gait Analysis of Patients with Tumor Prosthesis around the Knee)

  • Prosthetic replacement is one of the most common methods of reconstruction after resection of malignant tumor around the knee. Gait analysis provides a relative objective data about the gait function of patients with prosthesis. The purpose of this study was to compare the gait pattern of the patients who underwent limb salvage surgery with prosthesis for distal femur and that of patients with prosthesis for proximal tibia. This study included ten patients (4 males, 6 females, mean age 22.7 years, range 14-36) who underwent a wide resection and Kotz hinged modular reconstruction prosthesis replacement and six normal adult(Control). The site of bone tumor was the distal femur (Group 1) in six patients and proximal tibia (Group 2) in 4 patients. The follow-up period ranged from 15 to 82 months (mean : 33 months). The evaluation consisted of clinical assessment, radiographic assessment, gait analysis using VICON 370 Motion Analysis System. The gait analysis included the linear parameters such as, walking velocity, cadence, step length, stride length, stance time, swing time, single support and double support time and the three-dimensional kinematics (joint rotation angle, velocity of joint rotation) of ankle, knee, hip and pelvis in sagittal, coronal and transverse plane. For the kinetic evaluation, the moment of force (unit: Nm/kg) and power (unit: Watt/kg) of ankle, knee and hip joint in sagittal, coronal and transverse plane. In the linear parameters, cadence, velocity, step time and single support were decreased in both group 1 and group 2 compared with control. Double support decreased in group 2 compared with control significantly(p<.05). In contrast to our hypothesis, there was no significant difference between group 1 and group 2. Mechanical and biological complication rates of the modified lateral-screw-retained implant prosthesis in the posterior region: an alternative to the conventional Implant prosthetic system

  • PURPOSE. The modified lateral-screw-retained implant prosthesis (LSP) is designed to combine the advantages of screw- and cement-retained implant prostheses. This retrospective study evaluated the mechanical and biological complication rates of implant-supported single crowns (ISSCs) inserted with the modified LSP in the posterior region, and determined how these complication rates are affected by clinical factors. MATERIALS AND METHODS. Mechanical complications (i.e., lateral screw loosening [LSL], abutment screw loosening, lateral screw fracture, and ceramic fracture) and biological complications (i.e., peri-implant mucositis [PM] and peri-implantitis) were identified from the patients' treatment records, clinical photographs, periapical radiographs, panoramic radiographs, and clinical indices. The correlations between complication rates and the following clinical factors were determined: gender, age, position in the jaw, placement location, functional duration, clinical crown-to-implant length ratio, crown height space, and the use of a submerged or nonsubmerged placement procedure. RESULTS. Mechanical and biological complications were present in 25 of 73 ISSCs with the modified LSP. LSL (n=11) and PM (n=11) were the most common complications. The incidence of mechanical complications was significantly related to gender (P=.018). The other clinical factors were not significantly associated with mechanical and biological complication rates. CONCLUSION. Within the limitations of this study, the incidence of mechanical and biological complications in the posterior region was similar for both modified LSP and conventional implant prosthetic systems. In addition, the modified LSP is amenable to maintenance care, which facilitates the prevention and treatment of mechanical and biological complications.

스캔이 가능한 치유지대주를 이용한 구치부 단일 임플란트 수복 증례 (Posterior single implant prosthesis using scannable healing abutment)

  • 임플란트 보철 치료의 성공에 정확한 인상 채득은 매우 중요한 과정이다. 이러한 임플란트 보철의 인상을 채득하는 방법에는 임프레션 코핑과 인상재를 이용한 전통적인 방법과 구강 스캐너와 디지털 인상 코핑(scanbody)을 이용한 디지털 인상채득법이 있다. 하지만 인상용 코핑과 scanbody는 모두 구강 내 치유 지대주(healing abutment)를 제거하고 장착해야 한다. 때문에 술자는 치유 지대주와 임프레션 코핑 또는 scanbody를 여러 번 체결하고 제거하는 과정을 반복해야 한다. 뿐만 아니라 임프레션 코핑 또는 scanbody는 교합면보다 높게 솟아 있어 환자는 지속적으로 개구 상태를 유지해야 하는 불편을 주기도 한다. 이러한 기존의 방법을 극복하고자 최근에는 치유 지대주에 scanbody의 스캔 가능 부분(scannable part)을 부여하여 치유 지대주를 제거할 필요 없이 구강 내에서 직접 구강 스캐너로 스캔이 가능한 치유지대주(scannable healing abutment)가 소개되었다. 스캔이 가능한 치유지대주를 이용하여 구치부 단일 임플란트를 수복한 증례를 소개하고자 한다.

상악 전치부 단일 임플란트의 심미 보철 수복 증례 (Single implant restoration with esthetic prosthodontic treatment in maxillary anterior tooth: A case report)

  • 상악 전치부의 단일 임플란트 수복 치료 시, 치은 퇴축 및 골결손 문제를 가질 경우 임플란트 보철의 심미적인 결과를 얻는 것은 쉽지 않다. 장기적으로 심미적 안정성이 유지되기 위해서는 임플란트를 이상적인 위치에 식립하는 것이 중요하며 연조직의 회복 및 보철물과의 조화가 중요하다. 이러한 심미성이 더욱 요구되는 임플란트 보철물의 제작 시 이상적인 임플란트의 위치 뿐만 아니라, 주변 연조직과의 조화 역시 중요하다. 본 증례에서는 상악 전치부에서 골결손과 치은 퇴축이 진행된 47세 남자환자에서 골유도재생술 및 치은재형성술을 통해 심미 임플란트 치료를 진행하였다.

단일치 상실을 레진 접착 고정성 보철물로 수복한 증례 (Resin bonded fixed prosthesis for single tooth restoration: A case report)

  • 단일 치아 상실 시 전부피개관을 유지장치로 사용하는 고정성 보철이나 임플란트를 이용한 치료가 우선적으로 선택되어 사용되고 있다. 하지만 전부 피개관 지대치 형성을 위해서는 건전한 치질을 많이 삭제해야 한다는 단점이 있다. 임플란트의 경우 인접 치아의 손상을 피할 수 있으나 환자의 구강 상태와 전신 상태에 따라 불가능하거나 일정기간 연기해야 하는 경우도 있다. 본 증례에서는 최소한의 치질 삭제로 젊은 환자와 치은 퇴축이 있는 환자의 단일 치아 상실을 수복하기 위해 레진 접착 고정성 보철물을 선택하였다. 모든 환자들이 심미적, 기능적으로 만족하였기에 이를 보고하는 바이다.
