폐암 진단에 적용 가능한 측면 유동 면역 형광 분석법 개발 (Development of Lateral Flow Immunofluorescence Assay Applicable to Lung Cancer)

  • 탄소나노점@실리카를 신호 형질 소재로 이용한 측면 유동 면역 형광 분석법을 개발하여 폐암 바이오마커 중에 하나인 레티놀 결합 단백질 4의 농도를 분석하는 데 적용하고자 하였다. 측면 유동 면역 형광 분석법에서 항원 검출을 위해 바이오리셉터로 주로 사용하였던 항체 대신 좀 더 경제적이고, 장기간 보관성이 용이하며, 특정 표적 단백질에 대해 친화력이 강한 압타머를 니트로셀룰로오스 멤브레인에 사용하였다. 레티놀 결합 단백질 4에 특이적이며 5' 말단을 비오틴으로 변형한 압타머를 뉴트라비딘과 반응시켜 비오틴과 뉴트라비딘의 강한 결합력에 의해 압타머가 니트로 셀룰로오스 멤브레인에 고정되도록 하였다. 압타머가 고정된 스트립에 레티놀 결합 단백질 4 항체를 공유결합으로 고정한 탄소나노점@실리카 블루 형광 신호 형질 나노입자와 레티놀 결합 단백질 4 항원을 측면 유동 방식으로 흘려 주어 샌드위치 복합체를 형성하였다. 이렇게 형성된 샌드위치 복합체에서 탄소나노점@실리카 나노입자에 의한 형광 신호를 측정하여 항원 농도를 분석하기 위한 최적의 조건을 선정하기 위해 전개 완충용액에 첨가된 계면활성제의 농도, 이온 세기를 변화시키면서 블로킹 시약을 추가적으로 사용하였다. 그 결과 150 mM NaCl 및 0.05% Tween-20을 포함하는 10 mM Tris 완충용액(pH 7.4)에서 0.6 M 에탄올아민을 블로킹 시약으로 사용하였을 때 니트로셀룰로오스 멤브레인에 도포된 압타머와 레티놀 결합 단백질 4 항원 및 탄소나노점@실리카 나노입자로 레이블링한 항체가 결합하여 최적의 형광분석신호를 내는 것을 확인 가능하였다. 이러한 결과는 현장진단검사 키트로 현재 각광을 받고 있는 측면 유동 면역 형광 분석법에서 항체 대신 압타머를 니트로셀룰로오스 멤브레인에 고정함으로써 좀 더 경제적이며, 장기간 보관이 용이한 측면 유동 면역 형광 분석 칩을 제작하여 폐암 질환 진단용 바이오마커 검출이 가능함을 시사하였다.

두정부 백질 물질을 이용한 수소 자기 공명 분광 분석 (Analysis of $^1H$ MR Spectroscopy of parietal white matter material Phantom)

  • 두정부 백질 물질 NAA, Choline(Cho), Creatine(Cr), choline chloride, glutamin 등으로 자체 제작한 phantom을 이용하여 고 자장(4.7T)과 중자장(1.5T)에서의 신호강도 대 잡음비, 스펙트럼 분해능을 알아 볼 수 있는 선폭과 $T_2$, 그리고 $T_E$값의 변화에 따른 각 대사물질의 변화별 스펙트럼 등을 구하여 이론적인 정보와 실제적인 정보와의 차이를 알아보고자 한다. 이용된 기기는 Bruker Biospec 4.7T와 1.5T GE SIGNA를 이용하여 MRS의 결과를 얻었다. 관심영역에서 얻은 정보에서 분광 peak의 면적을 구하였다. 통계처리는 Microsoft사의 Excel program내에 있는 통계 package를 이용하였다. 중자장(1.5T)과 고자장(4.7T)에서의 각각의 대사물질의 스펙트럼을 얻어 본 결과 자장의 균일도와 SNR과 $T_2$값의 차이가 이론적인 값보다는 직선성을 보이지 않았다. 1.5T에서의 선폭은 Cho, Cr, NAA 순서로 $5.30{\pm}1.07,\;4.81{\pm}0.14,\;5.49{\pm}0$, 이에 반해 4.7T에서는 $9.14{\pm}0.55,\;8.87{\pm}0.67,\;9.65{\pm}0.56$ Hz 값이 나왔다. 평균 SNR은 NAA의 물질의 경우를 3회 측정하였는데 그 평균치는 63%의 증가를 보였다. $T_E$ 변화로 스펙트럼을 분석했을 때 $T_E=60ms$일 때 가장 SNR이 좋은 값을 보였고 $T_E=135ms$일 때 가장 낮은 값을 나타나 보였다. 두정부 백질 물질 Phantom을 제작하여 자장의 강도 즉 중자장(1.5T)과 고자장(4.7T)에 따라 분석하게 되었는데 고자장의 영역으로 갈수록 분해능, SNR은 좋아져 보이나 자장의 세기가 커질수록 이론적인 수치보다는 상당히 적은 증가를 보이고 있다는 것을 이 연구를 통해 알게 되었다.

뇌전이 종양의 발견에 있어서 Doble dose Gd-DTPA를 이용한 3 T MRI와 1.5 T MRI간의 비교연구 (Evaluation between 3.0 T vs 1.5 T MRI in Detection of Brain Metastasis using Double Dose Gd-DTPA)

  • 목적: 작은 뇌전이 종양의 조기 발견은 중요하다. 이 연구의 목적은 1.5 T MRI와 3.0 T MRI 간의 크기에 따른 뇌전이 종양의 발견율을 비교하는 것이다. 대상 및 방법: 폐암으로 진단 받은 162명의 환자를 대상으로 TNM 병기를 위해 뇌 MRI를 시행하였다. Gd-DTPA 를 2배 용량으로 투여 후, 3.0 T MRI에서 훼손경사회복획득으로 촬영하였으며 그 후 1.5 T MRI에서 T1 스핀 에코로 촬영하였다. 3명의 방사선과 전문의가 합의하여 MRI를 판독하였으며 정성 평가를 시행하였다. 3.0 T와 1.5 T MRI에서 크기에 따라 민감도, 양성 예측률, 정확도를 평가하였다. 신호 강도를 사용하여 전이 종양과 인접 조직간의 신호강도 비를 계산하였다. 결과: 162명의 환자 중 31명에서 1.5 T 또는 3.0 T MR에 뇌전이 종양이 발견되었다. 3.0 T MRI에서 143개의 종양이 발견되었으나 1.5 T MRI에서 137개의 종양이 발견되었다. 6개의 종양이 3.0 T MRI에서만 발견되었으며 크기는 모두 3 mm 미만이었다. 3.0 T MRI의 민감도, 양성 예측률, 정확도는 각각 100%, 100%, 100%이며, 1.5 T MRI에서는 각각 95.8%, 88.3%, 85.1% 이다. SI ratio는 1.5 T MRI보다 3.0 T MRI에서 유의하게 높았다(p=0.025). 결론: Double dose Gd-DTPA를 이용한 3.0 T MRI는 3 mm미만의 뇌전이 종양을 발견하는데 있어서 1.5 T MRI 보다 우수하다.

의인화몸통팬텀에서 PET/CT SUV 비율 (PET/CT SUV Ratios in an Anthropomorphic Torso Phantom)

  • 표준섭취계수(SUV) PET과 영상 재구성 방법에 따라 크게 달라진다. 삼성 서울병원 핵의학과에 설치된 GE사의 Discovery MIDR와 Discovery Ste의 영상 재구성 알고리즘을 이용하여 의인화몸통팬텀 내의 심장, 간과 배경영역에서 표준섭취계수를 측정하여 실제 SUV와 비교하였다. 영상 재구성 알고리즘은 MIDR에서는 VPFX-S (TOF+PSF), QCFX-S-350 (Q.Clear+TOF+PSF), QCFX-S-50와 VPHD-S (OSEM+PSF), Ste에서는 VUE Point (OSEM)와 FORE-FBP를 사용하였다. 방사선사의 방사선 피폭을 감소시키기 위하여 소량의 18F-FDG 선원을 물과 혼합하였다: 52.5 ml 심장에는 2.28 MBq, 1,290 ml 간에는 20.3 MBq와 9,590 ml 배경영역에는 45.7 MBq을 주입하였다. 심장에서의 표준섭취계수는 MIDR의 VPFX-S, QCFX-S-350, QCFX-S-50, VPHD-S와 Ste의 VUE Point (OSEM)와 FOR-FBP 알고리즘에서 각각 27.1, 28.0, 27.1, 26.5, 8.0와 7.4 이었으며, 기대치는 5.9이었다. 배경역역에서는 4.2, 4.1, 4.2, 4.1, 1.1와 1.2 이었으며, 기대치는 0.8이었다. 각 영역에서 표준섭취계수는 PET과 알고리즘에 따라 크게 차이가 있었지만, 심장과 배경영역의 SUV 비율은 비교적 일정하여 6개 영상 재구성 알고리즘에 대하여 6.5, 6.8, 6.5, 6.5, 7.3와 6.2 이었으며 기대치는 7.8이었다. 심장에서의 평균 신호 대 잡음비(SNR)는 각각 6개 알고리즘에 대하여 8.3, 12.8, 8.3, 8.4, 17.2와 16.6이었다. 결론적으로 PET 성능 평가는 각 영역에서의 절댓값 보다는 두 영역 사이의 SUV 비율이 적절하며, 비교적 소량의 방사능으로도 확인할 수 있는 가능성이 있다.

  • 텔레뷰어는 초음파를 이용하여 공벽을 검층하는 기법이다. 시추공 중심에서 방사된 초음파 빔이 공벽에서 반사되어 다시 중심으로 되돌아 왔을 때의 초음파 빔 진폭 및 주시는 공벽 암반의 물성 및 시추공 내벽 상태 파악을 위한 기본자료가 된다. 이러한 측정 과정이 공벽 전체를 대상으로 이루어지면 그 결과는 2차원(심도 ${\times}$ 방사각) 진폭 및 주시 분포(진폭이미지 및 주시이미지)로 표현된다. 여기서 초음파 빔의 초점화는 절리의 분산현상을 함께 유발하게 되며 그에 따라 큰 폭으로 하락된 진폭치는 주위 암반의 것과 크게 차별화되어 절리의 특성(예 : 크기, 열린상태)이 이미지에서 훌륭하게 반영된다. 한편, 시추 후의 공벽상태는 암반이 연약할수록 그만큼 거칠게 됨이 관찰되고 있으며 이에 대한 빔의 산란은 별도의 진폭하락을 초래하고 있다. 따라서, 텔레뷰어 진폭치는 공벽 암반의 임피던스 이외에도 암반의 견고성파 연관됨을 볼 수 있으며 특히 풍화암일 경우에는 더욱 그러하다. 이러한 현상은 무엇보다 암층분리의 정확성을 높히는 것이 된다. 텔레뷰어 주시이미지는 바로 3D caliper log로 쉽게 전환될 수 있으며 그 결과는 우선 공벽의 크기 및 거친 정도뿐만 아니라 나아가서 시추공 주위 응력분포에 대한 정보도 제시할 수 있다. 따라서 본 논문은 상기 특유의 초음파 빔 특성과 또한 양질의 텔레뷰어 이미지를 바탕으로 개발된 다양한 분석기법들을 소개하고 있으며 동시에 그에 따른 결과가 특히 토목설계 및 시공에서 요구하는 지반조사 매개변수를 훌륭하게 대변하고 있음을 보여주고 있다.>이용이 검토되어질 수 있다.에 비해 결코 우월한 위치에 있지 않다. 오히려 많이 낙후되었다고 할 수 있다. 경제규모와 위상과 발전상태에 비추어 보면 실로 부끄러운 일이 아닐 수 없다. 이렇게 된 배경에는 연약지반이 사회의 문제로 대두되는 일이 미미하여 이 분야에 쓸리는 관심이 상대적으로 적은 점이 주원인이라고 볼 수 있다. 연약지반이 기술현장에서 문제로 떠오르기 시작한지도 째 오랜 시간이 경과하였다. 이제 더 이상 우리의 기술수준을 이대로 방치할 수는 없는 시점에 와 있는 것이다. 연약지반에 몸담고 있는 우리 스스로가 위상을 지키려는 노력을 배가하여야 한다. 연약지반 공학자들은 스스로 고급의 데이터를 생산하고 평가하고 예측하는 기법을 활발하게 적용하고 발전시킴으로써 자신들의 위치를 지켜야 할 것이다.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.
