Effect of Hip Flexion and Internal Rotation on the Hip Abductor Muscle Activity During Side-Lying Hip Abduction in Subjects With Gluteus Medius Weakness

  • Background: Many previous studies recommended the side-lying hip abduction (SHA) exercise for targeting the gluteus medius (Gmed) and gluteus maximus (Gmax) muscle activity while the decreasing tensor fasciae latae (TFL) activation. Mischoice of hip position and angle in SHA may increase the risk of lower extremity injuries and undesirable muscle activation. However, information is limited on the effect of composite hip flexion angles and hip rotation on the gluteal muscle activity during SHA. Objects: This study aimed to compare muscle activity (Gmed, TFL, and Gmax) and activity ratios (Gmed/TFL, Gmax/TFL, and Gmed/Gmax) using surface electromyography (EMG) during SHA exercise at three different hip flexion angles either with or without internal rotation (IR) in subjects with Gmed weakness. We hypothesized that applying hip flexion and IR during SHA would increase gluteal muscle activity and decrease TFL activity. Methods: Muscle activity and activity ratios in 20 volunteers with Gmed weakness during 6 different SHA were investigated with surface EMG. One-way repeated-measures analysis of variance was used to determine the statistical significance. Results: Significant differences were found among the six different exercises for Gmed ($F_{2,41}=11.817$, p<.001) and Gmax ($F_{3,52}=5.513$, p=.003) muscle activity, and Gmed/TFL ($F_{3,54}=8.735$, p<.001) and Gmax/TFL ($F_{2,37}=4.019$, p=.028) activity ratios. Conclusion: Applying hip flexion is an effective method for increasing gluteal activity, and it elicits great Gmed/TFL and Gmax/TFL activity ratios during SHA in subjects with Gmed weakness.

Consideration of the Exercise position for Facilitating Gluteus Medius Maximally in Normal Adults

  • Purpose: This study examined the most efficient exercise position to activate the gluteus medius (GM) and tensor fascia latae (TFL) in hip abduction in side-lying (HA-SL), clam in side-lying (CL-SL), and sling bridging in side-lying (SB-SL), which are the most representative GM exercises. Methods: Twenty-four healthy male adults aged from 20 to 40 years, whose body mass index was under 25, participated in this experiment. While all participants conducted three different positions with a counterbalanced manner, such as in AB, CL, and BR, activation of the GM and TFL was measured using 8-channel wireless EMG. Exercise was performed for 10 seconds three times in total with a five minute-break session. Results: Significant differences in GM and TFL activation were observed among the three positions (p<0.05). The highest activation of 60.69 was observed in BR followed in order by 46.03 and 12.92 in HA-SL and CL, respectively. TFL activation in HA-SL was 42.01, followed in order by 35.98 and 14.01 in BR and CL, respectively. On the other hand, there was no significant difference in TFL muscle activation between BR and HA-SL. Conclusion: These findings suggest that both BR and HA-SL in GM can be done selectively. CL has remarkably low muscle activation in GM and TFL, which makes it less valuable in GM and TFL exercise. In conclusion, selective BR and HA-SL exercise should be applied to maximally and effectively activate the GM.

Relationship Between the Number of Hip Abduction Performance With Contralateral Adduction in Side-lying and the Lateral Pelvic Shift Distance During One-leg Lifting

  • Background: The gluteus medius (Gmed) plays a critical role in maintaining frontal plane stability of the pelvis during functional activities, such as one-leg lifting. Side-lying hip abduction (SHA) has been used as a dynamic test to evaluate Gmed function. However, the abduction force of the lower leg against the floor is not controlled during SHA. Therefore, hip abduction performance with contralateral adduction in the side-lying position (HAPCA) can be proposed as an alternative method to assess performance of hip abduction. If the number of HAPCA is related to the lateral pelvic shift distance, a new quantitative measurement for hip abductor function may be presented. Objects: This study aimed to investigate the relationship between the number of successful HAPCA and the lateral pelvic shift distance during one-leg lifting. Methods: Thirty healthy participants were recruited, and lateral pelvic shift distance was measured during one-leg lifting test using two-dimensional analysis. The number of successful HAPCA was counted when participants touched both target bars at the beat of a metronome. Results: There was a negative correlation between the number of HAPCA and lateral pelvic shift distance during one-leg lifting (r = -0.630, p < 0.05). The number of HAPCA accounted for 39.7% of the variance in the lateral pelvic shift distance during one-leg lifting (F = 18.454, p < 0.001). Conclusion: The number of successful HAPCA is significantly correlated with lateral pelvic shift distance during one-leg lifting. This finding suggests that HAPCA can be proposed as a new measurement for hip abductor performance and more research is needed on its relationship with hip abductor strength.

Reliability and Validity of the Side-lying Instability and Prone Instability Tests in Patients with Lumbar Segmental Instability

  • PURPOSE: The purpose of this study is to conduct inter-rater and intra-rater reliability tests in patients with low back pain (LBP) using the prone instability test (PIT) and side-lying instability test (SIT). We have analyzed the Korean version Oswestry disability index (K-ODI) correlations and radiograph finding (RF) for validity. METHODS: Individuals (n = 51) (mean age of 40.27 ± 13.28) with LBP for at least over a week were recruited, together with two participating physical therapist examiners. The measurement consisted of PIT, PST, K-ODI, and RF. Sensitivity (Sn), specificity (Sp), positive predictive value, negative predictive value, prevalence index, agreement %, Cohen's kappa, and prevalence-adjusted bias-adjusted kappa (PABAK) were calculated. The PIT and SIT were compared with RF for validity analysis, while PIT, SIT, K-ODI, and RF were calculated for the correlation analysis. RESULTS: The intra-rater reliability test measured for the PIT (kappa = .79, PABAK = .88) and SIT (kappa = .73, PABAK = .84), and inter-rater reliability test measured for the SIT (kappa = .80, PABAK = .88) showed good agreements. The PIT (Sn = .65, Sp = .63) and SIT validities (Sn = .68, Sp = .70) were compared with RF, showing a significant correlation in PIT and RF (r = .69), SIT and RF (r = .73), and PIT and K-ODI (r = .53). CONCLUSION: The SIT is a more comfortable position test than the PIT in patients. Both PIT and SIT have acceptable reliability and validity.

축 안정화 디자인이 상이한 토릭소프트콘택트렌즈의 회전 양상 차이 (Difference in Rotation Pattern of Toric Soft Contact Lenses with Different Axis Stabilization Design)

  • 목적: 본 연구에서는 축 안정화 디자인이 상이한 두 토릭소프트콘택트렌즈의 회전축과 회전량이 자세와 응시방향에 따라 달라지는 지를 알아보았다. 방법: 20~30대 52안을 대상으로, Lo-Torque$^{TM}$디자인 및 ASD 디자인(accelerated stabilized design) 토릭소프트콘택트렌즈를 피팅한 후 정면을 포함한 5가지 응시방향과 누운 자세에서의 회전량을 측정하였다. 결과: 정자세에서 정면 및 수직방향 응시 시 Lo-Torque$^{TM}$ 디자인 렌즈는 코쪽, ASD 디자인 렌즈는 귀쪽으로 회전하는 비율이 높은 것으로 나타났다. 수평방향 응시시에는 두 렌즈 모두 응시방향과 반대로 축회전이 일어났다. 위쪽을 응시할 때의 회전량이 가장 작았으며 코쪽을 응시할 때의 회전량이 가장 많았다. $5^{\circ}$ 이내의 회전량을 보인 경우는 정면과 수직방향을 응시할 때의 Lo-Torque$^{TM}$ 디자인 렌즈에서 더 많았으며, 수평방향 응시 시에는 ASD 디자인 렌즈에서 더 많았다. 또한, 누운 직후에는 Lo-Torque$^{TM}$ 디자인 렌즈의 회전량이 더 적었으나 1분 후에는 ASD 디자인 렌즈의 회전량이 더 적었다. 결론: 본 연구에서는 토릭소프트콘택트렌즈 착용 후 응시방향 및 자세에 의해 유발되는 축회전이 축 안정화 디자인에 따라 달라짐을 확인하였다.

옆으로 누워 엉덩관절 벌림운동 시 복부드로우-인, 복부브레이싱, 골반압박벨트가 중간볼기근과 몸통 근육의 활성도에 미치는 영향 (Effects of Abdominal Draw-in Maneuver, Abdominal Bracing, and Pelvic Compression Belt on Muscle Activities of Gluteus Medius and Trunk During Side-Lying Hip Abduction)

  • Background: Improvement of lumbo-pelvic stability can reduce the compensatory action of the quadratus lumborum (QL) and selectively strengthen the gluteus medius (GM) during side-lying hip abduction (SHA). There are abdominal draw-in maneuver (ADIM) and abdominal bracing (AB) as active ways, and pelvic compression belt (PCB) as a passive way to increase of lumbo-pelvic stability. It is necessary to compare how these stabilization methods affect the selective strengthening of the GM. Objects: To investigate the effects of ADIM, AB, and PCB during SHA on the electromyography (EMG) activity of the GM, QL, external oblique (EO) and internal oblique (IO), and the GM/QL EMG activity ratio. Methods: A total of 20 healthy male adults participated in the study. The subjects performed three conditions in side-lying in random order: SHA with ADIM (SHA-ADIM), SHA with AB (SHA-AB), and SHA with PCB (SHA-PCB). To compare the differences among the three conditions, the EMG activities of the GM, QL, EO and IO, and GM/QL EMG activity ratio were analyzed using one-way repeated ANOVA. Results: The EMG activity of the QL was significantly higher in SHA-AB than in SHA-ADIM and SHA-PCB. The GM/QL activity ratio was significantly higher in SHA-PCB than in SHA-ADIM and SHA-AB. In addition, the figure for SHA-ADIM was significantly higher than that for SHA-AB. In the case of the EO, the figure for SHA-AB was significantly higher than corresponding values for the other two conditions. The figure for SHA-ADIM was significantly higher than that for SHA-PCB. The EMG activity of the IO was significantly higher in SHA-AH than in SHA-PCB. Conclusion: It can be suggested that wearing the PCB can more selectively strengthen the GM than to perform ADIM and AB during SHA. In addition, the ADIM can be recommended when there is a need to strengthen abdominal muscles during SHA.

This study put emphasis on lying in which place Hwaam cave in Tong-myon, Chong son-gun, Kangwondo is situated and having in which stand is in comparison with other districts, and how the geological structure which determine the cave forming cause is.(omitted)

내경-후교통 동맥류 수술시 전 상상돌기 절제 유무에 대한 술전 혈관조영술의 의의 (Preoperative Angiographic Value in Anterior Clinoidectomy for Surgery of Internal Carotid-Posterior Communicating Artery(IC-PC) Aneurysms)

  • Objective : Internal carotid-posterior communicating artery(IC-PC) aneurysms can be clipped easily without any special preparations. Occasionally, however, it is difficult to clip the low-lying IC-PC aneurysms without some kinds of additional procedures. Clinical Material and Methods : We experienced four cases of low-lying IC-PC aneurysms, which the intradural anterior clinoidectomy and/or anterior petroclinoid fold(APF) resection was essential to expose the proximal side of the aneurysmal neck and/or proximal control. One patient harbored two low-lying IC-PC aneurysms bilaterally. The patients were divided into two groups according to the necessity of anterior clinoidectomy : Group I(n=4) that needed an intradural clinoidectomy and/or APF resection and Group II(n=29) that had IC-PC aneurysms, easily clipped without any special preparation. Also, various radiometric parameters were measured through the preoperative angiograms. Results : The incidence of such aneurysms was 12% among a total of thirty-three surgically treated IC-PC aneurysms during lasr 3 years. Among four cases, three cases presented with subarachnoid hemorrhage and all aneurysmal sac projected to postero-inferior direction. In our study, We initially considered the necessity of intraoperative anterior clinoid process(ACP) removal and/or resection of APF in cases of shorter distance less than 5.6mm between the proximal aneurysmal sac and tip of the ACP(p<0.001), and the proximal portion of aneurysmal neck has located below the interclinoid line(p=0.001). Conclusion : Through a careful preoperative evaluation, some radiometric parameters can be used to determine whether the ACP should be removed in clipping of the low-lying IC-PC aneurysms. Unlike to total removal of the ACP, the intradural partial anterior clinolidectomy and/or APF resection, which are more familiar to surgeons, reduce the risks of the premature rupture, operative time, and also contribute a more precise clip placement with proximal control than the extradural clinoidectomy.

엉덩 관절 벌림 운동 시 압력 생체 되먹임 장치와 골반 벨트의 사용이 엉덩 벌림근의 선택적 근 활성도에 미치는 영향 (The Effects of Using a Pressure Bio-feedback Unit and a Pelvic Belt on Selective Muscle Activity in the Hip Abductor during Hip Abduction Exercise)

  • Purpose : The purpose of this study was to examine the effects of using a pressure bio-feedback unit (PBFU) and a pelvic belt (PB) on the electromyographic (EMG) signal amplitude of the gluteus medius (Gmed) and the quadratus lumborum (QL) during hip abduction exercise when lying on the side. Methods : Twenty able-bodied volunteers (10 male, 10 female) were recruited for this study. The EMG signal amplitude was randomly measured during hip abduction with preferred hip abduction (PHA), with PBFU, and with PB. The surface EMG signal was recorded from the Gmed and the QL. Data were analyzed using a one-way repeated ANOVA. Results : Muscle activity of Gmed was significantly higher in PBFU and in PB than in PHA (p<.05). There were no significant difference between PBFU and PB(p>.05). Muscle activity of the QL was significantly lower in PB than in PHA(p<.05). The Gmed/QL muscle activity ratio was also significantly higher in PBFU and in PB than in PHA(p<.05), with no significant difference between PBFU and PB (p>.05). Conclusion : Based on these findings, using a PBFU and a PB is an effective method to disassociate QL use from Gmed use during hip abduction exercises when lying on the side.

Effect of Horizontal Adduction Force on Infraspinatus and Deltoid Activities During the Side-Lying Wiper Exercise Using Pressure Biofeedback

  • Background: Shoulder external rotation exercises are commonly used to improve the stabilizing ability of the infraspinatus. Although the side-lying wiper exercise (SWE) is the most effective shoulder external rotation exercise to maximize infraspinatus activity, the effect of adduction force on the infraspinatus and posterior deltoid has not been demonstrated. Objects: This study was conducted to investigate whether horizontal adduction force increases infraspinatus activity and decreases posterior deltoid activity. Methods: Twenty-eight healthy subjects (male: 21, female: 7; $age=23.5{\pm}1.8years$; $height=170.1{\pm}7.4cm$; $weight=69.4{\pm}9.6kg$) were recruited. Subjects were asked to perform the SWE under two conditions: (1) general SWE and (2) SWE with adduction force using pressure biofeedback. Surface electromyography (EMG) signals of the infraspinatus and posterior deltoid were recorded during SWE. Paired t-tests were used to compare the EMG activity of the infraspinatus and posterior deltoid between the two conditions. Results: Posterior deltoid muscle activity was significantly decreased following SWE with adduction force ($7.53{\pm}4.52%$) relative to general SWE ($11.68{\pm}8.42%$) (p<.05). However, there was no significant difference in the infraspinatus muscle activity between the SWE with adduction force ($28.33{\pm}12.16%$) and the general SWE ($26.54{\pm}13.69%$) (p>.05). Conclusion: Horizontal adduction force while performing SWE is effective at decreasing posterior deltoid activity.
