Ictal Hyperperfusion of Cerebellum and Basal Ganglia in Temporal Lobe Epilepsy: SPECT Subtraction (측두엽 간질에서 발작기 소뇌와 기저핵의 뇌혈류 변화: SPECT 감영영상)

  • Purpose: The ictal perfusion patterns of cerebellum and basal ganglia have not been systematically investigated in patients with temporal lobe epilepsy (TLE). Their ictal perfusion patterns were analyzed in relation with temporal lobe and frontal lobe hyperperfusion during TLE seizures using SPECT subtraction. Materials and Methods: Thirty-three TLE patients had interictal and ictal SPECT, video-EEG monitoring, SPGR MRI, and SPECT subtraction with MRI co-registration. Results: The vermian cerebellar hyperperfusion (CH) was observed in 26 patients (78.8%) and hemispheric CH in 25 (75.8%). Compared to the side of epileptogenic temporal lobe, there were seven ipsilateral hemispheric CH (28.0%), fifteen contralateral hemispheric CH (60.0%) and three bilateral hemispheric CH (12.0%). CH was more frequently observed in patients with additional frontal hyperperfusion (14/15, 93.3%) than in patients without frontal hyperperfusion (11/18, 61.1%). The basal ganglia hyperperfusion (BGH) was seen in 11 of the 15 patients with frontotemporal hyperperfusion (73.3%) and 11 of the 18 with temporal hyperperfusion only (61.1%). In 17 patients with unilateral BGH, contralateral CH to the BGH was observed in 14 (82.5%) and ipsilateral CH to BGH in 2 (11.8%) and bilateral CH in 1 (5.9%). Conclusion: The cerebellar hyperperfusion and basal ganglia hyperperfusion during seizures of TLE can be contralateral, ipsilateral or bilateral to the seizure focus. The presence of additional frontal or basal ganglia hyperperfusion was more frequently associated with contralateral hemispheric CH to their sides. However, temporal lobe hyperperfusion appears to be related with both ipsilateral and contralateral hemispheric CH.

  • The purpose of this study was to investigate treatment effectiveness of TENS and bite plane for bruxism and TMJ dysfunction patients. The electromyograms were made on males aged 23 to 25 with sound stomatognathic system, 5 males bruxism aged 24 to 27, and 1 male and 4 females TMJ dysfunction patients (right TMJ pain) aged 14 to 33. The electromyographic study was limited to the middle of masseter muscle and anterior temporal muscle. The electromyographic study was carried out with 8-channel EM2 (Myotronics Research Inc.) and was taken 5 mandibular positions of clinical rest position, clench intercuspal position, clench protruded, cluch right, clench left. The 2 pairs of surface electrodes were used exactly, with the ear lobe as reference point. The recording were subjected to determine the mean voltage. The results were as follows; 1. In the clinical rest position, the muscle activities of bruxism and TMJ dysfunction patients were higher than those of normal at the before treatment, but that were lower or similar to those of normal at the TENS after and after bite plane wearing. 2. In the clench I.C.P., the muscle activities of TMJ dysfunction patients were decreased as the order of the before treatment, after TENS, after occlusal bite plane wearing, but those of bruxism were irregular. 3. In the clench right and the clench left, the muscle activities of the middle of masseter muscle of the non-working side of bruxism were higher than those of the working side at the before treatment and after TENS, but the muscle activities of after occlusal bite plane wearing were similar to those of the normal. 4. In the clench right and the clench left, the muscle activities of the middle of masseter muscle and anterior temporal muscle of the working side of TMJ dysfunction patients were higher than those of non-working side as like the normal at the before treatment, after TENS, and after occlusal bite plane wearing.

  • This paper deals with the design of a microphone array to measure location and spectral characteristics of railway noise sources. A microphone array estimates the direction of a noise source assumed as a point source using the delayed-sum beamforming method and its performance is determined in terms of resolution and side-lobe level. A 48 channel microphone array was already developed to measure noise sources of KTX trains and a new array with 96 microphones has been designed to enhance the performance. This paper simulates the performance the microphone arrays according to the configuration of microphones and verifies it through on-site tests.

The First Record of the Genus Glycinde (Polychaeta: Goniadidae) from Korea

  • The goniadid species, Glycinde bonhourei Gravier, 1904, is newly reported with the description and illustration from Korean waters. Our Korean materials of G. bonhourei have the following characteristics that are generally known as distinctive features from its congeners: the area II-1 on proboscis possesses unidentate papillae bearing broad base; the area IV on proboscis bears the papillae of duct's foot-shaped with rounded teeth; the area V on proboscis has straightly conical papillae bearing slightly bifid tip; 4-16 micrognaths are arranged on the dorsal side; all parapodia have one neuropodial presetal lobe; uniramous parapodia are present on 19-26 anterior segments. As a result of the present study, the genus Glycinde is newly reported from Korean waters.

  • Microwave or optical photonic band-gap(PBG) structures are conventionally realized by cascading distributive elements in a periodic pattern. However, the frequency bandwidth obtained through such plainly periodic arrangement is typically narrow, corporate with a relatively high rejection side-lobe band. To alleviate such problems, a design involving a chirping and tapering technique is hence introduced and employed. The design has been applied in both a planar stratified dielectric medium as well as a strip-line transmission line structure, and results are validated when compared with the corresponding conventional PBG structure.

Design of slotted waveguide antenna with slot depth variation (슬롯깊이 변화에 따른 도파관 슬롯 안테나의 설계)

  • We design a slotted waveguide antenna for X-band which a 1.4m length with slot depth variation. A 54 slot array is modeled and simulated by CST, and the side lobe level is suppressed effectively in a certain range of frequency. The experimental results of the antenna show that its gain, maximum sidelobe level and half power beam-width are 21.8dB, -31dB and 2.0deg, respectively.

Clinical evaluation of bronchiectasis (기관지확장증의 임상적 고찰)

  • We experienced 48 operations in 46 surgical patients of bronchiectasis admitted to the Department of Thoracic and Cardiovascular Surgery of Jeonbug National University Hospital from January, 1975 to August, 1982. Among 46 patients, 27 patients [59%] was age group between 21 to 30 years. Common symptoms were cough with sputum, hemoptysis, dyspnea, fever and chilliness, and chest pain. The duration of the symptoms was variable between below one year and above 10 years. The most frequent associated disease, probably the cause of the bronchiectasis, was secondary bacterial infection after viral infection. The left lower lobe and lingular segment was involved most frequently, and the most frequent pathologic type was mixed type [40%]. Single lobectomy, and combined lobectomy and segmentectomy were performed in 77% of the patients. Bilateral resection was performed in three patients with good result. In those patients, the isolated pulmonary function test on each side of the lung performed 2 month later primary lung resection could make them be prevented from pulmonary insufficiency after secondary lung resection. The results were good except two patients who developed pulmonary insufficiency and chronic empyema with bronchopleural fistula.

  • The imaging characteristics of a 2-D interferometric synthetic aperture radiometer, such as an angular resolution, depend largely on the type of an antenna array. In this paper, different array configurations of antenna are studied and compared with each array types to get more high resolution image in spatial. T-, X- and Y- types of antenna array are considered and the performances of each type are analyzed considering spatial resolution. The simulation results of candidate antenna types are presented in this paper. In case of Y-type the coverage area of the visibility function is wide and the angular resolution is high more than the others. X-type array shows the good performance for side lobe level.

Tuberculous Empyema Thoracis which was misled to Anterior Mediastinal Tumor (종격동 종양으로 오인된 결핵성 농포)

  • Tuberculosis is one of the most common chronic disease. While the disease process may involve anywhere of the body, tuberculosis of anterior mediastinum which forming tumor like mass was not recognized commonly. We experienced a surgical case of tuberculous empyema thoracis at anterior mediastinum in 16 year old boy. Preoperatively, he was diagnosed to anterior mediastinal tumor such as teratoma or dermoid cyst by routine study. Operation was performed by midline sternotomy extending over right 4th intercostal space. The mass was elongated football shaped [20x16x15] and markedly adhered to right side of pericardium, upper 8< lower lobe of the right lung. Opening the mass, enormous pus-like material was evacuated and excised segmentally with decortication. Postoperative pathologic diagnosis was tuberculous empyema thoracis and granuloma.

Initial-phase Sensitivity Analysis of Harmonic Measurements via Windowed DFT

  • When the windowed DFT algorithm is applied in harmonic measurements, the problem of initial-phase sensitivity will be encountered, this has an effect on harmonic amplitude accuracy. In this paper, the origin of initial-phase sensitivity is analyzed and the main factors that influence the level of initial-phase sensitivity are demonstrated. A method of reducing initial-phase sensitivity is proposed to increase the stability of harmonic measurements. We found that initial-phase sensitivity is determined by the side lobe peak level of the window functions when synchronous deviation is fixed. In addition, increasing the length of the time recorded can be used to remove initial-phase sensitivity. The correctness and validity of our conclusions have been confirmed through numerical results and field tests.
