탄성밴드를 이용한 측방 보행 훈련이 뇌졸중 환자의 보행 및 균형에 미치는 영향 (Effects of Side Walking Training with Elastic-Band on Gait and Balance of Stroke Patients)

  • Purpose: The aim of this study was to evaluate the effect of side walking training with an elastic-band on gait and balance ability of stroke patients. Methods: Twenty three patients with stroke participated in the study. Participants were randomly assigned to the side walking with elastic-band group (n=7), the side walking without elastic-band group (n=8), and the walking on the treadmill group (n=8);. 10 m walking test (10MWT), Dynamic Gait Index (DGI), Berg Balance Scale (BBS), and modified Functional Reach Test (mFRT) were performed for evaluatione of pre- and post-intervention in gait and balance ability of participants. Results: Significantly differences in 10 MWT, DGI, BBS, and mFRT were observed between pre- and post-intervention in three groups (p<0.05). Improvement of pre- and post-intervention of mFRT showed significant difference (p<0.05). The highest rate of change was observed in the side walking with elastic-band group and rate of change showed in the order of the side walking without elastic-band group, walking on the treadmill group. Conclusion: This study suggests that side walking training with an elastic-band may help to improve gait and balance ability of stroke patients.

동력 철도차량 기관실의 효과적인 환기에 관한 실험적 연구 (Experiments on the Effective Engine Room Ventilation in a Power Car)

  • Experimental study was made to find the effective ways for cooling of engine room in a power car through natural ventilation, forced ventilation and wind tunnel test of 1:10 model. Through the measurements of inner temperature of engine room and pressure distribution of model surface for the various opening combinations and the fan locations, the following results are obtained. For natural ventilation, side and top opening combination is more effective than the side openings only and optimal opening combination is all side openings with top center opening. For forced ventilation with fan on the roof, the combination of all sides and top opening is more effective than the combination of side and top opening, and the optimal location of fan is top center. When the model is located in the air stream, the combination of side and top opening is more effective than the side openings only, and the optimal location of top opening is the front opening. With both air stream and forced ventilation, the optimal location of fan is the side region, and the direction of side grill installation is found to have negligible effect on the room ventilation.

암환자의 치료 부작용, 가족지지, 삶의 질과의 관계 연구 (The Relationships of Treatment Side Effects, Family Support, and Quality of Life in Patient with Cancer)

  • Purpose: The study was done to identify the relationship of treatment side effects, family support, and quality of life in patients with cancer, and factors influencing quality of life. Method: A convenience sample of 106 patients who were receiving cancer treatment at W hospital were interviewed using the Side Effects scale by Hur, Family Support scale by Kang, and QOL scale by Ro. Result: Results indicate that women experienced more severe side effects than men. There was a negative relationship between side effects and quality of life, and a positive relationship between family support and quality of life. The most bothersome side effects were changes in taste and appetite, followed by general weakness and fatigue. Side effects such as loss of hair, nausea, dizziness, numbness, pins and needles in fingers and toes, and dry mouth were also experienced. General weakness and family support were analysed as to whether they were factors influencing quality of life. Conclusion: The results revealed that relieving general weakness should be given high priority in nursing interventions for patients undergoing cancer treatment. In addition, nursing programs should be developed that can reinforce family support.

Effect of Paretic Side and Non-paretic Side Arm Training on Trunk Control and Upper Limb Functions in Stroke Patients

  • Background: Stroke patients usually have arm weakness, which affects trunks and arms. Objective: To investigate the effects of paretic side and non-paretic side arm training on trunk control and upper limb functions. Design: Randomized Controlled Trial (single blind). Methods: Twenty patients with stroke in hospital were enrolled in the study. Twenty subjects were randomly assigned to paretic side arm training group (PATG, n = 10) or non-paretic side arm training group (NATG, n = 10). Trunk impairment scale (TIS) was used for trunk control, and box and block test (BBT) was used for upper limb function. Training was conducted for 4 weeks. Results: PATG showed significant difference in TIS (static balance, dynamic balance, coordination, total score) and BBT. NATG showed significant differences in static balance, and dynamic balance and total score except for coordination and BBT. PATG also showed a more significant difference in BBT and coordination and total score than NATG. Conclusions: The arm training performed on the paretic side are more effective than those performed on the non-paretic side in improving both upper limb function and trunk control in stroke patients.

Aerodynamic modification of setback distance at half height of the tall building to minimize the wind effect

  • The present study focuses on aerodynamic parameters behaviors and control on the single and double side setback building models at the buildings mid-height. The study is conducted by computational fluid dynamics (CFD) simulation. This study estimates the face wise pressure coefficient on single side setback buildings with a setback range of 20%-50% and double side setback buildings with setbacks ranging from 10%-25%. The polynomial fitted graphs from CFD data predict the Cp on different setback model faces within permissible limit ±13% error. The efficient model obtained according to the minimum drag, lift, and moment consideration for along and across wind conditions. The study guides the building tributary area doesn't control the drag, lift, and moment on setback type buildings. The setback distance takes a crucial role in that. The 20% double side setback model is highly efficient to regulate the moment for both along and across wind conditions. It reduces 17.5% compared to the 20% single side setback and 14% moment compared to the 10% double side setback models. The double side setback building is more efficient to control 4.2% moment than the single side setback building

하복부수술 후 경막외 부피바케인과 펜타닐에 첨가한 케타민과 미다졸람의 제통효과 (Influence of Ketamine and Midazolam on the Analgesic Effect of Epidural Bupivacaine and Fentanyl after Low Abdominal Surgery)

  • Background: There are many ways to provide superior analgesia for postoperative pain after abdominal surgery of which epidural analgesics with opioids and local analgesics are the most useful. In an effort to maximize the level of analgesia and to minimize the side effects, ketamine, midazolam, clonidine, and adrenalin can be co-administrated as an adjuvant. This study examined the analgesic effect and side effects of midazolam compared with those given an epidural injection of bupivacaine, fentanyl and ketamine. Methods: In a double blind randomized controlled trial, 50 patients received either fentanyl $0.3{\mu}g/kg/h$ and ketamine 0.1 mg/kg/h (Group FK) or fentanyl $0.3{\mu}g/kg/h$, ketamine 0.1 mg/kg/h and midazolam 0.4 mg/h (Group FKM), added to 0.125% of bupivacaine at a rate of as much as 2 ml/h, for patient controlled epidural analgesia (PCEA) after low abdominal surgery. Ten minutes before surgery, the patients received either 10 ml of 0.125% bupivacaine with 0.5 mg/kg of ketamine or 10 ml of 0.125% bupivacaine with the same amount of normal saline, added to fentanyl $50{\mu}g$. The pain score and the side effects were recorded at 1, 3, 6, and 24 hours after surgery. Results: There was no difference in the pain score except for the VAS on coughing 1 hour after surgery. FKM group had fewer side effects. Conclusions: There was a better analgesic effect and fewer side effects with the addition of epidural midazolam to bupivacaine and fentanyl with ketamine formula. However, more study on the dose and route of administration will be needed.

Effect of perioperative buccal fracture of the proximal segment on postoperative stability after sagittal split ramus osteotomy

  • Objectives: Buccal fracture of the mandibular proximal bone segment during bilateral sagittal split ramus osteotomy (SSRO) reduces the postoperative stability. The primary aim of this study is to evaluate the effect of this type of fracture on bone healing and postoperative stability after mandibular setback surgery. Materials and Methods: Ten patients who experienced buccal fracture during SSRO for mandibular setback movement were evaluated. We measured the amount of bone generation on a computed tomography scan, using an image analysis program, and compared the buccal fracture side to the opposite side in each patient. To investigate the effect on postoperative stability, we measured the postoperative relapse in lateral cephalograms, immediately following and six months after the surgery. The control group consisted of ten randomly-selected patients having a similar amount of set-back without buccal fracture. Results: Less bone generation was observed on the buccal fracture side compared with the opposite side (P<0.05). However, there was no significant difference in anterior-posterior postoperative relapse between the group with buccal fracture and the control group. The increased mandibular plane angle and anterior facial height after the surgery in the group with buccal fracture manifested as a postoperative clockwise rotation of the mandible. Conclusion: Bone generation was delayed compared to the opposite side. However, postoperative stability in the anterior-posterior direction could be maintained with rigid fixation.

Effect of Step Height and Visual Feedback on the Lower Limb Kinematics Before and After Landing

  • Background: Landing from a step or stairs is a basic motor skill but high incidence of lateral ankle sprain has been reported during landing with inverted foot. Objects: This study aimed to investigate the effect of landing height and visual feedback on the kinematics of landing and supporting lower limbs before and after the touch down and the ground reaction force(GRF)s. Methods: Eighteen healthy females were voluntarily participated in landing from the lower (20 cm) and the higher (40 cm) steps with and without visual feedback. To minimize the time to plan the movement, the landing side was randomly announced as a starting signal. Effects of the step height, the visual feedback, or the interaction on the landing duration, the kinematic variables and the GRFs at each landing event point were analyzed. Results: With eyes blindfolded, the knee flexion and ankle dorsiflexion on landing side significantly decreased before and after the touch down. However, there was no significant effect of landing height on the anticipatory kinematics on the landing side. After the touch down, the landings from the higher step increased the knee flexion and ankle dorsiflexion on both landing and supporting sides. From the higher steps, the vertical GRF, anterior GRF, and lateral GRF increased. No interaction between step height and visual feedback was significant. Conclusion: Step height and visual feedback affected the landing limb kinematics independently. Visual feedback affected on the landing side while step height altered the supporting side prior to the touch down. After the touch down, the step height had greater influence on the lower limb kinematics and the GRFs than the visual feedback. Findings of this study can contribute to understanding of the injury mechanisms and preventing the lateral ankle sprain.

MC를 이용한 측면 연삭에 관한 연구 (A Study on the Side-cut Grinding using the Mactining Center)

  • A problem in the grinding process using the machining center(MC) with a small diametric wheel is the machining error due to due to decrease of quill diameter. In this paper, a side-cut grinding is performed with a vitrified bonded CBN wheel by the MC, and the relation between grinding force and machining error for grinding conditions is investigated experimentally. It is show that the normal force has a significant effect on the machining error.

X-선관 각도 변경에 따른 Anode Heel Effect (The Anode Heel Effect caused by changing the Angle of X-Ray Tube)

  • 본 연구는 X-선관 각도 변경에 따른 경사효과(Anode Heel Effect)의 변화를 알아보고자 실시하였다. 실험조건은 70 kV, 30 mAs, 초점-검출기간의 거리 100 cm, 조사야는 $35{\times}43cm^2$, 측정점은 조사야의 정중앙점에서 좌우 3.5 cm 간격으로 나누어 양극쪽으로 A1, A2, A3, A4, A5, A6 점을 설정하고 음극쪽으로 C1, C2, C3, C4, C5, C6 점으로 설정하였다. X-선관을 수직으로 하여 측정점인 A6에서 C6까지 각각 입사표면선량을 측정하였다. 다음에 X-선관을 양극쪽으로 15도 30도로 변경 하면서 각각 측정하고 같은 방법으로 음극쪽으로 15도 30도로 변화시켜 측정하였다. 결과로 X-선관이 수직인 경우 A5보다 C5점이 3배정도 입사표면선량이 높게 나타나 수직 촬영 시 방사선감수성이 높은 장기가 위치해 있는 쪽으로 양극을 위치시키면 피폭을 줄일 수 있었다. X-선관의 각도를 주고 촬영할 경우에는 음극측으로 각도를 주는 것이 양극과 음극측의 입사표면선량 차이를 줄일 수 있으며 상, 하 두께 차이가 있는 부위를 촬영할 경우에는 음극측이 두꺼운 부위를 향하게 각도를 주는 것이 입사표면선량의 차이를 줄여 좀 더 균일한 영상을 만들 수 있었다.
