요천추부 신경근병증의 치료를 위한 경추간공 경막외 스테로이드 주입과 요추 및 미추 경막외 스테로이드 주입의 비교 (Comparison of Transforaminal Epidural Steroid Injection and Lumbar/Caudal Epidural Steroid Injection for the Treatment of Lumbosacral Radiculopathy)

  • Background: An epidural steroid injection (ESI) is usually used for the treatment of low back pain with radiculopathy. An ESI can be performed by two procedures: I) a lumbar or caudal epidural steroid injection and II) a transforaminal epidural steroid injection. Methods: Ninety-three patients, who had undergone transforaminal epidural steroid injection (Group II), and either a lumbar or caudal epidural steroid injection (Group I), were retrospectively studied. The authors assessed the pain, walking, standing improvement and side effects after each procedure, which were evaluated as being very good, good, fair or poor. Data were collected from the patients medical records and analyzed using the chi-squared test. P < 0.05 was considered significant. Results: There were no statistically significant differences in the pain, walking, standing improvement and side effects between the two groups. However, there was a statistically significant difference in the pain improvement following transforaminal epidural steroid injection in those not effectively responding to an initial lumbar or caudal epidural block in Group II. Conclusions: A transforaminal epidural steroid injection is a useful alternative to a lumbar or caudal epidural steroid injection for low back pain with radiculopathy.

Effects of Piston Shapes and Intake Flow on the Behavior of Fuel Mixtures in a GDI Engine

  • The purpose of this study is to investigate the stratification of fuel vapor with different in-cylinder flow, piston cavity and injection timings in an optically accessible engine. Three different piston shapes that are F(Flat), B(Bowl) and R(Re-entrance) types were used. The images of liquid and vapor fuel were captured under the motoring condition using Laser Induced Exciplex Fluorescence technique. As a result, at early injection timing of 270 BTDC, liquid fuel was evaporated faster by tumble flow than swirl flow, where most of fuel vapor were transported by tumble flow to the lower region and both sides of cylinder for the F-type piston. At late injection timing of 90 BTDC, tumble flow appears to be moving the fuel vapor to the intake side of the cylinder, while swirl flow convects the fuel vapor to the exhaust side. The concentration of mixture in the center region was highest in the B-type piston, while fuel vapor was transported to the exhaust side by swirl flow in F and R-type pistons. At the injection timing of 60 BTDC, the R-type piston was better for stratification due to a relatively smaller bowl diameter than the others.

Clinical Study on Safety of Cantharidin Sodium and Shenmai Injection Combined with Chemotherapy in Treating Patients with Breast Cancer Postoperatively

  • Objectives: To assess side effects on Cantharidin sodium and Shenmai injection combined with chemotherapy in treating patients with breast cancer postoperatively. Method: Patients with breast cancer receiving postoperative chemotherapy were retrospectively collected, and divided into four groups: group A with cantharidin sodium injection combined with chemotherapy; group B with Shenmai injection combined with chemotherapy; group C with both cantharidin sodium and Shenmai injection combined with chemotherapy; while group D (control group) received chemotherapy alone. All patients were administered docetaxel at a dose of $75mg/m^2$ on day 1, epirubicin hydrochloride at a dose of $60mg/m^2$ on day 1, and cyclophosphamide at a dose of $500mg/m^2$ on day 1 for 3 cycles (repeated at 21 day intervals). After ${\geq}$ three courses of treatment, quality of life and side effects were evaluated. Results: There were a total of 78 patients in this study, and the incidence of leukopenia and gastrointestinal reactions in groups A and B were lower than those in the control group and lowest in group C (p<0.05). Conclusions: Thus cantharidin sodium and Shenmai injection combined with chemotherapy reduce side effects and deserve to be further investigated in randomized clinical control trials.

피동 주입 시험 장치의 척도 해석 및 설계 방법론 연구 (Study on Scaling Analysis and Design Methodology of Passive Injection Test Facility)

  • A design methodology of the modeled test facility to conserve an injection performance of a passive safety injection system is proposed. This safety injection system is composed of a core makeup tank and a safety injection tank. Individual tanks are connected with pressure balance line on the top side and injection line on the bottom side. It is important to conserve the scaled initial injection flow rate and total injection time since this system can be operated by small gravity head without any active pumps. Differential pressure distribution of the injection line induced by the gravity head is determined by the vertical length and elevation of each tank. However, the total injection time is adjustable by the flow resistance coefficient of the injection line. The scaling methodology for the tank and flow resistance coefficient is suggested. A key point of this test facility design is a scaling analysis for the flow resistance coefficient. The scaling analysis proposed on this paper is based on the volume scaling law with the same vertical length to the prototype and can be extended to a model with a reduced vertical length. A set of passive injection test were performed for the tanks with the same volume and the different length. The test results on the initial flow rate and total injection time showed the almost same injection characteristics and they were in good agreement with the design values.

가스사출성형을 이용한 자동차 내장부품 개발 사례에 관한 연구 (A Case Study on Development of Automotive Interior Parts using Gas Assisted Injection Molding Process)

  • Gas assisted injection molding (GAIM) is an innovative low-pressure injection molding technique that can provide numerous benefits such as reduced part warpage, excellent surface quality without sink marks, low injection pressure and greater design flexibility. However, adoptions of GAIM may cause unexpected defects since it requires many subtle design factors such as resin shot size, delay time and gas injection pressure, which wouldn't be considered in conventional injection molding process. Therefore, experiences applying GAIM should be collected and examined in order to establish design rules of the new technique. The purpose of this paper is to summarize developing cases of three automotive interior parts such as instrument panel, map pocket folding and center facia side panel so that possibilities and limitations of GAIM were examined. As a result, it is necessary to consider characteristics of GAIM at the initial stage of part design in order to obtain various advantages of the GAIM process without occurring severe defects, which would increase time and cost required to the part development.

  • SITVC 시스템의 이차분사 노즐 분사 조건 변화에 따른 시스템 성능 변화를 수치적으로 연구하였다. 해석에 사용된 형상은 3차원 종형 수축-팽창 노즐이고 측면에 8개의 이차분사 노즐을 가진다. 노즐 내부 유동은 전압이 70bar이며 300K의 cold flow로 가정하였다. 이차 유동의 유량 변화와 노즐 작동 조건 변화를 고려하였다. 상용코드인 Ansys Fluent v.13을 통해 해석하였고, 난류모델은 Spalart-Allmaras model(1- equation)를 사용하였다. 충격파의 수치적 진동을 막고 충격파의 불연속성을 잘 해석하기 위해 AUSM+ scheme을 사용하였다. Axial thrust, side force, system specific impulse ratio 와 같은 성능 변수를 사용해 시스템 성능을 평가하였다.

Gravity-Injection Core Cooling After a Loss-of-SDC Event n the YGN Units 3 & 4

  • In order to evaluate the gravity-injection capability to maintain core cooling after a loss-of-shutdown-cooling event during shutdown operation, the plant conditions of the Yong Gwang Units 3&4 were reviewed. The six cases of possible gravity-injection paths from the refueling water tank (RWT) were identified and the thermal-hydraulic analyses were performed using the RELAP5/MOD3.2 code. The core cooling capability was significantly dependent on the gravity-injection path, the RCS opening, and the injection rate. In the cases with the pressurizer manway opening higher than the RWT water level, the coolant was held up in the pressurizer and the system pressure continued increasing after gravity-injection. The gravity injection eventually stopped due to the high system pressure and the core was uncovered. In the cases with the injection path and opening on the same leg side, the core cooling was dependent on whether the water injected from the RWT passed the core region or not. However, in the cases with the injection path and opening on the different leg side, the system was well depressurized after gravity-injection and the core boiling was successfully prevented for a long-term transient. In addition, from the sensitivity study on the gravity-injection flow rate, it was found that about 54 kg/s of injection rate was required to maintain the core cooling and the core cooling could be provided for about 10.6 hours after event with that injection rate from the RWT. Those analysis results would provide useful information to operators coping with the event.

  • Objectives : The aim of this study was to find out the therapeutic effect of bee venom acupuncture in the treatment of patient with trigger finger accompany side effects by steroid injection. Methods : As the effect of bee venom acupuncture about various articulation diseases observed, we tried to treat a case that was diagnosed trigger finger with side effects by steroid injection by using bee venom acupuncture. Result : After three times bee venom acupuncture treatments, 'VAS' score and triggering grade decreased quite noticeably, and other symptoms as have improved as well. Conclusions : Bee venom acupuncture have shown fairly effective to trigger finger accompany side effects by steroid injection.

Efficacy of submucosal injection of hyaluronidase after mandibular third molar surgery: a randomized controlled trial

  • Objectives: This study aimed to investigate the efficacy of postoperative submucosal injection of hyaluronidase (HUD) for reducing sequelae and quality of life (QOL) after mandibular third molar (M3M) surgery. Materials and Methods: Participants with bilateral impacted M3M underwent surgical extraction with a split-mouth randomized controlled study design. M3M were removed by the same surgeon in 2 sessions, one a control and the other experimental. Submucosal injection of HUD was performed in the experimental session and submucosal injection of saline in the control session. Mouth opening, facial swelling, and pain intensity were measured before surgery, and then 2 and 7 days after surgery. The QOL of participants following surgery was evaluated by means of a patient-centered outcome questionnaire (PCOQ). Results: A total of 36 patients was included in the final data analysis. There was a significant reduction in the maximal mouth opening and postoperative pain in the experimental side at the 2 and 7 days after surgery (P<0.05), and a remarkable difference in facial swelling was reported on the experimental side 7 days after surgery (P<0.05). The PCOQ demonstrated that participants reported less pain and swelling on the experimental side. Conclusion: The present study provides clinical evidence that submucosal administration of HUD immediately after M3M surgery reduced postoperative discomfort and improved patients' QOL.

Management of Disc Displacement with Retrodiscitis after Macrotrauma: A Case Report

  • We present the case of a 60-year-old male with post-macrotrauma disc displacement and retrodiscitis, in which temporomandibular joint (TMJ) injection and manual therapy were used to alleviate his symptoms. He visited our clinic with complaints of pain and swelling in his right facial area and malocclusion of his right side teeth after being hit on the right side of his face five days earlier. During clinical and radiological examinations, the inflammatory state of the joint and disc displacement on the right side, which led to malocclusion, were noted. At the initial visit, we performed TMJ intracapsular injection and prescribed medications to control pain and inflammation. Simultaneously, manual manipulation was performed to relocate the disc. The same treatments were employed two days later. However, 10 days after the first visit, his symptoms did not mitigate substantially. We also performed magnetic resonance imaging (MRI), prescribed nortriptyline, and created a stabilization splint. MRI images depicted inflammatory disc displacement and joint effusion in the right TMJ. Based on the accurate diagnosis, we kept administering a stabilization splint, intra-articular injection, and medication. His signs and symptoms were alleviated 20 days after the initial visit and did not reoccur for the next 40 days.
