한방병원 종사자의 근골격계 증상과 유해요인평가 (A Survey for Ergonomic Work Risk Factors of Musculoskeletal Disorders in a Oriental Medicine Hospital)

  • Objectives: The purpose of this study was to evaluate work-related risk factors of musculoskeletal disorders (MSDs) in various departments and tasks at an oriental medicine hospital. Methods: 269 hospital workers were selected for the study. The questionnaire survey included general characteristics, work-related characteristics and MSDs. Cross-tabulation analysis, multiple logistic regression, adjusted for age, work time, job demand and workload, were used to evaluate the effects of work-related factors on MSDs. Results: There were differences in MSDs depending on gender, age, occupation, and work-related factors. Prevalence rates of MSDs were 23.79% shoulder, 14.13% back, 10.41% neck. Risk factors for MSDs was packaging of drugs, transfer, and exercise therapy of physical therapist. Shoulder pain was significantly influenced by using power tools, over-reaching, and job demand while back pain was significantly influenced by weight-lifting operations and keyboard typing. Conclusions: The results of this study suggest that consideration of MSDs characteristics of workers in oriental medicine hospital is necessary to change unsuitable work environments and educate on preventing MSDs.

거동장애를 가진 전동휠체어 사용자를 위한 근전도 기반의 휴먼-컴퓨터 인터페이스 (A Novel EMG-based Human-Computer Interface for Electric-Powered Wheelchair Users with Motor Disabilities)

  • Electromyogram (EMG) signal generated by voluntary contraction of muscles is often used in rehabilitation devices because of its distinct output characteristics compared to other bio-signals. This paper proposes a novel EMG-based human-computer interface for electric-powered wheelchair users with motor disabilities by C4 or C5 spine cord injury. User's commands to control the electric-powered wheelchair are represented by shoulder elevation motions, which are recognized by comparing EMG signals acquired from the levator scapulae muscles with a preset double threshold value. The interface commands for controlling the electric-powered wheelchair consist of combinations of left-, right- and both-shoulders elevation motions. To achieve a real-time interface, we implement an EMG processing hardware composed of analog amplifiers, filters, a mean absolute value circuit and a high-speed microprocessor. The experimental results using an implemented real-time hardware and an electric-powered wheelchair showed that the EMG-based human-computer interface is feasible for the users with severe motor disabilities.

VDT 증후군 환자에 물리치료와 인간공학적 개입: 사례 연구 (Physical Therapy and Ergonomic Interventions in Patients with VDT Syndrome: 4 Cases Study)

  • Purpose: Among computer users, the awkward posture and workstation setups of workers contribute to work-related upper limb musculoskeletal disorders. The purpose of this study was to evaluate the efficacy of a traditional physical therapy and ergonomic intervention by physical therapists in 4 computer users. Methods: After checking Visual Analogue Scores (VAS), four subjects who were treated by physical therapy for neck and shoulder problems related to VDT syndrome were enrolled in the study. All subjects spent at least 40 hours per week at a computer workstation. All subjects had pain and ergonomic states evaluated using methods such as VAS, Neck Disability Index (NDI), Workstyle short form, Rapid Upper Limb Assessment (RULA), and OSHA VDT checklist (Occupational Safety and Health Administration video display terminal) before a physical therapy + ergonomic intervention. Participants were re-evaluated 1 month later. Results: Participants showed more improvement of their neck pain after being treated with a physical therapy plus ergonomic intervention than when treatment consisted only of physical therapy. Improvements in RULA, Workstyle short form, and OSHA VDT checklist also were achieved. Conclusion: This case study suggests the importance of examining the work habits and work-related postures of subjects who complain of neck and shoulder pain that is exacerbated by computer use. Personalized ergonomic interventions and physical therapy can lead to improvement of patients with VDT syndrome.

PNF 패턴에 기초한 탄력밴드 훈련이 척수손상 환자의 상지 기능에 미치는 영향 (The effect of elastic band based of PNF pattern on the U/E function in the spinal cord injury patient)

  • The purpose of this study was to investigate the effect of elastic band based of PNF pattern on the U/E function in the spinal cord injury patient. 11 subjects with spinal cord injury participated in this study. They took elastic band excercise 5 times per week for 8 weeks. One time excercise spent 35minutes. The therapeutic effect was evaluated by Cybex 6000, BTE Work Simulator, how many seconds they needed to walk 100 meters. 11 cases were examined before, after 8 week, elastic band excercise. The results of this study are as follows; 1. Isokinetic power by Cybex 6000 : The elbow flexion, shoulder flexion and extension were significant difference between test-retest(p<.05). but elbow extension were not significant difference between test-retest. 2. BTE Work Simulator : Wheelchair Propulsion and Steering Torque were significant difference between test-retest(p<.05). 3. Wheelchair Propulsion velocity : There were significant difference between test-retest(p<.05). The findings suggest that SCI patients can improve their Isokinetic power on shoulder and elbow joint, wheelchair propulsion and Steering Torque by BTE Work Simulator, Wheelchair Propulsion velocity through elastic band based of PNF pattern.

광범위 회전근개 파열의 수술적 치료 후 발생한 단기 합병증 (Early Complications after Repair of Massive Rotator Cuff Tear)

  • Purpose: To investigate early complications after repair of massive rotator cuff tears and to find out factors that compromise the results. Materials and Methods: Fourteen patients who had two or more cuff tendons involved were included. All patients were operated by open acromioplasty and rotator cuff repair. At 3 months after operation, we investigated whether there were any early complications or not. We used ASES scoring system for preoperative and follow up evaluation. In addition, various preoperative factors, such as duration of symptom, degree of tendon retraction, degree of fatty degeneration, and acromio-humeral distance, were compared between the complicated patients and non-complicated patients. Results: At 3 months after operation, the ASES score and pain were improved in any degree in all patients. But 5 patients complained persisting pain, and three of them showed major complications such as re-rupture of rotator cuff or deltoid rupture. But no preoperative factors in complicated patients were significantly different from those in non-complicated patients. Conclusion: None of the preoperative factors were related to the complications. There was a tendency of overestimation of fatty degeneration in MRI. Some factors in surgical technique and rehabilitation were highly suspected to be related to the complications.

  • Epicondylitis, as a tendinopathy characterized by fibroblast and microvascular hyperplasia, is a common musculoskeletal problem especially related with repetitive hand and wrist motion. It has a prevalence of between 0.2% and 5% in general population depending on the amount of exposure to manual labor jobs. Although it is known that the pathological lesions lie in the flexor or extensor common tendons, there could be collateral ligament lesions and/or reactive synovitis accompanied, which may make a case unresponsive to the treatment aimed only at the tendinopathy. Epicondylitis is easy to diagnose with typical pain, tenderness, and positive provocation tests. However, many conditions can mimic epicondylitis that further imaging or electrodiagnostic studies should be undertaken to exclude other possible problems. Ultrasonography provides information about the existence and extent of tendinopathy with relatively high specificity. Magnetic resonance imaging is often required to rule out other problems and confirm the diagnosis of the cases intractable to long term treatment. Many options of treatment are available for epicondylitis while numerous conflicting evidences have been noted, debating one treatment method is better than the others. Since it was reported that over 80% of epicondylitis improved within a year no matter what was done as treatment, it is a challenge to make accurate diagnosis and combine effective therapeutic regimens for the 20% of intractable cases.

LIGASTIC 인공인대를 이용한 급성 견봉 쇄골 탈구의 수술적 치료 (Surgical Treatment of the Acute Acromioclavicular Joint Dislocation with a LIGASTIC Artificial Ligament)

  • Purpose: To find out the early results after surgical treatment of the acute acromioclavicular dislocation with LIGASTIC artificial ligament. Material and Method: 6 patients who were diagnosed as acute acomioclavicular joint dislocation and treated with LIGASTIC artifical ligament through March 2005 to July 2005. The radiologic and clinical results using Imatani evaluation system were analyzed. Results: By clinical evaluation, 4 cases(67%) were excellent and 2 cases(33%) were good. By radiologic evaluation, 3 cases(50%) were excellent and 3 cases(50%) were good. All cases showed satisfactory results. Till the final follow up, there were no complication. Conclusion: Surgical treatment of the acute acromioclavicular dislocation with LIGASTIC articifial ligament is simple, but provides enough stability for early postoperative rehabilitation, decreases arthritis of acomioclavicular joint and there is no burden of removal of the fixture, so it is thought as a very effective surgery.

Effects of Inspiration and Expiration Exercise Combined with Upper Extremity Proprioceptive Neuromuscular Facilitation on Forced Volume Vital and Peak Expiratory Flow

  • Purpose: The purpose of this study was to examine the effects of inhalation and exhalation exercise combined with upper extremity proprioceptive neuromuscular facilitation pattern on two spirometry values: forced volume vital (FVC) and peak expiratory flow (PEF). Methods: Thirty-two healthy adults were divided into two groups: 1) a combined group, which performed upper extremity D2 flexion pattern (shoulder flexed/abducted/external rotated, forearm supinated, wrist radial deviated, and finger extended) during exhalation and D2 extension pattern (shoulder extended/adducted/internal rotated, forearm pronated, wrist ulnar deviated, and finger flexed) during inhalation; and 2) reverse combined group, which performed the D2 flexion pattern during inhalation and the D2 extension pattern during exhalation. The inverse application of upper extremity movements during inhalation and exhalation induced selective resistance or assistance on respiration. FVC and PEF were measured at two time points, before and after four weeks. Results: In both groups, the pre-post intervention comparison showed significant increases in FVC and PEF (p < .05). In the between-groups comparison, the reverse combined group showed a significantly higher PEF than the combined group at four weeks post intervention (p < 0.05). Conclusion: The combined respiration exercise with reverse PNF upper extremity patterns using selective resistance showed an effective increase in PEF in healthy adults. Clinicians and researchers might consider using selective resistance as a widely applicable and cost-effective option for respiratory rehabilitation planning.

Biomechanical Test for Repair Technique of Full-thickness Rotator Cuff Tear

  • The arthroscopic rotator cuff repair is now considered a mainstream technique with highly satisfactory clinical results. However, concerns remain regarding healing failures for large and massive tears and high revision rate. In recent decades, various repair strategies and construct configurations have been developed for rotator cuff repair with the understanding that many factors contribute to the structural integrity of the repaired construct. The focus of biomechanical test in arthroscopic repair has been on increasing fixation strength and restoration of the footprint contact characteristics to provide early rehabilitation and improve healing. These include repaired rotator cuff tendon-footprint motion, increased tendon-footprint contact area and pressure, and tissue quality of tendon and bone. Recent studies have shown that a transosseous tunnel technique provides improved contact area and pressure between rotator cuff tendon and insertion footprint, and the technique of using double rows of suture anchors to recreate the native footprint attachment has been recently described. The transosseous equivalent suture bridge technique has the highest contact pressure and fixation force. In this review, the biomechanical tests about repair techniques of rotator cuff tear will be reviewed and discussed.

Focal Bone Marrow Lesions: A Complication of Ultrasound Diathermy

  • Ultrasound diathermy is widely used for the treatment of musculoskeletal disorders and other soft tissue injuries. Its use as a therapeutic modality is believed to be safe, with very few reported complications. Here, we report two patients who developed focal bone marrow abnormalities after receiving ultrasound diathermy. Both patients' magnetic resonance (MR) evaluations revealed linear subchondral bone lesions of the superolateral humeral head similar to those in osteonecrosis. The patients' symptoms subsequently improved, and available follow-up MR evaluation revealed near complete resolution of bone lesions. These findings suggest that ultrasound diathermy, and its interaction with bone tissue through thermal mechanisms, can cause focal bone marrow abnormalities. Furthermore, the bone marrow abnormalities seem to be transient, resolving upon cessation of ultrasound diathermy, therefore osteonecrosis should be differentiated from this temporal lesion.
