Treatment of Proximal Humeral Fracture Using Polarus Nail and Philos Plate

  • Background: To compare the treatment of the proximal humerus fracture using a Polarus nail or Philos plate, we aimed to analyze the functional recovery and the factors affecting the selection between the two types of surgery. Methods: The study included 107 patients with proximal humerus fracture who underwent surgery at our institution. Of these patients, 67 underwent surgery with Polarus nails (G1) and 40 with Philos plates (G2). In G1, the cases of two- and three-part fractures were 60 and 7 cases, in G2, the cases of two-, three-, and four-part fractures were 28, 10, and 2 cases, respectively. The average age was 61 years old, and the average follow-up period was 32.5 months. We compared radiological results, the functional recovery retrospectively. Results: The radiological union time was 6.8 weeks and 8.7 weeks on average in G1 and G2 (p < 0.05). At the one-year follow-up period, these were visual analogue scale (VAS) 1.355, forward flexion (FF) 130.968, external rotation (ER) 50.161, internal rotation (IR) L2 in G1, and VAS 0.781, FF 135.806 ER 51.25, IR L1 in G2, respectively, showing no significant differences between the two groups (p > 0.05). Similar observations were made at the final follow-up. In terms of functional recovery, no significant differences were seen at the one-year or at the final follow-up period (p > 0.05). Conclusions: For the surgical treatment of proximal humeral fracture, the selection of the type of surgery is affected by the fracture pattern. However, both methods give satisfactory outcomes and do not show significant differences in the functional outcome after the surgery.

Operative Treatment with Intramedullary Fibular Strut Allograft for Osteoporotic Proximal Humerus Fracture

  • Background: The purpose of this study was to investigate the clinical and radiological outcomes of locking plate fixation with fibular strut allograft to manage unstable osteoporotic proximal humerus fractures. Methods: We retrospectively reviewed 15 patients who underwent open reduction and locking plate fixation with fibular strut allograft for osteoporotic proximal humerus fracture between July 2011 and June 2015. For functional evaluation, we evaluated visual analogue scale (VAS) pain score, American Shoulder and Elbow Surgeons (ASES) score, University of California Los Angeles (UCLA) shoulder score, and active range of motion. For radiological evaluation, shoulder true anteroposterior (AP) and AP in $20^{\circ}$ external rotation, as well as the axillary view were taken at two weeks, six weeks, three months, six months, and one year. And the neck-shaft angle was measured on the AP view in $20^{\circ}$ external rotation view. Results: At the one-year follow-up, mean VAS pain score and all shoulder scores, including ASES score and UCLA shoulder score, exhibited satisfactory clinical outcomes. All patients obtained bone union between three and six months post-procedure. Moreover, the mean immediate postoperative neck-shaft angle was $138^{\circ}{\pm}4^{\circ}$, and at one-year follow-up, the neck shaft angle was $137^{\circ}{\pm}5^{\circ}$. There was no significant difference between the preoperative and postoperative values (p=0.105). Conclusions: For the unstable two-part and three-part osteoporotic proximal humerus fractures with medial calcar comminution, the use of fibular strut allograft with locking plate fixation was effective in maintaining the initial status of reduction and exhibiting the satisfactory functional and radiological outcomes.

A Study on the Corset Type Long-Line Brassieres Pattern Development for Adult Women : focusing on women in their 20s (성인 여성용 코르셋형 롱라인브래지어 패턴개발 - 20대 여성을 대상으로 -)

  • The study aims to design the corset type long-line bra patterns that compensate part-somatotype of women in their 20s. The corset type long-line research bra patterns consist of 4 sections: cup, front bodice, back bodice, and shoulder strap. The full cup is made up of 2 lower part pieces and 1 upper part piece, and the shoulder strap is 16 millimeters wide. The method to design the patterns is shown in figure 4 and figure 5. The result of wearing test showed that, while the wearing testers recognized some significant differences in 19 out of 26 criteria, the examiners did in 17 out of 23. The criteria more than 1 in an average value between research bras and commercial bras were 'appropriateness of breadth of shoulder strap', 'appropriateness of pressure of armhole circumference'. 'appropriateness of pressure of waist', 'suitability of side line and body curve', 'appropriateness of positional pressure of chest circumference', 'appropriateness of positional pressure of upper part line', 'appropriateness of positional pressure of bottom part line', and 'satisfaction of whole silhouette of side and back'. The research and commercial bras were evaluated 4.23 and 3.44 respectively by the testers, and 4.25 and 3.40 by the examiners. The former ones were scored higher by both parties. They were also appreciated higher in general silhouettes of fitting and appearance.

A Study on Shoulder Angle (견 각도에 관한 연구)

  • In order to design a good ready-made cloth, it is important to consider not only body size but also morphological factor. However, most dissatisfaction of customer is focusing on the size of ready-made cloth which is graded with elements of body size. Various researches on body type have been doing but the study on part of body type is insufficient compared to the study on whole body type. The objective of this paper is to provide relationship of shoulder angle between value in pattern and real measured value after analysis and comparison with these two values.

Open Intramedullary Nail with Tension Band Sutures on Proximal Humeral Fracture (상완골 근위부 골절에 시행한 긴장 대 봉합을 동반한 관혈적 골수강내 고정술)

  • Purpose: to determine the results after open intramedullary nailing and tension band suture technique in proximal humerus fracture for improving the stability and decreasing the complications. Materials and Method: Authors reviewed 27 patients treated by open intramedullary nailing and tension band suture technique. Mean follow-up period was 39 months (24-59months). Surgical neck fracture were 6 cases, surgical neck fracture with shaft fracture were 3 cases, three part fracture with greater tuberosity fracture were 17 cases, four part fracture was 1 case and fracture and dislocation were 2 cases Results: We got the bony union in 26 cases. Average pain scale was 1 point (0-6), Neer score was 86 point(45-99) and ASES score was 85 point(40-100). We separate all cases in two groups based on age (65 years), L-spine t-score (-2.5) and Neer classification (2 and 3 part). There is no significance in pain scale, Neer score and ASES score between each group. Conclusion: As a method of surgical treatment on severe proximal humeral fractures, we recommend intramedullary nailing and tension band suture technique and it may have particular advantages in early exercise and satisfactory functional outcome.

Subjective Symptoms of Musculoskeletal Disorders in Women Workers (경인지역 여성근로자의 근골격계장애 자각증상)

  • Purpose: To investigate subjective symptoms of musculoskeletal disorders in women workers Method: From February to April, 2006, 292 women workers working in 16 companies were surveyed using KOSHA CODE (H-30-2003). Result: Of the workers, 84.0% had at least one musculoskeletal disorder. The mean score of the severity of musculoskeletal disorder symptoms was 7.41 out of 25. As for the prevalence rate by body part, shoulder was the most common and severe body part of musculoskeletal disorders, and significant differences were shown in neck, shoulder, and lower limb according to the type of working. The severity of married workers was significantly higher than that of unmarried ones. Significant difference was shown in the severity of disorders by the type of working and in body parts according to the type of working. Conclusion: The self-reported symptoms of musculoskeletal disorders were very frequent in women workers, but the severity of the symptoms was relatively low, suggesting the early symptoms of disorders. Specifically, given the highly frequent self-reported symptoms in the shoulder part found in married women workers, intensive prevention is recommended. Furthermore, an ergonomic aspect should be concerned to consider physical characteristics of women workers.

Unrecognized bony Bankart lesion accompanying a dislocated four-part proximal humerus fracture before surgery: a case report

  • Proximal humerus fractures are the third most common fractures, totaling 4% to 5% of all fractures. Here, we present the case of a 39-year-old man with a dislocated four-part fracture of the proximal humerus with a huge bony Bankart lesion. Preoperatively, the bony Bankart lesion of the glenoid was not visualized on computed tomography scans or magnetic resonance imaging because the fracture of the proximal humerus was comminuted, displaced, and complex. It was planned for only the humerus fracture to be treated by open reduction and internal fixation using a locking plate. However, a fractured fragment remained under the scapula after reduction of the dislocated humeral head. This was mistaken for a dislocated bone fragment of the greater tuberosity and repositioning was attempted. After failure, visual confirmation showed that the bone fragment was a piece of the glenoid. After reduction and fixation of this glenoid part with suture anchors, we acquired a well-reduced fluoroscopic image. Given this case of complex proximal humerus fracture, a glenoid fracture such as a bony Bankart lesion should be considered preoperatively and intraoperatively in such cases.

The Effect of Electro-acupuncture Stimulation of Tiaoko(S38)-Chengsan(B57) on Shoulder ROM with Frozen Shoulder Patient (조구(S38)-승산(B57)혈의 전침 자극이 동결견 환자의 관절가동범위에 미치는 영향)

  • Objectives : The purpose of this study was to identify the effect of electro-acupuncture stimulation regarding restoration of range of motion on patients with frozen shoulder. Methods : Experimental group measured with shoulder flexion and abduction angle at pre and post electro-acupuncture stimulation. Results : There were statistically significant difference regarding change of flexion angle of electro-acupuncture stimulation(p<0.01) and the testing value of all levels had a significant difference with the value of final post-test(p<0.01). There were statistically significant difference regarding change of abduction angle of electro-acupuncture stimulation(p<0.01). Conclusions : Electro-acupuncture stimulation of distal part increased range of motion of frozen shoulder. Therefore, application of electro-acupuncture should be developed as a therapeutic method in treatment of frozen shoulder. Also, it can contribute as one of good therapeutic methods offered to patients through approach of oriental rehabilitation medicine.

Effects of Deficit Irrigation on the Reduction of Green Shoulder Fruits in Fresh Tomato (관수량 조절이 토마토 과실의 착색불량과 발생 억제에 미치는 영향)

  • Effects of deficit irrigation by different soil moisture-based water potential on the reduction of green shoulder fruits in fresh tomato were investigated. Deficit irrigation with -30 kPa treatment significantly reduced green shoulder fruit by 53.7% compared to the -15 kPa treatment. The total concentrations of potassium in levels of whole fruit was not different between normal red fruit (NRF) and green shoulder fruit (GSF). However, the concentrations of potassium in distal part of GSF lower than that of a basal part of GSF Total soluble sugar contents in NRF and GSF were increased during the maturation periods. Contents of starch in NRF and distal part of GSF were decreased during the maturation periods. However, contents of starch in basal part of GSF was not change during the maturation periods. Activity of sucrose phosphate synthase was higher in NRF than that of GSF. Activity of acid invertase in NRF and GSF decreased rapidly during the maturation periods. Activity of acid invertase was significantly different in distal and basal part of GSF. Total activities of peroxidase and alcohol dehydrogenase in NRF were higher than that of GSF.

Analysis of breast Replicas of Elementary School Girls (초등학교 여학생의 유방 레플리카 분석)

  • This study is to develop the well-fit brassieres by observing the changes in the breast sizes and shapes, the surface area of the breast through the arm movements of 3 types (0$^{\circ}$ , 90$^{\circ}$ and 180$^{\circ}$) in vortical motion. The 11 elementary girls who are aged 12 participated in this experiment. To obtain the measures regarding the surface area of the breast, replicas are made at each motion. The results of this study are as fellows: 1. The changes in the breast sizes and shapes at each motion By increasing the motions of the arm movement, the following measure items are inclined to decrease: Shoulder length, Upper blast girth, Bust girth, Under bust girth, Upper bust depth, Bust depth, Under bust depth, Nipple to nipple breadth, Horizontal distance of bust, Horizontal length of the cup, Bust height, Cup size. By increasing the motions of the arm movement, the fellowing measure items are inclined to increase: Center point of shoulder~B.p., Shoulder point~B.p., Center point of shoulder~armpit, Upper Bust breadth, Bust breadth, Under bust breadth, Vertical distance of Bust. 2. The changes in the surface area of breast at each motion By making the replica to observe changes in the surface area, which are sectioned to 4 parts(area 1 to area 4), and volume of breast at each motion, the results are as follows: At 0$^{\circ}$and 180$^{\circ}$, the sizes of each part are ordered as the following: area2>area1>area4> area3. At 90$^{\circ}$, the sizes of each part are ordered as the following: area2>area1>area3>area4. Through these orders, it is found that the upper and inside part of the breast huts the inclination to increase so along as the motions of the arm movement increase. Also, the total surface area increases so long as the motions of the arm movement increase. As a reset of the F-test on the changes in the each surface areas, the surface area by arm movements, the significant differences among the each surface areas are found.
