PACE 프로그램이 퇴행성관절염 노인의 자기효능감과 통증 및 관절기능에 미치는 효과 - 미국이민 한국노인을 중심으로 - (The Effects of PACE Program on Self-efficacy, Pain and Joint Function in Korean Immigrant Elderly with Osteoarthritis.)

  • The PACE (People nth Arthritis Can Exercise) is an exercise program developed by the Arthritis Foundation to improve muscle strength and joint flexibility for patients with arthritis. The purpose of this study was to explore the effects of PACE program on self-efficacy, pain, and joint function in the Korean immigrant elderly. The PACE program was held twice a week for 6 weeks for Korean immigrant elderly who had osteoarthritis. Twenty four subjects completed the program, who were recruited in two places : 10 elderly in a senior residential apartment, and 14 elderly in a senior center supported by Congregated Meal Program for Korean Elderly. Self-efficacy(Sherer et al., 1982), pain severity(by using Visual Analogue Scale), and number of painful joints were measured before and after the PACE program. To examine the joint flexibility and strengthening, the followings were measured : the extent of the upward arm reach in both sides(flexibility of shoulder), the ability to touch fingertips of the both hands in back pat and rub(flexibility of arm), the degree of range of motion(ROM) of both ankles in their dorsiflexion(flexibility of ankle) and plantarflexion with standing with toe(strengthening of ankle), and the degree of knee extension. Wilcoxon signed rank test was used for data analysis and the significance of the differences in the variables was examined to compare the data obtained before and after the PACE program. After the PACE, followings were found : 1. Self-efficacy was significantly increased. 2. Pain severity and number of painful Joints was significantly decreased. 3. The flexibility of both shoulders and arms were significantly improved, but the flexibility of knee was not changed. 4. The flexibility and strengthening of both ankle was significantly improved. In conclusion, PACE was clearly proved to be an effective exercise program to promote self-efficacy, to reduce pain, and to enhance joint function in the elderly with osteoarthritis. It is suggested that the PACE program should be recommended as one of the useful and appropriate nursing interventions for elderly with osteoarthritis.

The humeral suspension technique: a novel operation for deltoid paralysis

  • Isolated deltoid paralysis is a rare pathology that can occur after axillary nerve injury due to shoulder trauma or infection. This condition leads to loss of deltoid function that can cause glenohumeral instability and inferior subluxation, resulting in rotator cuff muscle fatigue and pain. To establish dynamic glenohumeral stability, a novel technique was invented. Humeral suspension is achieved using a double button implant with non-resorbable high strength cords between the acromion and humeral head. This novel technique was used in two patients with isolated deltoid paralysis due to axillary nerve injury. The results indicate that the humeral suspension technique is a method that supports centralizing the humeral head and simultaneously dynamically stabilizes the glenohumeral joint. This approach yielded high patient satisfaction and reduced pain. Glenohumeral alignment was improved and remained intact 5 years postoperative. The humeral suspension technique is a promising surgical method for subluxated glenohumeral joint instability due to isolated deltoid paralysis.

어깨뼈 안정화운동과 등뼈관절가동술이 어깨뼈 기능이상이 있는 성인의 어깨 기능에 미치는 영향 (The Effect of Scapular Stabilization Exercise and Thoracic Joint Mobilization on the Scapular Function in Adults with Scapular Dysfunction)

  • 본 연구는 어깨뼈 기능이상이 있는 사무직원에 대해 어깨뼈 안정화 운동과 등뼈관절가동술을 시행한 후 어깨뼈의 위치와 움직임 및 어깨기능장애지수에 미치는 영향을 분석하기 위한 목적으로 실행되었다. 연구 대상자는 충남의 N대학에서 근무하는 사무직원 중에서 어깨뼈 기능이상 검사(Scapular Dyskinesis Test, SDT)를 하여 이에 해당되는 42명을 연구 대상자로, A그룹은 어깨뼈 안정화운동 적용군(N=14), B그룹은 등뼈관절가동술 적용군(N=14), 그리고 C그룹은 어깨뼈 안정화운동과 등뼈관절가동술을 결합한 실험군(N=14)으로 나누었다. 각 중재는 4주간 주 3회, 회당 25분씩 총 12회 실시하였다. 연구 결과, 어깨뼈 위치 변화의 그룹 간 비교에서는 C그룹이 다른 그룹에 비해 유의한 차이가 있었다(p<.05). 어깨뼈 움직임의 변화에서 위팔뼈 벌림 45°에서는 C그룹이 다른 그룹과 비교하여 그룹 내 및 그룹 간 비교에서 유의한 차이를 보였다(p<.001). 벌림 90°에서도 C그룹이 그룹 내 및 그룹 간 비교에서 유의한 차이를 보였다. 어깨기능장애지수(Shoulder pain and disability index, SPADI)에서는 C그룹이 다른 그룹에 비해 유의한 차이를 보였다(p<.05). 본 연구의 결과, 어꺠뼈 안정화운동과 등뼈관절가동술의 결합은 어깨뼈 기능이상이 있는 사무직원의 어깨뼈 위치와 움직임 그리고, 상지기능장애지수의 향상에 유의한 효과가 있었다.

Applying Focused and Radial Shock Wave for Calcific Tendinitis of the Shoulder : Randomized Controlled Study

  • Objective: Extracorporeal shock wave therapy (ESWT) is a nonsurgical treatment alternative to surgery for various musculoskeletal diseases that have traditionally been difficult to treat conservatively, including calcific tendinitis, tennis elbow, and plantar fasciitis. This study evaluated the effect of focused and radial shock wave therapy for calcific tendinitis of the shoulder. Design: Randomized controlled study Methods: Forty participants with calcific tendinitis were randomized into focused shock wave therapy (FSWT, n=20) and radial shock wave therapy (RSWT, n=20) groups. Patients were examined before and one week after treatment. Pain intensity was subjectively assessed using the visual analogue scale and function was assessed using the Constant-Murley score (CMS) and range of motion (ROM). Results: The results showed a significant decrease in pain and significant increase in shoulder mobility and function in both groups. However, FSWT was significantly more effective than RSWT, based on CMS and ROM assessment. Conclusions: Although it is possible to raise the energy intensity of RSWT to increase the depth at which the energy becomes dispersed, higher energy intensity is associated with a greater risk of severe neurovascular damage, and that high-intensity stimulation can cause adverse effects such as pain and petechiae. Therefore, FSWT is considered to be a safe and effective method for treating tendinous lesions while minimizing adverse effects. In conclusion, both FSWT and RSWT can reduce pain and increase mobility and function. FSWT can be considered as an alternative for calcific tendinitis of the shoulder.

닫힌사슬에서의 PNF 대각선 패턴을 이용한 몸통 전·후방 이동운동이 뇌졸중환자의 상지 기능 및 일상생활에 미치는 영향 -단일 사례 연구- (Effect of Forward-and-Backward Shift Trunk Exercise Using Proprioceptive Neuromuscular Facilitation Diagonal Pattern in Closed Kinematic Chain Exercises on Upper Limb Function and ADL in Stroke Patient -A Single-Subject Design-)

  • Purpose: The purpose of this study was to investigate the effects of forward-and-backward shift trunk exercise using a proprioceptive neuromuscular facilitation (PNF) diagonal pattern in a closed kinematic chain exercise on the upper limb function and activity of daily living (ADL) in a stroke patient. Methods: One subject participated in this study. The study used a reversal A-B-A' design, where A and A' were the baseline period (no intervention), and B was the intervention period. The intervention was a forward-and-backward trunk shift exercise, using a PNF diagonal pattern on both a stand-on-hand position and a quadruped position of closed kinematic chain exercises, for 20 min per day for 2 weeks. The range of motion (ROM) of the shoulder joint was measured and a Fugl-Meyer assessment of upper extremity (FMA-UE) and a functional independence measure (FIM) were performed to measure upper limb function and activity of daily living (ADL). Results: ROMs of shoulder joint (flexion, extension, abduction, and external rotation) increased in the intervention phase. The FMA-UE score increased (from 28 to 36) in the intervention phase. The FIM score increased (from 20 to 25) in the intervention phase. These increases were maintained after intervention (Baseline II). Conclusion: These results suggest that forward-and-backward shift trunk exercises using a PNF diagonal pattern in a closed kinematic chain exercise have a positive effect on stroke patients' upper limb function and ADL ability.

Determination of In-Vivo Glenohumeral Translation During Loaded and Unloaded Arm Elevation

  • The purpose of this study was to investigate humeral translation relative to the glenoid invivo during loaded and unloaded shoulder abduction. CT scans of 9 healthy shoulders were acquired and 3D models were created. The subject was positioned in front of a fluoroscope and motions were recorded during active abduction. The subjects performed two trials of holding a 3kg weight and unload. 3D motions were determined using model-based 3D-to-2D registration to obtain 6 degrees of freedom kinematics. Glenohumeral translation was determined by finding the location on the humeral head with the smallest separation from the glenoid. Humeral translation was referenced to the glenoid center in the superior/inferior direction. The humerus moved an average of 2 mm, from inferior to central on the glenoid, during arm abduction for both conditions. The humeral head was centered within 1mm from the glenoid center above $70^{\circ}$. There were no statistically significant differences for both conditions. The standard deviation decreased gradually over the motion, with significantly lower variability at the end of abduction compared to the initial unloaded position. We assumed that the humeral translation to the center of the glenoid provides maximum joint congruency for optimal shoulder function and joint longevity. We believe this information will lead to better strategies to prevent shoulder injuries, enhance rehabilitation, and improve surgical treatments.

Diagnostic value of a preoperative acromioclavicular injection for symptomatic acromioclavicular osteoarthritis: a retrospective study of cross-sectional midterm outcomes

  • Background: It is essential to distinguish between symptomatic- and asymptomatic radiographic acromioclavicular (AC) osteoarthritis (OA) because AC-targeted physical examinations are dubious. This study aimed to determine the diagnostic value of a preoperative AC injection in discriminating between symptomatic- and asymptomatic radiographic AC OA based on patient arthroscopic distal clavicle resection (aDCR) outcomes. Methods: Forty-eight patients who underwent aDCR for AC OA were included. Their satisfaction was objectified using a 5-point Likert scale and patient willingness to repeat the surgery. The Oxford Shoulder Score (OSS), the Subjective Shoulder Value (SSV), and the Numerical Rating Scale (NRS) were used to assess postoperative shoulder function and pain. Patients were subdivided into groups based on their good or minimal reaction to an AC injection (good reaction: ≥7 consecutive days of pain reduction, Minimal reaction: <7 consecutive days of pain reduction). Results: Twenty-seven patients had a good reaction and 21 patients had a minimal reaction to the AC injection (median follow-up, 45.0 months; range, 31.0-52.8 months). No significant differences were found in level of satisfaction (P=0.234) or willingness to repeat the surgery (P=0.861). No significant differences were found in OSS (P=0.612), SSV (P=0.641), NRS at rest (P=0.684) or during activity (P=0.422). Conclusions: This study found no significant differences between patients with a good reaction or a minimal reaction to an AC injection after aDCR surgery. The outcomes of this study seem to suggest that a distinction between symptomatic and asymptomatic radiographic AC OA is unnecessary, as all patients were equally satisfied with the outcome.

유방절제술 환자의 신체기능과 피로 (Physical Function and Fatigue in Mastectomy Patients)

  • 목적 : 본 연구는 유방절제술 환자의 신체기능과 피로를 파악하여 수술 후 적응을 돕고 삶의 질을 향상시키기 위한 중재 개발에 기초 자료를 마련하고자 실시되었다. 방법 : 2003년 3월부터 6월까지 서울 C 대학병원에서 유방절제술을 받은 환자 63명을 대 상으로 어깨관절 가동범위, 어깨관절 기능, 신체적 자각증상 및 피로 정도를 측정하였다. 결과 : 환측의 어깨관절 가동범위는 건측에 비해 유의하게 낮았다. 어깨관절 기능이 가장 높았던 문항은 '바지 입기'였으며, 가장 낮았던 문항은 '등지퍼 올리기'였다. 신체적 자각증상의 정도가 가장 높았던 문항은 '감각이 둔하다'였으며, 가장 낮았던 문항은 '가렵다'였다. 4. 피로 때문에 가장 방해가 되었던 문항은 '일상적인 생활(집 밖의 일과 매일하는 허드렛 일)'이었다. 결론 : 유방절제술 후 신체기능을 향상시키고 피로를 감소시키기 위한 효과적인 중재가 필요하다.

견관절 굴곡 각도가 남·녀 대학생의 악력 변화에 미치는 영향 (The Impact of Shoulder Flexion Angle on Hand Grip Strength in Male and Female Undergraduate Students)

  • Purpose : This study's purpose is consideration about change of the hand grip strength according to different posture and shoulder flexion angle. The shoulder joint permits the greatest mobility and carries out the important function of stabilization for hand use. Hand grip activity is important to evaluate while assessing loads of shoulder in hand mobilities. Methods : Thirty(15 male, 15 female) college students with unknown shoulder dysfunction participated subject in five different positions of elbow extension with sitting and standing posture, different positions is followed : (1) shoulder $0^{\circ}$ flexion (2) shoulder $45^{\circ}$ flexion (3) shoulder $90^{\circ}$ flexion (4) shoulder $135^{\circ}$ flexion (5) shoulder $180^{\circ}$ flexion. Results : On the average, in the hand grip strength, the standing posture is higher than sitting posture. Sitting posture showed a most high level at the man's $0^{\circ}$ and woman's $135^{\circ}$. And standing posture showed a most high level at the man's $135^{\circ}$ and woman's $90^{\circ}$. Conclusion : The paired t-test was used to determine the different in grip strength between sitting and standing posture by shoulder angle change. There was no significant difference between the five position by sitting and standing posture. In man, correlation analysis revealed significant connection for all five position by sitting and standing posture. And in woman, correlation analysis revealed connection for all five position by sitting and standing posture.

In Vivo Three-Dimensional Evaluation of the Functional Length of Glenohumeral Ligaments

  • Glenohumeral ligaments play an important role in stabilizing the shoulder. However, it is impossible to know how they function in vivo during shoulder motion. To help elucidate this stabilizing role, we conducted in vivo three-dimensional kinematics of the normal shoulder joint using a markerless bone-registration technique. Magnetic resonance images of 14 shoulder joints of 7 healthy volunteers were acquired for 7 isometric abduction positions between $0^{\circ}$ and $180^{\circ}$. We then calculated three-dimensional shortest paths between the origin and insertion of each ligament based on anatomical study in each abduction position. At $0^{\circ}$ of abduction, the posterior band of the coracohumeral ligament displayed the maximum length. At $30^{\circ}$ of abduction, the superior glenohumeral ligament displayed the maximum length. At $60^{\circ}$ of abduction, the anterior band of the coracohumeral ligament and the middle glenohumeral ligament displayed the maximum length. At $120^{\circ}$ of abduction, the anterior band of the inferior glenohumeral ligament displayed the maximum length. We think that the maximum length of these results is an important influence on the function of the soft tissue stabilizer.
