Osteochondroma of the Distal Clavicle: A Rare Cause of Impingement and Biceps Tear of the Shoulder

  • Hereditary multiple exostosis (HME) is an autosomal dominant disorder manifested by the presence of multiple osteochondromas. Although the lesions are benign in nature, exostoses are often associated with characteristic progressive skeletal deformity and displaying clinical symptoms such as mechanical irritation or impingement. We present the successful arthroscopic resection in a 24-year-old HME male with impingement syndrome and long head tendon tear of the biceps caused by osteochondroma arising from the distal clavicle.

Effect of Motor Control Training with Strengthening Exercises on Pain and Muscle Strength of Patients with Shoulder Impingement Syndrome

  • Purpose: The purpose of this study was to investigate the effect of movement training based on motor control theory on pain and average power of muscles in patients with shoulder impingement syndrome and to develop more effective training methods. Methods: We studied 35 patients with shoulder impingement syndrome. Patients were randomly assigned to an experimental group or a control group according to the intervention. The therapeutic modalities such as superficial heat, deep heat, and electronic stimulus, and motor control training with strengthening exercises were applied to the experimental group and therapeutic modalities only were applied to the control group. All interventions were done 3 times a week for 4 weeks. Before the intervention and again after the 4 weeks, we measured pain utilizing a visual analog scale. We measured average power using isokinetic equipment. Results: After application of the intervention, pain significantly decreased in both the experimental group and the control group. There was a significant difference between the extent of the decrease between experimental and control groups. After the intervention, the average power between the two groups were significant at an angle of $60^{\circ}$/sec and $180^{\circ}$/sec for external rotator and internal rotator muscles. They also showed significant improvement in all variables post-intervention compared to pre-intervention. Conclusion: This study indicates that there is an effect in giving appropriate motor control training to patients with shoulder impingement syndrome. It is assumed that there will be a need for more surveys on various variables for motor control training from now on.

관절경적 감압술에 의한 견봉하 충돌증후군의 치료 (Arthroscopic Decompression of Subacromial Impingement Syndrome)

  • The authors performed the arthroscopic decompression and cuff debridement on 47 cases in 45 consecutive patients with either stage II or stage III impingement syndrome from July 1990 to January 1994. The summarized results are as follows. 1. 47 cases in 45 consecutive patients had arthroscopic decompression for the subacromial impingement syndrome. 2. There were 31 males and 14 females and the mean age was 40 years for men and 46years for women. 3. The follow up duration was from 3 years 9 months to 1 year (average 2 years 1month). 4. Among 47 cases, 19 cases were found to have no cuff tear (stageII) 13 cases partial thickness cuff tear (stage IIIa) 10 cases complete tear on cuff less than 3 cm long (stage IIIb) and 5 cases complete tear on cuff more than 3 em in length (stage IIIc). 5. The arthroscopic subacromial decompression and rotator cuff debridement was a good treatment method in stage II and stage IIIa and stage IIIb. in the case of stage IIIc rotator cuff tear, it was useful for pain relief and improvement of shoulder function.

재활 프로그램을 적용한 코칭 운동과 자가 운동이 견관절 충돌증후군 환자의 견부통증, 근력 및 일상생활기능에 미치는 효과 (Effects of Rehabilitation Programs with Couching Exercise or Home Exercise on Shoulder Pain, Muscle Strength, and Daily Living Functions in Patients with Shoulder Impingement Syndrome)

  • Purpose: This study aimed to investigate the effects of rehabilitation programs with coaching exercise (CE) or home exercise (HE) on shoulder pain, muscle strength, and daily living functions (DLF) in patients with shoulder impingement syndrome. Methods: With quasi-experimental design, thirty patients (15 CE and 15 HE) participated in rehabilitation programs for 12 weeks. At first, data were collected for pain and DLF using questionnaires with testing muscle strength and range of motion. Then, subjects were allowed to receive training on rehabilitation with structured training materials. CE group visited sports center three times a week at P hospital and HE group did self-exercise at home. Data were analyzed using SPSS program. Results: After 12 weeks, both groups showed statistically significant changes in improving pain, muscle strength, and DLF. Compare to HE group, CE group showed a statistically significant improvement in shoulder pain (p=.021), muscle strength in internal rotation (p=.001) and abduction (p=.013) and DLF (p=.012). Conclusion: Rehabilitation programs for patients with shoulder impingement syndrome were effective to improve pain, muscle strength, and DLF. Although CE would bring the better effects, it is possible to apply HE through structured education to patients who are difficult to visit hospitals.

Comparisons of Shoulder Muscle Activity and Muscle Activity Ratio during Serratus Anterior Exercise between CrossFit Practitioners with/without Shoulder Impingement Syndrome

  • Purpose: Due to the high incidence of shoulder injuries, including shoulder impingement syndrome (SIS), among CrossFit practitioners due to frequent overhead movements, serratus anterior exercises are considered crucial for scapular stabilization in both intervention and prevention. Objective: The objective of this study is to compare the muscle activity and ratios of scapular stabilizing and shoulder girdle muscles between individuals with and without SIS during serratus punch and wall slide exercises, both targeting the serratus anterior muscle, in CrossFit training practitioners. Methods: Surface electromyography was used to compare the muscle activity and activity ratio of scapular stabilizing muscles and shoulder muscles during serratus punch and wall slide exercises in two groups of 20 CrossFit practitioners: ten with SIS and ten without symptoms. Results: The group with SIS showed higher activation of the pectoralis major, upper trapezius, and a higher pectoralis major/serratus anterior and upper trapezius/serratus anterior muscle activation ratio during the serratus punch exercise compared to the group without SIS. Similarly, during the wall slide exercise, the group with SIS exhibited higher activation of the upper trapezius and a higher upper trapezius/serratus anterior muscle activation ratio compared to the group without SIS. However, no significant difference in serratus anterior muscle activation was observed between the two groups. Conclusions: This study highlights the higher activation of the pectoralis major and upper trapezius muscles in CrossFit practitioners with SIS during the serratus anterior exercise, suggesting the importance of minimizing the overactivation of these muscles to prevent impingement syndrome in this population.

견봉하 충돌증후군의 치료에서 스테로이드 국소주사의 효유성에 대한 분석 (An Analysis of the Efficacy of Local Steroid Injections for the Treatment of Subacromial Impingement Syndrome)

  • From May 1994 to May 1995, we performed a prospective controlled double-blind clinical study of 85 patients(85 shoulders). For inclusion in the study all patients met the following criteria: 1) a diagnosis of impingement syndrome by lidocaine injection test; 2) symptoms for at least three months; 3) no previous subacromial injection; 4) no evidence of rotator cuff tear. We excluded the patients from the study who showed a instability or other clear primary cause. The patients were randomized to receive either 5/subcc/ 2% lidocaine with 4ee of 20mg/cc Depomedrol(Group 1) in 45 cases or 5/subcc/ 2% lidocaine solely(Group 2) in 40 cases. At three month follow-up, all results were better in steroid group than control group. But at one year, improvement of pain and daily activity showed no difference between two groups. So we conclude that the use of subacromial steroid injection is effective for short-term therapy in the treatment of subacromial impingement syndrome, but the long-term effectiveness is doubtful.

어깨 충돌 증후군을 가진 중년 여성에게 모션테이핑이 근경직도 및 근긴장도와 통증에 미치는 영향 (Effects of Motion Taping on Muscle Tone, Muscle Stiffness, and Pain in Middle-Aged Women with Shoulder Impingement Syndrome)

  • Purpose: This study investigated the effects of motion taping on muscle stiffness, muscle tone, and pain in middle-aged women with shoulder impingement syndrome. Methods: The participants comprised 30 middle-aged women with shoulder impingement syndrome. This study measured muscle stiffness, muscle tone, and pain. In the experimental group, the tape was attached after physical therapy; the control group only received physical therapy. In the intervention period, the treatment program was conducted three times a week for a total of four weeks, and the values before and after the intervention were compared and analyzed. For statistical processing, a correspondence test was performed before and after the intervention at a significance level of 0.05, and the comparison between the groups, before and after the intervention, was made using an independent test. Results: In the experimental group, there was a difference in muscle stiffness, muscle tone, and intensity of pain. In addition, there was a significant difference between the control group's emotional domain regarding muscular stiffness and pain and the experimental group. However, there was no significant difference in muscle tone and grade of pain, and intensity of pain for the control group. Conclusion: This study showed that, when physical therapy and motion taping were administered to middle-aged women with shoulder impingement syndrome, it had a more positive affect on the sensory and emotional parts of the muscle tone and pain compared to physical therapy alone.

견관절 만성 충돌 증후군의 관절경적 견봉하 감압술 (Arthroscopic Subacromial Decompression for Chronic Impingement)

  • The purpose of this study was to assess the results of arthroscopic subacromial decompression in patients with chronic impingement and to evaluate the results according to the rotator cuff pathology. We evaluated the clinical results of treatment for chronic impingement syndrome in 28 patients from Feb 1996 to Feb 1997. There were twenty men and eight women in age from 24 to 72 years (mean age 51) with dominant arm involvement in sixteen patients. Follow up evaluations averaged 15(range 12-24)months. The average duration of symptoms were 15(range 6­60)months. The final diagnoses which were based on the physical examination, plain radiographs and arthroscopic findings, were stage II impingement in 16 patients and stage ill impingement in 12 patients. We excluded the patients with acromioclavicular arthritis or glenohumeral instability in this study. All patients were managed non-operatively a minimum of six months. During the operation we performed contouring and smoothing the acromial undersurface and only resecting of the anterolateral band of the coracoacromial ligament. The clinical results were quantitated using UCLA shoulder rating score. Satisfactory results were obtained in 23(80%) patients. Unsatisfactory results were obtained in 5(18%) patients with posterior cuff tear. The average UCLA pain score showed significant improvement from 2.8(constant pain) to 7.2(present during heavy activities) at final follow up. The function and active forward flexion scores also increased from their preoperative value. There was no significant differences according to the surface and severity of tear and NeeI' stage (P>0.05). These results compared favorably with those reported following open acromioplasty. While arthroscopic subacromial decompression is a demanding technique with a learning curve, it is a reliable treatment for chronic impingement syndrome. A less aggressive approach to subacromial decompression and preserving the posteromedial band of the coracoacromialligament does not appear to compromise results.

운동조절과 근력강화 운동이 유방절제술 후 견관절의 기능부전으로 유발된 견관절 충돌 증후군에 미치는 영향 (The Effects of Motor Control and Strengthening Exercise on Secondary Shoulder Impingement Syndrome at Postmastectomy)

  • 본 연구의 목적은 운동조절과 근력강화 운동이 유방절제술 후 기능부전이 악화되어 유발된 견관절 충돌 증후군 환자에게 미치는 영향을 알아보고자 한다. 유방절제술 후 기능부전이 악화되어 이차적으로 유발된 1-2단계 견관절 충돌 증후군 20명을 무작위로 실험군 10명, 대조군 10명으로 나누었다. 두군 모두 보조적 치료 4주간 주당 4회 40분씩 받았다. 실험군은 추가적으로 운동조절과 근력강화 운동을 60분간 실시하였다. 평가 측정은 실험군과 대조군 모두 실험 전과 후에 악력, 통증강도, 상지둘레, 상지의 불편감정도와 관절가동범위를 측정하였다. 본 연구의 결과는 실험군과 대조군간에 관절가동범위, 상지의 불편감정도에서 유의한 차이(p<.05)를 보였고 악력, 통증강도, 상지둘레는 유의한 차이를 보이지 않았다. 이런 결과들은 운동조절과 근력강화 복합운동프로그램이 유방절제술 후 기능부전이 악화되어 이차적으로 유발된 견관절 충돌 증후군에 안전하면서 효과가 높은 방법임을 알 수 있었다.
