  • It is well known that the essential pathology in traumatic anterior dislocation of the shoulder is the avulsion of the glenohumeral ligament complex from the glenoid. However, there are reports that the avulsion can occurs from the humeral attachments. We have experienced the 42 year-old male patient who had a traumatic lateral avulsion of the subscapularis and glenohumeral capsule from the humerus and reported the results of treatment.

  • 자기공명 관절 조영술(Shoulder MR Arthrography)는 견관절(Shoulder joint) 의 복잡한 해부학적 구조에 관하여 정확한 영상평가를 진단하기 위해 시행 한다. 우리는 어께 상완골(Shoulder humerus) 의 다양한 position인 Neutral position, Internal rotation position, External rotation position 에 관하여 Shoulder joint의 해부학 적인 회전 변화가 견관절 자기공명(Shoulder MRI) 영상에 어떤 진단적 결과를 가져오는가에 대해 상호 비교 하였다. 또한 환자의 정확한 촬영자세 유지를 위해 촬영 보조기구를 만들었다. 이 보조기구는 Modeling 설계에 의해 우리가 직접 제작한 자기공명 견관절 조영술 보조 기구이다. 이 보조기구를 사용하여 촬영한 결과 다음과 같은 결론을 도출해 내었다. External rotation position 에 의한 Shoulder MRArthrography 검사가 Shoulder joint의 중요 해부학적 구조인 Biceps tendon, Supera-spiatus tendon, Sub-scapularis tendon, Labrum, Sub-acromial space 의 해부학적 평가에서 제일 적합하다는 진단적 평가를 얻었다.

  • 저자들은 충돌 증후군 진단하에 관절경적 견봉하 감압술 중 우연히 발견된 Buford 복합체를 가진 환자를 본 후, 술후 이 환자를 추시한 결과가 Buford 복합체가 없는 다른 환자의 경우와 크게 다르지 않은 것을 보고, 충돌 증후군의 증상을 보이는 환자에서 삭형 중 견와상완 인대와 전상방견와 순의 결여가 있는 경우 이 복합체가 술후 예후에 영향을 미치지 않는 것으로 결론을 내리고 이를 보고하는 바이다.

Posterior and Multidirectional Instability

  • The posterior and multidirectional instability of the shoulder is a complex problem in terms of diagnosis and treatment. Increased joint volume by redundant capsular ligament has been regarded as a major pathogenesis of the posterior and multidirectional instability. Distinct from multidirectional hyperlaxity, multidirectional instability has symptoms related with increased translations in more than one direction. Recent report that shoulder symptom originates from labral lesion which was created by excessive rim-loading of the humeral head on the posteroinferior glenoid labrum during repetitive subluxation helps us to understand the pathogenesis of such instability. Painful jerk and Kim tests indicate labral lesion in the multidirectionally loose shoulder, suggesting multidirectional instability. Also, painful jerk test is a prognostic sign of failure of nonoperative treatment. The labral lesion can be an incomplete tear or a concealed lesion which often has been underestimated. Operative treatment is indicated when nonoperative treatment has failed. Arthroscopic capsulolabroplasty is a reliable procedure, which not only provides capsular balance, but also restores the labral height.

  • Historically, the decision to perform a hemiarthroplasty (HHR) versus a total shoulder arthroplasty (TSA) is based on the status of the glenoid and the status of the soft tissues (rotator cuff). In disease processes where the glenoid articular cartilage is relatively well preserved such as avascular necrosis and complex proximal humerus fractures, most orthopaedists recommend performing a HHR while preserving the native glenoid articular surface. At the other end of the spectrum, if the glenoid has excessive bone loss or is unreconstructible, a HHR is the preferred procedure. In patients who have deficient so(t-tissues (rotator cuff) such as rotator cuff tear arthropathy and, occasionally, rheumatoid arthritis, a HHR is the procedure of choice. The indications for HHR in osteoarthritis remain somewhat controversial. There is mounting evidence that performing a HHR for osteoarthritis is inferior to TSA. Recent developments, or 'third generation techniques and materials', in shoulder arthroplasty are expected to improve the longevity of TSA, particularly the glenoid component. In addition, newer designs of reverse-ball prostheses are entering the market with promising early results in patients with deficient rotator cuff mechanisms.

상부 견갑 현수 복합체의 3중 붕괴 - 5년 추시 증례 보고 - (Triple Disruption of the Superior Shoulder Suspensory Complex - Case Report at 5-year-follow up -)

  • 상부 견갑 현수 복합체(Superior shoulder suspensory complex)의 3중 붕괴는 몇몇 저자들에 의해서만 보고된 극히 드문 손상이다. 저자들은 오구돌기, 견갑극 골절 및 견봉-쇄골 관절의 탈구가 동반된 상부 견갑 현수 복합체의 3중 붕괴 환자를 경험하였기에 보고하고자 한다. 본 환자의 경우 견봉-상완 및 오구-상완 간격을 유지하기 위하여 관혈적 정복 및 내고정을 시행하여 상부 견갑 현수 복합체의 3중 붕괴 모두를 해부학적으로 복원하여 치료하였다. 술 후 6개월에 증상 없이 관절운동범위를 모두 회복하였다. 최종 추시인 5년에는 상부 견갑 현수 복합체의 기능이 완전히 회복된 것을 확인할 수 있었으며, 환자는 수술 결과에 대하여 크게 만족하였다.

Primary Total Elbow Replacement for Treatment of Complex Distal Humerus Fracture: Outcomes of Short-term Follow-up

  • Background: High complication rate after open reduction and internal fixation can lead to use of primary total elbow replacement (TER) in treatment of complex distal humerus fractures in elderly patients. The purpose of this study was to investigate the short-term outcomes and complications after primary TER in patients with complex distal humerus fracture. Methods: Nine patients with acute complex distal humerus fracture were treated by primary TER using the semiconstrained Coonrad-Morrey prosthesis. The mean age of patients was 72.7 years (range, 63-85 years). Clinical and radiographic outcomes were evaluated over a mean follow-up of 29.0 months (range, 12-65 months) using visual analog scale (VAS) score for pain; Mayo elbow performance score (MEPS); Quick Disabilities of the Arm, Shoulder, and Hand (Quick-DASH) score; and serial plain radiographs. Complications were also evaluated. Results: At the final follow-up, mean VAS, MEPS, and Quick-DASH scores were 1.2, 80.5, and 20, respectively. The mean range of motion was 127.7° of flexion, 13.8° of extension, 73.3° of pronation, and 74.4° of supination. There was no evidence of bushing wear or high-grade implant loosening on serial plain radiographs. Three complications (33.3%) comprising two periprosthetic fractures and one ulnar neuropathy were observed. Conclusions: Primary TER for treatment of complex distal humerus fractures in elderly patients yielded satisfactory short-term outcomes. However, surgeons should consider the high complication rate after primary TER.

Short-term comparative outcomes between reverse shoulder arthroplasty for shoulder trauma and shoulder arthritis: a Southeast Asian experience

  • Background: Reverse shoulder arthroplasty (RSA), first introduced as a management option for cuff tear arthropathy, is now an accepted treatment for complex proximal humeral fractures. Few studies have identified whether the outcomes of RSA for shoulder trauma are comparable to those of RSA for shoulder arthritis. Methods: This is a retrospective, single-institution cohort study of all patients who underwent RSA at our institution between January 2013 and December 2019. In total, 49 patients met the inclusion criteria. As outcomes, we evaluated the 1-year American Shoulder and Elbow Surgeons (ASES) and Constant shoulder scores, postoperative shoulder range of motion, intra- and postoperative complications, and cumulative revision rate. The patients were grouped based on preoperative diagnosis to compare postoperative outcomes across two broad groups. Results: The median follow-up period was 32.8 months (interquartile range, 12.6-66.6 months). The 1-year visual analog scale, range of motion, and Constant and ASES functional scores were comparable between RSAs performed to treat shoulder trauma and that performed for arthritis. The overall complication rate was 20.4%, with patients with a preoperative diagnosis of arthritis having significantly more complications than those with a preoperative diagnosis of trauma (34.8% vs. 7.7%). Conclusions: Patients who underwent RSA due to a proximal humeral fracture or dislocation did not fare worse than those who underwent RSA for arthritis at 1 year, in terms of both functional and radiological outcomes.

  • MR arthrography is a useful modality for evaluating the labrocapsular ligamentous complex(LOLC) of the shoulder. This study was performed to describe normal anatomic variations and pitfalis in image interpretation related to evaluation of the LOLC. MR Arthrogram of 56 shoulders in 41 asymptomatic young, active volunteers were prospectively reviewed to evaluate the labral shapes. capsular insertions and images which may mimic the lesions of glenohumoral instability. The anterior and posterior parts of the labra. respectively. varied in shape: triangular$(72\%,\; 36\%)$. round $(13\%,\; 35\%)$. cleaved$(8\%,\; 1\%)$. notched$(2\%,\; 0\%)$. flat$(5\%,\; 24\%)$ and absent$(0\%,\; 4\%)$. The anterior and posterior capsular insertions. respectively. varied in sites: Mosely and Oevergaard type I$(82\%,\;62\%)$, type II$(13\%,\; 3\%)$ and type III $(5\%,\; 2\%)$. A number of pitfalls in image interpretation were discovered. Articular cartilage undercutting the labrum$(29\%)$ and middle glenohumoral ligament in proximity to anterior labrum $(5\%)$ simulated a labral tear. Joint fluid interposed in the central. superior portion of the sublabral sulci$(25\%)$ simulated a SLAP lesion. Synovial fold$(38\%)$ in axillary pouch resembled a loose body. Knowledge of normal variations and pitfalls in MR arthrogram image interpretation of labral capsular - ligamentous complex will help the orthopedist to accurately detect debilitating derangements associated with the glenohumeral instability.

Immediate Effect of the Complex Rotational Stretching Method on the Proprioceptive Sensation of the Shoulder Joint, Subacromial Space, Range of Motion, Shoulder Instability and Dynamic Function

  • PURPOSE: This study compared the effects of proprioceptive sensation, subacromial space, and dynamic function according to proprioceptive neuromuscular facilitation (PNF), static stretching (SS), and complex rotational stretching (CRS). METHODS: Thirty students without any musculoskeletal disease who volunteered to participate were included in this study. The following metrics were measured to evaluate the function and stability under the normal conditions, with the PNF, SS, and CRS: special test and flexion, extension, abduction, adduction, internal rotation, external rotation (shoulder range of motion) and reaching distance on the medial (ME), superolateral (SL), inferolateral (IL), and subacromial space and proprioceptive sensation were evaluated. All measures were analyzed using one-way ANOVA and repeated measures of ANOVA. RESULTS: A clear difference in adduction in the range of motion was observed in all groups (p < .05). Significant differences could not be identified in all values in the error test, except for Ab (p < .05). Significant differences in reach were noted in all directions of the SS and PNF in the Me, SL, and IL (p < .05). After the intervention, significant differences in the average values could be identified in all groups except for the SS group after rest (p < .05). After the intervention, there was a significant difference between the CRS and SS and PNF groups (p < .05). CONCLUSION: The application of CRS is as helpful as the existing SS and PNF for improving the joint range of motion improvement, shoulder balance, and subacromial space.
