The purpose of this study was to compare the circumference and skinfold thickness of upper and lower limb and the leg strength of the casted limb with those of the normal limb after removal of a leg cast. The subjects for the study were orthopedic patients who had had long and short leg casts or splints due to tibial, fibulal, metatarsal, calcaneus fracture or ankle sprains. The subjects were divided into two groups, those who had the cast on for less than 40 days and those for over 41 days. Circumference and skinfold thickness of the upper and lower limb on the side on which the cast was ap-plied were compared with those of the contralateral side after removal of the cast. Circumference and skinfold thickness of the upper and lower limb, and leg strength for those in a cast for under 40 days were compared with those of over 41 days for both the side to which cast was applied and the contralateral side. Measurements were made after removal of the cast. Skinfold thickness was measured by fat caliper, circumference was measured by tape and lower extremity strength was determined with flat foot pressing on an electronic digital health meter in the sitting position. The results can be summarized as follows : 1. The circumference of the upper and lower leg on the side on which the cast was applied, when measured after the cast was removed, were significantly less than those of the normal side, 93.88%, 93.11% each. 2. Skinfold thickness of the quadriceps and gastrocnemius on the side on which the cast was applied were significantly less than those of the normal side when measured after removal of the cast, 85.98%, 82.85% respectively. 3. Leg strength on the side where the cast was applied was significantly 1ss than that on the normal side, 60.20%. 4. There was no difference in the circumference of upper and lower limbs, skinfold thickness or leg strength on the side where the cast was applied between the group which had the cast applied for under 40 days and the group that had it applied for over 41 days. 5. The circumference of the upper arm and lower leg on the normal side for the group that had the cast applied for over 41 days was significantly greater than the group that had the cast application for under 40 days. T ere was no difference between the two groups in the circumference of the forearm and upper leg, skinfold thickness and leg strength in the normal side. From these results, it may be concluded that muscle atrophy was apparent in the casted limb compared to the normal limb, and the circumference of the upper arm and lower leg, and leg strength on the normal side increased after removal of the cast in the group which had the cast on for more than 41 days.

Circumference, skinfold thickness, and strength of the normal and casted lower limb following removal of the cast were compared with those prior to the application of the cast. Measurements were made before cast application and after removal of the cast. Skinfold thickness was measured by fat caliper, circumference was measured by tape and strength was determined by measuring the length of time the leg was held elevated at 45$^{\circ}$. The results can be summarized as follows 1. There was no change in the normal limb in the circumference of the midthigh and midcalf after casting as compared to before cast application. 2. In the casted limb the circumference of the midthigh decreased by 3.23% and that of the midcalf decreased significantly by 7.49% during the period of casting. 3. In the normal limb skinfold thickness of the quadriceps decreased and that of gastrocnemius increased by 20.63% during the period of cast application. 4. In the casted limb skinfold thickness of the quadriceps decreased significantly by 12.37% and that of gastrocnemius decreased by 10% during the period of cast application. 5. Strength of the normal lower extremity decreased significantly by 48.37% and that of casted lower extremity decreased remarkably by 73.07% during the period of cast application. 6. Circumference of the midthigh and the midcalf decreased significantly by 7.6% and 9.4% respectively on the casted side as compared to the normal side. Skinfold thickness of the quadriceps and the gastrocnemius on the casted side decreased by 6.12% and 18.55% respectively as compared to the normal side and strength in the lower extremity on the casted side decreased significantly by 44.32% as compared to the nor-mal side. From these results, it may be concluded that muscle atrophy occurs in the casted lower limb and muscle strength of the normal lower limb are also reduced during the period of application of a leg cast.

The authors studied 74 patients with cal therapy Kosin University Medical March 1994 through February 1995. The results were, as follows: 1. Average age of the subject patients was 59.3 years old, their age ranged from 20 to 84 years old, and the incidence between male and female was 1:1.24 2. PURPOSE: This study measured the impact of a 12-week fall-prevention exercise program on balance, ambulatory ability, lower limb strength, and psychosocial characteristics in older adults diagnosed with dementia. METHODS: The participants comprised 31 older adults (9 men, 22 women) diagnosed with Alzheimer's or vascular dementia at a long-term care hospital located in Gunsan City. A fall-prevention exercise program was provided to the experimental group, while the control group was only provided with instruction and materials related to the fall-prevention exercise program. The participants were evaluated before the intervention, 6 weeks after the intervention, and 12 weeks after the intervention on static and dynamic balance abilities (using Timed Up and Go test: TUG, Tinetti-balance scale, one-leg standing test: OLS), gait (Tinetti-balance scale, 6-minute walk test: 6MWT), lower limb strength (sit to stand test; STS), and psychosocial characteristics (Short Form 36 Health Survey-Korean version, Korean Mini-Mental State Exam). RESULTS: An independent samples t-test and repeated measures analysis of variance were used for the statistical analysis. There were statistically significant improvements after the intervention (p<.05) in dynamic balance abilities (TUG and OLS using the left foot), gait (6MWT), and lower limb strength (STS) for the experimental group, but not for the control group. No difference was seen in psychosocial characteristics. CONCLUSION: Older adults with dementia who participated in the fall-prevention exercise program showed significant improvements in their static and dynamic balance abilities, lower limb strength, and ambulatory ability after the intervention.
Prevention of acute rejection in composite tissue allotransplantation without continuous immunosuppression lacks reports in worldwide literature. Recently dendritic cells (DC) gained considerble attention as antigen presenting cells that are also capable of immunologic tolerance induction. This study assesses the effect of alloantigen-pulsed dendritic cells in induction of survival in a rat hindlimb allograft. We performed hindlimb allotransplantation between donor Sprague-Dawley and recipient Fischer344 rats. Recipient derived dendritic cells were harvested from rat whole blood and cultured with anti-inflammatory cytokine IL-10. Then donor-specific alloantigen pulsed dendritic cells were reinjected into subcutaneous tissue before limb transplantation. Groups: I) untreated (n=6), II) DC injected (n=6), III) Immunosuppressant (FK-506, 2 mg/Kg) injected (n=6), IV) DC and immunouppressant injected (n=6). Graft appearance challenges were assessed postoperatively. Observation of graft appearance, H-E staning, immunohistochemical (IHC) study, and confocal immunofluoreiscece were performed postoperatively. Donor antigen pulsed host dendritic cell combined with short-term immunosuppression showed minimal mononuclear cell infiltration, regulator T cell presence, and could prolong limb allograft survival.
Purpose: Among computer users, the awkward posture and workstation setups of workers contribute to work-related upper limb musculoskeletal disorders. The purpose of this study was to evaluate the efficacy of a traditional physical therapy and ergonomic intervention by physical therapists in 4 computer users. Methods: After checking Visual Analogue Scores (VAS), four subjects who were treated by physical therapy for neck and shoulder problems related to VDT syndrome were enrolled in the study. All subjects spent at least 40 hours per week at a computer workstation. All subjects had pain and ergonomic states evaluated using methods such as VAS, Neck Disability Index (NDI), Workstyle short form, Rapid Upper Limb Assessment (RULA), and OSHA VDT checklist (Occupational Safety and Health Administration video display terminal) before a physical therapy + ergonomic intervention. Participants were re-evaluated 1 month later. Results: Participants showed more improvement of their neck pain after being treated with a physical therapy plus ergonomic intervention than when treatment consisted only of physical therapy. Improvements in RULA, Workstyle short form, and OSHA VDT checklist also were achieved. Conclusion: This case study suggests the importance of examining the work habits and work-related postures of subjects who complain of neck and shoulder pain that is exacerbated by computer use. Personalized ergonomic interventions and physical therapy can lead to improvement of patients with VDT syndrome.
Background: There is minimal literature on the morphology of partial distal biceps tendon (DBT) tears. We sought to investigate tear morphology by retrospectively reviewing 3-Tesla magnetic resonance imaging (3T MRI) scans of elbows with partial DBT tears and to propose a basic classification system. Methods: 3T MRI scans of elbows with partial DBT tears were retrospectively reviewed by two experienced observers. Basic demographic data were collected. Tear morphology was recorded including type, presence of retraction (>5 mm), and presence of discrete long-head and short-head tendons at the DBT insertion. Results: For analysis, 44 3T MRI scans of 44 elbows with partial DBT tears were included. There were 9 isolated long-head tears (20%), 13 isolated short-head tears (30%), 2 complete long-head tears with a partial short-head tear (5%), 5 complete short-head tears with a partial long-head tear (11%), and 15 peel-off tears (34%). Retraction was seen in 5 or 44 partial tears (11%), and 13 of the 44 DBTs were bifid tendons at the insertion (30%). Conclusions: Partial DBT tears can be classified into five sub-types: long-head isolated tears, short-head isolated tears, complete long-head tears with partial short-head involvement, complete short-head tears with partial long-head involvement, and peel-off tears. Classification of tears may have implications for operative and non-operative management. Level of evidence: III.
Chronic lymphedema is caused by an impairment of the lymphatic system due to primary or secondary causes. Vascularized lymph node transplantation (VLNT) is currently the most promising and frequently used technique besides lymphaticovenous anastomosis. However, the vessel anatomy in the lateral thoracic region is sometimes quite variable. Based on our experiences with vascular anatomical inconstancy in the lateral thoracic region, we planned a lateral intercostal artery perforator flap for VLNT in a female patient with chronic stage II lymphedema of both legs after cervical cancer treatment. After surgery, the patient reported significant improvement in limb volume and the accompanying symptoms. The limb circumference was reduced by an average of 19.2% at 6 months postoperatively. Despite having a short pedicle and small vessel caliber, the lateral intercostal artery perforator flap can safely be used for VLNT in lymphedema patients with anatomical variants.
A 72-year-old woman with metastatic lung cancer to bone and brain and with left external iliac vein thrombosis was under the care of a community palliative care provider. She experienced an acute pain crisis due to acute limb ischemia of the left lower limb. Goals-of-care discussions were held with the patient and her family; she prioritized symptom control and end-of-life care at home. The family and patient were aware of her short prognosis. Her complex pain was managed by the community palliative team, and her family was empowered to give subcutaneous injections. We illustrate a case showing the importance of community health services with palliative care support in providing symptom management and support to patient and family caregivers throughout the course of a life-limiting illness. It also highlights family caregivers' potential psychological distress in delivering subcutaneous injections in terminal care for a patient at home.
