당뇨 및 고혈압 환자에서 혈액투석에 따른 Jitter와 Shimmer 요소값 변화 측정 및 분석 (Variation Measurement and Analysis of Jitter and Shimmer Parameter Value by Hemodialysis in Diabetic and Hypertensive)

  • 현대사회에서 만성질환은 고령인구의 건강한 삶을 위협하는 요소이며 당뇨 및 고혈압은 신장 기능의 파괴를 초래하고 악화될 경우 혈액투석, 인공장기, 장기이식술 등의 치료를 받아야 하는 위험한 질환이다. 따라서 본 논문에서는 당뇨 및 고혈압 환자들을 대상으로 혈액투석 치료에 따른 성대 진동 요소의 변화를 측정하여 혈액투석 치료가 성대 진동에 미치는 영향을 연구하였다. 이를 위해 발음에 문제가 없는 당뇨 및 고혈압 환자들을 피실험자로 선정하고 이들의 혈액투석 전과 후의 음성을 수집하여 성대 진동과 관련된 Jitter 및 Shimmer 분석 요소를 적용하여 변화되는 정도를 비교, 분석하는 실험을 수행하였다. 결론적으로 당뇨 및 고혈압 환자를 대상으로 한 혈액 투석 치료가 신장 기능의 향상에 도움이 되는 것을 성대 진동의 안정성이 향상되는 실험 결과로 도출하였다.

  • 1. Objectives and Methods Sasang Constitutional Medicine is the original Korean Medicine. The purpose of this study was to objectify the diagnosis of Sasang Constitution. 212 Women's sentences were analyzed into 228 factors like Pitch, APQ, Shimmer, Octave and Energy, etc. Women's sentences were classified into 3 categories: total group, under 54 years old group and over 55 years old group. 2. Results 1) In Total group Soyangin's Center feq.(3) was significantly high compared with Taeyangin and Taeumin groups. Taeumin's Pitch2 was significantly high compared with Soeumin and Taeyangin groups. Taeyangin's Pitch S.D. was significantly high compared with Soyangin group. Taeyangin's Octave6 was significantly high compared with Soeumin group. There were no significant differences among constitutional groups in APQ and Shimmer segment. On the point of Energy, Taeyangin's G Tot E(1), G# Tot E(1), G dev.(1), G# dev.(1), G Tot E(2), G# Tot E(2), G dev.(4) and G# dev.(4) were significantly high compared with other groups. Soyangin's A#S.D.(2) was significantly high compared with Taeyangin group. Taeyangin's A#S.D.(3) was significantly high compared with Taeumin group. Taeyangin's F S.D.(5), F# S.D.(5) and Max Average were significantly high compared with Soeumin group. Taeumin's Peak3 and Peak4 were significantly high compared with Taeyangin group. Taeumin's PeakValue1 was significantly high compared with Soeumin group. Taeyangin's PeakValue2 was significantly high compared with Soeumin group. Taeyangin's PeakValue3 and PeakValue5 were significantly high compared with Other groups. 2) In Under 54 years old group, there were no significant differences among constitutional groups in APQ, Shimmer and Octave segment. Taeumin's Center freq.(2) was significantly high compared with Taeyangin and Soyangin groups. Taeumin's Pitch(2) and Pitch(3) were significantly high compared with Taeyangin and Soeumin groups. Taeyangin's and Taeumin's Pitch S.D. were significantly high compared with Soyangin group. Taeyangin's and Soyangin's Octave2 were significantly high compared with Taeumin group. On the point of Energy, Taeyangin's and Soyangin's A# S.D.(2) were significantly high compared with Soeumin group. Soeumin's G# Shimmer(1) and G# Shimmer(2) were significantly high compared with Soyangin group. Taeyangin's Octave5 and Octave6 were significantly high compared with Soeumin group. On the point of Energy, Soyangin's C S.D., F# S.D.(1), F# S.D.(2) and G dev.(2) were significantly high compared with other groups. Soyangin's F# S.D.(3) was significantly high compared with Taeumin and Soeumin groups. Taeyangin's and Taeumin's G# S.D.(2) and G# S.D.(3) were significantly high compared with Soyangin group 3. Conclusions From above result, there is the possibility of efficient standard guide for constitution diagnosis by analysis of voice

사상 체질별 음성과 건강 수준 관련 가능성에 대한 고찰 (Study on Correlation between Voice and Health Condition in the Sasang Constitution)

  • In this work, we investigate the correlation between health condition and voice to study the validity and value of voice diagnosis. For this purpose, we collected voices, Health index questionnaires (Short form 36, Psychological Well Being Index) and Sasang Constitution informations on 197 males at the age of twenties. Pitch, jitter, shimmer variables were analyzed by ANOVA and Pearson correlation coefficient. There were no significant correlations between pitch, jitter, shimmer and health questionnaire score in total group regardless of Sasang Constitution. However, We found tendency of correlation between shimmer variables and health questionnaire scores in Taeeumin and Soyangin. In Soeumin and Soyangin, zitter and pitch variables were found to be slightly correlated with health questionnaire scores. Our study suggests the possibility that voice might be related with both health condition and Sasang Constitution. Our finding may motivate research activities towards diverse clinical applications of voice diagnosis and studies of voice characteristics in the Sasang constitution.

신생아 울음의 의사소통 의도와 관련된 음향학적 특성 (Acoustic and Physiologic Characteristics of Newborn Infants' Communication Intent via Crying)

  • The purpose of this study was to investigate the acoustic characteristics of crying infants according to the communication intents such as hunger and pain in terms of acoustic differences in the fundamental frequency ($F_0$), jitter, shimmer, noise-to-harmonic ratio(NHR), habitual pitch, and intensity. The subjects were 20 healthy, normal infants, less than seven days old, from the city of Seoul and were born after 38 to 42 weeks(full term) of pregnancy. The sound of crying was recorded for three minutes. The crying due to pain was induced by means of the inborn metabolism error test, whereas the crying due to hunger was verified by means of the rooting reflex by waiting for the designated eating time. The results were as follows: (1) the fundamental frequency, noise-to-harmonic ratio(NHR), and intensity of the infants' crying due to pain was higher than that by hunger, showing a significant difference between the mean values. (2) the infants' crying due to hunger and that by pain did not have a significant difference in the mean jitter and shimmer values but both of them were largely outside of the normal threshold values(jitter by 1.04% and shimmer by 3.81%). This study was significant in the sense that it showed the acoustic characteristics of infants' crying from hunger and pain were very different from each other according to the communication intents in terms of the six acoustic parameters.

심도 청각장애 아동의 발성특성: 강도, 음도 및 그 변동률을 중심으로 (The Phonatory Characteristics of Voice in Profoundly Hearing-Impaired Children: with Reference to F0, Intensity, and their Perturbations)

  • This study investigates the differences in mean F0, intensity, jitter, and shimmer across hearing aid, gender, and vowels. For this study, 18 hearing-impaired children, 18 cochlear implanted children, and 18 normal hearing children as a control group were asked to read seven Korean vowels (/$\alpha$, $\wedge$, o, u, w, i, $\varepsilon$/). Subjects' readings were recorded by NasalView and analyzed by Praat. Results showed that the means of F0 were significantly higher in the hearing impaired group than in the normal hearing group; in the female group than in the male group; and in high vowels than in low vowels. Second, intensity was significantly higher in the hearing impaired group than in the normal hearing group; in the female group than in the male group; and in low vowels than in high vowels. Third, jitter was significantly higher in the normal hearing group than in the hearing impaired group; in the female group than in the male group; and in back vowels than in front vowels. Finally, shimmer was significantly higher in the CI group than in the normal hearing group or the hearing aided group; in the male group than in the female group; and in low vowels than in high vowels.

흡연환자 음성의 음향학적 특성에 관한 연구 (Acoustic Characteristics of the Smoking Patients in the Voice Disorders)

  • Background and Objectives: Smoking has been identified as one of the main determinants of negative changes on the larynx histology. The purpose of this study is to investigate the voice characteristics and correlation between voice parameters of the smokers with vocal polyp or nodule or both of them. Materials and Method: MPT, $F_0$, jitter, shimmer, NHR of Korean /a/ vowel from 54 smokers and 50 nonsmokers diagnosed as vocal polyp or nodule were analyzed. A Computerized Speech Lab (4400) was for the analysis of each voice sample and statistical analysis was done by one-way ANOVA and Pearson correlation coefficient. Result and Conclusion: It showed that we can find difference between smokers and nonsmokers group in MPT, $F_0$, jitter, shimmer were different except NHR. each group shows difference in correlation coefficient between MPT, $F_0$, jitter, shimmer.

음성장애 주관적 평가와 객관적 평가 간의 상관성 연구 (A Study of the Correlation between Subjective and Objective Evaluation of Voice Disorders)

  • The purpose of this study was to examine the relationship between subjective and objective evaluation in speakers with voice disorders. Subjective evaluation indicates the self-reports of voice problems by dysphonic speakers. The relating protocol is the Voice Handicap Index (VHI) and the self-awareness index of voice problems (SAIVP-14). A total of 48 individuals with voice disorders replied to the questionnaire and participated in a voice assessment. Objective evaluations included the perceptual judgement of G grade in GRBAS, acoustic measurements (jitter, shimmer, NHR) by MDVP (CSL 4400), and aerodynamic measurements (MPT, MFR, psub) by PAS (Phonatory Aerodynamic System, KayPentax, USA). Pearson and Spearman correlations were used for the analysis. In the correlation with perceptual judgement (G grade) and VHI-Total, VHI-Physical, and SAIVP-14, there was a significant correlation, but the overall correlation was poor. NHR, jitter, and shimmer were significantly correlated with overall VHI and SAIVP-14. Specifically, the correlation with shimmer was stronger compared to the other measurements. In aerodynamic measures, MFR and MPT showed a significant correlation with VHI-Total, VHI-Emotional, and SAIVP-14, but their correlation was poor. The results of this study suggested that subjective evaluation of self voice problems is meaningfully correlated with objective evaluations, but more data in the multidimensional voice assessment should be collected and analyzed for the reliability and validity of the voice handicap questionnaire.

장음 특징 변수의 회귀 분석을 통한 장 운동성 추정법 (Estimation Algorithm of Bowel Motility Based on Regression Analysis of the Jitter and Shimmer of Bowel Sounds)

  • Bowel sounds (BS) are produced by the movement of the intestinal contents in the lumen of the gastro-intestinal tract during peristalsis and thus, it can be used clinically as useful indicators of bowel motility. We devised an estimation algorithm of bowel motility based on the regression modeling of the jitter and shimmer of BS signals measured by auscultation. Ten healthy males ($23.5\pm2.1$ years) were examined. Consequently, the correlation coefficient, coefficient of determination and standard error between the colon transit times (CTT) measured by a conventional radiograph and the values estimated by our algorithm were 0.98, 0.96 and 2.86, respectively. Also, through k-fold cross validation, the average value of the absolute differences between them was $5.0\pm2.5$ hours. This method could be used as a complementary tool for the non-invasive measurement of bowel motility.

성대 용종 환자의 후두미세수술 전후 음성 평가에서 OperaVOXTM와 Multi-Dimensional Voice Program 간의 신뢰도 연구 (Reliability of OperaVOXTM against Multi-Dimensional Voice Program to Assess Voice Quality before and after Laryngeal Microsurgery in Patient with Vocal Polyp)

  • Background and Objectives OperaVOXTM (Oxford Wave Research Ltd.) is a portable voice analysis software package designed for use with iOS devices. As a relatively cheap, portable and easily accessible form of acoustic analysis, OperaVOXTM may be more clinically useful than laboratory-based software in many situations. The aim of this study was to evaluate the agreement between OperaVOXTM and Multi-Dimensional Voice Program (MDVP; Computerized Speech Lab) to assess voice quality before and after laryngeal microsurgery in patient with vocal polyp. Materials and Method Twenty patients who had undergone laryngeal microsurgery for vocal polyp were enrolled in this study. Preoperative and postoperative voices were assessed by acoustic analysis using MDVP and OperaVOXTM. A five-seconds recording of vowel /a/ was used to measure fundamental frequency (F0), jitter, shimmer and noise-to-harmonic ratio (NHR). Results Several acoustic parameters of MDVP and OperaVOXTM related to short-term variability showed significant improvement. While pre-operative value of F0, jitter, shimmer, NHR was 155.75 Hz (male: 125.37 Hz, female: 183.37 Hz), 2.20%, 6.28%, 0.16, post-operative values of these parameter was 164.34 Hz (male: 129.42 Hz, female: 199.26 Hz), 2.15%, 5.18%, 0.14 Hz in MDVP. While pre-operative value of F0, jitter, shimmer, NHR was 168.26 Hz (male: 135.16 Hz, female: 201.37 Hz), 2.27%, 6.95%, 0.26, post-operative values of these parameters was 162.72 Hz (male: 128.267 Hz, female: 197.18 Hz), 1.71%, 5.36%, 0.20 in OperaVOXTM. There was high intersoftware agreement for F0, jitter, shimmer with intraclass correlation coefficient. Conclusion Our results showed that the short-term variability of acoustic parameters in both MDVP and OperaVOXTM were useful for the objective assessment of voice quality in patients who received laryngeal microsurgery. OperaVOXTM is comparable to MDVP and has high intersoftware reliability with MDVP in measuring the F0, jitter, and shimmer
