Usefulness of Cepstral Peak Prominence (CPP) in Unilateral Vocal Fold Paralysis Dysphonia Evaluation (일측성 성대마비 환자 평가에서 Cepstral Peak Prominence의 유용성)

  • Background and Objectives : The purpose of this study was to compare the usefulness of Cepstral peak prominence (CPP) with parameter of Multiple Dimensional Voice Program (MDVP) in evaluating unilateral vocal fold paraylsis patients with subjective voice impairment. Materials and Methods : From July 2014 to August 2016, 37 patients with unilateral vocal fold paralysis who had been diagnosed with unilateral vocal fold paralysis and had received two or more voice tests before and after the diagnosis were evaluated for maximum phonation time (MPT), MDVP and CPP. Respectively. Voice tests were performed with short vowel /a/ and paragraph reading. Results : The CPP-a (CPP with vowel /a/) and CPP-s (CPP with paragraph reading) of the Cepstrum were statistically negatively correlated with G, R, B, and A before the voice therapy. Jitter, Shimmer, and NHR of MDVP were positively correlated with G, R, B. Jitter, Shimmer, and NHR of the MDVP were significantly correlated with the Cepstrum index. G, B, A and CPP-a and CPP-s showed a statistically significant negative correlation and a somewhat higher correlation coefficient between 0.5 and 0.78. On the other hand, in MDVP index, there was a positive correlation with G and B only with Jitter of 0.4. Conclusion : CPP can be an important evaluation tool in the evaluation of speech in the unilateral vocal cord paralysis when speech energy changes or the cycle is not constant during speech.

Acoustic Outcomes After Laryngomicrosurgery for Reinke's Edema (라인케 부종에서 후두미세수술 후의 음성 결과)

  • Background and Objectives : The management of Reinke's edema includes usually medical treatment and voice therapy. Laryngomicrosurgery (LMS) is also necessary, especially to improve airway obstruction. However, voice outcome after LMS has not been determined well. The aim of this study was to evaluate effectiveness of LMS for Reinke's edema and analyze the voice outcomes after LMS. Materials and Methods : Twenty-five patients with Reinke's edema who underwent LMS from September 2007 to December 2016 were enrolled in this study. We analyzed reflux finding score (RFS), reflux symptom index (RSI), and acoustic parameters before and after surgery. Results : Male was 15 (60%) and female was 10 (40%), and mean age was 49.6 years. Preoperative mean value of RFS decreased significantly up to 3 months after LMS ($18.3{\pm}2.2$ and $10.0{\pm}2.2$ at preoperative and 3 months postoperatively, respectively). The mean value of Jitter decreased significantly before and after surgery ($2.71{\pm}2.81%$ and $1.06{\pm}1.21%$ before and after LMS, p=0.041). The mean value of Shimmer also decreased significantly before and after surgery ($7.97{\pm}3.63%$ and $4.83{\pm}1.85%$, respectively, p=0.006). Conclusion : LMS is effective in the treatment of Reinke's edema. It results in favorable acoustic outcomes and laryngoscopic findings in properly selected patients.

Feasibility of Galaxy Smartphone Recording as Portable Recorder for Acoustic Analysis of Voice (음향분석에 사용할 녹음장비로 갤럭시 스마트폰 녹음기능의 유용성)

  • Background and Objectives : Acoustic analysis of voice could be influenced so much by the quality of voice files which were recorded by recording device. In clinical practice, voice files that were recorded by analysis program directly or portable digital recording device were analyzed mostly. This study examined the feasibility of using Galaxy smartphone recordings for acoustic analysis of voice. Materials and Methods : Acoustic measures were compared between voice signals recorded from 30 normal speakers (15 males and 15 females) through Galaxy smartphone, portable digital recording device and CSL. Fo, jitter, shimmer, NHR (Noise-Harmony ratio) and Formant frequencies were analyzed by MDVP. Results : Fo, Jitter, Shimmer, NHR and formant frequencies from 3 devices were no significantly difference. The intraclass correlation coefficient (ICC) was higher between each of the voice perturbation measures. Conclusion : The findings indicated that Galaxy smartphone recording system was useful device for acoustic analysis of voice. Furthermore, Galaxy smartphone can be applied widely in various way for acoustic analysis of voice.

The Effects of Changing the Respiratory Muscles and Acoustic Parameters on the Children With Spastic Cerebral Palsy (체간 조절을 통한 앉기 자세 교정이 경직형 뇌성마비 아동들의 호흡근과 음향학적 측정치들의 변화에 미치는 효과)

  • The purpose of this study was to investigate the effects postural changes on respiratory muscles and acoustic parameters of the children with spastic cerebral palsy. Nine children with spastic cerebral palsy who required assistance when walking were selected. The ages of the children ranged from 6 to 9 years old. The phonation of the sustained vowel /a/ and the voice qualities of each child such as fundamental frequency($F_0$; Hz), pitch variation (Jitter; %), amplitude variation (Shimmer; %) and noise to harmonic ratio (NHR) were analyzed by Multi-Dimensional Voice Program (MDVP). The muscle activity of three major respiratory muscles: pectoralis major muscle, upper trapezius muscle and rectus abdorminalis muscle, were measured by examining the root mean square (RMS) of the surface EMG to investigate the impact of changes in the adjusted sitting posture of each subject. However, the RMS of pectoralis major muscle showed a significant differences (p<.05). Secondly, there were no significant differences in $F_0$, Jitter and Shimmer between pre and post posture change, but there was a significant difference in NHR (p<.05). The data were collected in each individual; once prior and once after the sitting posture change. The data were analyzed by Wilcoxon signed ranks-test using SPSS version 14.0 for Windows. The findings of this study were as follows; Firstly, the RMS of upper trapezius and rectus abdorminalis muscle were not significant different between pre and post sitting posture changes. From the result, it is concluded that changes in the adjusted sitting posture decreases the abnormal respiratory patterns in the children with spastic cerebral palsy which is characterized by the hyperactivity of the respiratory muscles in breathing. Also, there is increased on the voice qualities in children with spastic cerebral palsy.

  • Vocal nodule is one of the representative chronic diseases of vocal folds, and it can be cured by surgical removal or voice therapy. The aim of this study is to evaluate the effect of the accent method, one of the popular effective voice therapy, in the patients with vocal nodule. Authors executed the accent method in 17 patients with vocal nodule who visited the Voice & Speech Therapy Clinic, Pusan National University Hospital analysed the voice before and after treatment using the local findings, acoustic analysis and aerodynamic analysis MPT. The voice was analysed with MDVP of CSL and MPT was checked using stop watch. The parameters included Fo, Jitter, Shimmer and noise to harmonic ratio(NHR) as acoustic analysis. The results were obtained as follows. In the evaluation by the local findings, it was improved to 77% in the patients of vocal nodule. Jitter and Shimmer were shown to be improved significantly. In particular, it was shown to be improved significantly in patients with vocal nodule. As the result of this study, the improvement of aerodynamic aspect was more statistically significant than that of acoustic parameters. When I generalized the above mentioned results, we suggest that it is a useful voice therapy which can be helpful to the improvement of voice, applying the accent method to the vocal nodule patients, and there are currently many methods to be used in the voice therapy, but it is thought which the accent method is the good treatment as the alternatives of keeping the continuous medical treatment.

Acoustic and Physiological Characteristics of Pre-term and Full-term Infants' Cries (미숙아와 만삭아 울음의 음향 및 생리학적 특성)

  • The purpose of this study is to first discriminate and assess those infants who appear healthy in appearance but who could face possible risk factors in the future and, secondly, to identify those infants who may have difficulties in their developmental stages. The subjects of this study consisted of 35 full-term infants (39-40 weeks) and 33 pre-term infants (34-35 weeks). The infants' voices were recorded for three minutes, for which EDIROL by Roland and a stand-type microphone made by SONY were used. This was done to discern the value of the Breath unit (B-unit) and the fundamental frequencies ($F_0$). It was found that there were significant differences in terms of F0 since the pre-term infants had higher F0 than the full-term infants, showing a result of 436.4 Hz for the full-term infants and 460 Hz for the pre-term infants (p<.05) There was an average rate of 4.01 for the full-term infants and 4.02 (SD=1.69) for the pre-term infants in shimmer. For NHR, it was observed .44 for the full-term infants and .50 for the pre-term infants, thus revealing no significant differences in these observations. This study shows that the crying of newborn babies is related to their physical conditions and it is a sensatory response to these conditions. Furthermore, this study could be helpful for the early detection and measurement of newborn babies who look clinically healthy but could be at risk through acoustic and physiological analyses.

A Study about Voice of Patients with Chronic Obstructive Pulmonary Disease/Asthma before & after ${\beta}_2$-agonist (${\beta}_2$-촉진제 사용전후에 따른 만성폐쇄성폐질환/천식 환자의 음성 연구)

  • An inhaled salbutamol and salmeterol for chronic obstructive pulmonary disease(COPD) and asthma have been used worldwidely. But there has been few study about the voice change evoked from the post-medicine effect. To evaluate the voice influenced of short-acting and long-acting ${\beta}_2$-agonists, two experiments were carried out: one was salbutamol experiment 1 with eight patients, the other was salmeterol experiment 2 with six patients. Experiment 1 was made of two stages: premedication & postmedication. Experiment 2 was four stages: stageI was premedication, stageII was postmedication & pregaggling, stageIII was postmedication & postgaggling(100 ml with water), and stageIV was postmedication & 30 minutes later. Measured parameters were F0, F0_SD, Jitter_rap, Shimmer_apq11, HNR, BW(1, 2, 3), Intensity, and H1-H2. The mean data collected from 3 repetitions each was statistically analyzed by Wilcoxon signed rank test for experiment 1 and repeated measures ANOVA for experiment 2. In experiment 1, significant differences were found in the Jitter_rap(Z= -2.10, p=0.036). The findings indicated that the postmedicated voice was worse than premedicated voice. In experiment 2, there wasn't significant difference, but values of parameters related to voice quality(Jitter_rap, Shimmer_apq11, HNR, and H1-H2) showed changes toward stageⅣ, that is, the voice quality was worse under medication.

  • One of the most important means in modern NQ emphasized smart society is the communication skill. Especially, effects on improving pronunciation accuracy, it is mostly necessary to accurately express his or her own idea due to the personal relation influence 38% of voice. For this, this paper proposed the voice influence analysis of carbonate drink and eucalyptus fragrance. In particular, in the case of carbonate drink, the amounts of drinking accumulation is verified for analysing the drinking accumulation influence. Also, eucalyptus fragrance is reported for influencing the pronunciation accuracy. For this, jitter, shimmer, pitch and intensity of voice is analyzed. Finally, we accomplish an voice analysis of quantization, objective and visualization for such carbonate drink and eucalyptus fragrance.

Efficacy of laughing voice treatment (SKMVTT) in benign vocal fold lesions (양성성대질환의 웃음 음성치료(SKMVTT))

  • The purpose of this study was to evaluate the efficacy of a multiple voice therapy technique ($SKMVTT^{(R)}$) using laughter for the treatment of various benign vocal fold lesions. To achieve this, 23 female patients diagnosed with vocal nodules, vocal polyp, and muscle tension dysphonia through videostroboscopy were enrolled in vocal hygiene and $SKMVTT^{(R)}$. All of the patients were treated once a week for 4 to 12 sessions. The GRBAS scale was used to confirm the changes in voice quality before and after the treatment. Acoustic analysis was performed to evaluate jitter, shimmer, NHR, fundamental frequency variation, amplitude variation, PFR, and dB range. Videostroboscopy was performed to confirm the changes in the laryngeal features before and after the treatment. After the $SKMVTT^{(R)}$, the results of the perceptual evaluation demonstrated that the G, R, and B scales significantly improved. An acoustic evaluation also demonstrated that jitter, shimmer, NHR, vAm, vFo, PFR, and dB range also significantly improved after the $SKMVTT^{(R)}$. In comparison to the videostroboscopic findings, the size of the vocal nodules and vocal polyp decreased or disappeared after the treatment. In addition, the size of the cuneiform tubercles decreased, the length of the aryepiglottic folds became longer, and the laryngeal findings of the supraglottic compressions improved after the $SKMVTT^{(R)}$. These results suggest that the $SKMVTT^{(R)}$ is effective in improving the vocal quality of patients with benign vocal fold lesions. In conclusion, it seems that laughter and inspiratory phonation suppressed abnormal laryngeal elevation and lowered laryngeal height, which seems to have the effect of improving hyperfunctional phonation.

  • The purpose of this study was to compare voice parameters in auditory conditions (normal/noise/music) between a teacher group and a control group. Results of statistical analysis showed that the teacher group had higher jitter (%) and shimmer (%) values than the control group. It indicated that the teacher group had larger variations in pitch and dynamic of their voice. In the teacher group, the voice under noisy condition showed a higher value of fundamental frequency than that under normal condition. though its fundamental frequency did not show any significant difference between the noisy condition and the musical condition. In the control group, however, although the voice under noisy condition also showed a higher value of fundamental frequency than that under normal condition, its fundamental frequency was significantly different between the noisy condition and the musical condition.
