Extramedullary Plasmacytoma on Unilateral Upper Eyelid in a Shih-Tzu Dog

  • A 9-year-old spayed female Shih-Tzu was presented with a mass on the upper eyelid of the right eye (OD). The eyelid mass continued to enlarge along the eyelid margin and the eyelid skin. Throughout the follow-up examinations, the mass did not affect the palpebral conjunctiva OD. Until 9 months since the mass was first identified, the dog did not show any clinical signs related to the mass. However, after 11 months, the owner complained of the dog's discomfort on OD and decided on surgical excision of the mass. There were no abnormalities in the complete blood count and serum chemistry profiles. The pentagonal resection was performed to completely remove the mass in the eyelid. There were no suspicious findings of metastasis on whole-body computed tomography scan immediately before the surgery. On histopathology and immunohistochemistry for multiple myeloma oncogene-1, the eyelid mass was diagnosed as extramedullary plasmacytoma (EMP). Surgical margin of the mass was clearly cut. No recurrence was observed during the 5-month follow-up. EMPs on the eyelid have rarely been reported in dogs. Although EMP is a benign tumor, the mass showed invasive growth and grew toward the skin rather than the palpebral conjunctiva. No recurrence occurred when surgically removed with clean margins.

Surgical Management of Urethral Plugs in a Male Shih-Tzu Dog (수컷 시츄견에서 발생한 유레쓰랄 플러그의 수술적 관리)

  • A 6-year-old castrated male Shih-Tzu was admitted to the Gyeongsang Animal Medical Center with hematuria and vomiting. Serum chemistry indicated increases in blood urea nitrogen and creatinine values. Radiography showed radiopaque materials attached to the ventralis of the inner bladder wall. Penile urethra also was filled with radiopaque materials. Ultrasonography revealed bilateral perineal effusion in the kidney, thinned bladder wall, and materials with acoustic shadowing at the caudal extremity of the os penis. Because urination was not identified by urinary obstruction, cystocentesis was performed. Hematuria was diagnosed by urinalysis, but clear crystals were not identified. Owing to failure of securing patency to the bladder using retrohydropropulsion, cystotomy and urethrotomy with retrohydropropulsion were performed. In this report, we describe the clinical and radiographic findings of urethral plugs in a male Shih-Tzu dog, wherein the urethral plugs were treated successfully with hydropropulsion, cystotomy, and urethrotomy. It provides an option for the treatment of urethral plugs in a dog.

  • A 5-year old female shih-tzu referred to the veterinary Teaching Hospital, College of Veterinary Medicine, Konkuk University. The patient was presented with a mass of ventral to pubic region and intraperineal region. CBC, chemistry, x-ray, fine needle aspiration, biopsy and MRI was performed. The dog was definitively diagnosed as lipoma based on cytologic and histopathologic examination. In order to remove the mass, an approach with two direction was made. First, intraperineal region was removed about 40%, and then ventral to pubic was removed the remainder. And inserted penrose drain for obliterate dead space to prevent the accumulation of serous discharge. The dog is clinically normal and being monitored for development of any abnormal signs for the time being. In this case, diagnostic MRI was very useful procedure. it revealed accurate location of a lipoma and useful manner for decision of surgical approach to remove.

  • A 3-year-old, intact female, Shih-tzu dog was presented with a 15-day history of vomiting,depression, and anorexia. On physical examination, systolic ejection murmurs with precordial thril atthe left heart base were detected. A diagnosis of congenital pulmonic stenosis (PS) was made mainlyfrom the thoracic radiography, electrocardiography, and echocardiography. On complete blood counts andconfirmed that main pulmonary artery was tremendously buldged and electrocardiography was suggestiveof severe right ventricular hypertrophy. Echocardiographic findings revealed the pulmonic valve stenosiscontaining valvular dysplasia and poststenotic dilation. On Doppler echocardiography, ejection velocityof the lesion accounted for 3.38 m/sec, meaning mild velocity through the stenotic area. The dog'sproblem and resulted in death. However, there has been no reliable relation between PS and CRF. Primarymalformation of pulmonic valve was confirmed at necropsy after death.

Gastric Dilatation and Volvulus in Shih-tzu (시츄에서 발견된 위확대염전)

  • Gastric dilatation and volvulus (GDV) is an acute and life-threatening disease most commonly affecting large- and giant-breed dogs. However a 17-year-old Shih-tzu (4 kg, spayed female) was hospitalized for acute GDV. Repeated unproductive retching, lethargy, and excessively enlarged abdomen were observed. Physical examination indicated that the patient had suffered from hypothermia ($36.5^{\circ}C$), tachycardia (240 bpm), slowed capillary refill time (> 2 sec.), and pale mucous membrane. Grade III murmur with normal lung sound was auscultated. Abdominal palpation revealed that tympanic regions existed in both the left and right sides. Systolic blood pressure decreased gradually from 220 to 40 mmHg within 4 hours. In blood analysis, slight azotemia was observed by blood urea nitrogen (BUN; 29.1 mg/dl) and creatinine (1.6 mg/dl). Blood lactate concentration (8.13 mmol/l) was severely elevated. Additionally, dilatation and volvulus of the stomach was observed by radiograph. Supportive oxygen, heat, fluid, and drugs were administered with gastric decompressions (e.g., gastrocentesis and nasogastric tube). However the patient entered into comatose status with uncontrollable systolic blood pressure, despite the administration of dobutamine intravenously. The case was closed by euthanasia, considering welfare and age. We finally diagnosed the patient as a GDV, thus this is the first GDV case report in small-breed dog such as Shih-tzu.

  • One nine-minth-old female crossbred dog of Maltese and Shih-tzu with chief complaints of left hindlimb lameness and pain was diagnosed as Legg-Calve-Perthes disease(LCPD) in radiographic examination. Femoral head and neck ostectomy for the treatment of LCPD obtained effective result.

  • A castrated male, 6 year-old Shih-Tzu, weighing 4.6 kg with a discharge from the external ear canal and a mal-odor was referred to Verterinary Medical Teaching Hospital, Seoul National University. Radiographic findings were narrowing and focal loss of an air density in the left ear canal and bilateral thickening of the osseous bullae. For more detail examination, computed tomography was used. In computed tomographic findings, bilateral thickening of the bulla wall and increased opacity of the bulla space were observed. Thus this dog was diagnosed as bilateral otitis media. Then surgical correction, total ear canal ablation, was performed successfully and the patient showed a good prognosis thereafter. It is considered that computed tomography might serve as an useful imaging tool for otitis media and that it supplies information not obtained with a conventional radiography.

  • A 5-year-old castrated male Shih-tzu dog and a 9-year-old intact female Schnauzer dog were presented with anorexia and depression. These 2 dogs were diagnosed based on which criteria as primary immune-mediated hemolytic anemia (IMHA). Blood examination showed anemia, spherocytosis, auto-agglutination and total bilirubinemia in 2 cases. These dogs were recovered by treatment of mycophenolate mofetil and prednisolone, and showed good prognosis until now. Mycophenolate mofetil is recommended as initial treatment in canine IMHA.

  • A male, 11-month-old, Shih-tzu dog weighing 5.2kg showing knuckling secondary to severe cranial bowing of left antebrachium with valgus deformity of the carpus was presented to the Veterinary Medicine Teaching Hospital of Seoul National University. Radiographic findings were a premature closure of the distal ulnar physis, a cranial bowing of the radius, subluxation of the elbow joint, and degenerative changes in the carpal joint. The patient was treated by performing an oblique osteotomy of the radius and ulna, followed by application of External Skeletal Fixations. At 18 weeks postoperatively, the radius was healed. An incongruency of the elbow joint and radial angular deformity was corrected.

Immune Mediated Hemolytic Anemia in a Dog (개에 있어서 면역매개성 용혈성빈혈 일례)

  • A 3-year-old, 4.0 kg, intact male shih-tzu dog with anorexia, depression, pale mucous membranes, tachypnea, tachycardia was referred to the Veterinary Medical Teaching Hospital. Autoagglutination was observed by naked eye when blood was collected in an EDTA-tube and many spherocytes were found on a Diff-Quik stained blood smear. PCV was 6% and indirect bilirubin was increased markedly. So the immune-mediated hemolytic anemia was diagnosed. Autoagglutination was too severe to perform cross-matching test. Blood was not transfused as it might accelerate or precipitate hemolytic crisis, and regeneration of erythrocytes was very good. Thus corticosteroid of immunosuppressive dose and fluid were administered and PCV was monitored. Although blood was not transfused, PCV increased from 6 to 15.9% in a day and to 30% 7 days later. Therapy for liver was concurrently conducted because liver enzyme activities were high. Corticosteroid tappering therapy was conducted for 75 days and PCV was recovered to 46% after 4 months form start of the treatment.
