근무형태별 간호사의 스트레스와 불안 조사: 타액 코티솔과 자가보고 척도를 이용한 탐색적 연구 (An Exploratory Study on Occupational Stress and Anxiety Through Salivary Cortisol and Self-Report Scale in Korean Nurses on Shift and Regular Work)

  • Purpose: The purpose of the study was to compare salivary cortisol levels, self-reported occupational stress, and anxiety before duty to those after duty in the shift and regular work nurses, and the relationships among them. Methods: Thirty nurses working in a tertiary hospital participated in the study. Salivary specimens were collected before and after daytime duty for 2 days. The occupational stress and state anxiety were also evaluated using self-report questionnaires. Results: The average level of salivary cortisol was $0.40{\mu}g/dL$ before duty and $0.20{\mu}g/dL$ after duty in Korean nurses. Overall levels of salivary cortisol, self-reported occupational stress, and anxiety were higher in shift working nurses than regular working nurses. In shift working nurses, the salivary cortisol gap between before and after duty was attenuated on the first day of daytime duty. Significant positive correlation was found between the level of cortisol before duty and anxiety after duty. Conclusion: Strategies to alleviate occupational stress and anxiety are needed for shift working nurses, especially for those with elevated levels of cortisol. It seems to require 3days to recover from attenuated cortisol rhythm during nighttime duty in shift working nurses.

교대근무 형태에 따른 대학병원 간호사의 수면 질, 피로, 우울 및 건강증진 행위 간의 관계 (Relationships between Sleep Quality, Fatigue and Depression on Health Promoting Behavior by Shift-Work Patterns in University Hospital Nurses)

  • Purpose: The objective of this study was to investigate the relationships between sleep quality, fatigue, depression and health promoting behavior by shift-work patterns in university hospital nurses. Methods: A cross-sectional, descriptive survey was conducted using a convenient sample. The self-administered questionnaires were given to 165 nurses (shift workers 91, non-shift workers 74) employed in two university hospital located in Seoul and Gyeunggi-do. Data were collected from February to March, 2010. Results: The shift workers showed significantly lower sleep quality and health promoting behavior, and higher fatigue than non-shift workers. There were significant correlations between fatigue, depression and health promoting behavior in shift workers. There were significant correlations between sleep quality, fatigue, depression and health promoting behavior in non-shift workers. In multiple linear regression analysis, after controlling for total work experiences, predictor of health promoting behavior in shift workers was only fatigue (${\beta}$=-.29, p<.001). Predictors of health promotion behavior in non-shift workers were depression (${\beta}$=-.37, p=.002) and fatigue (${\beta}$=-.27, p=.023). Conclusion: This study failed to provide a clear explanation why sleep quality does not have a comparable effect on health promotion behaviors of shift-workers as well as non-shift workers. However, future research is needed to determine the various risk factors influencing on health promotion behaviors by which shift-work pattern exacerbates fatigue.

교대근무 간호사의 효율적인 근무형태 개발을 위한 실태조사 (A Study for Developing the Effective Working Pattern for Nurses in Shift Work)

  • Purpose: This study was conducted to provide baseline data to develop work patterns for nurses that are suitable to Korean hospitals. Shift work patterns and nurses' preferences for working condition were examined. Methods: Questionnaires for nurse departments and nurses were mailed to 478 hospitals from June 1 to July 30, 2012. Nurse departments of 174 hospitals and 1,409 nurses responded to the questionnaire. Data were analyzed using SPSS statistical program. Results: Of the nurses, 89.4% were working three shifts. Of the hospitals, 17.3% had implemented fixed night work. Although 80% of nurses preferred fixed time work, only 24.9% of hospitals had fixed working hours. Of the nurses, 57% agreed with night work as fixed but potential night nurses accounted for only 17% of the respondents. Sixty-three percent of the nurses agreed on short time work during busy hours. There were preference differences according to age, marital status and clinical career. Conclusion: As many nurses reported a preference for various flexible working patterns rather than three shift work, there is a need for new decisions on hospital policy to ensure improved working conditions for nurses and enhanced job security. Legal requirements also need to be considered for various work patterns.

이부교대(二部交代) 근무제도(勤務制度)에 대한 임상간호사(臨床看護師)의 요구도(要求度)에 관한 연구(硏究) (A Study on the Need of Clinical Nurses about the System of 12-Hour Work Shift)

  • The 12-hour work shift can be influenced on the nurse's job satisfaction, effective personnel administration, and quality of patient care. The purpose of this study was to explore the perception on the 12-hour work shift by nurses. A total of 516 nurses were selected by convenience sampling from six general hospitals in Seoul. Based on the literature review, the questionnaire on the need of the 12-hour work shift was designed by the investigator. The data were analyzed using descriptive statistics, ANOVA and Duncan's method as post-hoc test. The results of this study are as follows ; 1. The need of 12-hour work shift The mean needs of 12-hour work shift in the nursing practice, psychological, physical, socioeconomic, and environmental perspectives were 3.05, 2.72, 2.66, 3.08, and 4.22. 2. The need of 12-hour work shift by demographic data For the nursing practice, there was a statistically significant relationship between marital status(p=.021), satisfaction on the 8-hour work shift(p=.038), the perception on the 12-hour work shift(p=.001) and the need of 12-hour work shift. For the psychological perspectives, there was a statistically significant relationship between marital status(p=.016), the perception on the 12-hour work shift(p=.0001), and the need of 12-hour work shift. For the physical perspectives, there was a statistically significant relationship between age(p=.002), marital status(p=.005), the number of children(p=.005), the duration of job career(p=.014), the job position(p=.002), the work shift(p=.030), the perception on the 12-hour work shift(p=.0001) and the need of 12-hour work shift. For the socioeconomic perspectives, there was a statistically significant relationship between the job position(0=.002), the work shift(p=.006), the perception of the 12-hour work shift(p=.002) and the need of 12-hour work shift. For the environmental perspectives, there was a statisitically significant relationship between the traffic method(p=.05), the duration of the job career(p=.026), the perception on the 12-hour work shift(p=.309) and the need of 12-hour work shift. 3. The need of 12-hour work shift by the demographic data There was a statistically significant difference between marital status(p=.014), the number of children(p=.038), the job position(p=.007), the work shift(p=.037), and the perception on the 12-hour work shift(p=.0001) for the need of 12-hour work shift.

중환자실 간호사의 2교대와 3교대근무 간 근무 만족도, 수면, 피로, 삶의 질과 환자안전사고 비교 (Comparison of Shift Satisfaction, Sleep, Fatigue, Quality of Life, and Patient Safety Incidents Between Two-Shift and Three-Shift Intensive Care Unit Nurses)

  • Purpose : The aim of this study was to compare shift satisfaction, sleep, fatigue, quality of life (QOL), and patient safety incidents between a newly implemented two-shift system and a traditional three-shift system. Methods : A total of 127 intensive care unit nurses (48 two-shift nurses and 79 three-shift nurses) working in a tertiary hospital in Seoul were recruited from January 1, 2017, to March 31, 2017. They completed a self-reported questionnaire about their work hours, shift satisfaction, sleep patterns, sleep quality, fatigue, QOL, and patient safety incidents in the past 2 weeks. Data were analyzed using SPSS version 23.0. Results : The two-shift group showed higher shift satisfaction scores compared with the three-shift group (6.93 vs. 4.37, p<.001). Sleep latency was shorter and sleep quality was better in the two-shift group compared with the three-shift group. There were no significant differences in other sleep parameters, fatigue, QOL, and patient safety incidents between the two groups. Conclusion : Although a two-shift system did not improve nurses' fatigue or QOL in this study, it may effectively serve as an alternative shift-work system that can increase sleep quality and shift satisfaction without increasing patient safety incidents.

Relationship Between Shift Intensity and Insomnia Among Hospital Nurses in Korea: A Cross-sectional Study

  • Objectives: This study explored the relationship between shift intensity and insomnia among hospital nurses. Methods: The participants were 386 female hospital nurses who underwent a special health examination for night workers in 2015. The Korean Insomnia Severity Index (ISI), indices of shift work intensity, and other covariates such as amount of exercise, level of alcohol consumption, employment duration, and hours worked were extracted from the health examination data. The indices for shift intensity were (1) number of 3 consecutive night shifts and (2) number of short recovery periods after a previous shift, both assessed over the prior 3 months. Multiple logistic regression analysis adjusted for the aforementioned covariates was performed to evaluate the association of shift intensity with insomnia, defined as an ISI score of ≥8. Results: The nurses with insomnia tended to be younger (p=0.029), to have worked 3 consecutive night shifts more frequently (p<0.001), to have experienced a greater number of short recovery periods after the previous shift (p=0.021), and to have worked for more hours (p=0.006) than the nurses without insomnia. Among the other variables, no statistically significant differences between groups were observed. Experiences of 3 or more consecutive night shifts (odds ratio [OR], 2.33; 95% confidence interval [CI], 1.29 to 4.20) and 3 or more short recovery periods (OR, 2.01; 95% CI, 1.08 to 3.73) were associated with increased odds of insomnia. Conclusions: The results suggest that decreasing the shift intensity may reduce insomnia among hospital nurses working rotating shifts.

간호사의 야간전담 및 유연근무제도 정착을 위한 실태 조사 (Study on the Establishment of Fixed Night Shifts and Flexible Work Arrangements for Nurses)

  • Purpose: The purpose of this study was to investigate the welfare status of nurses and the status of fixed night shifts and flexible work arrangements in both general hospitals and advanced general hospitals in Korea. Methods: A total of 167 hospitals participated in this study, done from April to October 2018. Results: There were 103 hospitals utilizing the fixed night shift system, 22 in the two-shift system, 3 in the fixed holiday shift, 79 in the fixed shift system, and 39 in the short-time working system. The average welfare benefits for nurses increased to 33.02 million won on average. However, welfare services for the nurses were poor: 41.8% of the respondents had a nurses' lounge, 81.7% were given dormitory space at their place of employment, 41.8% had access to welfare facilities, and 56.4% were provided with welfare support countermeasures. Conclusion: The results show an increasing trend of hospitals to utilize fixed night shift and flexible work arrangements. An institutional strategy at the national policy level should be established to implement flexible work arrangements including the fixed night shift system, which has a poor level of welfare.

교대근무 기혼여성 간호사의 일-가정 양립 경험 (Shift-Working Married Female Nurses' Experience of Work-Family Balance)

  • 본 연구는 교대근무 기혼여성 간호사의 일과 가정, 일상생활에 대한 생생한 경험과 교대근무의 특수한 상황적 맥락을 파악하는 것이다. 이를 위해 교대근무 기혼여성 간호사 10명을 대상으로 이들의 심층적인 내면을 탐색하기 위해 면담을 진행하였으며, 수집된 자료는 Giorgi의 현상학적 연구 방법을 적용하여 분석하였다. 분석 결과, 총 120개의 의미 단위와 16개의 하위 구성 요소, 5개의 구성 요소가 도출되었다. 5개의 구성 요소는 '교대근무로 인한 일-가정양립 현실 인식', '일-가정 양립 어려움', '일-가정 양립 원동력', '직무수행에서의 만족감 요소', '해결되어야 할 과제들'이다. 본 연구는 교대근무 기혼여성 간호사의 일-가정 양립 경험의 심층적 탐색을 통해 이들이 직면한 상황을 파악하여 교대근무 기혼여성 간호사를 위한 일-가정 양립에 정교한 개선 전략 수립에 대한 경험적 근거를 제시하였다는 점에서 의의가 있다.

병원간호사의 근무형태와 주간수면과다증에 따른 직무몰입 (A Study on Job Involvement according to Working Pattern and Daytime Sleepiness among Hospital Nurses)

  • Purpose: The purpose of this study was to examine job involvement according to working pattern and daytime sleepiness in hospital nurses. Methods: At 2 hospitals in affiliation of university, after obtaining participant's consent form, data were collected from October to November, 2007. Twohundred fifty nurses participated in the study. Questionnaire consisted of Epworth Sleepiness Scale (ESS), Job Involvement. Collected data was analyzed with SPSS 14.0 program, which was used for frequency, percentage, mean, standard deviation, t-test and Pearson correlation coefficients. Results: Major findings of this study were as follows 1) The nurses for 3 shift work was 172 (68.8%), the nurses for 2 shift work was 3 (1.2%) and the nurses for day fixation was 75 (30.0%). 2) Mean of ESS was 5.94 (3.28), daytime sleepiness was 13.2% and Job involvement had a mean of $21.27.{\pm}4.61.3$) There were significant differences between shift work and day fixed work on ESS (t=4.33, p<.001), job involvement (t=6.54, p<.001). Higher ESS were significantly related to lower job involvement (r=-.185, p=.003). Conclusion: The finding of this study gives useful informations about sleep and work involvement of hospital nurses. It is need to develop systemic management for shift work nurses by hospital, nurse organization, and government.

Association of Grit and Body Composition with Fatigue and Burnout among Shift-work Nurses

  • Purpose: This study aimed to investigate the effects of grit and body composition on fatigue and burnout in shift-working nurses. Methods: A descriptive cross-sectional design using self-report questionnaires was employed. Data were collected between February and April 2021 from 192 shift-working nurses in 22 units of C tertiary hospitals. Of the 192, 175 nurses returned their completed questionnaires (return rate: 91.1%). The participants objectively measured their body composition for three consecutive days using a home body composition measurement scale. Results: Nurses with higher consistency of interest were more likely to have lower chronic fatigue (B = -5.23, p= .013), lower emotional exhaustion (B = -2.75, p< .001), and decreased depersonalization (B = -1.08, p= .014). Perseverance of effort was not statistically significant for fatigue; however, it was statistically significant for higher personal accomplishment among the subdomains of burnout (B= 2.50, p< .001). Skeletal muscle mass and body mass index had no significant effect on fatigue and burnout. Conclusion: To reduce fatigue and burnout in shift-working nurses, comprehensive efforts at the organizational and individual levels should be implemented to increase their grit. Further research is needed to determine whether body composition affects fatigue and burnout in shift-working nurses.
