Influences of Hydrocortisone, DHEA, Estradiol and Testosterone on the Polyamine Metabolism of Mouse Brain, Kidney, Liver and Intestine (Glucocorticoid 및 성(性) Steroid 홀몬에 의한 뇌(腦) 및 복부내(腹部內) 장기(臟器)의 Polyamine 대사(代謝)의 변동(變動)에 관한 연구(硏究))

  • The bilateral castration of male mice was operated under light ether anesthesia, and the sham operated mice were considered as the uncastrated. The treatments of mice with the following steroids were started one hour after operation. Hydrocortisone 50 mg/kg (HC), dehydroepiandrosterone 250 mg/kg (DHEA), ${\beta}-estradiol$ 5 mg/kg (E2), and testosterone 20mg/kg (TS) were subcutaneously injected into male ICR mice at noon for four days. Animals were sacrificed in the next-morning (at 10-12 A.M.) after the last injection. The intestinal putrescine(PT) content was lower and the liver and intestinal spermine(SM) contents were higher in castrated mice(CM), comparing with those of uncastrated mice (UCM). The intestinal PT content of UCM was markedly increased HC. But all brain polyamines of CM were significantly decreased by it. And HC also increased the spermidine(SD) content of kidney and liver and the intestinal PT content in CM. E2 induced the marked increase of liver PT content with the moderate increase of renal SD in UCM. And E2 significantly increased the brain and liver PT contents and the all renal polyamine contents in CM. Both of DHEA and TS induced the increase of renal PT content in UCM, and they also induced the marked increases of all renal polyamines of CM. In addition, TS increased the brain SM of CM. These results suggest that the steroidal regulation mechanism of brain, kidney, liver, and intestine seems to be different from one another, and the renal activity of polyamine synthesis can be markedly enhanced by sex steroids.

Establishment of a library of fragments for the rapid and reliable determination of anabolic steroids by liquid chromatography-quadrupole time of flight-mass spectrometry

  • Anabolic steroids have similar structures to testosterone, both of which promote the growth of muscle mass and increase strength. However, the side effects of anabolic steroid use may lead to heart attacks or strokes. Additionally, the excessive use of steroids inhibits the production of the sex hormones in the body via a negative feedback loop, which results in testicular atrophy in males and amenorrhea in females. Currently, the method of choice used to test for the presence of anabolic steroids is GC-MS. However, GC-MS methods require chemical derivatization of the steroid sample to ensure compatibility with the analytical method; therefore, analysis of many different samples is difficult and time consuming. Unlike GC-MS, the liquid chromatography-quadrupole-time of flight mass spectrometry (LC-Q-TOF-MS) method is suitable for many samples. Twenty-two different anabolic steroids were analyzed by LC-Q-TOF-MS with various collision energies (CE). Accurate mass spectral data were obtained using a Q-TOF-MS equipped with an electro-spray ionization source and operated in the positive MS/MS mode for several classes of steroids that are often the targets of testing. Based on the collected data, fragmentation pathways were carefully elucidated. The high selectivity and sensitivity of the LC-Q-TOF-MS instrument combined with these fragmentation pathways offers a new approach for the rapid and accurate screening of anabolic steroids. The obtained data from the 22 different anabolic steroids will be shared with the scientific community in order to establish a library to aid in the screening of illegal anabolic steroids.

Treatment Results of Vocal Process Granuloma: Intubation Versus Contact Granuloma (삽관성 육아종과 접촉성 육아종에 대한 치료 결과 분석)

  • Background and Objectives Vocal process granulomas (VPGs) are benign lesions of the larynx, typically contact granulomas (CG) and intubation granulomas (IG). The two diseases are known to have different clinical manifestations despite having the same pathological features. The purpose of this study was to analyze the treatment results for CG and IG and to obtain clinical information. Materials and Method We retrospectively reviewed the medical records of patients diagnosed with VPG between January 2015 and December 2018. The patient's age, sex, medical history, lesion size, lesion type, reflux finding score, response to treatment, duration of treatment, and follow-up period were compared. Results In total, 32 patients were included in the study, of which 18 were CG and 14 were IG. In the CG group, males were dominant (n=15, 83.3%), whereas in the IG group, females were dominant (n=11, 78.6%) (p=0.0009). The response to medical treatment using proton pump inhibitor and steroid inhaler was better in the IG group (11/14, 78.6%) than in the CG group (7/18, 38.9%) (p=0.036). Of the 14 patients who did not respond to medical treatment, 5 received botulium toxin injections, and all 5 had complete remission. The duration of medical treatment was significantly longer in the IG group (p=0.0029). Conclusion IG was more common in female, and CG was more dominant in male. IG had better response to medical treatment using proton pump inhibitor and steroid inhaler than CG.

Role of Endocrine Disrupting Chemicals in the Occurrence of Benign Uterine Leiomyomata: Special Emphasis on AhR Tissue Levels

  • Although benign uterine leiomyomata (LMA) is the most common reproductive tumor in premenopausal women, its etiology is largely unknown. We aimed in the present study to demonstrate the potential role of environmental factors with estrogenic activity in tumor etiology by focusing on the role of aryl hydrocarbon receptor (AhR) which mediates the effects of many environmental endocrine disruptors and contributes to the loss of normal ovarian function in polluted environments. This case-control study aimed to compare the interactions between AhR and lifestyle factors in a clinical setting for the first time among 138 newly diagnosed LMA patients and 138 normal controls who lived in Tehran and Mashhad, respectively, during the last 10 years. To conduct immunohistochemical studies using appropriate monoclonal antibodies, 30 cases were selected retrospectively from 2009-2011 from the pathology departments of two university hospitals in Tehran. Although the levels of sex steroid receptors were similar in adjacent myometrium and uterine leiomyomas of all cases, AhR was significantly overexpressed (p=0.034, OR=1.667) in uterine LMA and this overexpression was correlated with living in Tehran [(p=0.04, OR=16 (1.216-210.58)], smoking[P=0.04, OR=2.085 (1.29-3.371)], living near polycyclic aromative hydrocarbon producing companies [p=0.007, OR=2.22 (1.256-3.926)] and eating grilled meat [p=0.042, OR=1.28 (1.92-3.842)]. Our study contributes to the understanding of the effects of EDCs on AhR levels as well as women's health and points out possible risk factors for the development and growth of uterine LMA. It seems that the development of LMA could be the result of interactions between hormonal and environmental factors.

Androgen Hormone Inhibits Expression of iNOS and COX-2 Protein in Rat Vascular Smooth Muscle Cell

  • We investigated the effects of testosterone and dihydrotestosterone on inflammatory response of iNOS and COX-2 expression in rat vascular smooth muscle cells. Rat vascular smooth muscle cells (VSMC) stimulated with bacterial lipopolysaccharide $(LPS;\;10{\mu}g/ml)$ for 24 hours were incubated with increasing amounts of testosterone and dihydrotestosterone (1 and 100 nM). LPS was found to induce inflammatory response of iNOS and COX-2 mRNA and protein in VSMC. These processes were affected by male sex steroid hormones. For 3 hours, however, pretreatment of the cells with 100 nM each of testosterone and dihydrotestosterone suppressed LPS induced iNOS and COX-2 protein expression. RT-PCR analysis revealed that testosterone and dihydrotestosterone did not inhibit mRNA expression of iNOS and COX-2 stimulated by 24 hours of LPS incubation. Proliferation rate was slower in VSMC treated with testosterone and dihydrotestosterone. Testosterone enhanced androgen receptor expression, and LPS significantly reduced androgen receptor protein expression in VSMC. These results indicate that the expression of both iNOS and COX-2 proteins was suppressed by testosterone and dihydrotestosterone in LPS stimulated VSMC and leading to reduction of vascular inflammation.

수컷 흰쥐 생식기관에서의 5-HT 수용체 아형의 유전자 발현과 조절

  • Serotonin(5-hydroxytriptamine, 5-HT)은 biogenic amlne류 신경전달물질로써, 다양한 생리조절활성을 갖고있다. 생식과 관련된 5-HT 기능으로 최근 사정 기능의 조절 가능성이 제시되었는데, 항우울제로 흔히 사용되는 selective serotonin reuptake inhibitor(SSRI) 계 약물을 장기 투여할 때 Premature ejaculation이 개선된다는 임상적인 증거들이 보고되었다. 본 연구는 수컷 흰쥐를 사용하여 생식기관, 특히 사정과 관계되는 기관들에서의 5-HT 수용체 아형들의 유전자 발현 여부와 그 조절 기작을 조사하였다. 흰쥐 수컷의 생식장기들인 고환, 부정소, 정관, 정낭에서 사정현상에 관여하리라 추정되는 세로토닌 수용체 아형들(type 1A, 1B, 2C)의 유전자 발현을 RT-PCR과 Southern blot으로 확인하였다. SSRI(sertraline)을 흰쥐에 매일 투여하는 모델(25mg/개체, 2주간)에서 1A 아형의 발현의 경우 정낭에서는 감소하였으나 정관에서는 증가하였고, 1B 아형의 발현은 두 장기에서 공히 증가하였다. 고환 제거후 testosterone(T) 보충 실험 모델을 사용한 실험에서, 정낭에서의 1A와 1B 발현은 T 보충에 의해 감소하였고, 정관에서는 큰 변화가 없었다. 한편 고환, 정낭과 정관에서의 세로토닌 수용체 아형 1A와 1B의 발현은 사춘기의 개시와 함께 증가하였다가 이후 점차 감소하는 경향을 보였다. 본 연구 결과는 사정 현상에 있어서 말초성 세로토닌 시스템이 중요한 역할을 담당할 가능성을 시사하는 것으로써, (i) 고등 포유동물에서의 사정 기작의 조절에 대한 과학적인 이해를 증진시키고, (ⅱ) 세로토닌 수용체 아형간의 특이한 발현과 작용에 대한 이해를 통해 보다 효과적인 사정 부전 치료법 개발을 시도할 수있고, (ⅲ) ontogeny와 sex steroid 의존성에 관련된 연구 시도는 노화와 관련된 사정능력의 변화와 같은 남성과학 분야로의 접목을 기할 수 있다고 사료된다.

Identification of Genes Differentially Expressed in the MCF-7 Cells Treated with Mitogenic Estrogens

  • Estrogens, a group of steroid compounds functioning as the primary female sex hormone, play an important role in the development and progression of breast cancer. In this study, using a novel annealing control primer-based GeneFishing PCR technology, five differentially expressed genes (DEGs), expressed using 10nM mitogenic estrogens, $17{\beta}$-estradiol (E2) and $16{\alpha}$-hydroxyestrone ($16{\alpha}$-OHE1), were selected from the estrogen receptor (ER)-positive MCF-7 human breast cancer cells. The DEGs, MRPL42, TUBA1B, SSBP1, KNCT2, and RUVBL1, were identified by comparison with the known genes via direct sequencing and sequence homology search in BLAST. Quantitative real-time PCR data showed that two DEGs, tubulin ${\alpha}1b$ and kinetochore associated 2, were greater than 2-fold upregulated by E2 or $16{\alpha}$-OHE1. Both genes could be new biomarkers for the treatment and prognosis of cancers, and further study may provide insights into the molecular mechanisms underlying development and progression of breast cancer.

Instrumentation Failure after Partial Corpectomy with Instrumentation of a Metastatic Spine

  • Objective : To identify the perioperative factors associated with instrument failure in patients undergoing a partial corpectomy with instrumentation (PCI) for spinal metastasis. Methods : We assessed the one hundred twenty-four patients with who underwent PCI for a metastatic spine from 1987 to 2011. Outcome measure was the risk factor related to implantation failure. The preoperative factors analyzed were age, sex, ambulation, American Spinal Injury Association grade, bone mineral density, use of steroid, primary tumor site, number of vertebrae with metastasis, extra-bone metastasis, preoperative adjuvant chemotherapy, and preoperative spinal radiotherapy. The intraoperative factors were the number of fixed vertebrae, fixation in osteolytic vertebrae, bone grafting, and type of surgical approach. The postoperative factors included postoperative adjuvant chemotherapy and spinal radiotherapy. This study was supported by the National Research Foundation grant funded by government. There were no study-specific biases related to conflicts of interest. Results : There were 15 instrumentation failures (15/124, 12.1%). Preoperative ambulatory status and primary tumor site were not significantly related to the development of implant failure. There were no significant associations between insertion of a bone graft into the partial corpectomy site and instrumentation failure. The preoperative and operative factors analyzed were not significantly related to instrumentation failure. In univariable and multivariable analyses, postoperative spinal radiotherapy was the only significant variable related to instrumentation failure (p=0.049 and 0.050, respectively). Conclusion : When performing PCI in patients with spinal metastasis followed by postoperative spinal radiotherapy, the surgeon may consider the possibility of instrumentation failure and find other strategies for augmentation than the use of a bone graft for fusion.

Effect of progesterone on insulin-like growth factors(IGFs) and IGF-binding proteins(IGFBPs) in female rat (암컷 랫트에서 Progesterone투여가 Insulin-like Growth Factors(IGFs) 및 IGF-binding proteins(IGFBPs)에 미치는 효과)

  • The sex steroid hormone progesterone is essential for normal development and maturation of the endometrium in preparation for the embryo implantation and the maintenance of pregnancy. Insulin-like growth factor (IGF) system that is composed of IGF-I, IGF-II, IGF binding proteins (IGFBPs) is also involved in the maintenance of pregnancy. In addition, liver, kidney, and uterus is a target tissue for IGF system. However, the effect of exogenous progesterone on IGF system was not elucidated in female rats. Therefore, we investigated the effect of progesterone on insulin-like growth factors (IGFs) and IGF-binding proteins in serum, liver, kidney, and uterus in female ovariectomized rats. IGFs concentration was measured by radioimmuoassay (RIA) and IGFBPs levels by western ligand blotting(WLB). IGF-I concentration was increased in serum, liver, and uterus, but not in kidney of progesterone-treated ovariectomized rats, compared to control (P<0.05). IGF-II concentration was decreased in liver, but not in serum, kidney, and uterus of progesterone-treated rats, compared to control (P<0.05). IGFBP-3 was increased in serum, but not in liver of progesterone-treated rats, compared to control. IGFBP-2 was decreased in kidney, but not in others tissues of progesterone-treated rats, compared to control. These results suggest that progesterone may exert diverse physiological functions via the tissue-specific regulation of IGFs/IGFBPs system in female rats.

A sclerosing stromal tumor of the ovary with masculinization in a premenarchal girl

  • A sclerosing stromal tumor of the ovary is an extremely rare benign tumor; it usually is found during the second and third decades of life. Patients present with pelvic pain or a palpable abdominal mass. Hormonal effects such as masculinization are uncommon. Here, an 11-year old premenarchal girl presented with deepening of the voice. In addition, clitoromegaly and hirsutism with a male suprapubic hair pattern were observed. The laboratory findings showed that the testosterone level was elevated to 3.67 ng/ml, andostenedione to above 10 ng/ml, dehydroepiandrosterone-sulfate to 346 ${\mu}g$/dl and 17-hydroxy progesterone (17-OHP) to 11.28 ng/ml. The chromosome evaluation revealed a 46,XX female karyotype. An adrenocorticotropic hormone stimulation test was performed. The 17-OHP to cortisol ratio in 30 minutes was 0.045, which suggested a heterozygote for the 21-hydroxylase deficiency. However, the CYP21A2 gene encoding steroid 21-hydroxylase showed normal. The pelvic ultrasound showed a heterogeneous mass consisting of predominantly solid tissue in the pelvic cavity. The pelvic magnetic resonance imaging revealed an $8.9{\times}6.2{\times}6.6$ cm mass of the left ovary. A left oophrectomy was performed and microscopic examination confirmed a sclerosing stromal tumor. Immunohistochemical studies showed that the tumor was positive for smooth muscle actin and vimentin, but negative for S-100 protein and cytokeratin. Following surgery, the hormone levels returned to the normal range and the hirsutism resolved.
