Objectives: The purpose of this study was to review predictive factors associated with weight loss in moderate to severe obesity treatment. The direction of the treatment for moderate to severe obesity will be suggested in consideration of various factors. Methods: Authors searched the articles published from 2018 to 2023 in three international databases (PubMed, Embase and the Cochrane Central Register of Controlled Trials) and two domestic databases (Research Information Sharing Service, Korean studies Information Service System). Studies including treatment with moderate to severe obese patients were selected. Results: A total of 43 studies were included. The main factors of weight loss were unchangeable predictors such as low initial degree of obesity, younger age, non-diabetes and high resting energy expenditure with changeable predictors such as increase in protein, physical activity, self-efficacy, initial weight loss and attendance of the treatment. Conclusions: Our review results suggest that based on the characteristics of moderate to severe obese patients, predictors of weight loss can be used to determine treatment and prognosis in various aspects.
The aim of the study was to assess the prevalence of young adults obesity, morbid obesity and severe obesity and to identify associated factors. Data for 50,095 participants aged 20-39 from Community Health Survey 2019. Chi-square test and multiple logistic regression were used for the analysis. Prevalence for young adults obesity, morbid obesity and severe obesity were 23.60%, 5.86% and 1.31%. Aged of 20's, male, low income, low educational level and marital status were found to be associated with all stages of obesity. Current smoking, high risk drinking, physical activity, health promotion practices, subjective health, EQ-5D, stress, depressive symptoms and comorbidities increased the prevalence of obesity in young adults. Health check-up, subjective health, EQ-5D, stress, depressive symptoms and comorbidities increased the prevalence of severe obesity in young adults. Based on the results, it is necessary to develop and provide focused intervention consisting of characteristics of young adults and stages of obesity.

Background: This study purposed to examine the difference in the prevalence of obesity at each stage among people with and without disabilities considering the severity and type of disability. Methods: The study targeted a total of 1,315,967 people, including 68,418 disabled and 1,247,549 non-disabled, who completed the national health screenings. Logistic analysis and average marginal effect analysis were conducted in three stages (pre-obesity, obesity, severe obesity). Those analyses were conducted considering the severity and type of disabilities. Results: People with disabilities were more likely to be at all stages of obesity than non-disabled people. In severely disabled people, the probability of obesity was higher than non-disabled people at all stages of obesity, but mildly disabled people had a higher only in the severe obesity stage, no difference in obesity stage, and a low in the pre-obesity stage. In physical and mental disabilities, the probability of obesity was higher than non-disabled people at all stages of obesity, but external physical function and internal organs disabled had a lower in the obesity and pre-obesity stage, and no difference in severe obesity stage. Conclusion: This study found that people with disabilities had a higher relationship with obesity than people without disabilities. In addition, severity and types of disabilities have different effects on the stage of obesity. Therefore, it is necessary to care about the health inequality and health of disabled people considering their severity and types of disabilities.
Obesity is a chronic disease associated with severe complications. A major complication of obesity is depression, which can worsen obesity and vice versa. In addition, most antidepressants or antipsychotics cause weight gain, and the relationship between obesity and depression is clinically critical. However, treatment of obese patients with major depressive disorder is complicated. Bariatric physicians should provide appropriate behavioral interventions alongside pharmacological treatment, considering psychiatric symptoms, drug side effects, and drug interactions. Two successful cases of moderate-to-severe obese patients with major depressive disorder who had been treated for obesity using behavioral intervention therapy along with liraglutide will be discussed. This report highlights the safety and efficacy of liraglutide treatment of obesity in patients with depression who take antidepressants and antipsychotics.
This study investigated the influence of obesity on depression and stress by analyzing the results of the national survey on health and nutrition. Relationships between normal, obese, and severely obese groups were investigated. The results of the study are as follows. First, obesity caused by demographic variables showed a relationship to both gender and average monthly income. In the case of the seriously obese group, females had higher rates of depression than males, and the high obesity group had lower average monthly income than the normal and obese groups. Second, obesity caused by health-related variables found that high blood pressure, hyperlipidemia, and diabetes that was not angina-related displayed higher rates of depression in the serious obesity group than in the other groups. As for subjective evaluations of one's own health condition, the serious obesity group showed a high tendency to evaluate their own health conditions negatively. Third, the difference between depression and stress related to obesity level showed no difference between the normal weight group and the obesity group; however, the seriously obese group demonstrated relatively higher occurrences of depression and higher stress scores. Fourth, the result for depression and stress level caused by demographic variables, health-related variables, and obesity demonstrated a direct relationship to gender, subjective level of satisfaction with one's personal finances, average monthly income, subjective perception of one's own health and severely obesity status. More specifically, it was found that the ratio of depression and stress score was higher when the subject was female, among those whose subjective evaluation of their own finances and health condition was negative, and those who suffered from severe obesity.

Purpose: With remarkable increase in the prevalence of childhood obesity, nonalcoholic fatty liver disease increased, The aim of this study is to evaluate the prevalence of the increase liver enzymes, lipid levels and fasting blood glucose level in normal and obese children. Methods: A total of 2206 elementary students were grouped according to obesity index; normal group and obesity group(mild, moderate, severe). Aspartate aminotransferase(AST, SGOT) and alanine aminotrausferase(ALT, SGPT) were measured with tota1 cholesterol, triglyceride and fasting blood glucose. Results: Compared with the 2.7% of ALT > 50 IU/L in normal group, obese groups showed significantly higher prevalence; 6.7% in mild obesity group, 11.8% in moderate group and 15.0% in severe group. The prevalence of hypertriglyceremia was 12,4% in normal weight group, which is significantly lower than obesity group(mild obesity group 24.8%, moderate and severe 32.1% each). Conclusion: The prevalence of nonalcoholic fatty liver increased along with severity of obesity. Of Nutritional assessment, intervention, and preferably prevention are necessary for health promotion elementary students.

Background: Musculoskeletal pain is associated with obesity; however, information on factors associated with pain in adults with obesity and severe obesity is limited. The purpose of this study was to assess the prevalence of musculoskeletal pain by site and intensity of pain and associated factors in individuals with severe obesity (body mass index ≥ 35.0 kg/㎡). Methods: Baseline data from the DieTBra Trial study evaluating pain symptoms in nine body regions over the last seven days using the Nordic Questionnaire on Musculoskeletal Symptoms and Numerical Pain Scale. The variables analyzed using multiple Poisson regression with hierarchical analysis were: sociodemographic, lifestyle, food consumption, clinical, and anthropometric, and the outcome was moderate and intense pain. Results: In 150 participants, there was a high prevalence of ankle and foot pain (68.7%), lower back pain (62.7%), pain in the knees (53.3%) and upper back pain (52.0%), with a predominance of intense pain. Factors associated with pain according to specific sites were: type 2 diabetes with hand/wrist pain; sedentary time with hip pain; insomnia with pain in the hip and knee; edema in the lower limbs with pain in the lower back and ankles/feet; degree of obesity with ankle/foot pain; and percentage of total fat with ankle/foot pain. Conclusions: There was a high prevalence of pain and intense pain in individuals with severe obesity and an association with clinical variables, the degree of obesity, and sedentary lifestyle.
Objectives: The purpose of this study is to review clinical studies related to the effect of Traditional Chinese Medicine (TCM) on moderate to severe obese patients with hypertension. Methods: Clinical studies from the China National Knowledge Infrastructure (CNKI) were searched by specific keywords and criteria. Total of 7 randomized controlled trials were selected and analyzed. Results: Various evaluation methods were used to see the effect of TCM compared to conventional western medicine. Most of the studies used herbal medicine combined with conventional anti-hypertensive western medicine. The effective rate of anti-hypertension was higher when TCM was added to conventional anti-hypertensive medicine. Traditional medicine treatment showed significant effect on lowering blood pressure and body mass index. Conclusions: Based on the results of the clinical studies from China, TCM can be a valuable option for moderate to severe obese patients with hypertension. Applying Traditional medicine has a significant effect on lowering blood pressure and weight loss. Combining herbal medicine can be worthy of clinical promotion and application for moderate and severe obese patients with hypertension.

Obstructive sleep apnea syndrome (OSAS) has negative effects on health, including increased mortality, risk of cardiovascular disease, and neurocognitive difficulties. OSAS is common in obese patients and obesity is an important risk factor of OSAS. A 41-year-old female OSAS patient with severe obesity (body mass index [BMI] ${\geq}35$) who failed dietary weight loss underwent bariatric surgery. After surgery, there were improvements in BMI (from 36.9 to $31.7kg/m^2$) and polysomnographic data, including the apnea-hypopnea index (from 25.1 to 11.2 events/hr) and minimum SaO2 (from 69 to 82%). This case demonstrates that bariatric surgery may be an effective therapeutic option to reduce sleep-disordered breathing in severely obese patients with moderate OSAS. Bariatric surgery as a treatment option for OSAS should be considered in OSAS patients with severe obesity who failed dietary weight loss.
Purpose: To analyze the associations among the degrees of nonalcoholic fatty liver disease (NAFLD) by ultrasonography and metabolic syndrome, degrees of obesity in children, and degrees of parental obesity. Methods: A total of 198 children with obesity who visited a pediatric obesity clinic were prospectively enrolled in this study. The severity of NAFLD based on ultrasonography was classified into no, mild, moderate, or severe NAFLD group. The degree of obesity based on the percentage over standard weight for height per sex was classified into mild, moderate, or severe. Results: Of 132 patients evaluated for the degree of NAFLD and metabolic syndrome, the p-value of correlation between the two factors was 0.009. Therefore, metabolic syndrome might significantly affect the degree of NAFLD. Of 158 patients evaluated for the degree of NAFLD and the degree of obesity, the p-value of correlation between the two factors was 0.122. Of 154 patients evaluated for the degree of obesity and father's obesity, the p-value was 0.076. Of 159 patients evaluated for the degree of obesity and mother's obesity, the p-value was 0.000, indicating that mother's obesity could significantly affect the degree of obesity in children. Of 142 patients evaluated for the degree of obesity and metabolic syndrome, the p-value was 0.288. Conclusion: Metabolic syndrome might significantly affect the degree of nonalcoholic fatty liver in children. In addition, mother's obesity might be a significant factor that affects the degree of obesity in children.
