편식하는 비만 아동의 식행동, 맛 선호도 및 영양 섭취 상태 (The Dietary Behaviors, Taste Preferences, and Nutrient Intakes of Obese Children Consuming Unbalanced Diets)

  • This study was designed to evaluate the nutritional status and dietary problems of obese elementary students consuming unbalanced diets. Blood parameters, body-image perceptions, taste preferences, and nutrient intakes of 40 obese elementary students with unbalanced diets were compared to those of 40 additional obese elementary students as a control. The averages for age, height, weight, and obesity index were 11.5 years, 142.0 cm, 51.0 kg, and 35.4% in the study group and 11.8 years, 144.0 cm, 53.5 kg, and 36.3% in the control group, respectively. The major disliked foods by the students in the study group were vegetables (65%) particularly Kimchi (15%), as well as fish(15%), and pulses (10%). Mean serum cholesterol, blood glucose, GOT/GPT, and hemoglobin levels were 175.1 mg/dL, 89.2 mg/dL, 24.1/18.9 IU/L, and 14.3 g/dL in the study group and 182.8 mg/dL, 91.3 mg/dL, 28.3/33.9 IU/L, and 14.7 g/dL, in the control group, respectively. Approximately 60${\sim}$80% of the subjects responded that they eat a meal everyday, and the remainder skipped at least one meal. The preference for salty tasting foods was significantly lower in the study group, but the preference for hot tasting food was higher as compared to the obese control group(p<0.05, p<0.05). The daily energy intakes of the study and control groups were 1,768.3 kcal and 1,572.7 kcal, respectively, but there was no significant differences between them. Daily intakes of animal protein, fat, animal fat, and vitamin $B_2$ were significantly higher in the study group than in the control group. In conclusion, this study found that obese children consuming unbalanced diets strongly disliked vegetables, particularly Kimchi, as well as hot tasting foods; however, they preferred salty foods. Daily intakes of animal protein and fat were higher in the study group as compared to obese children not consuming unbalanced diets.

Phloxine O, a Cosmetic Colorant, Suppresses the Expression of Thymic Stromal Lymphopoietin and Acute Dermatitis Symptoms in Mice

  • Cosmetics are primarily applied to the skin; therefore, the association of cosmetic dyes with skin diseases or inflammation is a topic of great interest. Thymic stromal lymphopoietin (TSLP) is an interleukin 7-like cytokine that activates dendritic cells to promote Th2 inflammatory immune responses. TSLP is highly expressed in keratinocytes under inflammatory conditions, which suggests that it may play a critical role in the development of skin diseases, such as atopic dermatitis. Therefore, we investigated whether cosmetic dyes influenced the production of TSLP by keratinocytes. Phloxine O, also known as D&C Red No.27, is one of the most common red synthetic pigments and is widely used in colored cosmetics. Our results showed that Phloxine O downregulated phorbol 12-myristate 13-acetate-induced production of TSLP in a murine keratinocyte cell line (PAM212). Phloxine O also suppressed TSLP expression in KCMH-1 cells, which are mouse keratinocytes that constitutively produce high levels of TSLP. To investigate the in vivo effects of Phloxine O, we induced TSLP expression in mouse ear skin by topically applying MC903, a vitamin D3 analogue that is a well-known inducer of atopic dermatitis-like symptoms. Topical application of Phloxine O prevented MC903-induced TSLP production in mouse ear skin, attenuated the acute dermatitis-like symptoms and decreased serum IgE and histamine levels in mice. Suppression of TSLP expression by Phloxine O correlated with reduced expression of OX40 ligand and Th2 cytokines in mouse ear skin. Our results showed that Phloxine O may be beneficial to prevent dermatitis by suppressing the expression of TSLP and Th2 cytokines in skin.

Relationship among Nutritional Intake, Duration of Outdoor Activities, Vitamin D Status and Bone Health in High School Girls

  • In this study, we examined the effects of dietary 1,3-diacylglycerol (DG) compared to conventional triacylglycerol (TG) oil on the postprandial response of total and chylomicron TG, glucose, insulin, and free fatty acid (FFA). This study was conducted using a cross-over design. Ninety subjects participated in the high-fat meal tolerance test where they were randomly assigned to consume two experimental sandwiches containing mayonnaise with TG or DG oil with a seven-day interval. Blood samples were collected before ingestion and at 2, 3, 4 and 6 hr time point after ingestion and analyzed for total and chylomicron TG, glucose, insulin, FFA and phospholipid fatty acid composition. Both TG and DG ingestion had similar effects on postprandial TG response, but a different response from chylomicron TG. Compared with the TG group, TG levels were significantly lower only at 6 hr time point in the DG group. On the other hand, chylomicron TG rose steeply at 2 hr time point and decreased faster in this group. Also, the adjusted value to fasting levels was the same as the unadjusted level. Fasting levels and net differences in insulin were significantly lower at 3 hr time point where chylomicron TG levels were significantly lower in the DG group. But those of glucose and FFA in the TG and DG groups did not differ significantly. Fasting and postprandial levels of fatty acid composition in serum phospholipids in the two groups did not differ significantly. In conclusion, this study indicated that one could reduce the magnitude of postprandial lipemia without influencing glucose metabolism by consumning DG oil as a substitute for TG oil. Based on the correlation of coronary artery disease and postprandial lipemia, dietary DG ingestion might have a beneficial effect in treating such a disease. Further studies are required to clarify the long-tenn effects of dietary DG on blood lipid levels in humans.

유제품 섭취와 당뇨 예방 (Dairy Products Intake and Managing Diabetes)

  • Milk intake is widely recommended for healthy diet, not only for bone growth and maintenance, but also as a protein, calcium and magnesium sources as part of an adequate diet. Many research suggest that milk and dairy products are associated with a lower risk of type 2 diabetes mellitus (T2DM). Milk and dairy products are low Glycemic index (GI) and Glycemic load (GL) foods. The GI and GL are useful tools to choose foods to help control blood glucose levels in people with diabetes. The GI and GL of milk are 32~42 and 4~5, respectively, and which are about 1/2 and 1/5 of boiled rice. The mechanisms underlying the effects of dairy on T2DM development includes the calcium and vitamin D content in dairy foods and the possible positive effect of high milk and calcium intake on weight control. The role of dairy products on reducing the risk of diabetes can be inferred from the reports that lower serum IGF-1 levels were positively associated with diabetes and the girls with low milk intake had significantly lower IGF-1. Accumulating data from both patients and animal models suggest that microbial ecosystems associated with the human body, especially the gut microbiota, may be associated with several important diseases, such as inflammatory bowel disease, obesity, diabetes and cardiovascular disease. It was thought that fermented milk containing lots of probiotics can be useful for controling blood glucose levels and preventing complication of diabetes, but sucrose in commercial yogurt should be substituted. There are some reports of oligosaccharide, xylitol, and stevia as a potentially useful sweetener in the diabetic diet.

30대 남성의 결혼 여부에 따른 신체계측, 혈액성상, 건강상태, 식품섭취빈도 및 영양소 섭취량 비교 - 국민건강영양조사(2008~2015년)에 기초하여 - (Anthropometic Characteristics, Serum Profiles, Health Status, Food Intakes Frequency and Nutrient Intakes by Married Status of Men Aged 30-39 - Based on Korean National Health and Nutrition Examination Survey (2008-2015) -)

  • This study was conducted to investigate the anthropometric data, serum profiles, nutrient intakes and diet quality of men in their 30s. The subjects were divided into a married group and an unmarried group. For the study, we obtained data for analysis from the combined 2008~2015 Korean National Health and Nutrition Examination Survey (KNHANES). Mean height and weight of study population were 173.5 cm, and 74.0 kg in the married group and 173.1 cm, and 73.6 kg in the unmarried group, respectively. Systolic blood pressure in the unmarried group was significantly higher than that of the married group (P<0.001), while circulating vitamin D levels in the married group were significantly higher than those of the unmarried group (P<0.001). The proportion of obesity in the two groups was 42.41% and 38.40%, respectively. In the unmarried group, prevalence of depression was significantly higher than that those of the married group. Intakes of energy, water, protein, fat, carbohydrate and calcium in the married group were significantly higher than those of the unmarried group. In both groups, water and fiber intakes were low and sodium intakes were extremely high based on the KDRIs (Dietary Reference Intakes for Koreans). The mean adequacy ratio (MAR) of the married group was higher than that of the unmarried group. The index of nutritional quality (INQ) in the married group was also higher than that of the unmarried group. Therefore, we propose development of dietary guidelines and education programs for improvement of food and nutrient intakes, nutrition balance and dietary quality of unmarried men in their 30s.

소아의 만성신장질환의 치료 (Treatment of chronic kidney disease in children)

  • 만성 신질환 환아들의 치료는 신대체 요법과 더불어 적절한 식이 요법, 수분, 전해질 및 산-염기의 관리, 신장 골형성 장애의 예방 및 치료, 빈혈과 고혈압, 성장 지연의 치료 등으로 이루어진다. 식이 요법은 환아들의 성장이 충분히 이루어질 수 있도록 탄수화물, 지방, 단백질 및 열량의 공급이 적절하게 이루어져야 한다. 수분, 전해질 및 산-염기의 상태를 확인하고 적절하게 관리해야 한다. 혈청 칼슘, 인, 부갑상선 호르몬 농도가 사구체 여과율에 따른 적절한 목표치 안에서 유지될 수 있도록 인 결합 제제와 비타민 D 제제를 투여하여 신장 골형성 장애를 예방하여야 한다. 적혈구 생산 촉진제와 철분 제제를 투여하여 혈색소 값을 11-12 g/dl로 유지하여야 한다. 수분 및 염분의 균형과 적절한 항고혈압제의 투여로 고혈압을 조절해야 한다. 사람 재조합 성장 호르몬을 투여하여 최종 성인 키를 호전시키도록 한다.

일부 흡연 남성의 식사섭취와 대사증후군 위험도 (Nutrient Intake and Risk of Metabolic Syndrome in Male Smokers)

  • This study was intended to investigate the risk of metabolic syndrome (MS) in healthy 92 male smokers (mean age: $44.4\;{\pm}\;7.8\;yrs$). We investigated the anthropometric assessment and dietary intake survey for 2 days by 24-recall method, also blood pressure and serum lipids were measured. The average numbers of cigarettes smoking a day were 21.3/d, smoking duration were 21.5 years. The proportion of fat energy was 24.9% and intakes of vitamin B2, folate, calcium, potassum and fiber were lower than KDRI. The body mass index (BMI) and body fat % were 24.8, 23.9% respectively. The systolic ($134.1\;{\pm}\;1.4\;mmHg$) and the diastolic blood ($87.9\;{\pm}\;1.1\;mmHg$) pressure were in borderline hypertension. Among biochemical parameters, TG ($173.6\;{\pm}\;9.4\;mg/dL$) and fasting blood glucose ($109.0\;{\pm}\;2.4\;mg/dL$) levels were out of normal values. The most occurred problem among the risk factors related to MS was the borderline hypertension (63%) in subjects. Regarding the correlations of anthropometric data with biochemical factors, TG was significantly correlated with the BMI, body fat % and waist circumference. Smoking years showed positive correlation with AI. These results suggest that the smoking habit has significant relations with the risk factors of MS. Therefore, quitting is necessary to prevent MS, and nutrition education and dietary management program are required to prevent the degenerative disease.

광주 소재 한 종합병원을 방문한 성인에서 혈중 Osteocalcin과 HbA1c의 상관관계 (Correlation between Serum Osteocalcin and Hemoglobin A1c in Gwangju General Hospital Patients)

  • 최근 골대사와 당뇨병 발생 기전의 관계에 대해 많은 연구가 진행되고 있으며, osteocalcin이 제 2형 당뇨병을 위한 치료법에 관련될 수 있다는 것이 밝혀지고 있다. 따라서 본 연구는 osteocalcin과 HbA1c의 상관관계를 알아보고자 한다. 2015년 1월 1일부터 2016년 12월 31일까지 광주 소재 한 종합병원 수진자 중 osteocalcin과 HbA1c를 비롯하여 칼슘, 카르복시말단펩티드, 총 콜레스테롤, 중성지방, 고밀도 지질단백 콜레스테롤, 저밀도 지질단백 콜레스테롤, 25-수산화비타민D, 키, 몸무게를 측정한 40세 이상의 성인 총 714명($70.8{\pm}10.4$세)을 연구대상자로 최종 선정하였다. 성별은 남자(452명, 63.3%)가 여자(262명, 36.7%)보다 많았다. 당뇨환자군에서 osteocalcin과 HbA1c는 약한 음의 상관관계(r=-0.183, P<0.001)가 있었고, 다른 요인들을 모두 보정했을 때에도 여전히 음의 상관관계(r=-0.251, P<0.001)가 있었다. 본 연구는 당뇨환자에서의 osteocalcin과 HbA1c가 음의 상관관계가 있음을 보여주었다. 골대사와 당대사의 생물학적 기전 및 관련요인들이 미치는 영향을 바탕으로 골대사와 당뇨병 발생의 상호관계에 대한 연구가 지속되어야 한다.

미니어처 슈나우저에서 발생한 원발성 부갑상선기능저하증 (Primary Hypoparathyroidism in a Miniature Schnauzer Dog)

  • 7연령의 암컷 미니어처 슈나우저견이 갑작스런 발작과 근육 떨림 증상을 주증으로 내원하였다. 내원 당시 발열과 헐떡임이 두드러졌으며, 신체검사 상에서 현저한 앞다리 근육 강직증상, 근육부분수축과 뻣뻣한 걸음걸이가 관찰되었다. 실험실 검사에서 심각한 저칼슘혈증과 함께 고인혈증이 특징적으로 관찰되었다. 이와 더불어 관찰된 낮은 농도의 부갑상선 호르몬과 이온화된 칼슘농도, 뇨중 칼슘과 인의 배설 량 측정을 통하여 원발성 부갑상선기능저하증이 진단되었다. 이 환축은 10% 글루콘산칼슘 투여를 통한 초기 치료에 반응이 좋았으며, 비타민 D 유사체와 칼슘 보조제를 통한 유지 치료에 의해 잘 관리 되고 있다. 부작용으로 일시적인 고인혈증이 나타났으나, 이 역시 경구용인 흡착제인 sevelamer hydrochloride를 이용한 치료에 의해 안정화 되었다. 결론적으로 본 증례의 경우 개에서 나타나는 원발성 부갑상선기능저하증의 임상증상과 특징적인 실험실 검사 결과 그리고, 치료 반응에 대한 국내 첫 증례보고이다.

채식 성인여성의 식사형태 및 비타민, 영양제 복용과 혈중 지질, 혈당, 혈압과의 관련성에 관한 비교연구 (The Study of the Diet Style and Relationships among Vitamin and Nutrient Supplement Intakes, Serum Lipid Levels, Blood Sugar and Blood Pressure of Adult Female)

  • 조사대상자의 평균나이는 채식인 44.2세, 비채식인 40.5세, BMI는 각각 22.4, 21.0이었고, WHR은 0.8, 0.8였고, %BF는 28.7, 26.5였으며, 채식인의 평균채식기간은 13.1년이었다. 조사대상자중 비만인 사람의 식사 형태는 채식인은 볼륨형(58.8 %), 미식가형(20.6%), 다이어트형(8.8%), 패스트푸드형 (5.9%), 밸런스형(5.9%)의 순이었고 비채식인은 미식가형(33.3%), 볼륨형 (27.8%), 다이어트형 (16.7%), 패스트푸드형 (11.1%), 밸런스형(11.1%)의 순이었다. 채식인은 식사의 양이 많은 사람에게서 비채식인은 미식가형에서 비만이 많은 것으로 나타났다. 정상체중인 사람의 식사형태는 책식인은 볼륨형(45.1 %), 미식가형 (32.4%), 밸런스형 (16.2%), 다이어트형 (4.2%), 패스트푸드형 (2.1%)의 순으로 볼륨형과 미식가형이 약 78% 정도를 차지하였다. 비채식인은 볼률형(34.2%), 미식가형(26.1 %), 패스트푸드형 (22.6%), 밸런스형 (10.6%), 다이어트형 (6.5 %)의 순으로 의외로 볼륨형이 가장 많았다. 조사대상자의 평균 중성지방은 채식인 136.7 mg/dL, 비채식인 130.5 mg/dL 였고, total-cholesterol은 채식인 161.4 mg/dL, 비채식인 189.6 mg/dL, HDL-cholesterol은 채식인 48.2 mg/dL, 비채식인 50.8 mg/dL, LDL-cholesterol은 채식인 86.6 mg/dL, 비채식인 111.1 mg/dL, AI는 채식인 2.4, 비채식인 2.9, 혈당은 채식인 90.8 mg/dL, 비채식인 103.6 mg/dL, 수축기 혈압은 채식인 107.5 mmHg, 비채식인 119.3 mmHg, 이완기혈압은 채식인 72.4 mmHg, 비채식인 73.6 mmHg이었다. 조사대상자의 total-cholesterol, LDL-cholesterol, HDL-cholesterol, AI, 수축기혈압 및 혈당은 비채식인이 유의적으로(p<0.05) 높았으며, 심혈관질환 예견지수인 HDL-cholesterol/total-cholesterol 비는 채식인이 유의적으로(p<0.05) 높았다. 조사 대상자의 비타민 및 영양제 복용실태는 비타민을 복용한다고 한 사람은 채식인 53명(22.6%), 비채식인 71명 (29.0%), 영양제를 복용한다고 한 사람은 채식인 24명(10.2%), 비채식인 15명 (11.0%), 비타민이나 영양제를 먹지 않는다고 한 사람은 채식인 158명(67.2%), 비채식인 147명(60.0%)으로 두 군이 비슷한 비율이었다.
