급성 약물중독에 합병된 일과성 혹은 지속성 고암모니아혈증의 특성 (General Characteristics for Poisoning-Induced Transient or Sustained Hyperammonemia)

  • Purpose: In patients with altered mentality caused by drugs or unknown causes, ammonia is checked to facilitate differential diagnosis or diagnose hepatic coma. This helps early prevention and treatment of brain damage due to hyperammonemia. This study was conducted to evaluate clinical characteristics of intoxicated adult patients with hyperammonemia. Methods: We evaluated 95 patients with hyperammonemia among intoxicated patients above the age of 15 who visited our ED from January 2013 to December 2015. We analyzed the demographic characteristics and type of poisoning substance, reason for ingestion, toxicological characteristics such as elapsed time from ingestion to hospital visit, lab, clinical progression and complications. Data were evaluated using the student's t test or Mann-Whitney U test for continuous variables, and Chi-square test and Fisher's exact test for frequency analysis of categorical variables. Results: When compared to healthy individuals, patients with hyperammonemia showed statistical significance on their SOFA score (p=0.016) and poison severity score (p<0.001). Additionally, patients with hyperammonemia showed significantly different initial serum AST level (p=0.012) and maximum serum AST level during the hospital stay (p=0.026) when compared to healthy individuals. Moreover, individuals with sustained hyperammonemia compared to transient hyperammonemia showed clinically significant SOFA scores (p<0.001), poison severity scores (p=0.007), mortality rates in the ICU (p=0.021), as well as different duration of hospital stay (p=0.037), serum creatinine level (p=0.002), erythrocyte sedimentation rate (p=0.025), and serum myoglobin (p=0.015). Conclusion: Most poisoning-induced hyperammonemia cases were transient and recovered without special treatment. Therefore, hyperammonemia is almost non-specific among poisoning patients.

한국 성인의 Vitamin D 영양 상태가 골밀도에 미치는 영향 (The Effect of Vitamin D Status on Bone Mineral Density of Korean)

  • This study was carried out to evaluate the effects of vitamin D nutritional status on bone mineral density of adults (21-49 years). To attain the aim, we measured bone mineral density (BMD) of the subjects at distal radius by single-photon absorptiometry (SPA). Serum level of 25-hydroxyvitamin D(25-(OH)D) , known to be the best indicator of indicator of vitamin D status in humans was analyzed . The factors affecting this vitamin D level were also investigated in autumn in 122 young adults. Serum level of 25-(OH)D was measured by high pressure liquid chromatography(HPLC) and biochemical variables, general health status, time spent outdoors, and dietary intakes of the subjects. BMD of the male subjects was significantly greater than that of female subjects. Weight, activity and total energy expediture (TEE) showed a positive correlation with distal BMD. The mean level of serum 25-(OH)D was 24.4$\pm$11.0 ng./ml and by sex, 26.0$\pm$6.8ng/ml for males and 23.3$\pm$12.3ng/ml for females , the level was significantly higher in male (p<0.01). there was significant correlation between BMD at distal-radius and s-25(OH)D levels (p<0.001). The serum level fo parathyroid hormone (PTH) showed a negative correlation with BMD(p<0.05), with the more obvious correlation in females. Vitamin D intake was estimated to be 3.75$\pm$2.19ug/day in average. Among the nutrients studied, protein ,fat, calcium , and vitamin D intake were positively correlated with distal BMD. When food frequencies were concerned , milk and dairy products showed a significant positive correlation with the BMD level, and driedfoods, eggs , fats and oils, and cereals also showed a positive correlation. Time spent outdoors was estimated to be about 70 minutes in average and positively correlated with the distal BMD level(p<0.01). During the day, the specific time between 12 :00pm and 2:00pm showed the most significant correlation with BMD (p<0.001). Multiple regression analysis with the variables showed that distal BMD could be fit 31.9% by the time spent outdoors a day, intake of Ca and vitamin D, and TEE. The standardized estimates were 0.344 for vitamin D intake, 0.284 for Ca intake 0.179 for the time spent outdoors a day and 0.273 for TEE. For males, s-25*OH)D level, TEE and time spent outdoors during a day showed a significant correlation. For females, intake of Ca and vitamin D could fit about 27.1% of the distal BMD.

Log-transformed plasma level of brain natriuretic peptide during the acute phase of Kawasaki disease is quantitatively associated with myocardial dysfunction

  • Purpose: Brain natriuretic peptide (BNP) has been considered a biochemical marker for myocarditis in Kawasaki disease. We performed this study to determine its quantitative significance. Methods: We attempted to correlate log-transformed BNP concentrations (log-BNP) and clinical, laboratory, and echocardiographic variables in 81 children with Kawasaki disease. Stepwise multiple linear regression analysis was used to determine the variables independently associated with log-BNP concentration. Results: Serum C-reactive protein level (P<0.0001), serum alanine aminotransferase concentration (P =0.0032), white blood cell count (P=0.0030), and left ventricular mass index (P=0.0024) were positively related with log-BNP, and hemoglobin level (P<0.0001), serum albumin level (P<0.0001), $Na^+$ concentrations (P<0.0001), left ventricular fractional shortening (P=0.0080), and peak early diastolic tissue velocity of the left ventricular basal lateral segment (P=0.0045) were negatively related to the log-BNP concentration. Multiple regression analysis showed that serum albumin concentration ($R_2$=0.31, P=0.0098) and left ventricular mass index ($R_2$=0.09, P=0.0004) were significantly associated with the log-BNP concentration. Conclusion: Elevated BNP levels during the acute phase of Kawasaki disease may be attributable to cardiac dysfunction associated with the increase in left ventricular mass, and log-BNP concentration may be a quantitative biochemical marker of myocarditis in Kawasaki disease.

대한민국 성인에서 비만과 Uric acid의 관련성 (The Association of Obesity and Serum Uric acid in Korean adults)

  • 본 연구는 일부 종합검진 수검자들을 대상으로 비만수준이 혈 중 uric acid에 미치는 영향을 검토하고자 2011년 1월부터 12월까지 광주광역시의 일개 종합병원 건강검진센터에서 종합건강검진을 받았던 20세 이상의 지역주민 1,118명(남자 636, 여자 482)을 분석대상으로 하였다. 우리는 혈청 uric acid, 요소 질소, creatinine 뿐만 아니라 신체계측 (SBP, DBP 및 BMI)에 대해 평가하고, 분석에 포함 하였다. Model I에서, 연령, 성별, SBP, DBP, TC, TG, HDL-C, FBG를 보정하였을 때, 남성(p<0.001)과 여성(p=0.036)에서 uric acid의 평균값은 비만수준이 증가할수록 증가하였다. Model II에서, BUN과 creatinine을 추가로 보정하였을 때, 남성에서는 uric acid의 평균값이 정상 체중군은 $4.89{\pm}0.07mg/dl$, 과체중군은 $5.01{\pm}0.09mg/dl$, 비만군은 $5.35{\pm}0.08mg/dl$로 비만수준이 증가할수록 증가하였다(p<0.001). 그러나 여성에서는 uric acid의 평균값이 정상체중군은 $5.03{\pm}0.08mg/dl$, 과체중군은 $5.19{\pm}0.11mg/dl$, 비만군은 $5.27{\pm}0.09mg/dl$로 유의한 차이가 없었다(p=0.191). 결론적으로, 남성에서 비만수준의 증가는 혈 중 uric acid의 증가와 관련이 있지만, 여성에서는 관련이 없었다.

고혈압 환자에 도움을 줄 수 있는 한방 다류 개발 (Development of Functional Tea Products for Hypertension Patients with the Mixed Medicinal Herbal Extracts)

  • 본 연구는 두충을 주 원료로 고안된 한방 혼합 다류가 고혈압 질환자의 혈압, 혈청지질 및 혈액학적 변화에 미치는 영향을 평가하기 위하여 수행하였다. 한방 혼합 다류를 시험 4주간 복용한 그룹과 시험 전 그룹을 비교한 결과, 수축기 혈압은 167.14 mmHg에서 134.86 mmHg로, 이완기 혈압은 100.00 mmHg에서 87.10 mmHg로 뚜렷하게 저하되었다. 혈청지질 중 총콜레스테롤(TC), 고밀도-콜레스테롤(HDL-C), 저밀도-콜레스테롤(LDL-C)의 농도 및 동맥경화지수(AI)는 유의적인 차이가 없었다. 반면, 한방 혼합 다류를 복용 시 중성지방(TG) 농도가 237.10 mg/dL에서 145.00 mg/dL로 강하게 감소되었다. 시험 4주 후와 시험개시 전의 혈액학적 지표(적혈구, 백혈구, 헤마토크리트, 헤모글로빈, 혈소판 및 호모시스테인)를 비교한 경우, 유의적 변화는 관찰되지 않는 반면 평균 적혈구 혈색소 농도(MCHC)는 유의적으로 2.70% 증가되었다. 이상의 결과로 부터 두충을 주 원료로 고안된 한방 혼합 다류는 중성지방 농도를 감소시킴으로써 혈압을 저하시키는 것으로 추정하며, 평균 적혈 수혈 색소 농도를 증가시킴을 확인하였다.

한국 여성에서 혈청 ferritin과 25-hydroxyvitamin D 및 대사 증후군의 관련성 (The association of vitamin D and urine microalbumin/creatinine and obesity in Korean adults)

  • 본 연구는 한국 여성에서 혈청 ferritin과 25-hydroxyvitamin D [25 (OH) D]와 대사 증후군 (MetS)의 관련성을 평가하기 위해 시행되었다. 다섯 번째 국민 건강 영양 조사 (KNHANES V) (2,010- 2,012)에서 ≥ 20세 이상 성인 9,256명(4,196 폐경 전 여성과 폐경 후 여성 4,340)의 데이터를 분석 하였다. 주요 연구 결과는 다음과 같다. 첫째, MetS가 없는 여성의 경우 관련 변수 (연령, 흡연, 음주, 규칙적인 운동, SBP, DBP, WM, TC, TG, HDL-C, FPG, Hb, Hct)를 조정 한 후, 비타민 D 상태는 혈청 ferritin 수치와 관련이 있었다 (폐경 전, p <0.001, 폐경 후, p = 0.027). 둘째, MetS를 가진 여성에서 비타민 D 상태는 혈청 ferritin 수치와 관련이 없었다 (폐경 전, p = 0.739, 폐경 후, p = 0.278). 결론적으로, 비타민 D 상태는 MetS가 없는 여성에서는 혈청 ferritin 수준과 양의 상관 관계가 있었지만 MetS가 있는 여성에서는 그렇지 않았다.

Physical activities due to the social distancing could change the serum lipid levels between the pre-and post-COVID-19 pandemic on Jeju Island

  • 코로나바이러스질병-2019 (COVID-19) 대유행은 감염병의 확산을 예방하기 위하여 사회적 거리 두기로 인해서 신체적 활동들을 변화시켜 왔다. 그러한 제한으로 인해서 신체활동과 혈청의 지질 수치들을 변화시킬 수 있다. 이 연구의 목적은 제주도 내에서 COVID-19 대유행 전과 후에 사회적 거리 두기로 인해서 혈청 내의 지질 수치들과 신체적 활동의 변화들에 대해서 연구해 보고자 하였다. 이 연구는 2018년 5월부터 2021년 12월까지 제주도에 소재한 단일 건강검진센터에서 검진을 시행한 총 5,373명의 대상자들이 포함되었다. 그들이 검진한 날들을 기준으로 두 군(COVID-19 전과 COVID-19 후)으로 나누어서 임상 변수들에 대해서 분석하였다. 임상적 변수들 중에서 평균연령 (P<0.014), HDL-콜레스테롤 (P=0.001), LDL-콜레스테롤 (P=0.039), 총콜레스테롤 (P<0.001)의 평균 수치들, 복부 비만 (P<0.001), 유산소 운동 (P=0.003), 신체활동 (P=0.008)을 가진 대상자들의 비율들이 코로나 대유행 전과 비교해서 더 높았다. 비록 제주도 내의 코로나 대유행 시기에 신체활동이나 유산소 운동을 하는 대상자들의 비율이 통계적으로 증가했음에도 불구하고, 복부비만이나 이상지질혈증을 가진 대상자들의 비율이 COVID-19 대유행 이전에 비해서 통계적으로 그 시기에 높았다.

지속성 외래 복막투석환자의 영양상태에 관한 연구 (Nutritional Assessment of the Continuous Ambulatory Peritoneal Dialysis Patients)

  • This study investigated the nutritional status of 32 CAPD patients. Their weight, height, triceps, mid arm circumference were measured and their dietary intake and the blood profiles were evaluated. They were 56.0${\pm}$11.8 years old. Their intake of energy and protein were 78% and 93% of RDA respectively. The energy from dextrose was 376.1${\pm}$83.2kcal. The men's and women's intake of protein was 84.6% and 102.1% of RDA respectively, According to the distribution of BMI, 23.5% of the men and 6.1% of the women were underweight and 5.9% of the men and 20% of the women were overweight. The serum albumin levels of the men and women were 3.30 and 3.71g/41 respectively. However, the average amount of serum ferritin, as a whole, comes within the normal range, which shows that stored iron was not decreased and that their anemia was not caused by iron deficiency. The subjects were divided into three groups according to the level of albumin, and their intakes of nutrients were compared with one another, The group with the high level of albumin showed that energy and protein intake was significantly larger and that BMI was also significantly higher than the other groups. There was a positive correlation between BMI and energy intake. Serum total protein had a positive correlation to energy intake ; hematocrit, to carbohydrate intake. BMI had a positive correlation to energy intake. A relative magnitude of factors affecting albumin level was analyzed by Stepwise multiple regression analysis. Overall results about relative influence of independent variables to dependent variable(albumin) indicated that the blood total protein(p <0.0001) was the most significantly correlated with serum albumin level in all subjects,1311owe4 by creatinine and total cholesterol. 무안군 주민의 양파와 영양소 섭취 실태 및 혈청 지질 농도의 분포에 관한 연구 (A Study on the Status of Onion and Nutrients Intake and Distribution of Serum Lipid Levels for Muan-gun Inhabitants)

  • The purpose of this study was to investigate the anthropometric variables, dietary behaviors, nutrient intakes and distribution of serum lipid levels for Muan-gun inhabitants. The subjects was consisted of 212 men and 317 women. There were significant differences in the height, weight, BMI and fat mass with ages. In the onion intake status. 90.6(79.9)% of men(women) liked. 56.8(46.4)% of men(women) intakes 3 times per day, 29.6(40.8)% of men(women) 50g per one time and 83.1(87.5)% of men (women) after cooking. The mean energy intake of the subjects were 2,043㎉ for men and 2,105㎉ for women. Men (women) consumed 88(90)g protein. 34(38)g lipid, 340(355)g carbohydrate, 10.5(11.7)g fiber, 655(757)mg calcium. 1,396(1,421) mg phosphorous. 14.7(15.9)mg iron. 7.061(7,244)mg sodium, 3.597(3,704)mg potassium, 1.983(1,369)RE retinol, 1.4(1.5)mg thiamin. 1.1(1.4)mg riboflavin. 19.4(20.4)NE niacin. 172(160)mg ascorbic acid and 232(239) mg cholesterol. respectively. The level of triglyceride(TG) was 136.0(114.2)mg/dl, total cholesterol(TC) was 179.1(174.9)mg/dl HDL-C was 43.3(43.3)mg/dl, LDL-C was 109.6(107.9)mg/dl. VLDL-C was 26.8(22.8)mg/dl and AI was 3.2(3.0) And 68.1(79.0)% of men(women) were assessed as Normocholesteroiemia(NC) and 66.2(69.9)% of men(women) as Normotriglyceridemia (NTG). Most of the subjects in Muan-gun preferred and ate onions routinely and had normal range of serum cholesterol and triglyceride level. Therefore it is suggested that intake of onions is effective for decreasing the serum cholesterol and TG levels.

Serum Amyloid A as an Independent Prognostic Factor for Renal Cell Carcinoma - A Hospital Based Study from the Western Region of Nepal

  • Objective: The objective of our present study was to assess the role of serum amyloid A (SAA) in stages and prognosis of renal cell carcinoma. Material and Methods: It was a hospital based retrospective study carried out in the Department of Medicine and Biochemistry of Manipal Teaching Hospital, Pokhara, Nepal between $1^{st}$ January 2008 and $31^{st}$ December 2011. The variables collected were SAA, CRP. Approval for the study was obtained from the institutional research ethical committee. Quantitative analysis of human SAA and C-reactive protein (CRP) was performed by radial immune diffusion (RID) assay for all cases. Results: Of the 422 total cases of renal cell carcinoma, 218 patients had normal and 204 abnormal SAA. SAA levels were grossly elevated in T3 stage ($122.3{\pm}SD35.7$) when compared to the mean for the T2 stage ($84.2{\pm}SD24.4$) (p value: 0.0001). Similarly, SAA levels were grossly elevated in M1 stage ($190.0{\pm}SD12.7$) when compared to the M0 stage ($160.9{\pm}SD24.8$) (p: 0.0001). There was no significant association with elevated CRP levels ($209.1{\pm}SD22.7$, normal $199.0{\pm}SD19.5$). Conclusion: The validity of SAA in serum as being of independent prognostic significance in RCC was demonstrated with higher levels in advanced stage disease.
