Analysis of Two Electrocution Accidents in Greece that Occurred due to Unexpected Re-energization of Power Lines

  • Investigation and analysis of accidents are critical elements of safety management. The over-riding purpose of an organization in carrying out an accident investigation is to prevent similar accidents, as well as seek a general improvement in the management of health and safety. Hundreds of workers have suffered injuries while installing, maintaining, or servicing machinery and equipment due to sudden re-energization of power lines. This study presents and analyzes two electrical accidents (1 fatal injury and 1 serious injury) that occurred because the power supply was reconnected inadvertently or by mistake.

A Development and Characteristics of Remote Emergency Stop Device for Conveyors (컨베이어용 리모트 비상정지장치 개발 및 특성)

  • Conveyors cause a high serious and fatal injuries in the industry. Every year people are hurt or killed as a result of accidents on conveyors. Most accidents occur during operation and maintenance when employees are working on moving, unguarded conveyors or when the conveyors starts unexpectedly. In 2008 there were 486 reported injures ranging from fatalities to injures. Of these 486 reported accidents, 8 were fatalities, 66.9%(325) of reported accidents occurred in manufacturing company and 43.8%(213) occurred at the belt conveyors. The objective of this study was to invent the remote emergency stop device because industrial accidents mainly occur at blind spots where usually do not have any safety guard or device rather than the normal working places. In principle, this new device will use with the existing safety system. Then, it will be powerful safety system for preventing injuries related with conveyors.

Accident Characteristics by Age Construction Workers (건설근로자의 연령별 재해 특성)

  • As the construction project gets bigger in scale, the actual construction itself is becoming more complex. The construction accidents occupy 26.18 percent of total death disaster according to the accidents report in 2007. It is the highest figure among the entire industry, and it shows how serious the construction accident is. In order to reduce accidents occurred in the construction industry, we should find the accident characteristics of construction workers by age. In this study, we investigated 589 cases of fatal accidents, and classified them by trades, type of works, the things that caused the accident, how they occurred, and size. We then identified accident characteristics for each group by analysing them in accordance with workers age. The result of analysis showed that in carpenter and common laborer, the number of death accidents increased as the workers age increased. Another characteristics were that the number of death accidents increased as the age increased in finishing work and reinforced concrete construction and in a smaller-scale businesses, respectively. Common labor was the trade with the highest number of death accidents and the laborers of 50 age or higher in age in finishing working type were the most vulnerable to death accidents.

Analysis of Factors influencing Severity of Motorcycle Accidents using Ordered Probit Model (순서형 프로빗모형에 의한 이륜차 사고심각도의 영향요인 분석)

  • PURPOSES : This study drew factors affecting motorcycle accidents in Seoul by severity using an ordered probit model and aimed to analyze and verify the drawn influence factors. METHODS : As the severity of the accidents could be classified into three types (fatal injury, serious injury and minor injury), this study drew the factors affecting accidents by a comparative analysis employing an ordered probit model, removed the variables that would not secure significance sequentially to construct a model with high explanatory power regarding the factors affecting the severity of motorcycle accidents, and calculated the marginal effect of each factor to understand the degree of each factor's impact on the severity. First, Model 1 put in all variables; Model 2 was constructed by removing the variables of the road surface conditions that could not meet the level of significance (p=0.608); Model 3 was constructed by removing gender variable (p=0.423); and Model 4 was constructed finally by removing age variable (p=0.320). RESULTS : As a result of an analysis, statistically significant variables were time of occurrence, type of accident, road alignment and motorcycle displacement, and it turned out that the impacts on the severity were in the following order: a road alignment of left downhill, the type of motorcycle-to-vehicle accidents and a road alignment of a flatland on the left. The significance of the models was tested using the likelihood ratio, the level of significance and suitability statistics about them, and as a result of the test, the significance level and suitability of the constructed models were all excellent. In addition, the model accuracy indicating the accuracy of a predicted value compared to that of the value actually observed was 70.3% for minor injury; 70.1% for serious injury; and 68.6% for fatal injury, and the overall accuracy was 70.2%, which was very high. CONCLUSIONS : As a result of an analysis of motorcycle accidents in Seoul through the ordered probit model and the marginal effect, it turned out that their severity increased in nighttime accidents as compared to daytime ones and gradually increased in the order of motorcycle-to-vehicle accidents, motorcycle-to-person ones and the ones involving motorcycle only. As a result of an analysis, the severity of accidents in road alignments of left downhill, left flatland and straight downhill increased as compared to those in a road alignment of straight flatland and that the severity of accidents of motorcycles with a displacement larger than 50cc was higher than that of those with a displacement smaller than 50cc.

  • In this paper, the factors of 1680 marine casualties occurred during 1986-1988 are analysed by the Principal Component Analysis. The main results are as follows: 1. Most of marine casualties result from the human factors such as careless operation and insufficient engine maintenance. Engine trouble is main part of accidents and great number of accidents is rated for fishing vessels. 2. Accidents are serious in case of cargo ships, passenger ships and tankers from the point of view of the damage of human life and properties. On the other hand, those are not so serious matter in case of fissing vessels and governmental vessels. 3. Grounding, collision and capsizal mainly result from careless operation, however material defect result in flooding

  • Construction accidents are very serious comparing to its workers. These kind of accidents consist in the particularity of construction. The accident prevention task in construction requires both into immediate approach and longer range approach of instruction, training and education. Information concerning each individual accident, if the information of investigation has been done well, it provides information of prevention accident. At this point, this paper by examining 800 cases of serious accident researched by the Korea Occupational Safety and Health Agency from Jan. 1992 to Dec. 2000 specifically targeting apartment and building construction develops the construction information system to suggest the proper information of construction safety.

  • As the scale of building construction is larger and more complex and the adaptation of new technology is increasing, the tendency of the risk serious accident cases is increasing now. Especially, the accident rate is below 10% at the building construction workers in Korea but among the total industrial accidents it exceeded by 35%. To prevent a construction accident, it is essential that not only to have a through grasp of the characteristic of safety problem but also to establish of efficient safety counter plan are required. According to the type and situation of building construction, the acquirement of necessary information is useful to prevent the hazard of work and establish the counter plan. Among the safety information it is a thing of especial importance that to estimate the hazard of work types for building construction to prepare the counter plan of safety. This study will analyze all of the1600 accidents of the building construction works from the data of KOSHA(Korea Occupational Safety &Health Agency) which were collected during 11 years(1992~2002) data relating to serious accidents of the building construction works, and suggest the counter plan according to the each work type in building construction works.

  • Recently, the importance of disaster prevention has emerged as serious accidents have occurred frequently, in which workers died or seriously injured at industrial sites. In response, the government enacted the 'Act on the Punishment of Major Disasters' to strengthen the responsibility, punishment intensity, and regulation of business owners for accidents at the site. In particular, the construction industry accounts for a large proportion of serious accidents, so safety levels need to be raised through on-site management. This study aims to reduce the occurrence of disasters at construction sites and to improve the work efficiency of safety managers. To derive detailed elements of the safety manager's work, domestic and overseas cases were investigated. The derived items were analyzed for the difficulty and preventive effects of the work through a survey of experts, and they will contribute to disaster prevention by presenting tasks to be focused and suggesting ways to improve inefficient tasks.

An Evaluation of the Quantitative Risk of Plastic Process Manufacturing Industries by Means of the 4M Method

  • This study includes a case study among plastic process manufacturing companies, based on which, the currently used 4M method is applied in terms of machine, media, man, and management, to conduct quantitative risk evaluation, and thus to contribute to reducing human and material loss as well as preventing accidents in industrial fields. The result of this study is analyzed based on the 4M-risk assessment to find out the hazardous risk elements, and the quantitative evaluation made it predictable the value of risk(frequency $\times$ intensity) in such classified levels as serious risk, critical risk, and intolerable risk. Further, Among the businesses with hazardous risk elements and high frequency of industrial disaster, risk analysis was conducted for each process, and as a result, 38 cases among 76, including those of serious risk, critical risk, and intolerable risk, were improved, and the risk was reduced. Besides, it is thought that with the engineering approach with 4M-Risk Assessment, the attempt to improve safety level contributes to prevention of accidents.

Comparison Analysis on the Structural Stability of Concrete Median Barriers Using Commercial Software (상용 프로그림을 이용한 콘크리트 중앙분리대 방호울타리들의 구조적 안정성에 대한 비교 분석)

  • The concrete median barriers are the most popular safety appurtenances that can be installed on narrow medians and are effective in keeping uncontrolled vehicles from crossing into opposing lanes of traffic. It is necessary to install and maintain median barriers because it is very difficult to reserve enough room required for medians in KOREA. And concrete median barriers are accepted as the actual alternatives for median barriers, mostly because they require almost no maintenance even after serious collisions. Typical concrete median barriers are 810mm high and have 596mm high glare screens on top of them. But we have experienced a number of “climb” and “roll-over” accidents of heavy vehicles until now, and most of all, there have been some serious accidents caused by the part of broken glare screens. We have performed many computer simulations for the evaluation of the crashworthiness of concrete median barriers, and through the simulations we have tried to find a proper type of concrete median barrier.
