폐격리증 2례 보고 및 대한 흉부외과학회지에 발표된 19례의 문헌 고찰 (Bronchopulmonary Sequestration - Report of 2 cases and review of 19 cases published in the Korean Journal of Thoracic and Cardiovascular Surgery -)

  • Bronchopulmonary sequestration is a rare congenital malformation of the lung, concerning about the abnormal feeding systemic artery, may happen a serious complication of heeding during operation if not recognized before operation. High index of suspicion of sequestration is the key to successful operative procedure with the aid of characteristic recurrent and long-standing symptoms, its location, and invasive or non-invasive diagnostic tools. We report 2 cases of intralobar type of bronchopulmonary sequestration and review 10 articles about the subject, totaling of 21 cases.

  • Bilateral intralobar and extralobar pulmonary sequestration is an extremely rare anomaly. At present, four pathologically proven cases and two possible cases have been reported in the literature insofar as we know. We have found no previous reports in the literature in which simultaneous bilateral intralobar and extralobar pulmonary sequestration were present. We report a first case presenting as bilateral intralobar and extralobar pulmonary sequestration, recently performed staged bilateral resection, and emphasize the importance of computed tomography and angiography in diagnosis and operation.

  • Carbohydrate antigen 19-9 (CA19-9) is a specific tumor marker of the biliary, pancreatic and gastrointestinal tracts. CA19-9 is occasionally elevated in serum in patiens with benign pulmonary diseases such as bronchiectasis, idiopathic interstitial pneumonia or collagen disease-associated pulmonary fibrosis. Intralobar pulmonary sequestration is an uncommon congenital lung anomaly. It is dissociated from the normal tracheobronchial tree and is supplied by an anomalous systemic artery. There have been some reports of elevation of CA19-9 in this lesion. We report a case of intralobar pulmonary sequestration with elevated serum CA19-9 in a 29-year-old man who was diagnosed with bronchiectasia of left lower lung field on general check up. He had no evidence of any malignant disease in pancreatobiliary or gastrointestinal tracts. Elevated serum CA19-9 level might be encountered with benign pulmonary disease such as pulmonary sequestration.

  • In this study, we investigated the effect of carbon sequestration and sink on the environmental impact assessment of a housing redevelopment project. Through the case study, we found that the amount of carbon sequestration and sink increased with the increase of the area of park and green space and, furthermore, the amount of carbon emission decreased slightly with implementation of district heating and renewable energy. Therefore, it is necessary for its land use plan to be established to minimize the amount of net carbon emission, taking account of both the amount of present carbon emission and the amount of the future carbon sink, sequestration, and emission.

폐격리증에 대한 임상적 고찰 (Clinical study of Pulmonary Sequestration)

  • Pulmonary sequestration occurs when some disturbance produces a cystic mass of nonfunctioning lung tissue which lacks normal communication with the tracheobronchial tree. Between 1971 and 1985, pulmonary sequestration was diagnosed in 11 patients, ranging age from 3 to 29 years. All sequestration were intralobar type. Definitive diagnosis can only be obtained by aortography and/or surgical exploration in 10 cases. The other one was confirmed by pathologic examination postoperatively. The presenting complaints were mostly recurrent local pulmonary infection, but in 2 cases mediastinal mass with respiratory symptoms was presented, and cardiac murmur was only finding in one case. Preoperative diagnostic procedure revealed 3 associated anomalies which were funnel chest, right aortic arch, and pulmonic stenosis with vascular ring. Operative treatment for sequestration was lobectomy in 10 cases, and a segmentectomy in one. There was no operative mortality, but 3 complications [empyema, B-P fistula, post-op bleeding] which were controlled by subsequent operations or conservative measure. Aortography is strongly advocated not only for its diagnostic value, but for its preoperative localization of the aberrant vessels that are the major concern to the surgeon.

동정맥 누공을 합병한 내엽형 폐 격절증의 치험례 (In tralobar pulmonary sequestration associated with A-V fistula)

  • Intralobar pulmonary sequestration is a rare congenital malformation characterized by a cystic portion of the lung that derives its arterial blood supply through aberrant vessel directly of systemic circulation. Two forms of pulmonary sequestration occur; intralobar sequestration is usually contained within the visceral pleura of a pulmonary lobe and its venous drainage to the pulmonary venous system. We presented on case of intralobar pulmonary sequestration with A-V fistula. The patient was 8 years old male and chief complaints are mild fever and exertional dyspnea. Aortogram revealed an aberrant artery originated from thoracic aorta just above the diaphragm and its vessel directly communicated pulmonary vein in sequestrated segment. At time of operation, sequestrated lobe measuring 6x5x5cm, well demarcated at the lower lobe was noted. Aberrant, measuring 1.0cm in diameter and 1cm in length and directly connected pulmonary vein. After division and ligation of the aberrant artery, only excision of sequestrated lobe was performed and complication is not during postoperative course.

폐국균증을 동반한 내엽형 폐격리증 -1례 보고- (Aspergillosis within an Intralobar Sequestration -A case report-)

  • 폐격리증이란 기형적 동맥에 의해 영양공급을 받는 대생 폐조직의 일부를 일컫는 용어이다. 이는 두 가지로 대별되며 -외협형과 내엽형- 다른 임상양상을 보인다. 본 과에서는 폐국균증을 동반한 내엽형 폐격리증 1례를 경험하였다. 환자는 32세 여자로 특별한 증상은 없었으며, 우연히 낭종성 병변을 지닌 폐종물이 발견되었다. 수술시야에서 하폐인대의 비정상적 동맥을 보어 폐격리증으로 진단하고, 우하엽 절제술을 시행하였다.

  • 저자들은 반복적인 폐렴의 병력을 가지고 있는 여성에서 도플러초음파를 이용하여 하행복부대동맥으로부터 분리폐로 체동맥의 주행을 확인하여 폐분리증을 진단하고 이를 수술로 확인하였기에 문헌고찰과 함께 보고하는 바이다.

  • Pulmonary sequestration is a rare congenital anomaly of the lung in which it is separately supplied from the aorta or one of its branches. Bilateral pulmonary sequestration is very rare, particularly in adults. In bilateral pulmonary sequestration, resection of both sides is usually recommended if both sides are infected and symptomatic. We report the case of a 37-year-old female patient with bilateral intralobar pulmonary sequestration treated by staged bilateral lower lobectomy.

Sequestration and Bioavailability of Organopollutants in Soil;Their Implication to the Risk Assessment

  • The bioremediation is an economic technology to remove the organopollutants from soil. It is often found that the remediation could not remove the compounds below the levels determined by vigorous extractions as required by regulatory agencies. The reason for the reduced bioavailability with increasing time of aging has been accredited to the sequestration of the compounds in remote sites within or between soil particles. Then, the aging could be defined as the time-dependent sequestration. Partitioning and entrapment have been suggested as mechanism for aging. The rate and extent of the sequestration varies among dissimilar soils. The bioavailability of aged pollutants in soil could be measured by bioassays, mild solvent extraction, and soild-phase extractions. The sequestration could be affected by many factors including various soil properties, wetting and drying cycle, and the presence of cosolutes and NAPLs etc. The bioavailability and sequestration should be considered to determine the environmentally acceptable endpoint.
