Purpose : This study examines the effects of pre-eccentric exercise and stretch ing to bicepsbrachii to prevent delayed onset muscle soreness and recovery of muscular function depending on the training intensity with 28 normal adults in their twenties. Methods : The subjects were divided into a control group, a group without any previous eccentric exercise, and a stretching group. Pre-eccentric exercise group conducted exercise with the intensity of 25% of maximal voluntary contraction. Pre-eccentric exercise and stretching was applied before to induce delayed onset muscle soreness and after, 24 hour post, 48 hour post, and 72 hour post. Measurements were conducted to examine pain and muscular function changes before, immediately after, and after inducing delayed onset muscle soreness. After inducing delayed onset muscle soreness, measurements were taken at the 24th hour, 48th hour, and 72nd hour. Results : The pre-eccentric exercise group and stretching group showed a significant difference from the control group by isometric contract ion power and mechanical pain threshold as a result of measuring delayed onset muscle soreness. Conclusion : From these results, electrical stimulation using presynaptic inhibition mechanism of transcutaneous electrical stimulation (TES) had positive effects for walking ability on inhibition of muscle tone in lower extremity. The motor level stimulation group experienced a more significant effect than the sensory level stimulation group. Therefore, the transcutaneous electrical stimulation (TES) is considered to be effective on walking ability increasing through inhibition of muscle tone in lower extremity for rehabilitation of post stroke hemiplegic patients.
Purpose: The aim of this study was to analyze the effects of neurofeedback training for reducing stress and enhancing self-regulation in late adolescence to identify the possibility of use for nursing intervention. Methods: A nonequivalent control group pre-post quasi-experimental design was used. Participants were 78 late adolescents assigned to the experimental group (n=39) that received the neurofeedback training and the control group (n=39). Data were collected on heart rate variability (HRV) and skin conductance level (SCL) to assess stress-biomarker response. The questionnaire contained 164 items from: Positive and Negative Affect Schedule (PANAS), Symptom Checklist-90-Revised (SCL-90-R) and Self-regulatory Ability scale. The neurofeedback training was based on the general adaptation syndrome and body-mind medicine. The intervention was conducted in a total of 10 sessions for 30 minutes per session with high-beta, theta and sensory motor rhythm training on scalp at central zero. Results: There were significant difference in standard deviation of normal to normal interval (p=.036) in HRV and SCL (p=.029) of stress-biomarker response between the two groups. Negative affect (p=.036) in PANAS and obsessive compulsive (p=.023) and depression (p<.001) in SCL-90-R were statistically significant. Self-regulation mode (p=.004) in self-regulation ability scale showed a significant difference between the two groups. Conclusion: The results indicated that the neurofeedback training is effective in stress-biomarkers, psychoemotional stress response and self-regulation. Therefore, neurofeedback training using neuroscientific approach based on brain-mind-body model can be used as an effective nursing intervention for late adolescents in clinics and communities for effective stress responses.
본 연구는 지속적인 곡예 운동이 여자 체조, 프리스타일 스키어 및 피겨 스케이터 선수의 동적자세 유지 능력에 미치는 영향을 분석하고자 하였다. 이 연구에 참여한 피험자는 운동경력이 4-6년 된 운동선수로서 체조선수 7명, 프리스타일 스키어 선수8명, 피겨 스케이터 7명과 일반 여학생 10명을 통제집단으로 분류하였다. 동적자세의 운동능력을 비교하고자 뉴로컴사의 동적자세측정기를 이용하여 6가지 각 조건에 맞는 이른바, 시각계, 전정계 및 체성감각계가 동원되는 상황을 연출하면서 실험에 임하였다. 자료처리는 일원변량분석을 실시하였고, 사후검증으로는 집단 간의 차이를 보고자 Scheffe로 하였고, 통계학적 유의수준은 .05로 하였다. 연구결과 운동집단 간에는 조건 5에서 프리스타일 선수와 체조선수 간의 유의한 차이가 있었으며, 운동선수 집단과 일반인 여학생 간에는 조건 2, 3, 4, 5, 6에서 유의한 차이가 나타났다. 결론적으로 지속적인 곡예 운동은 평형기관계를 자극하여 동적자세 조절의 향상에 효과가 있는 것으로 사료된다.
주의 집중력은 워킹메모리에 속하며 경두개직류전기자극은 워킹메모리를 향상시키는데 효과적이다. 본 연구는 경두개직류전기자극(Transcranial Direct Current Stimulation, tDCS)이 대학생들의 주의집중력에 미치는 영향에 대해 알아보기 위해 연구를 진행하였다. 본 연구는 대상자 37명의 대학생($23.08{\pm}3.33$세)을 대상으로 tDCS그룹과 대조군으로 두 그룹으로 나누어 2018년 9월 3일부터 28일까지 연구를 진행하였다. tDCS그룹은($n_1=19$) 2mA로 13분씩 2회 26분간 적용하였으며, 대조군($n_2=18$)은 패드를 붙인 후 작동을 시키지 않았고 13분씩 2회 26분간 적용하였다. 연구는 주 3회 총 4주간 총 12회 진행되었다. 주의 집중력을 확인하기 위하여 뇌전도(Electroencephalogram) 검사를 하였으며, 연구결과 tDCS그룹이 주의집중력이 증가할수록 증가하는 SMR파(Sensory motor rhythm, p<0.01, 95% CI: -1.955, -0.459), 중간 베타파(p<0.05; 95% CI: 0.027, 0.943) 그리고 파워 비율(SMR+중간베타파/ 세타파, p<0.01, 95% CI: -1.764, -0.315)에서 유의한 향상을 보였다. 그러나 집중력 하락시 증가하는 세타파는 유의한 감소를 얻지 못했다(p>0.05). 결과값과 같이 tDCS적용시 주의집중력의 유의한 향상을 보였으며 본 연구를 기반으로 발전시켜 주의력 결핍환자들에게 적용에 대한 기초 자료로 사용될 수 있을 것이다.
본 연구는 노인들의 낙상 위험 기전을 이해하고 필라테스 재활 운동 원리가 노인들의 균형 능력과 자세 안정화에 긍정적 효과를 기대할 수 있는지 선행 연구 자료를 고찰하였고, 재활 필라테스 운동의 낙상 예방 효과에 대한 문헌적 기초자료를 제시하는데 연구의 목적이 있다. 이에 다음과 같은 결론을 제시하고자 한다. 첫째, 재활 필라테스 운동은 척추와 골반의 안정화를 위한 신체 중심부의 강화 운동이 가능하고, 신경근을 촉진시켜 균형과 관절 안정화에 효과가 있다. 둘째, 노화에 따른 고유수용기 감각 저하와 근골격계 퇴행 질환은 균형 능력 상실과 자세 유지의 불안정성을 높여 운동 기능 수행의 어려움과 보행 장애로 낙상 손상 위험을 높이게 된다. 셋째, 재활 필라테스 운동은 노인들의 코어 근력 향상으로 균형과 반응시간 운동 기능 향상을 기대할 수 있으며, 낙상 예방에 관여되는 신체 불균형 개선과 움직임 안정성에 긍정적 영향을 미쳐 낙상 위험률 감소에 기여할 가능성을 제시할 수 있다. 결론적으로 재활 필라테스가 노인운동 프로그램으로서 신체의 근력 향상, 균형 감각 향상, 코어의 안정화 효과를 나타내 근골격계 퇴행에 따른 낙상 손상의 위험 요인을 감소시키고, 노인성 만성질환에 따른 심각한 활동 장애를 예방할 수 있음을 고찰하였다.
The purpose of this study was to investigate the effect of Modified Constraint-Induced Therapy (MCIT) on the effected upper extremity of children with hemiparesis. Four children with hemiparetic upper extremity caused by brain injuries were trained by MCIT for ten weeks. During the same period, all of the subjects were also involved in thirty-minute regular physical therapy and occupational therapy. During the treatment period, the unaffected upper extremities of the subjects were restrained by a specially designed hand splint or a mitten for five hours a day, five days per week. For two hours out of the five-hour restraint period, the affected upper extremities were intensively trained by performing various functional tasks, which were individually structured to emphasize use of the affected arm. A single-subject design with A-B-A reversal was employed in this study. The affected limb motor ability was evaluated by Melbourne Assessment, measuring the time to grasp and release nine pegs, and measuring grasping power. As a consequence of this study, the affected limb motor test scores of all four subjects in the baseline period were improved during the treatment period. Furthermore, the treatment effect was maintained during a one-month follow-up period. The results of this study support the assumption that MCIT is an effective therapeutic method to improve the sensory and motor abilities of hemiparetic children. It also increases the frequency of functional use of the hemiparetic hands of brain-injured children. Based on the results of this study, it can also be assumed that the modified CIT method is especially beneficial to these children by reducing the negative emotional effects of forceful restraint of the unaffected upper extremity. To optimize the functional recovery of the paretic upper extremity by CIT, the restriction period per day should be decided individually, according to the characteristics of the individual.
Guillain-$Barr{\acute{e}}$ syndrome and acute transverse myelitis manifest as demyelinating diseases of the peripheral and central nervous system. Concurrency of these two disorders is rarely documented in literature. A 4-year-old girl presenting with cough, fever, and an impaired walking ability was admitted to hospital. She had no previous complaints in her medical history. A physical examination revealed lack of muscle strength of the lower extremities and deep tendon reflexes. MRI could not be carried out due to technical problems; therefore, both Guillain-$Barr{\acute{e}}$ syndrome and acute transverse myelitis were considered for the diagnosis. Intravenous immunoglobulin treatment was started as first line therapy. Because this treatment did not relieve the patient's symptoms, spinal MRI was carried out on the fourth day of admission and demyelinating areas were identified. Based on the new findings, the patient was diagnosed with acute transverse myelitis, and high dose intravenous methylprednisolone therapy was started. Electromyography findings were consistent with acute polyneuropathy affecting both motor and sensory fibers. Therefore, the patient was diagnosed with concurrency of Guillain-$Barr{\acute{e}}$ syndrome and acute transverse myelitis. Interestingly, while concurrency of these 2 disorders is rare, this association has been demonstrated in various recent publications. Progress in diagnostic tests (magnetic resonance imaging and electrophysiological examination studies) has enabled clinicians to establish the right diagnosis. The possibility of concurrent Guillain-$Barr{\acute{e}}$ syndrome and acute transverse myelitis should be considered if recovery takes longer than anticipated.
목적 : 뇌성마비 아동의 자세조절을 향상시키기 위해 국내에서 사용되는 운동 중재의 방법과 효과를 알아보기 위해 체계적 고찰을 사용하여 근거를 제시하고자 하였다. 연구방법 : 누리미디어(DBpia), 학술연구정보서비스(RISS) 그리고 한국학술정보(KISS)의 데이터베이스를 사용하여 2006년 10월 이후로 게재된 국내학회지를 검색하였다. '뇌성마비', '자세조절', '균형'를 키워드로 검색하여 총 18개의 연구가 연구대상으로 선정되었다. 각 연구는 근거의 질적 수준과 방법론적 질적 수준으로 분석하였고 그 결과는 대상자, 중재, 대조군, 결과에 따라 Patient, Intervention, Comparison, Outcome(PICO)로 분석하였다. 결과 : 본 연구의 분석에 사용된 연구의 근거수준은 수준 I이 8개, 수준 III이 5개, 수준 IV가 2개, 수준 V가 3개이었고, 방법론적 질적 수준은 '우수'의 연구가 가장 많았다. 대상자는 경직형 양하지 마비, 실험설계는 무작위 대조군 실험 연구가 가장 많았다. 분석결과 국내에서 사용되는 자세조절 운동 중재는 뇌성마비 아동의 운동능력과 균형능력 향상에 긍정적인 영향을 보였다. 결론 : 국내에서 사용되는 운동 중재는 뇌성마비 아동의 자세조절 능력을 향상시켰다. 앞으로 뇌성마비 아동의 유형별 자세조절 향상을 위한 연구가 이루어져야 할 것이다.

반응성 애착장애 아동의 발달적 특징 및 정신병리적 현상에 대한 이해를 위하여 이들과 전반적 발달장애 아동과의 비교연구를 실시하였다. 연구대상은 1989년 5월부터 1991년 9월까지 서울대학교병원 어린이병원 소아정신과 외래에서 통원치료를 하거나 주간치료실에서 치료교육하였던 아동중, 소아정신과 의사에 의하여 진단한, DSM-111-R의 진단기준을 만족시키는, 만 2세에서 만 6세까지의 아동으로, 반응성 애착장애 아동이 20명(남 18,여 2),전반적 발달장애 아동이 20명(남 20) 이었다. 이들의 정신과적 진단을 모르는 평가자에 의해 Schopler의 교육심리적 평가(PEP ; Psycho-Educational Pronto)가 실시되었으며, 각 발달영역별 기능수준과 정신병리적 특성에 관하여 두군간의 차이를 검증하였다. 연구결과는 반응성 애착장애아동은 발달 전반에 걸쳐 지체가 되어 있었으며 지체의 정도는 지각능력>모방>대근육운동>인지수행, 눈-손 협응능력>인지언어, 소근육운동 순으로 지체되어 있었다. 반응성 애착장애 아동의 정신병리적 특성으로는 언어>놀이>사람과의 관계성>대인감정>감각순으로 심한 정신병리를 보였다. 반응성 애착장애 아동과 전반적 발달장애 아동의 두집단간 발달기능수준의 차이 검중에서는 지각영역의 싹트기 점수와 대근육운동의 싹트기 반응이 두군간 차이를 보였다. 정신병리 영역에서는 대인감정과 사람과의 관계에서 반응성 애착장군군은 '없음'에, 전반적 발달장애군은 '심함'에 해당되었다. 놀이 및 자료에 대한 흥미, 감각이상, 언어병리성에서 '없음'에 반응성 애착장애군이 전반적 발달장애군에 비하여 통계적으로 유의하게 높게 나타났다.

Objectives: Our school introduced a new curriculum based on faculty-directed, intensive, small-group teaching of clinical skills in the third-year medical students. To examine its effects, we compared the mean scores on an OSCE between the third- and fourth-year medical students. Methods: Third- and fourth-year students did rotations at the same five OSCE stations. They then completed a brief self-reporting questionnaire survey to examine the degree of satisfaction with new curriculum in the third-year students and clinical practice in the fourth-year students, as well as their perception of confidence and preparedness. We analyzed the OSCE data obtained from 158 students, 133 of whom also completed the questionnaire. Results: Mean OSCE scores on the breast examination and wet smear stations were significantly higher in the third-year group (P < 0.001). But mean OSCE scores of motor-sensory examination and lumbar puncture were significantly higher in the fourth-year group (P < 0.05). The mean OSCE scores had no significant correlation with satisfaction. In addition, the self-ratings of confidence had a high degree of correlation with satisfaction with new curriculum (r = 0.673) and clinical practice (r = 0.692). Furthermore, there was a moderate degree of correlation between satisfaction and preparedness in both groups (r = 0.403 and 0.449). Conclusions: There is no significant difference in the effect on the degree of clinical performance and confidence between an intensive-small group teaching and a 1-year clinical practice. If combined, intensive small group teaching and clinical practice would be useful to improve the degree of ability and confidence in medical students.
