  • The purpose of this study is to investigate the current characteristics of healthcare and welfare policies in Japan focused on the related policies in 2000's. In Japan, with the perspectives of entering the super-aged society by 2005 and the movement of all of the baby boom generation into the elderly generation by 2015, more rapid progress to the aging society is expected. From this situation, Japanese government has developed diverse elderly-oriented strategies such as "structural reform of social security", "nursing care system" and "gold plan 21" in 2000's. However, most of the related researches targeted the period to early 2000's, excluding the papers about the individual policies. Recently in our country, many related policies have been developed such as the enforcement of "long term care insurance system for the elderly" by 5 year period reevaluations and the preparation for 2nd step of "Plan for Ageing Society and Population"(2011~2015). At this moment, the investigation on the related policies of Japan could give appropriate references to us, a late starter of aging society.

  • The Korean Academy of Health Policy and Management (KAHPM) has shown remarkable achievements in the field of health policy and management in Korea for the last 30 years. The HPM invited authors, including former presidents of the KAHPM and current board members, to write about main issues in health policy and management. Although the HPM tried to set up an invited author on all subjects in the health policy and management field, 19 papers are published, that completed the peer review process by August, 2018. The authors of the special issue of the 30th anniversary of the KAHPM include six former presidents, a senior professor, and 12 board members. The subjects of this issue are reform of the healthcare delivery system, health insurance and medical policy, reform of health system governance, the role of National Health Insurance Service (NHIS), the Korea Institute for Health and Social Affairs (KIHASA) and the National Evidence-based healthcare Collaborating Agency (NECA), ethical aspects of health policy change, regional disparities of healthcare, healthcare accreditation, new healthcare technology evaluation system, globalization of the healthcare industry, the epidemiological investigator system, the quarantine system, safety and disaster, and official development assistance. There are some remaining topics to deal with for the KAHPM: aged society, anti-smoking, non-infectious disease, suicide, healthcare resources, emergency medical care, out-of-pocket money, medical fee payment system, medical aid system, long-term care insurance, industrial accident compensation insurance, community-centered health welfare system, and central government and local government of health. The HPM will continue to publish review articles on the main topics in health policy and management. This is because the KAHPM, which has been the leading academic society of Korea's health policy and management for the last 30 years, feels responsible for continuing its mission for the next 30 years.

  • Canada is a democratic country, yet it keeps a social democratic system in which the government is in charge of welfare of its people. And this is one of the most significant features about the country. Her public and private pension system has been effective since the 1920s, securing its people's fundamental income. In particular, the public medical system applies to its every citizen and performs its role. This system is called the National Medical System as well as "MEDICARE" named after its related law. However, there has been a significant change in the national medical and welfare policy due to the budget deficit. In other words, the policy was mainly implemented to welfare facilities in the past, but the policy changed to a welfare policy for the elderly with a concentration on the support for self-reliance of senior citizens since the reform. The purpose of this study is to provide data and implications for Korea through the analysis of the current situation and distinct features of the housing welfare system in Canada. This study has researched the literature on the subject with an analytic focus on three aspects that are the fundamental frame of the system, essential content (support for self-reliance and facility composition), and distinct features of the housing for the elderly. In other words, they are, first, how the fundamental frame of the housing welfare system for the elderly is composed; second, how the service for self-reliance welfare and facility service are composed; and third, what their scale and distinct spatial features of general houses for the elderly with self-reliance are. A comparative study was conducted in detail on courses and characteristics of the housing welfare system for senior citizens in Canada and the USA of North America. In particular, it reveals the scale and distinct spatial features of public houses for the elderly with self-reliance in British Columbia (BC) which is one of the main provinces of Canada.

  • Serious social problems related to the elderly have appeared in the Gangwon Province because the population in this area is rapidly aging. Gangwon has the highest suicide rate and the highest percentage of single households. In the process of preparing for the super-aging society, Time Bank (TB) System has been actively used in 34 countries, especially in the United States, the United Kingdom, and Japan. In Korea, the Gumi Senior Club has joined this system since 2004. TB is a multilateral exchanged system based on the philosophy that everyone's time is valued equally. Time credits are given to the person who provides social services, such as citizen participation and elderly care. People receive the desired services using time credits or donating them. If this system were applied to the Gangwon Province in accordance with its characteristics, it would help to reduce the elderly's prejudices and manage their diverse health problems. A virtual local currency using IT technologies is needed to boost the regional economy. The elderly's participation in TB is strongly needed. Future research about the effectiveness of health management is then discussed.

  • Objectives: The high readmission rate of patients with chronic obstructive pulmonary disease (COPD) has led to the worldwide establishment of proactive measures for identifying and mitigating readmissions. This study aimed to identify factors associated with readmission, as well as groups particularly vulnerable to readmission that require transitional care services. Methods: To apply transitional care services that are compatible with Korea's circumstances, targeted groups that are particularly vulnerable to readmission should be identified. Therefore, using the National Health Insurance Service's Senior Cohort database, we analyzed data from 4874 patients who were first hospitalized with COPD from 2009 to 2019 to define and analyze readmissions within 30 days after discharge. Logistic regression analysis was performed to determine factors correlated with readmission within 30 days. Results: The likelihood of readmission was associated with older age (for individuals in their 80s vs. those in their 50s: odds ratio [OR], 1.59; 95% confidence interval [CI], 1.19 to 2.12), medical insurance type (for workplace subscribers vs. local subscribers: OR, 0.84; 95% CI, 0.72 to 0.99), type of hospital (those with 300 beds or more vs. fewer beds: OR, 0.77; 95% CI, 0.66 to 0.90), and healthcare organization location (provincial areas vs. the capital area: OR, 1.66; 95% CI, 1.14 to 2.41). Conclusions: Older patients, patients holding a local subscriber insurance qualification, individuals admitted to hospitals with fewer than 300 beds, and those admitted to provincial hospitals are suggested to be higher-priority for transitional care services.

Results: Depending on income levels, there were differences in their oral health status as to whether they use oral hygiene products, take dental examinations, join private health-insurances, and delay dental treatments or not. And educational levels also made significant differences in their oral health status as to whether they smoke, drink alcohol, how many times they brush teeth a day, whether they use oral hygiene products, take dental treatments, and join private health-insurances. Regression analysis on the relationship between their oral health status and the oral health quality of life showed that there were significant differences depending on whether they take dental treatments, delay dental medical treatments, smoke, take oral examinations, how many times they brush teeth a day, and whether they use oral hygiene products, or join private health-insurances.
