  • 우리나라가 급속도로 고령화 사회로 진입하여 감에 따라 의료비 등 국민적 부담이 크게 증가하고 있는 추세에 있다. 본 논문에서는 노인들의 삶의 질을 제고하고, 국가적으로는 노인의료비 부담을 경감시킬 수 있는 노인케어 유비쿼터스 시스템 u-SCS(Ubiquitous Senior Care System)에 대하여 연구하였다. 본 u-SCS 시스템은 조도 센서에서 감지한 조도값의 차이를 분석하여 실내의 노인이 불을 끄고 켜고 한 정보를 파악한다. 이러한 점등 및 소등 시간에 대한 분석은 노인의 수면 시간 등 일상적인 노인 행동을 인지할 수 있는 정보이므로, 이를 기초로 노인들이 겪을 수 있는 건강상 문제나 일상 생활의 불편함을 해소하는데 중요한 기초 정보로 활용할 수 있다.

  • 선진각국에서 고령화 문제를 해결하기 위하여 고령친화 산업과 관련된 많은 연구들이 진행되고 있다. 본 논문에서는 노인 요양 또는 복지 시설 내 노인들의 거주 환경 변화 및 거주자의 활동 양상을 측정하는 유비쿼터스 노인케어 시스템 knu-SCS(KangNam Ubiquitous Senior Care System)에 대하여 연구하였다. knu-SCS 시스템은 조도, 습도, 온도 센서, 카메라 등으로 구성되며, 각종 센서들이 감지한 데이터를 저장, 가공, 분석하여 데이터베이스에 저장을 하고 저장된 데이터들을 사용자들에게 시각적으로 제시하기 위하여 센서 웹을 구현하였다. 센서 웹에서 제공하는 데이터들은 일상적인 노인 행동 및 생활환경을 인지할 수 있는 자료가 되므로, 이를 기초로 노인들이 겪을 수 있는 건강상 문제, 갑작스런 생활의 변화감지, 일상생활의 불편함을 해소하는 등 노인케어에 효과적으로 활용될 수 있다.

Effects of Health Status and Health Management on Activities of Daily Living among Urban-Dwelling Older Koreans (도시 재가노인의 건강상태, 건강관리형태 및 일상생활수행능력)

  • Purpose: The purpose of this study was to identify the effects of health status and health management on activities of daily living (ADL) in older, urban-dwelling Koreans. Methods: A total of 206 subjects were recruited from one senior welfare center, six senior citizen centers, and subjects' home in two cities. Data was collected with self-reported questionnaires in order to measure health management, health status, and ADL. Data was analyzed by t-test, ANOVA, and stepwise multiple linear regression using SPSS/WIN 22.0. Results: ADL in this subject were different depending on their age, education, marital status, type of family, and average monthly living expenses. Multivariate analysis showed that age (${\beta}=-0.35$, p<.001), exercise ability (${\beta}=0.20$, p<.001), diabetes (${\beta}=-0.17$, p<.001), osteoarthritis (${\beta}=-0.15$, p<.001), caregiver (${\beta}=0.14$, p=.005), frequency of health management (${\beta}=-0.13$, p=.006), smoking (${\beta}=-0.11$, p=.019), hypertension (${\beta}=-0.10$, p=.027), and type of family (${\beta}=-0.10$, p=.036) were significantly associated with ADL. Overall, approximately 60.2% of total variability in ADL could be explained by the 11 variables in this model ($R^2=0.602$, F=32.06, p<.001). Conclusion: This study suggests that individualized health care should be continued for older, community-dwelling Koreans in order to improve their ADL. Moreover, we need to develop self-care programs and encourage them to participate in those programs.

  • The purpose of this study was to examine the impact of education for welldying on the death anxiety and death reception of care workers who were most closely linked to death among workers who were engaged in senior welfare. It's basically meant to let care workers have a good understanding of death, death process and death-related factors to help elderly people close their life in a comfortable manner. The subjects in this study were the care workers who worked in J nursing home in the region of Gwangju. They received education in nine sessions, once a week, and the collected data were analyzed by the statistical package 15.0. The statistical analysis methods used in this study were reliability analysis, descriptive statistics analysis, t-test and ANOVA. The findings of the study were as follows: First, the welldying program participants showed a decrease in death anxiety. Second, the welldying program participants became more receptive to death.

  • The purpose of this research was to find the present condition on dementia patients clothes design including shapes, colors, construction and details in domestic. This research was for confirming the present problems, suggestions, and basic informations for development dementia patients clothes proto-types. The research methods and process were as follows; 1. The total 20 institutes (5 dementia specialty hospitals, 3 the senior hospitals and 11 nursing care centers) were researched during from June to August, 2005 by trained researchers who had experiences researches. 2. The total 30 nurses and care-givers were interviewed for find the present problems related the dementia patients clothes. 3. The total 39 suits for dementia patients clothes were collected and the design analysed such like shapes, colors, constructions and details by taking pictures and drawing flat works. 4. The present condition about the clothes and problems were found and suggest new ideas. The results were belows; 1 The most of institutes of domestic area had two piece type like general hospital patients' clothes for dementia patients clothes. It means there are no concerning now for old dementia patients. 2. The white colors were firstly used as fabric ground and secondly pale tone or pastel tone colors were used The blue and pink were used as pattern colors. The pattern colors were usually moderate tones. 3. The characteristics of constructions and details were round neckline, buttons, full-length sleeves, straight pants, ankle length pants. 4. New functional fabrics and designs for emotional satisfaction were demanded for dementia patients.

  • Aging our country is facing now is seeking qualitative and quantitative expansion of social welfare services. In particular, the rapid increase in the aging population is an urgent need to resolve the problems of inducing demand for medical care and various welfare related to the elderly, In addition, IT technology in Korea is improving daily, in particular, fields of ubiquitous have been studied actively. In this paper, U-Health Care System model technology for the elderly were studied. Existing residential and senior living environment dedicated to the Care system for the fusion of design, and content designed around the ubiquitous technology that combines the old Care system was studied. Existing community-based model that combines technology and IT systems and mobile elderly Care services in conjunction with other devices by providing an application, the existing Care and medical needs will be able to solve the problems.

Comparison of Spiritual Well-Being and Attitudes toward Death due to Taking a Hospice Care Subject in Nursing Students (간호 대학생의 호스피스 수강 유무에 따른 영적안녕과 죽음에 대한 태도 비교)

  • Hospice should help a patient maintain spiritual well-being and its attitudes toward death can have great effects on treatment and nursing of patients on terminal patient. The purpose of this study was to examine the differences in spiritual well-being and attitudes toward death due to taking a Hospice Subject in Nursing Students This is a comparative study design in which 31 senior nursing students of taking a hospice care subject and 27 senior nursing students of not taking a hospice care subject in college of nursing K university, that is located B metropolitan city in Korea. The instruments for spiritual well-being measuring tools was Kim's (2006) revision and complement of the translation by Choe (1990) on the basis of the spiritual well-being scale developed by Paloutzian and Ellison (1983). The instruments for attitudes toward death was researches revision and complement of the translation by Kim(1992) on the basis of the attitudes toward death Measuring Tool developed by Thorson and Powell (1988). Data were collected from June 1 through June 30, 2006; the researcher got permission from the subjcts, explained objectives of the research to subjects personally and got their permission, and distributed structured questionnaires to make a response in a self-administered basis. For data analysis, an SPSS WINDOWS 12.0 program was used for frequency, percentage, the mean, standard deviation, the minimum, the maximum, x2-test, ANCOVA, and Pearson's correlation coefficients. The main results of this research are as follows: 1. There were no differecnces the degree of spiritual well-being and the degree of attitudes toward death between nursing students who took a hospice subject and nursing students who did not take a hospice Subject, except several items of attitudes toward death. There was significant positive correlation (r= .508, p= .000) between subjects spiritual well-being and attitudes toward death. That is, the higher spiritual well-being, the more positive attitudes toward death. In conclusion, although there were no differecnces spiritual well-being and attitudes toward death between nursing students who took a hospice care subject and nursing students who did not take a hospice care subject, in nursing students, spiritual well-being was relatively high and their attitudes toward death was relatively positive. To help a nursing students make a positive change in attitudes toward death, a nursing intervention program is necessary to improve spiritual well-being.

Effects of Evidence Based Practice Integrated Critical Care Clinical Practicum (근거중심실무 연계 중환자간호 실습교육의 적용 및 효과)

  • Purpose: This study examines the effects of integrating Evidence Based Practice (EBP) into a critical care clinical practicum on nursing students' access and use of information resources and EBP competency. Methods: A one-group pretest-posttest design was used. Fifty senior nursing school students from a university participated. A critical care clinical practicum combined with EBP consisted of six full days of clinical practicum in intensive care units with EBP education. Group and individual training in EBP skills, lectures, small group discussion and conferences were provided. Data were analyzed using paired t tests for 50 participants. Results: The scores of evidence based practice competency increased significantly (p<.001) showing significant improvement in searching and classifying the evidence. Nursing students' access and use of research evidence improved (p=.004). Conclusion: This study showed that the integration of EBP into a clinical practicum was effective in improving accessibility and usefulness in research evidence such as guidelines and research articles, and increasing EBP competency in undergraduate students.

A Methodological Triangulation Study on the Experience of Horizontal Violence in Intensive Care Unit Nurses (중환자실 간호사의 수평적 폭력 경험에 관한 방법론적 트라이앵귤레이션 연구)

  • Purpose: The purpose of this study was to investigate the experience of horizontal violence in intensive care unit (ICU) nurses. Methods: This is a methodological triangulation study consists of quantitative and qualitative research methods. A total of 134 ICU nurses from 5 hospitals participated in a survey on nurse to nurse horizontal violence, and the qualitative data were collected through 3 focus group interviews. Results: 94.0% of the participants have experienced horizontal violence within the past 6 months. The question with the highest mean score was "dominating senior nurses mood", and the question with the lowest mean score was "physical assaults". The qualitative data analysis revealed 17 themes on the backgrounds, types, and influence of horizontal violence. Conclusion: The survey result shows that the problem of horizontal violence in ICU nurses is serious, and the focus group interview extracts additional types of horizontal violence such as "scolding in a wrong way", "pressing to resign", "leaving out in the cold", and "burning". Further research needs to be done to build a multidimensional model of horizontal violence in the nursing workplace.

An Analysis of University-based Continuing Care Retirement Communities' Services and Programs Management - With a Focus on Three Cases in the United States - (대학연계형 연속보호체계형 노인주거환경의 서비스 및 프로그램 운영 분석 연구 - 미국의 사례를 중심으로 -)

  • This study aimed to identify services and social and leasure programs that will be needed for helping senior residents to be aging healthfully and comfortably in the university-based continuing care retirement community (UBRC). This study conducted case study targeting three representative university-based CCRCs in the east area of United States such as Oak Hammock (OH) at Florida state, Kendal at Lexington (KL) and The Collonades (C) at Virginia state. There were three types of services in the UBRC, which were meal services, healthcare services, and personal services. Those services were different from the level of care such as independent living, assisted living, nursing home and memory care. Meal services and personal services were very similar among three UBRC, whereas healthcare services of OH were higher level of quality and diversity than the others. The social and leisure programs were categorized in-house programs, university outreach program and intergenerational programs between residents and students attending to related university. In-house programs and university outreach program were almost similar among three UBRC, whereas intergenerational programs of OH were well organized and actively operated than the others. This study results are expected to provides essential information for the development of Korean style services and programs of university-based CCRCs in the future.
