A Study on the Effect of Preoperative Nursing Informations on the Self-Care in Abdominal Surgery Patients. (수술전 간호정보 제공이 복부수술환자의 자가간호 수행에 미치는 영향)

  • The purpose of this study was to examine the effect of self-care of abdominal surgery patients in surgical intensive care unit after offering preoperative nursing information. This study worked out nonequivalant control group post test only design as suspected experimental study. This subjects of the study were 45 patients, 23 in experimental group and 22 in control group. This study was carried at a university hospital in Taegu between January 12, 1999 and March 13, 1999. The experimental group was supplied with preoperative nursing information and the control group received routine care in ICU. The measuring tools of this study were assessment of the self-care role behavior. The data were analyzed by SPSS computer program, $x^2-test$ and t-test and ANOVA were utilized for testing the difference between the experimental and the control group. The hypothesis was examined by t-test. The result of this study are summarized as follows ; 1) The hypothesis, 'The experimental group will show a higher level of self-care than the control group' was accepted(t=-8.958, p=.000). 2) We could not find any meaningful relation about general characteristics and disease associated characteristics between the experimental and the control group. 3) The self-care behavior scores according to the general characteristics and disease associated characteristics show significant statistical difference by economics status (p=.033). 4) The rate of self-care behavior scores of each questionnaire are below average in the questions that request active activity to prevent postoperative complications From the result of this study, the self-care of the experimental group was higher than those of the control group, and the nursing information can be regarded as an effective nursing intervention for preoperative patients.

A Survey on Knowledge of Diabetes and Self-care Behavior of Rural Patients with Diabetes Mellitus (농촌지역 당뇨병 환자의 당뇨지식 및 자가관리에 대한 연구)

  • Purpose: The purpose of this study was to examine the knowledge of diabetes and self-care behavior of patients with diabetes mellitus living in rural area. Methods: Participants in this study were 94 patients with type 2 diabetes mellitus. Data were collected from July 13 to September 14, 2012. The instruments were the knowledge of diabetes measurement developed by Park Oh Jang and the measurement of self-care behavior for patients with diabetes developed by Kim Young Ok. Data were analyzed using the SPSSWIN 18.0 program for t-test, one-way ANOVA and Scheffe test. Results: There were significant differences in knowledge of diabetes according to education level (p=.011), alcohol use (p=.010), regular exercise (p=.046), and duration of illness (p=.045). There were significant differences in self-care behavior of patients with diabetes according to satisfaction with income (p=.031), regular exercise (p<.001), experience with diabetes education (p=.001), number of diabetes education sessions (p<0.001), and complications (p=.001). Conclusion: Based on the result of this study, there is a need to develop and confirm the efficacy of education programs by examining their fit to characteristics of patients with diabetes mellitus living in rural areas.

A Study on the Development and Effects of Slides / Tape Program for the Hypertensive Patients진 Self-Care (고혈압환자의 자가간호증진을 위한 Slides / Tape 프로그램 개발 및 효과)

  • The self-efficacy as the mediating variable to connect knowledge to behavior was Identified as the most powerful predictor that determines behavior. The purpose of this study was, first, to develop the slides /tape program based on the self-efficacy theory of Bandura(1977) second, to investigate that the Slides /Tape program would increase self-efficacy and self-care in patients with hypertension. The subjects for this study were 54 hypertensive outpatients and data collection was carried out from January 9, 1995 until March 18, 1995. The design of this study was one-group pretest and posttest design and the results are as follows 1) The slides /tape program was developed to based on performance accomplishment and vicarious experience which are induction modes of efficacy expectation. It composed of 80 slides and 20 minutes long tape. 2) The effectiveness of the slides /tape program “The self-efficacy score after giving the slides /tape program will be higher than one before giving the slides /tape program.” was supported (t=3.67, p=0.001). “The self-care score after giving the slides /tape program will be higher than one before giving the slides /tape program.” was supported(t=5.23. 000), 3) A significant difference was found between the self-care and sex. Also, the significant difference was found between the self-care and job. From these results, the slides /tape program is effective to increase the hypertensive patients' self-care.

The Study Related to Perceived Stress, Stress Coping Method, Self-Care Behavior and Hemoglobin $A_1c$ of Elderly Diabetic Mellitus Patients (노인 당뇨병환자의 스트레스 지각, 스트레스 대처방식, 자가간호행위 및 당화혈색소와의 관계)

  • Purpose: The purpose of this studγ is to understand relations among perceived stress, stress coping method, self-rare behavior and Hemoglobin $A_1c$ of elderly diabetic mellitus patients. Method: Participants were 128 patients who were under medical treatment in outpatient department at C university hospital. The collected data were analyzed using technical statistics, frequency, percentage, T-test, ANOVA and person's correlation coefficient by SPSS 12.0. Result: There were positive correlation between self-care behavior and coping method based on emotions under the lower category of stress coping method. Hemoglobin $A_1c$ of patients had a negative correlation with physical exorcise of self coping method under the lower category. Perceived stress had also a negative correlation with emotions as coping method in the lower category and medication of self-care behavior. Conclusion: It was noted that elderly diabetic mellitus patients didn't much use a coping method based mainly problems and another method based emotions in case of perceived stress. The findings of this study will be useful for developing an nursing intervention program focused on elderly diabetic mellitus patients.

Self-Efficacy as a Predictor of Self-Care in Persons with Diabetes Mellitus: Meta-Analysis

  • Diabetes mellitus, a universal and prevalent chronic disease, is projected to be one of the most formidable worldwide health problems in the 21st century. For those living with diabetes, there is a need for self-care skills to manage a complex medical regimen. Self-efficacy which refers to one's belief in his/her capability to monitor and perform the daily activities required to manage diabetes has be found to be related to self-care. The concept of self-efficacy comes from social cognitive theory which maintains that cognitive mechanism mediate the performance of behavior. The literature cites several research studies which show a strong relationship between self-efficacy and self-care behavior. Meta-analysis is a technique that enables systematic review and quantitative integration of the results from multiple primary studies that are relevant to a particular research question. Therefore, this study was done using meta-analysis to quantitatively integrate the results of independent research studies to obtain numerical estimates of the overall effect of a self-efficacy with diabetic patient on self-care behaviors. The research proceeded in three stages : 1) literature search and retrieval of studies in which self-efficacy was related to self-care, 2) coding, and 3) calculation of mean effect size and data analysis. Seventeen studies which met the research criteria included study population of adults with diabetes, measures of self-care and measures of self-efficacy as a predictive variable. Computation of effect size was done on DSTAT which is a statistical computer program specifically designed for meta-analysis. To determine the effect of self-efficacy on self-care practice homogeneity tests were conducted. Pooled effect size estimates, to determine the best subvariable for composite variables, metabolic control variables and component of self-efficacy and self-care, indicated that the effect of self-efficacy composite on self-care composite was moderate to large. The weighted mean effect size of self-efficacy composite and self-care composite were +.76 and the confidence interval was from +.66 to +.86 with the number of subjects being 1,545. The total for this meta-analysis result showed that the weighted mean effect sizes ranged from +.70 to +1.81 which indicates a large effect. But since reliabilities of the instruments in the primary studies were low or not stated, caution must be applied in unconditionally accepting the results from these effect sizes. Meta-analysis is a useful took for clarifying the status of knowledge development and guiding decision making about future research and this study confirmed that there is a relationship between self-efficacy and self-care in patients with diabetes. It, thus, provides support for nurses to promote self-efficacy in their patients. While most of the studies included in this meta-analysis used social cognitive theory as a framework for the study, some studies use Fishbein & Ajzen's attitude model as a model for active self-care. Future research is needed to more fully define the concept of self-care and to determine what it is that makes patients feel competent in their self-care activities. The results of this study showed that self-efficacy can promote self-care. Future research is needed with experimental design to determine nursing interventions that will increase self-efficacy.

A Study on the Relationship of Self Care Behavioral Compliance and Perceived Self-Efficacy in Type II Diabetic Patient (제2형 당뇨병 환자의 자기간호행위 이행과 자기효능감에 관한 연구)

  • This study was designed to identify the relationship between perceived self-efficacy and self care behavioral compliance in type II diabetic patient. The subjects for this study were the 113 adults with type II diabetes that received hospital treatment or attended as outpatients in 2 general hospitals at Inchon City. The data were collected during the period from August 15 to semtember 15, 2000 by means of an interview. Collected data were analyzed by means of descriptive statistics, t-test, ANOVA, Pearson's correlation using the SPSS/PC+ program. The results of this study were as follows : 1. The mean score of self care behavioral compliance was 3.06. 2. There were statistically significant difference in self care behavioral compliance according to sex(p<0.05), age (p<0.001), BMI(p<0.05), education(p<0.001), occupation(p<0.001), experience of diabetic education(p<0.05) and complication(p<0.05). 3. The mean score of perceived self-efficacy was 3.45. Self care behavioral compliance and perceived self-efficacy had a positive correlation which was statistically significant (r=0.3879 p<0.001). 4. Stepwise multiple regression analysis was performed to identify the influencing factors for self care behavioral compliance. Education, experience of diabetic education and self-efficacy accounted for 41% of the variance in self care behavioral compliance. The results suggest that education, experience of diabetic education and self-efficacy are important variables in the compliance of self-care behavior with typeII diabetes.

Factors influencing Nursing Students' Health Behavior related to Particulate Matter (간호대학생의 미세먼지 관련 건강행위 영향요인)

  • Purpose: This study was conducted to identify factors affecting health behavior related to particulate matter (PM) of among nursing college students. Methods: A cross-sectional survey design was used. Data were collected from 261 volunteering nursing college students from three universities via an online survey. The survey questions measured PM risk perception, self-care agency, and health behavior related to PM. The study was conducted from June 22 to August 21, 2020. The data were analyzed using descriptive statistics, independent t-test, ANOVA, Pearson's correlation coefficient and multiple regression analysis. Results: The factor with the greatest influence on health behavior related to PM was PM risk perception, followed by self-care agency and the frequency of going out. These variables explained about 41% of the types of health behavior related to PM. Conclusion: PM risk perception and self-nursing agency were found to be significantly correlated with health behavior related to PM. This result suggests that there is a need to develop an educational program for nursing students to improve PM risk perception and self-nursing agency.

Development and Evaluation of a Self Care Education Program for Elderly Patients with Percutaneous Coronary Intervention (경피적 관상동맥 중재술을 받은 노인환자의 자가간호 교육프로그램 개발 및 평가)

  • Purpose: The purpose of this study was to identify the effects of the education program to promote self care for elderly patients with percutaneous coronary intervention. Method: The research was a quasi-experimental study using a nonequivalent control group non-synchronized design. The participants was 56 patients, 28 in the experimental group and 28 in the control group. A pretest and a posttest were conducted to measure main variables. The self care education program, consisting of flash animation, leaflet and telephone counseling, was given for the experimental group. Data were analyzed using ${\chi}^2$, Fisher's exact test, and independent t-test with SAS statistics program. Results: There were statistically significant increase in knowledge and self care behavior, and decrease in anxiety by difference between a pretest and a posttest in the experimental group compared to that in the control group. Conclusion: The result of this study indicate that the self care education program is effective in increasing knowledge and self care behavior, and in decreasing anxiety for elderly patients with percutaneous coronary intervention. Therefore, it can be usefully utilized in the field of nursing for elderly patients with percutaneous coronary intervention.

Self-care Behaviors And Physiological Index According to Cognitive Function of Hemodialysis Patients (혈액투석환자의 인지기능에 따른 자가간호 행위와 생리적 지표)

  • The purpose of this study was to investigate correlation with self-care behaviors and physiological index according to cognitive function of hemodialysis patients. The data were collected from 108 patients on hemodialysis in university hospital. The period of data collection was from January 4 to January 31, 2010. The instruments of research were MMSE, Self-care behavior scale for hemodialysis patients. The 17.6% of patients showed the cognitive disorder. Self-care behaviors of patients showed that normal cognitive function group was significantly higher than dysfunction group. Creatinine and parathyroid hormone of patients showed that normal cognitive function group was significantly lower than dysfunction group. The cognitive function of patients showed positive correlations with self-care behavior(r=.283, p=.003) and Kt/V(r=.214. p=.026), showed significant negative correlations with phosphorus(r=-.223, p=.020) and creatinine(r=-.318, p=.001). The results of this study can be used to develop nursing education program to improve self-care behaviors according to cognitive function of hemodialysis patients.

Factors Related Self-Care Behavior among Liver Cirrhosis Patients: Focusing on Disease Knowledge and Family Support (간경변증 환자의 자가간호 행동 관련 요인: 질병 지식과 가족지지를 중심으로)

  • This study examined the associations among the factors related to self-care behavior in patients with liver cirrhosis, focusing on disease knowledge and family support. The subjects were 120 liver cirrhosis patients recruited from the medical out-patient clinic of ageneral hospital in the U metropolitan city during the period of June-August, 2015. The data were analyzed with SPSS(Version 21) program. The results showed that the average scores of disease knowledge was $12.64{\pm}2.16$ out of 18; family support was $23.28{\pm}5.68$ out of 32; and self-care behavior was $35.66{\pm}8.67$ out of 60. The study subjects had moderate levels of disease knowledge and family support, but low levels of self-care behavior. The data showedthat self-care behavior was positively correlated with disease knowledge (r=0.675, p<0.001) and family support (r=0.804, p<0.001) of the subjects. The best predictors for self-care behavior were family support, disease knowledge, frequency of alcohol consumption per week, having liver cirrhosis patients among family or relatives, and educational level, which accounted for 69.7% of the variance. In conclusion, promoting the self-care behavior of liver cirrhosis patients should be planned based on a consideration of the patient's educational level, knowledge of their own disease, and family support.
