  • It has been suggested that people who suffer from impaired masticatory function may adapt food consistency to their oral status (which may lead to deficient nutrient intake) or rely on the digestive system to compensate for the lack of oral preparation of food (which may increase the likelihood of digestive diseases and decrease gut absorption). Masticatory deficiency thus may be detrimental to health. This article reviews evidence of the effects of masticatory deficiency on nutrition. The selection of relevant literature was based on Medline queries using the following key words: mastication, nutrition, digestion, diet, and disease risk. Earlier work not listed in Medline but related to the subject also was reviewed. Only publications available in English were selected for inclusion. It is difficult to draw conclusions from many of the reviewed studies due to issues related to study design, confounding variables, and the subjective nature of the measurements. In particular, data supporting a link between masticatory function and deficient dietary intake often are based on relatively weak correlations and cannot confer a causal relationship.

Clinical Applications of Office-Based Laryngeal Surgery With KTP Laser (KTP 레이저를 이용한 외래 기반 후두 수술의 임상적 적용)

  • The pulsed photoangiolytic 532-nm potassium-titanyl-phosphate (KTP) laser has emerged in recent years as an efficacious treatment modality for vocal fold lesions. It also has broadened the indications for other laryngeal laser surgery. Features of KTP laser that it is a fiber-based delivery system and its energy is selectively absorbed by oxyhemoglobin make it suitable for office-based laryngeal procedures. An office-based KTP laser surgery provides an alternative management option for benign laryngeal diseases and can be performed comfortably under flexible endoscopic guidance which is placed through the nose of a fully awake patient. Office-based laryngeal surgery with a KTP laser can alleviate the need for general anesthesia. However, there are some limitations to apply due to reduced visual precision and the fact that the vocal folds are moving during procedures. Clinicians should carefully weigh the advantages and disadvantages of office-based procedures before a treatment option is selected. Patient selection and standardized laser energy parameters may help in decreasing complications and improving the treatment results.

Agonist (P1) Antibody Converts Stem Cells into Migrating Beta-Like Cells in Pancreatic Islets

  • Tissue regeneration is the ultimate treatment for many degenerative diseases, however, repair and regeneration of damaged organs or tissues remains a challenge. Previously, we showed that B1 Ab and H3 Ab induce stem cells to differentiate into microglia and brown adipocyte-like cells, while trafficking to the brain and heart, respectively. Here, we present data showing that another selected agonist antibody, P1 antibody, induces the migration of cells to the pancreatic islets and differentiates human stem cells into beta-like cells. Interestingly, our results suggest the purified P1 Ab induces beta-like cells from fresh, human CD34+ hematopoietic stem cells and mouse bone marrow. In addition, stem cells with P1 Ab bound to expressed periostin (POSTN), an extracellular matrix protein that regulates tissue remodeling, selectively migrate to mouse pancreatic islets. Thus, these results confirm that our in vivo selection system can be used to identify antibodies from our library which are capable of inducing stem cell differentiation and cell migration to select tissues for the purpose of regenerating and remodeling damaged organ systems.

Considerations about Evolutionary Ecological Study of Psychiatry (정신의학의 진화생태학적 연구 시 고려사항)

  • Evolutionary research on mental disorders is relatively difficult compared to other medical studies. It is because the cause of mental disorder is unclear relative to other medical diseases, various proximate causations are involved. And it is difficult to distinguish cause and effect and to carry out experimental research. Despite these methodological difficulties, it is possible to establish an evolutionary hypothesis on mental disorders based on constructive reductionism, and to demonstrate actual data on the model based on this hypothesis. In this paper, I will discuss some conceptual definitions needed for applying ecological approaches to evolutionary psychiatric research. We will first discuss the appropriate level of explanations and the scope of the study subjects, then discuss the conceptual definition of behaviour and function, dysfunction and the appropriate level of selection.

Occupational Diseases and Injuries among Korean Nurses (한국 간호사의 직업관련성 건강문제)

  • Purpose: This study was done to learn the status of occupational injuries and illnesses among Korean nurses. Methods: Thirty four articles published between 1990 and 2007 were selected using keywords as nurse and back pain, musculoskeletal symptom, violence, needle stick injuries and latex allergy from the database. The 609 RN cases among the workers' compensation data from 2005 to 2007 were analyzed by descriptive statistics. Results: Based on the analysis of the articles, the most frequent work-related illness and injuries were the musculoskeletal symptoms including back pain. The prevalence of back pain was ranged from 36.5% to 98.5%. Tuberculosis cases were the most frequent and the musculoskeletal cases were the second rank in the workers' compensation data. In the case of violence, there has been an increasing tendency since 2000 in the articles and workers' compensation data. The relatively high percentage of compensation cases occurred among the ones working in small institutions and with less than one year experience. Conclusion: The further research on the nurses' work-related illness and injuries needs to consider the measurement tool selection and longitudinal design. The various types of contribution to the occupational health in nursing is suggested to improve the nurses' health and safety.

Characterization of Single Nucleotide Polymorphisms in 55 Disease-Associated Genes in a Korean Population

  • Most common diseases are caused by multiple genetic and environmental factors. Among the genetic factors, single nucleotide polymorphisms (SNPs) are common DNA sequence variations in individuals and can serve as important genetic markers. Recently, investigations of gene-based and whole genome-based SNPs have been applied to association studies for marker discovery. However, SNPs are so population-specific that the association needs to be verified. Fifty-five genes and 384 SNPs were selected based on association with disease. Genotypes of 337 SNPs in candidate genes were determined using Illumina Sentrix Array Matrix (SAM) chips by an allele-specific extension method in 364 unrelated Korean individuals. Allelic frequencies of SNPs were compared with those of other populations obtained from the International HapMap database. Minor allele frequencies, linkage disequilibrium blocks, tagSNPs, and haplotypes of functional candidate SNPs in 55 genetic disease-associated genes were provided. Our data may provide useful information for the selection of genetic markers for gene-based genetic disease-association studies of the Korean population.

A Design Methodology for the Temporary Isolation Room Based on the MERS-Cov Infection Control Guideline - In Case of Temporary Negative Pressure Isolation Room Using Shipping Container - (메르스 감염관리지침에 따른 감염병 임시 격리병동 계획방법에 관한 연구 - 컨테이너를 이용한 음압격리병동을 중심으로 -)

  • The purpose of this study is to propose a design methodology to build temporary isolation rooms when infectious diseases suddenly occur in a certain region, such as the case of MERS-Cov in South Korea in 2015. Although most big hospitals usually have isolation rooms, they are expensive and dangerous to run such facilities on normal and typical days. To deal with these problems in this research, shipping containers are chosen as devices used to build the temporary isolation rooms near the original hospital. To do so, firstly, a prototype for the temporary isolation room was designed with the three part modules. The first part is for the medical team; the second part including the isolation rooms is for patients; the third part is for medical selection rooms to test the specimens. Secondly, the plan was compared with the MERS-Cov infection control guidelines. Finally this prototype is applied into the Yong-in Yon-sei severance hospital and then evaluated through a CFD simulation using STAR-CCM+(ver.9.06) for checking infectious bacterium movement in this prototype. The result showed that the prototype is effectively safe for patients tested as negative, patients waiting to be tested, and the medical team.

Risk Assessment for Toluene Diisocyanate and Respiratory Disease Human Studies

  • Background: Toluene diisocyanate (TDI) is a highly reactive chemical that causes sensitization and has also been associated with increased lung cancer. A risk assessment was conducted based on occupational epidemiologic estimates for several health outcomes. Methods: Exposure and outcome details were extracted from published studies and a NIOSH Health Hazard Evaluation for new onset asthma, pulmonary function measurements, symptom prevalence, and mortality from lung cancer and respiratory disease. Summary exposure-response estimates were calculated taking into account relative precision and possible survivor selection effects. Attributable incidence of sensitization was estimated as were annual proportional losses of pulmonary function. Excess lifetime risks and benchmark doses were calculated. Results: Respiratory outcomes exhibited strong survivor bias. Asthma/sensitization exposure response decreased with increasing facility-average TDI air concentration as did TDI-associated pulmonary impairment. In a mortality cohort where mean employment duration was less than 1 year, survivor bias pre-empted estimation of lung cancer and respiratory disease exposure response. Conclusion: Controlling for survivor bias and assuming a linear dose-response with facility-average TDI concentrations, excess lifetime risks exceeding one per thousand occurred at about 2 ppt TDI for sensitization and respiratory impairment. Under alternate assumptions regarding stationary and cumulative effects, one per thousand excess risks were estimated at TDI concentrations of 10 - 30 ppt. The unexplained reported excess mortality from lung cancer and other lung diseases, if attributable to TDI or associated emissions, could represent a lifetime risk comparable to that of sensitization.

  • In this paper, the assessment items were selected for water by drought, water quality by drought, and thermal diseases by heat wave. We described the selection background for these assessment items. and then we weighted the selected vulnerability assessment items. The vulnerability assessment procedures also describe the calculation methods applied in phases and the actual cases. The vulnerability assessment database was analyzed and a climate change vulnerability assessment system for heat waves and droughts using these vulnerability assessment procedures was designed.

  • In an age when a small quantity of sperm can lead to pregnancy through in vitro fertilization or intracytoplasmic sperm injection, selecting healthy sperm is important. Sperm DNA fragmentation (SDF) is known to be higher in infertile men. Terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate nick end labeling (TUNEL) and the alkaline comet test are SDF tests that directly measure DNA damage and have shown closer correlations with assisted reproduction results than indirect tools such as the sperm chromatin structure assay or the sperm chromatic dispersion test. It is difficult; however, to endorse a single test as the best test overall; instead, it is best to select a testing method based on each patient's clinical condition and goals. In a couple struggling with infertility, if the male partner has a high level of SDF, he should aim to decrease SDF through lifestyle modifications, antioxidant treatment, and ensuring an appropriate duration of abstinence, and physicians need to treat the underlying diseases of such patients. If sperm DNA damage continues despite the patient's and physician's efforts, other methods, such as micromanipulation-based sperm selection or testicular sperm extraction, should be used to select healthy sperm with nuclear DNA integrity.
