한 민간종합병원에서 진단된 폐결핵 환자의 특성과 치료성적 (Clinical Characteristics and Treatment Outcomes of Patients with Pulmonary tuberculosis at a Private General Hospital)

  • 연구배경 : 1990년대 초반까지 민간 병의원에서의 폐결핵환자 치료효율은 보건소에 비해 낮았다. 보건소에 비해 민간 병의원에서 치료받는 결핵환자의 비율이 최근 계속 증가하고 있지만, 민간부문에서 치료 받는 환자들의 특성과 치료효율은 정확히 파악되지 못하고 있다. 본 연구는 서울에 소재한 한 민간 종합병원에서 최근 진단되고 치료한 폐결핵환자의 특성과 치료효율을 조사하고자 하였다. 방 법 : 2000년 8월 1일부터 2001년 1월 31일까지 6개월간 삼성서울병원에서 진단되어 관할 보건소로 신고된 232명의 성인 폐결핵 환자를 대상으로 진단 시 연령과 성별, 입원여부, 과거 폐결핵 치료력, 객담 항산균 도발과 배양 검사 결과, 약제감수성검사 결과, 치료약제의 종류 및 치료 기간, 치료 결과 그리고 도말 양성 환자에서의 치료성적 등을 조사하였다. 결 과 : 232명의 폐결핵 환자의 평균 연령은 47세(범위 16-86세)이고 남자 136명(58.6%), 여자 96명(41.4%)이었다. 82명(35.3%)의 환자는 진단 당시 입원치료를 받았고, 13명(5.6%)이 치료 도중 사망하였다. 과거 치료력이 없는 신환이 166명(71.6%), 재발이 32명(13.8%)이었다. 도말 양성 환자가 83명(35.8%), 도말 음성 배양 양성 환자가 27명(11.6%)으로 균 양성 환자가 110명(47.4%)이었다. 47명(20.3%)에서는 객담 검사가 시행되지 않았다. 초치료 환자 188명 중 130명(69.1%)에서 INH, RFP, EMB, PZA가 처방되었고, 21명(11.2%)에서 INH, RFP, EMB가 사용되었다. 전체 폐결핵 환자 232명 중 26명(11.2%)과 도말 양성 환자 83명 중 11명(13.3%)이 치료를 중단하였다. 도말 양성 초치료 환자 68명 중 29명(42.6%)가 완치되었고, 18명(26.5%)은 치료를 종결하였으나 균 음전을 확인할 수 없었다. 결 론 : 민간 병의원에서 진단된 폐결핵 환자의 치료효율을 높이고 치료 중단률을 낮추기 위해서는 폐결핵 환자의 진단과 치료에서 대한 결핵 및 호흡기학회의 진료기준을 충실히 따르려는 보다 적극적인 노력과 함께 우리나라 실정에 맞는 적절한 공공-민간 협력체계의 국가결핵관리모델의 개발이 필요하리라 사료된다.

  • The small group activities (SGA) was introduced into some of enterprises in Korea in 1967 and plant division, Saemaul undong Headquarters had encouraged Quality Control Circles (QCC) within the manufacturing corporations in assistance of central government. Registered small groups as of august 31, 1984 amounted up to 78,243 units and the number of members were 791,512. Meanwhile, small and medium industries have mostly introduced small group activities considerably later and less actively managed than large business. Main reasons for the less effectiveness of the activities are due to lack of management skills and less awareness of it from management and workers group. Effective small group activities are presumed to be successful only with labor management cooperation on the basis of human-oriented management philosophy. The small group activities are also prevalent in service sector. More derivative methods have been developed and more members are willingly participating in training programs. The small group which is basically a horizontal organization unit, promotes communication within the whole organization. In consideration of the social circumstances and traditions, the flexible model of the small group activities suitable to the corporate environment, will contribute to industrial development.

Current Status, Future Trend and General Policies of Forestry in China

  • Based on data collected by the State Forestry Administration (SFA) and other databases, this paper describes the current status, future trend and general policies in China's forestry. Forestry sector plays an important role in GDP growth of China. As a result forestry industry value output is increasing day by day. There is huge gap between demand and supply of timber projected in 2010. As a result it is necessary to develop fast growing, high yield forest following some specific policies. The ultimate goals of forestry are to construct or improve industry infrastructure, overall upgrade of forest industry structure, income improvement of forest employees and peasants, polices renewal and technology improvement, acceleration of forest processing with high-tech.

억새 처리에 따른 시설수박 생육과 토양 특성에 미치는 영향 (Effects of Soil Improvement and Growth of Watermelon on Plastic Film House by Soil Treatment of Miscanthus sinensis)

  • BACKGROUND: Silver grass (Miscanthus sinensis) No. 1 was developed for production of bio-ethanol, and for the purpose the silver grass growing sector was established in Geumgang basin, Iksan, Jeonbuk, in 2011. However, the other application potentials except for using as the bio-energy resources should be considered because of the drop in international oil prices. Therefore, there is the necessity of a scientific basis to use the silver grass instead of rice straw as the organic matter source that is used for improvement of soil quality in the plastic film house. METHODS AND RESULTS: The silver grass was applied at 5, 10, 15 and 20 Mg/ha and tilled before the watermelon was planted in the plastic film-house. The control plot was treated with 10 Mg/ha with rice straw, and watermelons have been cultivated for 3 years(2017~2019). Soil aggregation, soil chemistry, and the growth characteristics were investigated, when the watermelon was harvested every year. Soil aggregation levels at the 2nd and 3rd year of watermelon harvest were similar from the plot applied with the silver grass at 5 Mg/ha and the control plot, and increased in the silver grass treated plots with more than 10 Mg/ha. However, there was no statistically significant difference between the plots. The nitrogen mineralization of silver grass in the control plot tended to be similar to the 5 Mg/ha plot, but the silver grass treated plots with over 10 Mg/ha showed low nitrogen mineralization. Soil EC on harvest stage was proportional to the applied mass of the silver grass, but pH was in inverse with the applied mass. Soil organic matter content, available phosphate, and exchangeable cations increased with the continued use of silver grass. Watermelon weight found to be the best on more than 15 Mg/ha of silver grass, and the sugar content was highest when 10 Mg/ha was treated. CONCLUSION: The use of the silver grass at 10 Mg/ha annually as the organic source was effective in replacing rice straw while growing fruits and vegetables on the plastic film house.

수산부문 저탄소.녹색성장 패러다임 (Low Carbon.Green Growth Paradigm for Fisheries Sector)

  • Two of the most important topics of the 21st century are ensuring harmony between man and his environment and the emerging long-tail economy in which niche markets are becoming increasingly more important. Since the Industrial Revolution in 17th century, human beings have increasingly exploited the world's natural capital, such as the natural environment and its ecosystems. Now the world is facing limits to sustainable economic growth because of limits to this natural capital. Thus, most countries are beginning to adopt a new development paradigm, the so-called"Green Development Paradigm" which pursues environmental conservation in parallel with economic growth. Recently, the Korean government announced an ambitious national policy of Low Carbon & Green Growth for the next six decades. This is an important step that transforms the existing national policy into a new future-oriented one. The fisheries sector in particular has great potential for making a substantial contribution to this national policy initiative. For example, the ocean itself with its sea plants and phytoplankton has an enormous capacity for fixing carbon, and its vast areas of tidal flats have a tremendous potential for cleaning up pollutants from both the sea and the land. Furthermore, the fishing industry has great potential for the development of fuel-saving biodegradable technologies, and a long-tail economy based on digital technologies can do much to promote the production and consumption of green goods and services derived from the oceans and the fisheries. In order for this potential to be realized, the fisheries authority needs to develop a new green-growth strategy that is practical and widely supported by fishing communities and the markets, taking into account the need for greenhouse gas reduction, conservation of the ocean environment and ecosystems, an improved system for seafood safety, the establishment of strengthened MCS (monitoring control surveillance) system, and the development of coastal ecotourism. In addition, fisheries green policies need to be implemented through a well-organized system of government aids, regulations and compensation, and spontaneous (voluntary) orders in fishing communities should be promoted to encourage far more responsible fisheries.

실내 주행 및 승강 전동 휠체어를 이용하는 고령 여성의 사용성 연구 (Usability Study of the Elderly Women Using Indoor Driving and Elevating Electric Wheelchairs)

  • 본 논문에서는 기존 선행연구에서 설계된 시작품을 보완하여 1차적으로 완성된 시제품에 대하여 고령 여성들이 직접 사용성 평가를 수행함으로써 불편한 사항이나 안전과 직결되는 문제점을 찾아보고 개선이 필요한 사항을 도출하고자 하였다. 본 사용성 평가에서는 일상생활에서의 동작 및 신체적 활동을 수행할 수 있는 65세 이상의 고령자 여성 10명을 피실험자로 선정하였다. 고령 여성이 이승과 주행의 일생상활 시나리오를 기반으로 이승, 승강, 주행, 도착으로 사용성 평가항목들을 정의하고, 사용성 평가 실험환경을 구축하였다. 실험방법은 대상 제품을 사용하는 일상생활 시나리오에서 고령 여성과 기기 간의 주요 사용 접점 분석을 통해 주관적 만족도 설문평가, 동영상 촬영 자료를 활용한 관찰평가 등으로 수행하였으며, 프로토콜은 5단계로 나눠 진행하였다. 측정된 데이터는 최대값과 최소값을 제외하여 평균값과 표준편차를 구하여 그 결과들을 분석하였다. 사용성 평가결과, 승강부문의 개선사항으로는 승강 제어 패널과 주행 제어 패널을 분리해야 하며, 토글 등의 아래/위 방향으로 움직이는 버튼을 통해 직관성 향상이 필요하다는 것을 알 수 있었다. 주행부문에서는 앞바퀴 메커니즘 또는 주행 제어 알고리즘 개선과 UI 개선, 급정지 시스템 개선이 필요하다는 것을 알 수 있었다. 이승부문에서는 시트 좌면부의 크기 향상을 통해 안전성 확보와 힘을 지지할 수 있는 구조물 추가가 필요하다는 것을 알 수 있었다. 사용성 평가에서 나타난 불편사항과 개선사항을 토대로 한층 더 보완된 연구개발 제품이 출시된다면 거동이 불편한 고령 여성들이 타인의 도움 없이 스스로 실내에서 간단한 일상생활을 할 수 있으므로 사용자의 삶의 질은 한층 더 높아질 것이다.

음식물류폐기물처리시설의 악취관리대책에 관한 연구 (Odor control of Foodwaste Treatment Facilities)

  • 본 연구에서는 음식물류폐기물처리시설의 시설운영에 대한 전반적인 문제점 분석을 통해 음식물류폐기물처리시설의 효율적인 운영관리방안을 제시하고자 음식물류폐기물 관련 연구보고서와 자료를 수집 분석하였다. 음식물류폐기물처리시설에서 발생하는 악취의 특성을 알아보기 위하여 퇴비화시설과 사료화시설의 악취시료를 채취하여 분석하였다. 퇴비화 및 사료화시설의 주요 악취발생인이 큰 투입공정, 파쇄공정, 발효공정에 대하여 악취를 측정 분석한 결과 파쇄공정이 악취농도가 가장 높았다. 퇴비화시설의 주요 악취성분은 황화수소, 메틸머캅탄, 다이메틸설파이드, 암모니아였으며, 사료화시설의 주요 악취성분은 메틸머캅탄, 트라이메틸아민, 아세트알데하이드이었다.

부여지역 양송이농가 버섯파리 발생소장 및 사전방제기술 적용효과 (Seasonal occurrence of mushroom fly infestation and analysis of the effects of preemptive pest control technology: A case study in button mushroom farms in Buyeo County)

  • 본 연구는 3년간 버섯파리 발생소장 조사를 통해 발생 양상을 파악하였고 버섯파리 발생 최성기 이전에 물리적 방제기술을 적용하여 방제효과를 알아보았다. 버섯파리 사전방제를 위한 물리적 방제방법으로 재배사 출입구에 에어커튼을 설치하고, 출입구 주변으로 포충기와 끈끈이 트랩을 설치하여 버섯파리의 외부유입 차단과 내부 밀도 감소가 되게 하였고, 유기농업자재인 달마시안제충국 추출물을 주 1회 연무하여 화학적 방제기술을 병행처리 하였다. 또한 버섯파리 방제효과 조사를 위해 양송이 재배사 균상에 황색 끈끈이트랩(15×25 cm)을 3개 지점에 설치하여 사전방제기술 처리구와 무처리구의 버섯파리 발생 양상을 비교하였다. 관행대비 버섯파리 사전방제기술 적용 결과 버섯파리 발생 수준이 5월에는 15에서 6으로 약 60% 감소하였고, 6월에는 10에서 6으로 약 40% 감소함을 확인할 수 있었다. 종합적으로 버섯파리 급증시기에 사전방제기술 적용시 50% 이상의 방제효과를 얻을 수 있었지만, 외부 유입원 차단(재배사 문틈), 재배사 주변 청결관리(폐배지 재배사 옆 보관) 등 위의 결과를 보완하여 버섯파리 사전방제기술의 적용효과를 분석하고 응애 및 병원균과의 상관관계 분석 등 안전성 연구를 수행하여 양송이 안전생산 및 안정생산 기술이 농가에 보급될 수 있도록 추후 면밀한 연구가 진행되어야 할 것으로 판단된다.

  • Consultant and owner play a very important role in the construction sector which is one of the main sectors in economic development. There are many stakeholders in a construction project including but not limited to: owner, consultant, contractor, subcontractor, and supplier. The consultant is the party who is to stay in close touch with the project owner, which both consultant and owner are considered as the main key stakeholders to make the project happen. It's not a simple task for a consultant to just implement the same style of management and control within various projects that are of a different background and character of owners. Thus, the adjusting plan management and control to the owner environment has been an essential technique to drive the project to success. The objective of this paper is to identify the significant management techniques for adjusting a plan of management and control to the owner environment in Thailand. The methodology is the identification throughout the analogous to abduction from literature review and our empirical observations of Future Engineering Consultants Co, Ltd. We have been in consultation for twenty-five years (25) in Thailand with 209 projects from both public and private owners. As a result, there are (1) Project management styles, (2) Personal style, (3) Owner expectation management, (4) Scope Management, (5) Time Management, (6) Cost Management, (7) Quality Management, and (8) Environmental Management. The input are: a clear defined goal, showing respect by giving good service, maintaining high quality, keeping in touch with owners, and resources. This would give the output of the project's success by meeting the owner's expectation and requirements.

Review of Qualitative Approaches for the Construction Industry: Designing a Risk Management Toolbox

  • Objectives: This paper presents the framework and protocol design for a construction industry risk management toolbox. The construction industry needs a comprehensive, systematic approach to assess and control occupational risks. These risks span several professional health and safety disciplines, emphasized by multiple international occupational research agenda projects including: falls, electrocution, noise, silica, welding fumes, and musculoskeletal disorders. Yet, the International Social Security Association says, "whereas progress has been made in safety and health, the construction industry is still a high risk sector." Methods: Small- and medium-sized enterprises (SMEs) employ about 80% of the world's construction workers. In recent years a strategy for qualitative occupational risk management, known as Control Banding (CB) has gained international attention as a simplified approach for reducing work-related risks. CB groups hazards into stratified risk 'bands', identifying commensurate controls to reduce the level of risk and promote worker health and safety. We review these qualitative solutions-based approaches and identify strengths and weaknesses toward designing a simplified CB 'toolbox' approach for use by SMEs in construction trades. Results: This toolbox design proposal includes international input on multidisciplinary approaches for performing a qualitative risk assessment determining a risk 'band' for a given project. Risk bands are used to identify the appropriate level of training to oversee construction work, leading to commensurate and appropriate control methods to perform the work safely. Conclusion: The Construction Toolbox presents a review-generated format to harness multiple solutions-based national programs and publications for controlling construction-related risks with simplified approaches across the occupational safety, health and hygiene professions.
