위성영상을 활용한 하천환경 평가 세구간 설정 (Determination of Stream Reach for River Environment Assessment System Using Satellite Image)

  • 본 연구에서는 하천환경 평가체계에서 가장 우선적으로 이루어지는 하천 유형화 및 세구간 설정 작업을 위해 위성영상의 활용방안을 검토하였다. 국내 하천 환경 평가체계의 경우 하천 유형화 결과에 따라 저수로 폭의 10배 또는 20배를 기준으로 평가 세구간을 설정하기를 제안한다. 연구수행을 위해 청미천 본류를 대상으로 하천 관련 기본자료들을 이용하여 하천 유형화 작업을 수행하였으며, 이후 위성영상에 최적분류법을 적용하여 토지피복분류를 수행하였다. 위성영상의 경우 10 m 해상도의 개방형 자료인 Sentiel-2를 이용하였으며, 다양한 유량 조건을 고려하기위해 총 4개의 위성영상자료를 이용하였다: 2018년 2월 2일 (일유량 = 2.39 m3/s), 5월 23일 (일유량 = 15.51 m3/s), 6월 2일 (일유량 = 3.88 m3/s), 7월 7일 (일유량 = 33.61 m3/s). 토지피복분류 결과로부터 저수로 폭을 획득하였으며, 결과를 통해 하천환경평가를 위한 세구간을 설정하였다. 세구간 설정 결과는 하천 환경 물리적 지표 중 소의 다양성, 사행도, 하천횡단 형상, 하천횡단구조물에 대한 평가를 통해 검토되었다. 본 연구를 통해 하천환경 평가시스템을 위한 세구간 설정에 위성영상을 활용하는 경우 유량 조건에 대한 충분한 고려가 필요할 것으로 판단된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Comparisons between the valves were made on the transvalvular pressure drop, regurgitation volume and maximum valve opening area. The polymer valve showed smaller regurgitation volume and transvalvular pressure drop compared to the mechanical valve at higher heart rate. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. 일 대학병원 호스피스 병동 입원 환자의 간호활동시간 측정과 원가산정 (Determination of Cost and Measurement of nursing Care Hours for Hospice Patients Hospitalized in one University Hospital)

  • This study was designed to determine the cost and measurement of nursing care hours for hospice patients hostpitalized in one university hospital. 314 inpatients in the hospice unit 11 nursing manpower were enrolled. Study was taken place in C University Hospital from 8th to 28th, Nov, 1999. Researcher and investigator did pilot study for selecting compatible hospice patient classification indicators. After modifying patient classification indicators and nursing care details for general ward, approved of content validity by specialist. Using hospice patient classification indicators and per 5 min continuing observation method, researcher and investigator recorded direct nursing care hours, indirect nursing care hours, and personnel time on hospice nursing care hours, and personnel time on hospice nursing care activities sheet. All of the patients were classified into Class I(mildly ill), Class II (moderately ill), Class III (acutely ill), and Class IV (critically ill) by patient classification system (PCS) which had been carefully developed to be suitable for the Korean hospice ward. And then the elements of the nursing care cost was investigated. Based on the data from an accounting section (Riccolo, 1988), nursing care hours per patient per day in each class and nursing care cost per patient per hour were multiplied. And then the mean of the nursing care cost per patient per day in each class was calculated. Using SAS, The number of patients in class and nursing activities in duty for nursing care hours were calculated the percent, the mean, the standard deviation respectively. According to the ANOVA and the $Scheff{\'{e}$ test, direct nursing care hours per patient per day for the each class were analyzed. The results of this study were summarized as follows : 1. Distribution of patient class : class IN(33.5%) was the largest class the rest were class II(26.1%) class III(22.6%), class I(17.8%). Nursing care requirements of the inpatients in hospice ward were greater than that of the inpatients in general ward. 2. Direct nursing care activities : Measurement ${\cdot}$ observation 41.7%, medication 16.6%, exercise ${\cdot}$ safety 12.5%, education ${\cdot}$ communication 7.2% etc. The mean hours of direct nursing care per patient per day per duty were needed ; 69.3 min for day duty, 64.7 min for evening duty, 88.2 min for night duty, 38.7 min for shift duty. The mean hours of direct nursing care of night duty was longer than that of the other duty. Direct nursing care hours per patient per day in each class were needed ; 3.1 hrs for class I, 3.9 hrs for class II, 4.7 hrs for class III, and 5.2 hrs for class IV. The mean hours of direct nursing care per patient per day without the PCS was 4.1 hours. The mean hours of direct nursing care per patient per day in class was increased significantly according to increasing nursing care requirements of the inpatients(F=49.04, p=.0001). The each class was significantly different(p<0.05). The mean hours of direct nursing care of several direct nursing care activities in each class were increased according to increasing nursing care requirements of the inpatients(p<0.05) ; class III and class IV for medication and education ${\cdot}$ communication, class I, class III and class IV for measurement ${\cdot}$ observation, class I, class II and class IV for elimination ${\cdot}$ irrigation, all of class for exercise ${\cdot}$ safety. 3. Indirect nursing care activities and personnel time : Recognization 24.2%, house keeping activity 22.7%, charting 17.2%, personnel time 11.8% etc. The mean hours of indirect nursing care and personnel time per nursing manpower was 4.7 hrs. The mean hours of indirect nursing care and personnel time per duty were 294.8 min for day duty, 212.3 min for evening duty, 387.9 min for night duty, 143.3 min for shift duty. The mean of indirect nursing care hours and personnel time of night duty was longer than that of the other duty. 4. The mean hours of indirect nursing care and personnel time per patient per day was 2.5 hrs. 5. The mean hours of nursing care per patient per day in each class were class I 5.6 hrs, class II 6.4 hrs, class III 7.2 hrs, class IV 7.7 hrs. 6. The elements of the nursing care cost were composed of 2,212 won for direct nursing care cost, 267 won for direct material cost and 307 won for indirect cost. Sum of the elements of the nursing care cost was 2,786 won. 7. The mean cost of the nursing care per patient per day in each class were 15,601.6 won for class I, 17,830.4 won for class II, 20,259.2 won for class III, 21,452.2 won for class IV. As above, using modified hospice patient classification indicators and nursing care activity details, many critical ill patients were hospitalized in the hospice unit and it reflected that the more nursing care requirements of the patients, the more direct nursing care hours. Emotional ${\cdot}$ spiritual care, pain ${\cdot}$ symptom control, terminal care, education ${\cdot}$ communication, narcotics management and delivery, attending funeral ceremony, the major nursing care activities, were also the independent hospice service. But it is not compensated by the present medical insurance system. Exercise ${\cdot}$ safety, elimination ${\cdot}$ irrigation needed more nursing care hours as equal to that of intensive care units. The present nursing management fee in the medical insurance system compensated only a part of nursing car service in hospice unit, which rewarded lower cost that that of nursing care.

개인정보 보호를 위한 의료영상 발급 표준 업무절차 개발연구 (Development of Standard Process for Private Information Protection of Medical Imaging Issuance)

  • 목 적 : 기존 필름으로 발급되었던 의료영상은 IT기술의 발달로 디지털화 되어 CD로 발급되고 있다. 그러나 발급 시 신분확인을 하고 있는 의무기록과는 달리 필름을 사용하던 시절부터 의료영상은 별다른 신분확인을 하지 않는 의료기관이 많다. 이에 신청자의 개인의료정보 보호에 대한 인식 실태를 조사하고 여러 의료기관의 CD 또는 DVD 등의 매체를 통한 의료영상 복사 현황을 조사, 정보보안에 관련된 국, 내외 법률 및 권고안을 분석하여 국내 환경에 부합하는 의료영상 복사 발급과 절차를 마련하는 기준을 제시하고자 한다. 대상 및 방법 : 첫째, 2008년 5월 1일부터 7월 31일까지 수도권에 있는 33개 종합병원을 대상으로 의료영상복사 신청 시 구비서류, 발급절차 등을 전화를 통한 유선 조사를 시행하였다. 신청자에 따른 구비서류를 의료법 제 21조 2항에 의거 (1) 본인일 경우 신분증 확인, (2) 가족일 경우 신청자 신분증, 가족관계 서류(건강보험증, 가족관계증명서, 등본 등), (3) 제 3자 대리인일 경우 신분증, 위임장, 인감증명서로 기준을 마련하여 조사하였다. 둘째, 연구기간 동안 위의 기준에 따라 의료영상을 발급해 주고 있는 K 의료원에 복사를 신청하는 신청자들이 준비해온 구비서류 여부를 파악하였다. 셋째, 구비서류의 확인 및 미비 시 조치 등에 대한 발급절차의 기준을 정립하여 프로세스를 개발하였다. 결 과 : 수도권 33개 의료영상 발급현황을 조사한 결과 모든 조건을 충족한 병원은 16곳(49%), 신분증만 있으면 가능한 병원은 4곳(12%), 누구나 신청 가능한 병원 4곳(12%)이었으며 의료영상을 발급하는 부서가 아닌 진료과에서 신청하는 곳이 9곳(27%)으로 구비서류 조건여부는 알 수 없었다. 또한 신청자들이 복사 신청시 준비해온 구비서류가 조건에 충족한지 3개월간의 조사 결과 모두 준비한 경우(완비)는 629건(49%), 일부만 준비한 경우(일부 미비) 416건(33%), 모두 준비하지 않은 경우(미비) 226건(18%)이였다. 위의 연구결과를 근거로 의료영상 복사 신청 절차에 대한 프로세스를 정립하여 객관적인 응대를 할 수 있도록 하고, 환자와의 마찰을 줄이고 불편을 최소화 하면서 환자의 편의를 도모하고자 세분화된 발급절차 모형도를 작성하였다. 결 론 : 다른 전산 시스템과 달리 의료영상 시스템인 PACS가 의료기기로 분류되어 있는 것은 그만큼 의료정보의 중요성이 크다는 의미이다. 또한 의료영상의 학문적 성격으로 의학교육 및 연구에 많이 쓰이는데 이러한 이유로 쉽게 인용되고 남용 될 수 있다. 따라서 의료영상은 전문적인 교육을 받은 의료영상 관리자에 의해 적절한 발급 기준으로 발급, 관리되어야 할 것이며 이에 관한 개인정보보호와 의료영상에 대한 적극적인 홍보가 필요할 것이다.

치과병원 내원환자의 만족도 조사분석 (A study on the gratification of the patient in the Dental Hospital)

  • 오늘날의 시장 경제는 소비자 중심으로 빠르게 변모하고 있다. 이는 점점 가속화 되어가는 소비자의 생활수준과 교육수준의 향상, 다양한 대중매체를 통한 구매자의 상품에 대한 접근의 용이성에 기인한다. 다양한 선택의 대안 속에서 자신의 가치를 만족시킬 수 있는 상품을 선택할 수 있는 소비자 중심의 시장 경제체제는 소비자의 소득수준의 증가와 전 국민 의료보험의 실시로 인한 의료 수요확대를 통해 의료분야에서도 그 두각을 나타내고 있다. 최근 우리나라의 의료계는 의료기관의 수 및 의료 인력의 증가로 인해 경쟁이 강화되고 있는 실정이고, 이러한 경쟁 강화를 통해 소비자의 시장 선택의 폭은 넓어지게 되었다. 이런 시점에 소비자의 입장에서 본 의료서비스에 있어서의 고객만족도에 영향을 미치는 요인, 서비스의 문제점, 개선점에 대한 조사와 연구는 필요 불가결하다고 하겠다. 이 연구는 2006년 1월 23일부터 4월 15일 까지 서울시에 소재한 연세대학교 치과병원에 내원한 환자 784명을 대상으로 진행되었으며 구조화된 설문지를 이용하여 자기기입식 방법으로 자료를 수집하여 이를 분석하였다. 본 연구의 결과를 살펴보면 다음과 같다. 1. 응답자의 사회인구학적인 특성은 성별이나 결혼여부는 적절하게 표본이 추출 된 것으로 보이나 직업에서는 학생 및 대학(원)생, 내원이 유와 현재 받고 있는 치료에 있어서는 교정치료가 높은 비율로 나타났다. 2. 치과병원에 대한 전반적인 만족도는 전체의 74%가 높은 만족을 나타내었다. 3. 독립변수들 중 구체적 서비스 항목에 대해서는 '서비스시스템', '친절도', '설명', '대기시간설명', '예약시스템', '응급상황에의 대처', '치과진료경험의 풍부', '치과적 지식의 유무', '병원의 규모', '소독', '내부시설', '주차' 에 대한 만족도는 높게, '치료비', '치료 준비시간', '대기시간', '치료시간', '예약기간' 에 대한 만족도는 낮게 나타났다. 4. 독립변수들 간의 상관관계 분석을 실시하였고 이들 사이에는 높은 선형관계가 없었다. 5. 독립변수들과 종속변수 사이의 상호관련성을 평가하기 위한 프로빗 회귀분석 결과 총 34개의 독립변수 중에서 불편감 해소, 서비스 시스템, 친절도, 설명, 한 번 내원 시 치료시간, 예약시스템, 지식의 유무, 내부시설에 대한 만족도의 8개 독립변수가 유의수준 1%하에서 서로 높은 상호관련성을 보였다. 고객의 만족은 전적으로 고객의 주관적인 평가에 의존한다. 환자들의 욕구가 다양화되고 고급화되면서 의료서비스에 대한 기대수준이 높아지고 있기 때문에 이에 부합하는 서비스를 제공하는 것이 의료서비스의 고객인 환자를 만족시키는 필수 요건이 된다. 많은 의료기관이 환자를 중심으로 하여 고객만족을 위한 병원, 고객감동을 위한 병원이 되기 위하여 노력을 하고 있으며, 이는 치과의료 분야에 있어서도 적용되고 있는 현실이다. 내원환자들의 만족도를 향상시키기 위해서는 이 연구의 결과를 기초로 한 새로운 병원 경영 전략의 수립과 그로 인한 의료 서비스의 질 향상으로 고객이 만족하는 양질의 의료서비스를 제공하는 노력이 필요하리라 여겨진다.

수유 종류 및 수유 후 자세에 따른 위 배출 시간의 초음파적 연구 (Ultrasonic Assessment of Gastric Emptying According to Feeding Types and Postprandial Postures)

  • 목 적 : 신생아의 식도 특히 하부 식도 괄약근의 발달은 해부학적으로 미숙하기 때문에 하부 식도 괄약근의 수축력이 낮아 신생아에서 역류와 구토가 잘 일어난다. 식도는 15개월이 되어야 성숙하게 되므로 그 동안 역류와 구토의 빈도가 적도록 돌보아 주어야 한다. 역류와 구토의 빈도를 줄이는 방안으로 섭취한 음식이 위에 머므르는 시간을 단축시킬수 있다면 도움이 될 것이므로 수유의 종류와 수유 후 자세에 따른 위 배출 시간의 영향을 알아 보기 위하여 본 연구를 시행하였다. 방 법: 신생아를 대상으로 초음파검사를 이용하여 수유 종류(모유 및 분유)에 따른 위 배출 시간을 비교하고, 수유 후 자세(앙와위 및 복와위)에 따른 위 배출 시간을 비교해 보았다. 수유 종류에 따른 비교에서 대상아는 모유수유아 15명, 분유수 유아 15명이었고 수유 후 자세는 앙와위를 취하였다. 수유 후 자세에 따른 비교에서는 분유 수유를 하였고 앙와위 15명, 복와위 18명을 대상으로 하였다. 위 전정부 면적은 상장간막 동맥과 대동맥이 지나는 위치에서 5 MHz 컨벡스 초음파 탐촉자로 종단스캔을 하여 초음파 기계에 내장된 컴퓨터에 의해 구하였다. 위 배출 시간은 금식 후 4시간에 위 전정부 면적의 기본 크기를 측정하고 수유 후 즉시, 그 후 30분마다 측정하여 기본 크기로 돌아올 때까지 추적하여 측정하였고 기본 크기로 두번 계속 측정되면 측정을 마쳤으며 처음 기본크기로 돌아온 시간을 위 배출 시간으로 정하였다. 결 과: 1) 수유 종류에 따른 위 전정부 면적 및 위 배출 시간의 비교; (1) 모유수유아를 앙와위로 누인 경우의 위 전정부 면적은 금식 시 $0.59{\pm}0.13\;cm^2$이었고 수유 후 즉시는 $0.70{\pm}0.17\;cm^2$이었으며 수유 후 30분에 최고치에 달하여 $0.82{\pm}0.19\;cm^2$이었고 금식 시 면적의 1.39배였다. 위 배출 시간은 $76.0{\pm}20.02$분이었다. (2) 분유수유아를 앙와위로 누인 경우의 위 전정부 면적은 금식 시 $0.52{\pm}0.08\;cm^2$이었고 수유 후 즉시는 $0.68{\pm}0.10\;cm^2$이었으며 수유 후 30분에 최고치에 달하여 $0.85{\pm}0.14\;cm^2$이었고 금식 시 면적의 1.63배였다. 위 배출 시간은 $96.0{\pm}20.28$분이었다. 3) 모유수유아 및 분유수유아를 앙와위로 누인 경우의 위 전정부 면적은 금식 시와 수유 후 경과시간에 따른 차이가 없었으나 위 배출 시간은 모유수유아가 분유수유아 보다 유의하게 빨랐다(p<0.05). 2) 수유 후 자세에 따른 위 전정부 면적 및 위배출 시간의 비교; (1) 분유수유아를 앙와위로 누인 경우의 위 전정부 면적은 금식 시 $0.52{\pm}0.08\;cm^2$이었고 수유 후 즉시는 $0.68{\pm}0.10\;cm^2$이었으며 수유 후 30분에 최고치에 달하여 $0.85{\pm}0.14\;cm^2$이었고 금식 시 면적의 1.63배였다. 위 배출 시간은 $96.0{\pm}20.28$분이었다. (2) 분유수유아를 복와위로 누인 경우의 위 전정부 면적은 금식 시 $0.54{\pm}0.06\;cm^2$이었고 수유 후 즉시는 $0.63{\pm}0.13\;cm^2$이었으며 수유 후 30분에 최고치에 달하여 $0.81{\pm}0.12\;cm^2$이었고 금식 시의 1.50배였다. 위 배출 시간은 $85.0{\pm}22.43$분이었다. (3) 분유수유아에서 수유 후 자세를 앙와위 및 복와위로 취한 경우 위전정부 면적은 금식 시와 수유 후 경과에 따른 차이는 없었으나 수유 후 30분에는 앙와위로 누인 경우 $0.85{\pm}0.14\;cm^2$이었고 복와위로 누인 경우 $0.81{\pm}0.12\;cm^2$로 앙와위인 경우가 컸다(p<0.01). 위 배출 시간은 두 군간에 유의한 차이가 없었다(p>0.05). 결 론: 식도의 기능이 미숙하고 위의 연동운동이 덜 발달된 미숙아나 신생아는 역류와 구토의 빈도를 줄이고 경구 영양을 잘 진행해 나가기 위해서 위 배출 시간이 빠른 모유수유를 적극 권장하는 것이 좋을 것으로 사료된다. 수유 후 자세는 위 배출 시간에 영향을 주지는 않으나 역류나 구토가 일어났을 때 흡인의 가능성이 적어 안전하고, 편안히 잠잘 수 있으며 전통적으로 선호하는 자세를 취하면 적절할 것으로 사료된다.

국내 지진활동 및 지각구조 연구동향 (Recent Research for the Seismic Activities and Crustal Velocity Structure)

  • 유라시아판 동남쪽에 위치한 한반도는 판경계로부터 수백 km 떨어진 판내지역에 해당하며, 판내부 지진은 판경계부에서의 지진에 비해 발생하는 지진에 비해 숫자가 상대적으로 적고 크기도 작을 뿐만 아니라 발생 위치도 매우 불규칙한 편이다. 이는 단층의 활동주기가 매우 길다는 것을 의미하므로, 약 2,000년 동안의 지진역사가 기록되어 있는 우리나라는 이들 자료를 정확히 분석하여 역사시대 동안에 발생한 지진의 활동도 및 특성 등을 규명하여야 한다. 역사지진의 자료에 의하면 지진활동은 조선 중기 즉 16-18 세기에 특히 활발했으며 높은 지진활동의 기간은 중국 북동부와 일치하고 있다. 이는 이 두지역의 지진활동이 밀접하게 연관되어 있음을 시사한다. 역사지진 및 계기지진 자료에 의하면 대체로 서해안쪽이 활발하며, 한반도 남동부에서 서북서 방향으로 활발한 양상을 보여준다. 우리나라에서의 근대적 지진관측은 1905년 최초로 인천관측소가 설치되어 광복이전 6개소의 지진관측소를 운영하였다. 그 후 지진관측 공백기를 거쳐 1963년 서울에 세계표준지진계가 설치되었으며 1990 년 초에 기상청은 중앙집중식 12개소의 관측소를 본격적으로 운영하기 시작하였다. 그 후 지속적인 확장을 통해 기상청에서는 속도계관측소 35개소, 가속도 관측소 75개소를, 한국지질자원연구원은 32개소의 속도계관측소, 16개소의 가속도 관측소를, 한국원자력안전기술원은 4개소의 속도계 및 가속도 관측소를, 한국전력연구원은 13개소의 속도계 및 기속도 관측소를 운영하고 있다..27개 지진의 발생원인을 분석한 결과 한반도 및 인접지역에서 발생한 지진의 대부분은 주향이동 단층 운동에 의한 메카니즘과 다소의 역단층 운동이 첨가된 단층운동 특성을 보여준다. 한반도 및 주변에서 단층작용을 일으킨 주응력 방향은 거의 수평한 동북동-서남서 방향으로 같은 판내 지역인 북동부 중국 지역의 주응력 방향과 매우 유사하고 동해 동부와는 상당한 차이를 보인다. 이는 한반도 및 그 주변에서 지진을 일으키는 주응력은 동쪽에서 유라시아판 밑으로 침강하는 태평양판의 영향뿐만 아니라 서남쪽에서 충돌하는 인도판의 영향도 상당히 작용하는 것으로 해석된다. 지각 속도구조는 지진이 발생한 진원의 위치와 지진규모를 정확히 알아내는데 필수적이다. 그 동안 국내 지진관측망 미비, 한반도 내부의 낮은 지진 발생 빈도 등의 이유로 양질의 지진자료를 구하기 어려워 지진자료를 이용한 지각속도 구조에 대한 연구가 극히 제한적으로 이루어질 수밖에 없었다. 그러나 최근에 국내의 여러 지친관측망에서 축적된 지진기록과 반사 및 굴절 탄성파 탐사를 수행하여 종합적으로 지각 속도구조를 규명하기 시작하였다. 이와 같은 인공발파를 이용한 지각속도구조를 규명하기 위해서는 많은 인원과 예산을 필요로 하므로 관련분야의 전문가들의 적극적인 참여가 필요한 상황이다.

병원 간호행정 개선을 위한 연구 (A Study for Improvement of Nursing Service Administration)

  • Much has teed changed in the field of hospital administration in the It wake of the rapid development of sciences, techniques ana systematic hospital management. However, we still have a long way to go in organization, in the quality of hospital employees and hospital equipment and facilities, and in financial support in order to achieve proper hospital management. The above factors greatly effect the ability of hospitals to fulfill their obligation in patient care and nursing services. The purpose of this study is to determine the optimal methods of standardization and quality nursing so as to improve present nursing services through investigations and analyses of various problems concerning nursing administration. This study has been undertaken during the six month period from October 1971 to March 1972. The 41 comprehensive hospitals have been selected iron amongst the 139 in the whole country. These have been categorized according-to the specific purposes of their establishment, such as 7 university hospitals, 18 national or public hospitals, 12 religious hospitals and 4 enterprise ones. The following conclusions have been acquired thus far from information obtained through interviews with nursing directors who are in charge of the nursing administration in each hospital, and further investigations concerning the purposes of establishment, the organization, personnel arrangements, working conditions, practices of service, and budgets of the nursing service department. 1. The nursing administration along with its activities in this country has been uncritical1y adopted from that of the developed countries. It is necessary for us to re-establish a new medical and nursing system which is adequate for our social environments through continuous study and research. 2. The survey shows that the 7 university hospitals were chiefly concerned with education, medical care and research; the 18 national or public hospitals with medical care, public health and charity work; the 2 religious hospitals with medical care, charity and missionary works; and the 4 enterprise hospitals with public health, medical care and charity works. In general, the main purposes of the hospitals were those of charity organizations in the pursuit of medical care, education and public benefits. 3. The survey shows that in general hospital facilities rate 64 per cent and medical care 60 per-cent against a 100 per cent optimum basis in accordance with the medical treatment law and approved criteria for training hospitals. In these respects, university hospitals have achieved the highest standards, followed by religious ones, enterprise ones, and national or public ones in that order. 4. The ages of nursing directors range from 30 to 50. The level of education achieved by most of the directors is that of graduation from a nursing technical high school and a three year nursing junior college; a very few have graduated from college or have taken graduate courses. 5. As for the career tenure of nurses in the hospitals: one-third of the nurses, or 38 per cent, have worked less than one year; those in the category of one year to two represent 24 pet cent. This means that a total of 62 per cent of the career nurses have been practicing their profession for less than two years. Career nurses with over 5 years experience number only 16 per cent: therefore the efficiency of nursing services has been rated very low. 6. As for the standard of education of the nurses: 62 per cent of them have taken a three year course of nursing in junior colleges, and 22 per cent in nursing technical high schools. College graduate nurses come up to only 15 per cent; and those with graduate course only 0.4 per cent. This indicates that most of the nurses are front nursing technical high schools and three year nursing junior colleges. Accordingly, it is advisable that nursing services be divided according to their functions, such as professional, technical nurses and nurse's aides. 7. The survey also shows that the purpose of nursing service administration in the hospitals has been regulated in writing in 74 per cent of the hospitals and not regulated in writing in 26 per cent of the hospitals. The general purposes of nursing are as follows: patient care, assistance in medical care and education. The main purpose of these nursing services is to establish proper operational and personnel management which focus on in-service education. 8. The nursing service departments belong to the medical departments in almost 60 per cent of the hospitals. Even though the nursing service department is formally separated, about 24 per cent of the hospitals regard it as a functional unit in the medical department. Only 5 per cent of the hospitals keep the department as a separate one. To the contrary, approximately 12 per cent of the hospitals have not established a nursing service department at all but surbodinate it to the other department. In this respect, it is required that a new hospital organization be made to acknowledge the independent function of the nursing department. In 76 per cent of the hospitals they have advisory committees under the nursing department, such as a dormitory self·regulating committee, an in-service education committee and a nursing procedure and policy committee. 9. Personnel arrangement and working conditions of nurses 1) The ratio of nurses to patients is as follows: In university hospitals, 1 to 2.9 for hospitalized patients and 1 to 4.0 for out-patients; in religious hospitals, 1 to 2.3 for hospitalized patients and 1 to 5.4 for out-patients. Grouped together this indicates that one nurse covers 2.2 hospitalized patients and 4.3 out-patients on a daily basis. The current medical treatment law stipulates that one nurse should care for 2.5 hospitalized patients or 30.0 out-patients. Therefore the statistics indicate that nursing services are being peformed with an insufficient number of nurses to cover out-patients. The current law concerns the minimum number of nurses and disregards the required number of nurses for operation rooms, recovery rooms, delivery rooms, new-born baby rooms, central supply rooms and emergency rooms. Accordingly, tile medical treatment law has been requested to be amended. 2) The ratio of doctors to nurses: In university hospitals, the ratio is 1 to 1.1; in national of public hospitals, 1 to 0.8; in religious hospitals 1 to 0.5; and in private hospitals 1 to 0.7. The average ratio is 1 to 0.8; generally the ideal ratio is 3 to 1. Since the number of doctors working in hospitals has been recently increasing, the nursing services have consequently teen overloaded, sacrificing the services to the patients. 3) The ratio of nurses to clerical staff is 1 to 0.4. However, the ideal ratio is 5 to 1, that is, 1 to 0.2. This means that clerical personnel far outnumber the nursing staff. 4) The ratio of nurses to nurse's-aides; The average 2.5 to 1 indicates that most of the nursing service are delegated to nurse's-aides owing to the shortage of registered nurses. This is the main cause of the deterioration in the quality of nursing services. It is a real problem in the guest for better nursing services that certain hospitals employ a disproportionate number of nurse's-aides in order to meet financial requirements. 5) As for the working conditions, most of hospitals employ a three-shift day with 8 hours of duty each. However, certain hospitals still use two shifts a day. 6) As for the working environment, most of the hospitals lack welfare and hygienic facilities. 7) The salary basis is the highest in the private university hospitals, with enterprise hospitals next and religious hospitals and national or public ones lowest. 8) Method of employment is made through paper screening, and further that the appointment of nurses is conditional upon the favorable opinion of the nursing directors. 9) The unemployment ratio for one year in 1971 averaged 29 per cent. The reasons for unemployment indicate that the highest is because of marriage up to 40 per cent, and next is because of overseas employment. This high unemployment ratio further causes the deterioration of efficiency in nursing services and supplementary activities. The hospital authorities concerned should take this matter into a jeep consideration in order to reduce unemployment. 10) The importance of in-service education is well recognized and established. 1% has been noted that on the-job nurses. training has been most active, with nursing directors taking charge of the orientation programs of newly employed nurses. However, it is most necessary that a comprehensive study be made of instructors, contents and methods of education with a separate section for in-service education. 10. Nursing services'activities 1) Division of services and job descriptions are urgently required. 81 per rent of the hospitals keep written regulations of services in accordance with nursing service manuals. 19 per cent of the hospitals do not keep written regulations. Most of hospitals delegate to the nursing directors or certain supervisors the power of stipulating service regulations. In 21 per cent of the total hospitals they have policy committees, standardization committees and advisory committees to proceed with the stipulation of regulations. 2) Approximately 81 per cent of the hospitals have service channels in which directors, supervisors, head nurses and staff nurses perform their appropriate services according to the service plans and make up the service reports. In approximately 19 per cent of the hospitals the staff perform their nursing services without utilizing the above channels. 3) In the performance of nursing services, a ward manual is considered the most important one to be utilized in about 32 percent of hospitals. 25 per cent of hospitals indicate they use a kardex; 17 per cent use ward-rounding, and others take advantage of work sheets or coordination with other departments through conferences. 4) In about 78 per cent of hospitals they have records which indicate the status of personnel, and in 22 per cent they have not. 5) It has been advised that morale among nurses may be increased, ensuring more efficient services, by their being able to exchange opinions and views with each other. 6) The satisfactory performance of nursing services rely on the following factors to the degree indicated: approximately 32 per cent to the systematic nursing activities and services; 27 per cent to the head nurses ability for nursing diagnosis; 22 per cent to an effective supervisory system; 16 per cent to the hospital facilities and proper supply, and 3 per cent to effective in·service education. This means that nurses, supervisors, head nurses and directors play the most important roles in the performance of nursing services. 11. About 87 per cent of the hospitals do not have separate budgets for their nursing departments, and only 13 per cent of the hospitals have separate budgets. It is recommended that the planning and execution of the nursing administration be delegated to the pertinent administrators in order to bring about improved proved performances and activities in nursing services.
