Adaptive Power Control Strategy based on Spectrum Sensing for Cognitive Relay Networks (CR 넷워크를 위한 주파수 감지에 기번한 적응적인 전력 제어 전략)

  • An adaptive power control scheme is proposed for the cognitive relay networks with joint overlay and underlay spectrum sharing model. The transmit power of the secondary user is adjusted adaptively according to the spectrum sensing results and the interference channel condition. The outage probability of the secondary user is compared by Monte - Carlo simulations between the fixed power control scheme and pure overlay or underlay spectrum sharing schemes. The results show that, by employing the adaptive power control strategy, the interference probability of the secondary user to the primary user is decreased by 70 % ~ 80 % under the same outage probability. Also, the outage probability of the secondary user is reduced by 1 ~ 2 orders of magnitude under the same interference probability. Thus, the performance of the spectrum sharing is improved effectively.

Pulsed Radiofrequency Application for the Treatment of Pain Secondary to Sacroiliac Joint Metastases

  • Sacroiliac (SI) joint pain can result from degeneration, infection, malignancy, and trauma. Patients with metastatic bone pain who do not respond to conventional treatment may need more aggressive neuroinvasive approaches. Recently, pulsed radiofrequency (PRF) neuromodulation has emerged as a promising treatment alternative for refractory cases of SI joint pain. Nevertheless, there is no report on the treatment of pain arising from SI joint metastases with PRF. We are reporting about a 63-year-old woman suffering from buttock pain due to breast cancer metastases in the SI joint. We treated this patient with PRF neuromodulation of the L4-S3 primary dorsal rami and lateral branches using a rotating curved needle technique. The patient tolerated the procedures well, without any complications. She experienced about 70% reduction in pain, and pain relief was sustained for 10 months. This result suggests that PRF neuromodulation is a safe, effective treatment for pain from SI joint metastases.

Osteochondroma on distal tibiofibular joint in adult - One Case Report - (성인의 원위 경비골 관절에 발생한 골연골종 - 1예 보고 -)

  • Although osteochondromas are common lesions, osteochondroma accompanying diastasis of the ankle joint in a child or adult is an uncommon condition. No previous cases of distal tibiofibular diastasis caused by an osteochondroma in adult have been reported except in child. The authurs experienced a case of mild distal tibiofibular diastasis secondary to an osteochondroma in a 37-year-old female presented with right ankle pain. Osteochondroma of the distal tibiofibular joint were detected on magnetic resonance imaging (MRI). She was pain-free 4 weeks after excision of the osteochondroma. The patient has remained asymptomatic for 1 years postoperatively. Early excision obviates the need for complex reconstructive surgery to correct ankle deformity later.

  • 본 논문에서는 5G NR 시스템을 위한 동기 신호를 이용한 cell ID 검출 방법에 대한 성능을 비교하였다. 5G NR(fifth-generation new radio) 시스템의 송신기는 SS/PBCH (synchronization signal/physical broadcast channel) 블록을 송신하며, 수신기는 수신된 SS/PBCH 블록을 이용하여 주파수 및 타이밍 오프셋 (frequency and timing offset)을 추정 할 수 있으며, cell ID (cell identity)는 PSS (primary synchronization signal)와 SSS (secondary synchronization signal)를 통해 검출할 수 있다. 본 논문에서는 cell ID 를 검출할 수 있는 방법으로서 2-stage 디코딩 방법과 결합 최대우도 결정 규칙 (joint maximum-likelihood decision rule: joint ML) 디코딩 방법을 사용하였다. Joint ML 디코딩 방법은 2-stage 디코딩 방법에 비해 더 좋은 검출 성능을 보이지만, 복잡도 측면에서는 2-stage 디코딩 방법이 joint ML 디코딩 방법에 비해 더 낮은 복잡도를 갖는 것을 확인하였다.

Evaluation of Rock Fragmentation due to Artificial Joint Effect (인공절리에 의한 암석의 파쇄도 평가)

  • Since the rock fragmentation by blasting can affect the subsequent processes including loading, hauling and secondary crushing, its control is essential for the assessment of blasting efficiency as well as production cost. In this study, we were analyzed the rock fragmentation by the direction of artificial joint. The underground blasting experiments were performed after forming the vertical and horizontal artificial joints. The blast fragmentation was conducted by the split-desktop which is a 2D image processing program. As a result, it was found that the horizontal artificial joint was evaluated to have lower overall the size of muck pile than the vertical artificial joint and the distribution of the size of muck pile was varied. It is possible that the direction of artificial joint could suppress the occur of oversize muck pile and control to a certain size or less.

  • Acromioclavicular (AC) joint cyst is an uncommon presentation around the shoulder joint that is usually associated with chronic rotator cuff pathology and degenerative AC joint arthritis. Understanding the underlying pathophysiology of the shoulder joint is essential for appropriate interpretation of the AC joint cyst, assistance in determining the therapeutic strategy, and prevention of complications after treatment. We present a case of AC joint cyst with an aseptic fistula which was complicated by simple excision for recurred AC joint cyst after multiple aspirations. This case revealed a massive rotator cuff tear involving supraspinatus, infraspinatus, and subscapularis tendon on ultrasonography. We also reviewed the clinical approach and treatment options to be considered for its pathogenesis in the literature.

  • Anatomy: Facet joint syndrome most often affects the lower back and neck and refers to pain that occurs in the facet joints, which are the connections between the vertebrae in the spine that enable the spine to bend and twist. Many physicians have believed that the usual lesion of facet syndrome was an anatomical impairments of facet joint itself.. Facet joint injection using local anesthetics is a reliable method for the diagnosis and treatment for facet syndrome. Etiology: One of many possible causes is imbalances that can occur in stress levels, hormone levels, and nutritional levels. These imbalances can adversely affect posture, which can lead to neck and back pain. The common disorder called facet syndrome exhibits lower back pain, with or without, radiating pain to buttock and thigh due to facet joint arthropathy. Pain in the facet joint is supposedly the secondary effect of narrowing of joint space by sustained muscle contracture around joints. Syndrome: Facet joint syndrome tends to produce pain or tenderness in the lower back that increases with twisting or arching the body, as well as pain that moves to the buttocks or the back of the thighs. Other symptoms include stiffness or difficulty standing up straight or getting out of a chair. Pain can be felt in other areas such as the shoulders or mid-back area. Treatment: Non-drug treatments include hot packs, ultrasound, electrical stimulation, and therapeutic exercises. Stimulating blood flow using massage or a hot tub may also help. Alternative treatments include yoga and relaxation therapy. If your pain persists after trying these treatments, a surgical procedure called radiofrequency rhizotomy, which destroys the sensory nerves of the joint, may bring relief. Facet joint injection has been helpful in diagnosis and therapy for this facet syndrome. Radiofrequency thermocoagulation of medial branches is known to be an effective method of relieving pain caused by facet joint problems. We conclude that spasmolytic treatment of muscles connecting the two vertebral articular space would be better for treatment and diagnosis of facet syndrome rather than facet block with local anesthetic and steroid only.

Clinical Features of the Patients with Craniomandibular Disorders (두개하악장애환자의 임상양태에 관한 연구)

  • A prevalence study was carried out on 847 CMD patients who had visited the Department of Oral Medicine in Pusan National University from 1990 to 1993. To obtain the same type of information, all subjects were interviewed and examined clinically using a standardized examination form, The ratio of women to men was about 3:1 and all subjects were divided into acute and chronic groups on the basis of 6 months of duration. Diagnostic groups consisted of muscle disorder, joint disorder and muscle-joint disorder. As related to gender, duration and diagnosis subjective and objective symptoms in CMD were studied. The obtained results were as follows : 1. Muscle-joint disorder had the highest percent, followed by muscle disorder and joint disorder. 2. The most common reasons for CMD treatment were pain, joint noise and limited opening, while headache and neckache were relatively often reported as associated symptoms and dizziness, ringing in the ears also reported as secondary CNS excitatory effects. 3. Pain was more ofter seen in women, acute group and muscle-related disorder groups (p<0.05, p<0.01). Noise was significantly frequent in chronic group and joint-related groups (p<0.01). 4. Analysis of contributing factors presented that macrotrauma was found frequently in men (p<0.05), and that muscle-related groups were more related to stress than joint disorder grop (p<0.05). 5. Hard end feel was seen significantly often in joint-related disorder group (p<0.05). On the other hand, soft end feel was frequent in muscle disorder. 6. Reciprocal clicks and crepitation increased with chronicity. Subjects with joint-related disorder groups significantly often reported all kinds of noises (p<0.01). 7. Tender muscles and joints were more often reported in women and chronic group. Whereas muscle-related disorder groups revealed significantly more tender muscles (p<0.01). joint-related disorder groups presented significantly more tender joints (p<0.01).

Simultaneous Information and Power Transfer for Multi-antenna Primary-Secondary Cooperation in Cognitive Radio Networks

  • In this paper, cognitive radio and simultaneous wireless information and power transfer (SWIPT) are effectively combined to design a spectrum-efficient and energy-efficient transmission paradigm. Specifically, a novel SWIPT-based primary-secondary cooperation model is proposed to increase the transmission rate of energy/spectrum constrained users. In the proposed model, a multi-antenna secondary user conducts simultaneous energy harvesting and information forwarding by means of power splitting (PS), and tries to maximize its own transmission rate under the premise of successfully assisting the data delivery of the primary user. After the problem formulation, joint power splitting and beamforming optimization algorithms for decode-and-forward and amplify-and-forward modes are presented, in which we obtain the optimal PS factor and beamforming vectors using a golden search method and dual methods. Simulation results show that the proposed SWIPTbased primary-secondary cooperation schemes can obtain a much higher level of performance than that of non-SWIPT cooperation and non-cooperation schemes.
