  • Introduce of gene connected with disease and transformation system of ginseng, Squalene systhase(PSS) gene cloned from and disease resistant gene were carried out for expression and transformation of plant using Agrobacterium. PSS of 35S-35S-AMV-PSS-Tnos, has been constructed which were mobilized into Agrobacterium tumefaciens strain MP 90 disarmed Ti-plasmid. PSS gene were introduced into the binary vector pRD 400. The transgenic ginseg plants were propagated using repetitive secondary embryogenesis and introduced NPTII and PSS genes of the transgenic ginseng were successfully indentified by the PCR and survival test on the medium.

폐 국균종의 외과적 치료 (Surgical Treatment of Pulmonary Aspergilloma)

  • Pulmonary aspergilloma is a rare disease, most commonly presenting as secondary invasion of preexisting cavitary disease. Tuberculosis and bronchieetasis were the commonest pre-existing diseases. Between 1990 and 1994, 11 patients[7 male and 4 female underwent thoracotomy for treatment of pulmonary aspergilloma. The mean age was 45.7 years. The most common indication for operation was hemoptysis. Lobectomy was the m&t frequent operation. Major complication occurred in a patient with bronchopleural fistula and treated with open drainage. There was no operative death. It is concluded that elective pulmonary resection is recommended for all patients with aspergilloma who do not constitute prohibitive operative risks.

  • Caudal agenesis refers to the congenital malformation with the essential feature of the agenesis of the sacrococcygeal bone. It is associated with various types of spinal cord anomaly as well as with complex anomalies of genitourinary or gastrointestinal system. The wide spectrum of the disease can be attributed to its pathoembryological origin, the secondary neurulation. This review presents the definition, etiology, classification, and clinical features of the disease.

허혈성심질환 발생에 대한 대사증후군과 비만의 개별효과와 결합효과 (Combined Influence of Obesity and Metabolic Syndrome on Ischemic Heart Disease in Korean middle aged and older adults)

  • Purpose: The purpose of this study was to examine a combined influence of obesity and metabolic syndrome on ischemic heart disease in Korean middle aged and older adults. Methods: This study used secondary data from the 2013 Korea National Health and Nutrition Examination Survey. A total of 3,726 adults over age 40 were included. Logistic regression was used for analysis of complex samples. Gender, age, educational level, family income, family history of ischemic heart disease, physical activity, smoking, and heavy alcohol consumption were analyzed as covariates. Results: Ischemic heart disease was more prevalent among adults with metabolic syndrome regardless of obesity (non-obesity: Adjusted Odds Ratio [AOR]: 3.044, 95% Confidence Interval [CI]: 1.163-7.967, overweight: AOR: 2.805, 95% CI: 1.246-6.316, obese: AOR: 3.137, 95% CI: 1.548-6.358) compared to the reference group, defined as adults with non-obesity and non-metabolic syndrome. Odds of ischemic heart disease were not significant in the group with obesity and non-metabolic syndrome compared to the reference group. Conclusion: The results of this study show that the population with metabolic syndrome is an at-risk group for ischemic heart disease. Thus, management of metabolic syndrome is required for prevention of ischemic heart disease.

형방지황탕(荊防地黃湯)으로 호전된 초기 파킨슨병환자의 두통(頭痛) 변비(便秘) 치험례 (A Case report of Soyangin patient with early-stage Parkinson's disease treated with Hyeongbangjihwhang-tang)

  • Objectives The purpose of this case study is to report significant improvement of headache, constipation and a slight improvement of tremor after treatment with Hyeongbangjihwhang-tang in a Soyangin patient with Parkinson's disease.Methods This Soyangin patient is treated by Hyeongbangjihwhang-tang and acupuncture therapy. The progress was evaluated using the Unified Parkinson Disease Rating Scale before and after treatment. Secondary results assessments include Global Assessment Scale, change of patient's omniloquilism, Xerostomia and Cold hands & Feet Syndrome.Results and Conclusions The patient's symptoms of headache, constipation and tremor were improved after using Hyeongbangjihwhang-tang. and UDPRS score decreased in Part Ⅰ, Part Ⅱ, and Part Ⅲ. In conclusion, this study shows that Hyeongbangjihwhang-tang can be effective treatment for Soyangin patient with early-stage Parkinson''s disease. Overall, Sasang medicine can be a solution of the symptoms caused by Parkinson's disease. Through Sasang medicine we can contribute Parkinson's disease Patient's quality of life get better.

후천성 폐 동정맥루 -1례 보고- (Acquired Pulmonary Arteriovenous Fistula -A Case Report-)

  • Pulmonary arteriovenous fistula can be either congenital or acquired. The vast majority are congenital, and about 60% have been associated with hereditary hemorrhagic telangiectasia [Rendu-Osler-Weber disease . Secondary or acquired pulmonary arteriovenous fistula occurs with trauma, schistosomiasis, long-standing hepatic cirrhosis, metastatic carcinoma, and actinomycosis. Pulmonary hemorrhage secondary to acquired pulmonary arteriovenous fistula is a rare event associated with mortality. We have experienced 64 year-old female patient with the hemoptysis secondary to acquired pulmonary arteriovenous fistula due to the infection of pulmonary parasite. The chest PA and CT scan was showed calcified nodule to the distal portion of lateral segmental bronchus of RML. The bronchial angiogram was demonstrated slightly hypertrophied bronchial artery supplying RML bronchus and the presence of hypervascularization around the calcified nodule, rapid A-V shunting is noted by fluoroscopy. The patient was successfully treated by the right middle lobectomy.

A review of analysis methods for secondary outcomes in case-control studies

  • The main goal of a case-control study is to learn the association between various risk factors and a primary outcome (e.g., disease status). Particularly recently, it is also quite common to perform secondary analyses of the case-control data in order to understand certain associations between the risk factors of the primary outcome. It has been repeatedly documented with case-control data, association studies of the risk factors that ignore the case-control sampling scheme can produce highly biased estimates of the population effects. In this article, we review the issues of the naive secondary analyses that do not account for the biased sampling scheme, and also the various methods that have been proposed to account for the case-control ascertainment. We additionally compare the results of many of the discussed methods in an example examining the association of a particular genetic variant with smoking behavior, where the data were obtained from a lung cancer case-control study.

개심술후 재수술에 대한 임상적 고찰 (Reoperation after Open Heart Surgery -Clinical analysis of 27 cases-)

  • The emergence and expansion of cardiac surgery over the past decade has resulted in an increasing number of patients undergoing cardiac operations but many kinds of heart surgery was realized only palliative, resulting in increasing numbers of secondary cardiac procedures. From 1978 to 1988, 10 cases of various congenital heart diseases and 17 cases of acquired heart diseases were reoperated at Hanyang University Hospital. The leading indication of second operation was residual shunt or valvular malfunction due to technical failure in congenital heart disease and primary valve failure, endocarditis, paravalvular leakage were for acquired heart disease. The mortality of reoperation was 0% for congenital heart disease and 11.7%[2 death among the 17 patients] for acquired heart disease. The leading causes of death were myocardial failure, sepsis with endocarditis, acute renal failure and congestive heart failure.

Understanding insomnia as systemic disease

  • Sleep plays a critical role in homeostasis of the body and mind. Insomnia is a disease that causes disturbances in the initiation and maintenance of sleep. Insomnia is known to affect not only the sleep process itself but also an individual's cognitive function and emotional regulation during the daytime. It increases the risk of various neuropsychiatric diseases such as depression, anxiety disorder, and dementia. Although it might appear that insomnia only affects the nervous system, it is also a systemic disease that affects several aspects of the body, such as the cardiovascular, endocrine, and immune systems; therefore, it increases the risk of various diseases such as hypertension, diabetes mellitus, and infection. Insomnia has a wide range of effects on our bodies because sleep is a complex and active process. However, a high proportion of patients with insomnia do not seek treatment, which results in high direct and indirect costs. This is attributed to the disregard of many of the negative effects of insomnia. Therefore, we expect that understanding insomnia as a systemic disease will provide an opportunity to understand the condition better and help prevent secondary impairment due to insomnia.

소아 만성 콩팥병에서 나타나는 심혈관계 질환 (Cardiovascular Disease in Pediatric Chronic Kidney Disease)

  • 만성 콩팥병의 장기 예후를 결정하는 요인 중 심혈관계 합병증의 중요성은 잘 알려져 있다. 그러나 소아 환자의 경우 전형적인 증상 발현이 적어 그 임상적 중요성이 간과되는 경향이 있다. 현재까지 알려진 심혈관계 합병증의 위험인자로는 고혈압, 당뇨병, 이상지질혈증, 비만과 같은 전통적인 위험 인자와 빈혈, 이차성 부갑상선 기능항진증, 산화 스트레스, 염증과 같은 새로운 위험 인자 등이 알려져 있다. 소아 만성 콩팥병의 경우 주로 좌심실 비대나 경동맥 내중막 두께의 증가 및 관상동맥의 석회화 같이 대부분 겉으로 드러나지 않는 증상이므로 위험 인자에 대한 철저한 관리와 지속적인 추적 관찰이 필수적이다.
