  • Objectives : Ulcerative colitis and Crohn's disease are chronic inflammatory disease of the gastrointestinal tract was identified mainly for diarrhea, stomachache, hemafecia. The source and pathologic mechanism about ulcerative colitis and crohn's disease are still unknown but stress, infection, a genetic factor, environmental factors etc. may be the cause of chronic inflammatory disease of the gastrointestinal tract recently. The purpose of the present study is to evaluate the effects of herbal acupuncture with Artemisiae Argyi for the treatment to intestinal disease in the rat with 2, 4, 6 - trinitrobenzenesulfonic acid (TNBS) induced colitis. Methods : Artemisiae Argyi Herbal acupunctures were infected in HapGok (LI4) which is used to treat for intestinal disease in clinic. All animals were subjected to the injection of saline (300 ${\mu}l$, 500 ${\mu}l$) for a study control and TNBS (300 ${\mu}l$, 500 ${\mu}l$) into the lumen of the colon, 8cm proximal to the anus through the intestine. Artemisiae Agryi' Herbal (20 mg/ml, 0.4 ml) acupunctures were injected to the LI4 (both hands) at the secondary injection time of TNBS in rats. And body weight, RBC count, WBC count, total protein, Paw edema rate, rate of protein leakage into CMC-pouch fluid, IgG levels and IgM levels were observed to study the effects of Artemisiae Argyi' Herbal acupuncture in HapGok (Ll4). Results : Artemisiae Argyi' Herbal acupuncture in HapGok (Ll4) on TNBS-induced colitis inhibit the body weight lose rate but not RBC and WBC count. Furthermore, it inhibited the reduction of total protein concentration, paw edema, rate of protein leakage into CMC-pouch fluid, IgG levels and IgM levels. Conclusions : Herbal acupuncture with Artemisiae Argyi helps recover the TNBS-induced colonic damage and may be an important method for treatment of the colitis.

  • Purpose: The purpose of this study was to evaluate and compare the scintigraphic findings and diagnostic accuracy of double-phase Tc-99m sestamibi scan in primary and secondary hyperparathyroidism (HPT). Materials and Methods: We retrospectively reviewed 16 cases of primary (18 lesions) and 11 cases of secondary HPT (44 lesions) who underwent Tc-99m-sestamibi scan before the surgical intervention. Scan was performed using LEM camera (Siemens, Germany) after the injection of 740MBq of Tc-99m sestamibi. Routine image consisted of baseline and 3-hour delayed images and each image was obtained using both parallel and pine hole collimator. The study population was 27 patients (male/female=5/22, age: $49.1{\pm}10.8$). Results: Eighteen lesions of primary HPT consisted of 13 adenomas and S hyperplasias, while all lesions of secondary HPT were hyperplasias. Among the case of primary HPT, we could detect all the lesions of 13 adenomas but only 2 lesions of 5 hyperplasias (40%) could be detected by double phase scintigraphy. Three cases of primary lesion showed decreased uptake in delayed images compared with baseline. The sensitivity, specificity, positive predictive value and accuracy of primary and secondary HPT were 58.8% (10/17), 83.3% (10/12), 83.3% (10/12), 75.9% (22/29), and 37.5% (15/40), 50% (2/4), 88.2% (15/17), 38.6% (17/44), respectively. Overall sensitivity, specificity, positive predictive value and accuracy were 43.9% (25/57), 75% (12/16), 86.2% (25/29), and 53.4% (39/73). There were no statistical difference between the weight of primary and secondary HPT lesion (p>0.05). Conclusion: Tc-99m sestamibi scan is fairly good modality to detect parathyroid lesion in patient with primary HPT before the surgical intervention. However, since some of cases may reveal decreased uptake in delayed image, a careful attention to the findings of baseline images may be helpful. Still the low accuracy of sestamibi scan in diagnosis of secondary HPT prohibits routine use of it for this disease.

  • The study is designed to grasp school adminstrators' attitudes toward school health education and practices of school health education and to analyse relationships between characteristics of school administators and their attitudes to school health education and between their attitudes and practices of school health education in elementary and secondary schools, so that it may offer the basic materials for improving the school health education. This study was obtained from 740 school administrators all over the country, who have attended Certification Training of Elementary and Secondary Principal opened at Korea National University of Education from July, 1, 1991 to August 9, 1991. The results are summarized as follows: 1. School administrators' attitudes toward school health education 1) As for view of health education, those who believed that health is the means of education was 36.2%. 2) In methods of school health, those who want regular health education was 75% ; irregular health education 25%. 3) As for attitudes of those chose irregular health education as teaching methods : (1) In teaching hours, the morning and afternoon class meetings was highest 50.8% of whole. (2) In teaching staffs, home room teacher was 51.9% ; school nurse 34.8%. (3) In frequency of irregular health education, 1-2 times a week was 38%, 1-2 times a month 32.6%, 1-2 times a semester 11.8%. (4) As for teaching methods of irregular health education, practice through the health function showed higher frequency than other methods 4) As for attitudes of those chose regular health education as teaching methods : (1) Most of respondents had opinion that regular health education should be carried out by the subject of physical education in both elemetary and secondary schools. (2) Opinion that school nurse should be in charge of regular health education was a little higher than other ones. (3) More than a half of the whole thought that 1-2 hours a month was suitable for carrying out regular health education. (4) Therr-fourth of those who wanted regular health education chose the using of audiovisual material as a teching method. (5) Curriculum recommended first by respondents for the school health education were personal habits and health, prevention and control of disease, mental and emotional health etc. (6) As for impedimental factors in the development of school health education, it was shown as following order : the lack of professional health education instructors, the lack of budget and administrative support, the lack of instructional materials and instruments etc.

  • Tricuspid regurgitation has been considered as a secondary lesion when it is combined with left valvular heart diseases. However, there have been some reports which show that tricuspid regurgitation keeps going and results in congestive heart failure even after a successful operation for left valvular heart disease. So far, there are no definite operation indications and predictive factors for the tricuspid re-gurgitation which is resulted from the left sided valvular heart disease. We designed this study to evaluate the effects of pulmonary artery pressure and left ventricular ejection fraction on the prognosis of tricuspid regurgitation, and to make an operation indication for the patients with secondary tricuspid regurgitation. Material and Method: We reviewed the medical records of patients who underwent surgery for the left sided valvular heart disease with tricuspid regurgitation and were followed for more than 1 year with echocardiograms. There was a total of 114 cases. We compared the grades of tricuspid regurgitations and pulmonary artery pressures and left ventricular ejection fractions on the basis of echocardiograms which were checked preoperatively and on the last follow up. Result: There were 43 cases of tricuspid an-nuloplasty. In these patients, the grades of tricuspid regurgitations were improved in 42 cases (97.7%). But in 71 cases without annuloplasty, 29 cases (41%) were improved, 32 cases (45%) had no change, and 29 cases (14%) were aggravated. This finding shows significant differences in the prognoses of tricuspid regur-gitations between the two groups (p<0.05). There was no difference in pulmonary artery pressures and ejection fractions between the patients who showed progression of tricuspid regurgitations and those who didn't (p > 0.05). The improvements of tricuspid regurgitations are not statistically related to the changes of pulmonary artery pressures or left ventricular ejection fractions. Conclusion: This study shows that it is impossible to predict the prognoses of tricuspid regurgitations with preoperative pulmonary artery pressures or left ventricular ejection fractions. Also, the excellent results of tricuspid annuloplasty is proven in controlling the secondary tricuspid regurgitations. Therefore, when tricuspid regurgitation is detected preoperatively, the procedures to correct the tricuspid regurgitation at the time of the operation for the left-sided valvular heart disease must be considered positively, regardless of the grades of tricuspid regurgitations, to prevent sig-nificant tricuspid regurgitation that may develop later.

  • Pasteurella multocida, a Gram-negative facultative anaerobic bacterium, is a causative animal pathogen in porcine atrophic rhinitis and avian fowl cholera. For the development of recombinant subunit vaccine against P. multocida, we cloned and analyzed the gene for outer membrane protein H (ompH) from a native strain of Pasteurella multocida in Korea. The OmpH had significant similarity in both primary and secondary structure with those of other serotypes. The full-length, and three short fragments of ompH were expressed in E. coli and the recombinant OmpH proteins were purified, respectively. The recombinant OmpH proteins were antigenic and detectable with antisera produced by either immunization of commercial vaccine for respiratory disease or formalin-killed cell. Antibodies raised against the full-length OmpH provided strong protection against P. multocida, however, three short fragments of recombinant OmpHs, respectively, showed slightly lower protection in mice challenge. The recombinant OmpH might be a useful vaccine candidate antigen for P. multocida.

  • The recent advances in the basic hematology and immunology have significantly enhanced the understanding of histiocytic disorders. The Histiocyte Society which was established in 1985 enabled the randomized trials for these diseases, and important knowledge regarding pathogenesis, clinical presentation, diagnosis, therapy and late consequences has been obtained. The treatment of Langerhans cell histiocytosis (LCH) has varied greatly over last decades, and is still controversial. Therapy can be reduced for low risk patients, and it is possible to discriminate early the non-responding patients with risk disease who might require more intensified treatment. Current therapy of LCH recommended by the Histiocyte Society (LCH-III protocol) is activated in 2001. Hemophaocytic histiocytosis (HLH) is fatal if diagnosis is delayed and appropriate therapy is not instituted rapidly. The diagnostic criteria for HLH is revised by the Histiocyte Society for the current treatment protocol (HLH-2004) which consists of dexamethasone, etoposide, and cyclosporin in combination with intathecal methotrexate. Hematopoietic stem cell transplantation is usually necessary for the primary HLH and recurrent secondary HLH.

  • We present a rare case of critically compromised airway secondary to a massively dilated sequestered colon conduit after several revision surgeries. A 71-year-old male patient had several operations after the diagnosis of gastric cancer. After initial treatment of pneumonia in the pulmonology department, he was transferred to the surgery department for feeding jejunostomy because of recurrent aspiration. However, he had respiratory failure requiring mechanical ventilation. The chest computed tomography (CT) scan showed pneumonic consolidation at both lower lungs and massive dilatation of the substernal interposed colon compressing the trachea. The dilated interposed colon was originated from the right colon, which was sequestered after the recent esophageal reconstruction with left colon interposition resulting blind pouch at both ends. It was treated with CT-guided pigtail catheter drainage via right supraclavicular route, which was left in place for 2 weeks, and then removed. The patient remained well clinically, and was discharged home.

  • Angioneurotic edema is rarely seen in dental practice and manifested by acute attacks of swelling of the extremities, face, airway, or abdominal visera, occuring spontaneously and suddenly or secondary to trauma. Two types are recognized : hereditary and nonhereditary. Prophylatic therapy may be used by fresh frozen plasma or antifibrinolytic agents in hereditary type. Good supportive care for acute attacks, together with a knowledge of course of the disease, can prevent asphyxiation from airway obstruction. A case of acute angioneurotic edema of the facial area in a 19-year-old man is presented in conjunction with a review of the literature. Angioneurotic edema attacks him acutely after the extraction of the lower, right 3rd molar.

  • Conversion disorder is a psychologically produced alteration or loss of physical functioning suggestive of a physical disorder. Conversion symptoms are often superimposed on organic disease and can be overlooked. Psychological techniques are central to the management include the following: avoiding confrontation with the patients; avoiding reinforcement or trivializing the symptoms; reviewing results of tests and exams and creating an expectation of recovery; educating the patient before a treatment is begun; evaluating the patient's emotional adjustment and considering it at a treatment; using caution in labeling the condition; considering referral for psychotherapy; establishing particularly a treatment plan and making a definite treatment program; adjusting patient' s environment; letting participate a family at appropriate time; developing a reinforcement program for a treatment of chronic symptoms; developing a home program for outpatients. Use behavior therapy reinforcement may be helpful with more chronic or resistant symptoms, especially when there is a history of vague or excessive somatic complaints or significant secondary gain.
