Purpose: The present retrospective clinical study aimed to evaluate and compare the clinical and radiographic parameters, complications, and satisfaction in patients who received fixed prostheses supported by narrow-diameter implants (NDIs) in the anterior and posterior jaw. Methods: Patients aged ≥30 years who had NDI-supported fixed prostheses in the anterior or posterior region of either jaw for at least 2 years were included. Complications such as chipping of the crown; loosening or fracture of the screw, crown abutment, or implant; and loss of retention were recorded. Clinical peri-implant outcomes and crestal bone loss (CBL) were measured. A questionnaire was used to record responses regarding the aesthetics and function of the fixed restorations. Analysis of variance was used to assess the significance of between-group mean comparisons. The log-rank test was performed to analyze the influence of location and prosthesis type on technical complications. Results: Seventy-one patients (mean age: 39.6 years) provided informed consent with a mean follow-up duration of 53 months. Only bleeding on probing showed a statistically significant difference between NDIs in the anterior and posterior regions. The complication rate for NDIs in the posterior region was significantly higher than that for NDIs in the anterior region (P=0.041). For NDIs, CBL was significantly higher around splinted crowns than single crowns (P=0.022). Overall mean patient satisfaction was 10.34±3.65 on a visual analogue scale. Conclusions: NDIs in the anterior and posterior jaws functioned equally well in terms of periimplant soft and hard tissue health and offered acceptable patient satisfaction and reasonable complication rates.
Purpose: The causes of twelve cases of the mid-shaft clavicle nonunion and the results of internal fixation with plate and bone graft were investigated. Materials and method: From August 1997 to March 2003, twelve cases of the mid-shaft clavicle nonunion were operated with internal fixation with plate and bone graft. The duration of follow-up was average 13 months. Results: The causes of the mid-shaft clavicle nonunion included severe associated injury, severe initial displacement of the fracture fragments and insecure fixation. All cases were operated with internal fixation with plate and bone graft. According to the factor for evaluations of results, using a rating scale of excellent (no apparent factors), good (one factor), fair (two factors), poor (more than three factors), the results showed 10 excellent, 1 good and 1 poor. Screw loosening was developed in only one case . Conclusion: The internal fixation with plate and bone graft of the mid-shaft clavicle nonunion after failed conservative treatment achieved excellent results and seemed to be the procedure of choice for mid-shaft clavicle nonunion.
The purpose of this study was to evaluate how mandibular implant-supported fixed complete prosthesis, implant and mandible responded mechanically, according to curvature of arch, number and location of fixture, and amounts of load. The shape of mandibular arch was tapered or square form and, 4 or 6 fixtures were implanted in each arch model. A vertical load of 10kg was applied at the center of prosthesis and a vertical load of 20kg was applied at the location of the 10mm or 20mm cantilever posterior to the most distal implant. Three-dimensional finite element analysis was performed for stress distribution and deflection using commercial software(ABAQUS program) for Sun-SPARC Workstation. The results were as follows : 1. The case square arch form was more stable to compare with that of tapered arch form in respect of stress distribution and displacement under vertical load on the center of prosthesis. 2. 6-implants cases were more stable than 4-implants cases for decreasing bending torque under vertical load on the center of prosthesis. 3. Under vertical load on cantilever extension, the case of 10mm long cantilever was more stable than that of 20mm long cantilever in respect of stress distribution and displacement. 4. Under vertical load on cantilever extension, 6-implants cases had a tendency to reduce displacement and to increase the reaction force of supporting point due to increasing of the bending stiffness of the prosthesis than 4-implant case. 5. When the ends of 10mm or 20mm long cantilever were loaded, the most distal implant was under compressive stress but the second most distal implant was under the highest tensile stress and the remaining implants were under varying tensile stress. 6. Because 6-implants cases had smaller displacement than 4-implants cases, 6-implants cases were more favorable in respect of prevention of screw loosening under repeated loadings.
This paper investigates the Synergetics based Damage Detection Method (SDDM) for frame structures by using surface-bonded PZT (Lead Zirconate Titanate) patches. After analyzing the mechanism of pattern recognition from Synergetics, the operating framework with cooperation-competition-update process of SDDM was proposed. First, the dynamic identification equation of structural conditions was established and the adjoint vector (AV) set of original vector (OV) set was obtained by Generalized Inverse Matrix (GIM).Then, the order parameter equation and its evolution process were deduced through the strict mathematics ratiocination. Moreover, in order to complete online structural condition update feature, the iterative update algorithm was presented. Subsequently, the pathway in which SDDM was realized through the modified Synergetic Neural Network (SNN) was introduced and its assessment indices were confirmed. Finally, the experimental platform with a two-story frame structure was set up. The performances of the proposed methodology were tested for damage identifications by loosening various screw nuts group scenarios. The experiments were conducted in different damage degrees, the disturbance environment and the noisy environment, respectively. The results show the feasibility of SDDM using piezoceramic sensors and actuators, and demonstrate a strong ability of anti-disturbance and anti-noise in frame structure applications. This proposed approach can be extended to the similar structures for damage identification.
Objective : The retrospective study is undertaken to report clinical results of anterior cervical interbody fusion with an intradiscal cage with an integrated plate [PCB cervical plating system]. Methods : 38 patients underwent anterior cervical interbody fusion with PCB cervical plating system and followed $6{\sim}24\;months$. The authors investigated overall surgical results; clinical outcome, fusion rate, change of interspace height & lordotic angle, and complications. Results : No complication was observed during the operation. Clinical improvement was identified in 34 cases [89.5%]. Bone fusion observed in 44 out of 49 sites [90.7%]. After operation, the interspace height increased from $5.4{\pm}1.3mm$ to $7.8{\pm}1.5mm$ and maintained $7.4{\pm}1.1mm$ and, interspace angle went up from $4.2{\pm}0.7^{\circ}$ to $4.8{\pm}1.1^{\circ}$ and maintained $4.6{\pm}$0.9^{\circ}. The loosening of screw was observed in 6 cases, one of which had reoperation because of the expulsion of the device accompanied. Conclusion : PCB cervical plating system could restore interbody height and lordosis in anterior cervical interbody fusion. But, if the insertion of the spacer is not precise, the frequencies of hardware failure are relatively high. It is considered necessary for the operator to be careful in the procedure.
Dental implants are currently widely used as artificial teeth due to their good chewing performance and long life cycle. A dental implant consists of an abutment as the upper part and a fixture as the lower part. When chewing forces are repeatedly applied to a dental implant, gap at the interface surface between the abutment and the fixture is often occurred, and results in some deteriorations such as loosening of fastening screw, dental retraction and fixture fracture. To cope with such problems, a sealing-type abutment having a number of grooves along the conical-surface circumference was previously developed, and shows better sealing performance than the conventional one. This study carries out optimization of the groove shape by genetic algorithm(GA) as well as structural analysis in consideration of external chewing force and pretension between the abutment and the fixture. The overall optimization system consists of two subsystems; the one is the genetic algorithm with MATLAB, and the other is the structural analysis with ANSYS. Two subsystems transmit and receive the relevant data with each other throughout the optimization processes. The optimization result is then compared with that of the conventional one with respect to the contact pressure and the maximum stress. The result shows that the optimized model gives better sealing performance than the conventional sealing abutment.

Objectives: This study aimed to compare the effectiveness of a hybrid arch bar (hAB) with the conventional Erich arch bar (EAB) for the management of jaw fractures, focusing on their use for temporary fixation in patients undergoing open reduction and internal fixation (ORIF). Materials and Methods: Patients presenting with maxillary and mandibular fractures at our institution were included in this prospective, comparative study. Placement time and ease of occlusal reproducibility were recorded intraoperatively for Group A (hAB patients) and Group B (EAB patients). The primary outcome was comparison of the postoperative stability of the two arch bars. Postoperative measurements also included mucosal overgrowth, screw loosening or wire retightening, and replacement rates. The data were tabulated and computed with a P<0.05 considered statistically significant. Results: The study included 41 patients. A statistically significant difference was observed in postoperative stability scores (3) between Group A and Group B (85.0% vs 9.5%, P=0.001). The mean placement time in Group A (23.3 minutes) significantly differed from that in Group B (86.4 minutes) (P<0.001). The ease of intraoperative occlusion was not different between the two groups (P=0.413). Mucosal overgrowth was observed in 75.0% of patients (15 of 20) in Group A. Conclusion: The hAB was superior to EAB in clinical efficiency, maxillomandibular fixation time reduction, stability, versatility, and safety. Despite temporary mucosal overgrowth, the benefits of hAB outweigh the disadvantages. The choice between hAB and EAB should be based on specific clinical requirements.
The purpose of this study was to analyze the stress distribution at supporting bone according to the types of connection modality between implant and tooth in the superstrcture. This investigation evaluated the stress patterns in a photoelastic model produced by three different types of dental implants such as Branemark, Steri-Oss, IMZ and resin tooth using the techniques of quasi three dimensional photoelasticity. The teeth-supported bridge had a first molar pontic supported by second premolar and second molar as a control group. The implant and toothsupported bridge had a first molar pontic supported by second premolar and implant posterior retainer as an experimental group. Prostheses were mechanically connected to an adjacent second premolar by the rigid of nonrigid connection, Nonrigid connection used an attachment placed between the tooth-supported and fixture-supported component. The female(keyway) of attachment was placed on the distal end of the retainer supported by the tooth ; the male(Key) of attachment connected to the osseointegrated bridge was engaged into the keyway. All prostheses were casted in the same nonprecious alloy and were cemented and screwed on their respective abutments and implants. 16㎏ of vertical loads on central fossae of second premolar, first molar pontic, implant of second molar were applied respectively and 6.5㎏ of inclined load on middle buccal surface of first molar pontic was applied. The results were as follows : 1. Under the vertical load on the central fossa of first mloar pontic, the stress developed at the apex of tooth of implat was more uniformly distributed in the case of nonrigid connection than in the case of rigid connection. 2. Under the vertical load on the central fossa of first molar pontic, the stress developed around the cervical area of tooth of implant was larger in the case of rigid connection than in the case of nonrigid connection because the bending moment was more occured in the case of rigid connection than in the case of nonrigid connection. 3. Stress was more restricted to the loaded side of nonrigid connection than to that of rigid connection 4. Under the inclined load. The set screw loosening of implant was more easily occured in the case of nonrigid connection than in the case of rigid connection due to torque moment. 5. In the case of Branemark implant, the stress concentration in second premolar was larger and the stress developed around the cervical area of implant was lower than any other cases under the vertical load, because Branemark implant with the flexible gold screw was showed in incline toward second premolar by a bending moment. 6. The stress developed around the apex of tooth or implant was more uniformly distributed in the case of Steri-Oss implant with stiff screw than in the case of Branemark implant under the vertical load. But, the stress developed around the cervical area of the Steri-Oss implant was larger than that of any other implants because bending moment was occured by vertical migration of second premolar. 7. The stress distribution in the case of IMZ implant was similar to the case of natural teeth under small vertical load. But, the residual stress around the implant was showed to occurdue to deformation of IMC and sinking of screw under larger vertical load.
목적: 임플란트 지지 서베이드 고정성 보철물을 이용한 가철성 국소의치의 합병증을 임상적으로 조사하고, 임플란트 식립위치, 보철물 연결고정여부, 국소의치의 유지장치 종류, 케네디 분류, 대합치에 따른 연관 관계를 조사하고자 하였다. 대상 및 방법: 2000년부터 2016년까지 임플란트 고정성 서베이드 보철물을 이용한 가철성 국소의치로 수복을 받은 환자 11명(남자 7명, 여자 4명)을 대상으로 하였다. 평균 연령은 67.5세였으며, 상악 7개, 하악 4개의 국소의치가 제작되었다. 장착된 국소의치 11개와 이를 지지하는 37개의 임플란트 고정성 보철물의 기계적 합병증, 잔존 자연치아 상태, 주변 연조직 상태 등 모든 합병증과 그와 관련된 요소를 조사하였다. 결과: 장착된 국소의치 11개를 지지하는 총 임플란트는 37개로 국소의치마다 평균 3.4 개의 임플란트 지지 서베이드 고정성 보철물이 이용되었다. 평균 41.5 개월의 추시 기간 동안 발생한 합병증은 임시시멘트 접착 탈락, 대합치 파절/동요, 나사 파절, 나사 풀림, 클래스프 헐거움, 상부 도재파절, 임플란트 주위 골 흡수/동요, 국소의치 인공치아 파절 순으로 발생하였다. 이러한 합병증은 구치부보다 전치부에서, 연결한 보철물 보다 연결하지 않은 보철물에서, 가공선 클래스프를 유지장치로 이용한 보철물에서, 대합치가 의치를 포함하는 보철물보다 자연치열인 경우 합병증 발생이 좀 더 높은 빈도로 나타난 것을 확인 할 수 있었다. 케네디 분류에 따른 합병증 발생 빈도 차이는 유의한 차이를 확인할 수 없었다. 결론: 임플란트 융합 국소의치는 장기간 사용되고 있고 충분히 효과적으로 기능하고 있었다. 하지만 아직까지 이러한 치료의 예후에 대한 축적된 임상 결과는 많이 부족한 실정이므로 좀 더 장기적인 임상 연구가 필요한 상황이다.
연구목적 : 본 연구에서는 rotational freedom을 측정할 수 있는 기구를 개발하여 시중에서 유통되고 있는 국산 임플란트 및 다양한 국적의 임플란트들의 기계적인 가공오차들을 측정하여 다양한 임플란트 시스템의 component간의 기계적인 안정성을 평가하고자 한다. 또한 본 연구에서는 더 나아가 최근에 임플란트 abutment로 각광을 받고 있는 각종 ceramic abutment의 절삭 가공오차에 관한 항목을 측정하여 임플란트 제조사 및 임상의들에게 올바른 정보를 제공하고자 한다. 연구재료 및 방법 : 국내에서 유통되는 외부연결구조의 외국산 임플란트 시스템(Nobel Biocare, Anthorgyr)과 국산 시스템(Neobiotec)과 내부연결구조의 임플란트 시스템(외국산:Nobel Biocare, Anthorgyr, Straumann, Frident Dentsply, 국산:Dentium) 별로 임플란트 fixture, abutment, analog를 서로 교차 연결하여 회전각도측정기(rotational angle measuring device)로 freedom of rotational angle을 측정하였다. 국산 외부연결구조의 지르코니아 abutment(ZirAce)를 외부연결구조의 임플란트 시스템(Neobiotec, Nobel Biocare, Anthorgyr)의 fixture와 analog와 교차연결하여 freedom of rotational angle을 측정하였다. 연구결과 : 국산 외부연결구조의 임플란트 시스템은 약 2.67도(fixture와 abutment 연결시), 내부연결구조의 임플란트는 약 4.3도(fixture와 abutment 연결시)의 rotational freedom을 보였다. 국산 지르코니아 abutment는 외국산 및 국산 외부연결구조 임플란트 시스템과 상관없이 3도 이하(fixture와 연결시)의 결과를 보였다. 결 론 : 시제품으로 제작된 디지털 회전각도측정기는 높은 분해능을 갖고 있었으며, 국산 임플란트의 기계적 가공오차는 외국산 임플란트와 거의 유사했다. 국산 세라믹 abutment의 기계적 가공오차는 fixture 제조회사별로 다르게 나타났지만 같은 회사의 절삭가공된 금속 abutment와 비교시 가공오차가 더 적었다.
