치매평가검진 환자에 관한 임상연구 (청주 지역사회 환자의 치매진단을 중심으로) (The clinical research of dementia assessment examination-focused on the diagnosis of dementia for patient of Cheongju district.)

  • For this study, we carried out dementia assessment examination of 74 patients with memory disturbance who have come to Cheongju oriental hospital of Daejeon university from April 2005 to February 2006. This study classified the patients as none-dementia(ND), questionable dementia(QD), and dementia(DA) groups and analyzed the result of examination. As a result, the following conclusion was drawn. 1. Among the 3 groups, there was no significant differences in the sex distribution. But according to age distribution, the age of QD and DA groups showed significant difference from that of ND group. 2. MMSE-K and HDS-K scores showed the significant differences among all groups, and 7 MS result showed the significant difference between ND and the other groups. 3. The DA group significantly got lower scores than ND group in the items of the MMSE-K, Orientation, Registration, Recall, Attention, Copy two pentagons and Comprehension. Especially, significant difference also was shown in the orientation item between QD and DA groups. 4. The scores in the items of 7 MS, Benton temporal orientation, Enhanced cued recall and Clock drawing showed significant difference among all groups. Category fluency score showed significant difference between ND and the other groups. 5. The results of Brain CT and clinical chemistry test didn't show significant difference among all groups.

Validity of Breast Cancer Symptom Questionnaire and Its Relationship With Breast Ultrasonography in Young Female Night Workers

  • Background: This study aimed to identify the validity of breast cancer symptom questionnaire of worker's special health examination and its relationship with breast ultrasonography findings in young female night workers. Methods: The breast cancer symptom questionnaire data of worker's special health examination and breast ultrasonography results in young female shift workers who worked in one electronic manufacture company were collected from 2014 to 2018. Results: Of the 857 workers, 18 had a Breast Imaging Reporting and Database System category 4 or higher. Among other variables, shift work tenure alone was associated with the risk of having a Breast Imaging Reporting and Database System category higher than 4. The sensitivity, specificity, positive predictive value, and negative predictive value of the symptom questionnaire were 16.7%, 87.7%, 2.8%, and 98.0%, respectively. Conclusion: The current breast cancer symptom questionnaire of the worker's special health examination is inappropriate due to its low sensitivity and positive predictive value. In the future, female night workers will need alternative measures for more accurate screening for breast cancer.

Developing a Best-Evidence Pre-employment Medical Examination: An Example from the Construction Industry

  • The Dutch construction industry has introduced a compulsory preemployment medical examination (PE-ME). Best-evidence contents related to specific job demands are, however, lacking and need to be gathered. After the identification of job demands and health problems in the construction industry (systematic literature search and expert meeting), specific job demands and related requirements were defined and instruments proposed. Finally, a work ability assessment was linked to the instruments' outcomes, resulting in the modular character of the developed PE-ME. Twenty-two specific job demands for all Dutch construction jobs were identified, including kneeling/squatting, working under time pressure, and exposure to hazardous substances. The next step was proposing self-report questions, screening questionnaires, clinical tests, and/or performance-based tests, leading to a work ability judgment. "Lifting/carrying" is described as an example. The new modular PE-ME enables a job-specific assessment of work ability to be made for more than 100 jobs in the Dutch construction industry.

Prevalence and Risk Factors of Diabetic Retinopathy in Diabetes People using Korean National Health and Nutrition Examination Survey VII

  • Purpose: This study aimed to investigate the prevalence and risk factors of diabetic retinopathy (DR) in people with diabetes mellitus (DM) using Korean National Health and Nutrition Examination Survey VII (2017~2018). Methods: DM was defined as in two ways; 1) doctor's diagnosis (Group 1, n=549), 2) one of doctor's diagnosis, medication, or hyperglycemia (Group 2, n=849). The DR prevalence was measured as the prevalence proportion (%). Risk factors for developing DR were analyzed using multiple logistic regression, and odds ratios (ORs) and 95% confidence intervals (CIs) were calculated. Results: The prevalence of DR was 25.87% in Group 1 and 20.14% in Group 2. Risk factors for DR were identified as insulin therapy (Group 1: OR=5.31, Group 2: OR=5.27), DM duration ≥10 years (Group 1: OR=2.20, Group 2: OR=3.10), and systolic blood pressure ≥140 mmHg (Group 1: OR=2.26, Group 2: OR=2.23) for both groups. Conclusion: Considering the DR prevalence, eye examinations education is highly recommended as part of a diabetes management programs in the community. It is also proposed to shorten the eye examination cycle for people with risk factors and establish a referral system to link between screening to treatment.

중년남성의 국가암 검진수검 관련요인 (Factors Influencing the of Middle-Aged Men the NCSP(National Cancer Screening Program))

  • Objectives : The purpose of this study was to find factors influencing compliance with the national cancer screening program in middle-aged men. Methods : The data were collected from July 28, to October 31, 2015. Total subjects were 615 middle-aged men living in Busan. Results : The Rate of compliance with the NCSP(National Cancer Screening Program) for middle-aged men was 52.2%. There were significant differences in the compliance with the NCSP for the following barrier of exam(${\chi}^2=7.327$, p=.007), self-efficacy(${\chi}^2=23.074$, p<.001), age(${\chi}^2=38.823$, p<.001), marital status(${\chi}^2=19.012$, p<.001), cancer diagnosis in family(${\chi}^2=7.615$, p=.006), smoking(${\chi}^2=9.012$, p=.011), drinking(${\chi}^2=7.073$, p=.008), exercise(${\chi}^2=14.615$, <.001). Factors influencing the rate of compliance for the NCSP in middle-aged men were self-efficacy, age, marital status, exercise, and cancer diagnosis in family. Conclusion: To increase the rate of compliance to the NCSP in middle-aged men it is necessary to elevate the self-efficacy. Additional more positive support needed in men who are younger have no cancer diagnosis in their family are smokers and not m married encourage and improvve paticipation in the examination.

성인 비만의 건강특성에 관한 분석적 연구 (An Analytical Study on Health Characteristics of Obesity of Adulthood)

  • This study was carried out to offer the basic data for more qualitative lives of the adulthood with obesity through their health maintenance and control. So, the researcher reviewed and analyzed the health problems of the subjects according to sex, age, and positive rate on the screening tests. The subjects were 91 obese persons in K university hospital in Seoul from December 1, 2000 to January 31, 2001. A Questionnaire developed by researcher to obtain subject's general characteristics, main symptoms, medical examination, diagnosed disease was used as the tool for the study. The results of this study are as follows: 1. For the distribution of diagnosis in subjects, liver disease was most frequent and hypertension was second. 2 The most common complaints were indigestion in the digestive system, throat pain in the respiratory system, dyspnea in cardiovascular system, thirsty in endocrine system, frequent urine in the urinary system, vertigo in the hematologic system. 3. The main symptoms according to sex were for females, a statistically significant higher rate of complaints in the digestive system, musculoskeletal system as compared to males. 4. The main symptoms according to age were that in the fourth decade there were higher rate of complaints in the musculoskeletal system. 5. In screening tests according to sex, males showed a higher positive rate for Alk-Phosphatase and females, in FBS, urine-micro, VDRL. 6. In screening tests according to age, higher positive rates of BP, Chest X-ray, mammography, ESR, CRP were seen in the fourth decades. 7. Obesity was positively related to cardiovascular system, endocrine system, urinary system, musculoskeletal system, mental and nervous system in main symptoms. 8. Obesity was positively related to BP, Sonography, FBS, SGOT, SGPT in screening tests.

RBF와 LVQ 인공신경망을 이용한 요(尿) 딥스틱 선별검사에서의 요로감염 분류 (Classification of UTI Using RBF and LVQ Artificial Neural Network in Urine Dipstick Screening Test)

  • Dipstick urinalysis is used as a routine test for a screening test of UTI (urinary tract infection) in primary practice because urine dipstick test is simple. The result of dipstick urinalysis brings medical professionals to make a microscopic examination and urine culture for exact UTI diagnosis, therefore it is emphasized on a role of screening test. The objective of this study was to the classification between UTI patients and normal subjects using hybrid neural network classifier with enhanced clustering performance in urine dipstick screening test. In order to propose a classifier, we made a hybrid neural network which combines with RBF layer, summation & normalization layer and L VQ artificial neural network layer. For the demonstration of proposed hybrid neural network, we compared proposed classifier with various artificial neural networks such as back-propagation, RBFNN and PNN method. As a result, classification performance of proposed classifier was able to classify 95.81% of the normal subjects and 83.87% of the UTI patients, total average 90.72% according to validation dataset. The proposed classifier confirms better performance than other classifiers. Therefore the application of such a proposed classifier expect to utilize telemedicine to classify between UTI patients and normal subjects in the future.

Extended use of P504S Positive Primary Circulating Prostate Cell Detection to Determine the Need for Initial Prostate Biopsy in a Prostate Cancer Screening Program in Chile

  • Background: To determine the frequency of primary circulating prostate cells (CPC) detection according to age and serum PSA levels in a cohort of men undergoing screening for prostate cancer and to determine the diagnostic yield in those men complying with the criteria for prostate biopsy. Materials and Methods: A prospective study was carried out to analyze all men evaluated in a hospital prostate cancer screening program. Primary CPCs were obtained by differential gel centrifugation and detected using standard immunocytochemistry using anti-PSA, positive samples undergoing a second process with anti-P504S. A malignant primary CPC was defined as PSA+ P504S+, and a test positive if 1 cell/4ml was detected. The frequency of primary CPC detection was compared with age and serum PSA levels. Men with a PSA >4.0ng/ml and/or abnormal rectal examination underwent 12 core prostate biopsy, and the results were registered as cancer/no-cancer and compared with the presence/absence of primary CPCs to calculate the diagnostic yield. Results: A total of 1,117 men participated; there was an association of primary CPC detection with increasing age and increasing serum PSA. Some 559 men underwent initial prostate biopsy of whom 207/559 (37.0%) were positive for primary CPCs and 183/559 (32.0%) had prostate cancer detected. The diagnostic yield of primary CPCs had a sensitivity of 88.5%, a specificity of 88.0%, and positive and negative predictive values of 78.3% and 94.9%, respectively. Conclusions: The use of primary CPCs for testing is recommended, since its high negative predictive value could be used to avoid prostate biopsy in men with an elevated PSA and/or abnormal DRE. Men positive for primary CPCs should undergo prostate biopsy. It is a test that could be implemented in the routine immunocytochemical laboratory.

Significance of Human Telomerase RNA Gene Amplification Detection for Cervical Cancer Screening

  • Aim: Liquid-based cytology is the most often used method for cervical cancer screening, but it is relatively insensitive and frequently gives equivocal results. Used as a complementary procedure, the high-risk human papillomavirus (HPV) DNA test is highly sensitive but not very specific. The human telomerase RNA gene (TERC) is the most often amplified oncogene that is observed in cervical precancerous lesions. We assessed genomic amplification of TERC in liquid-based cytological specimens to explore the optimal strategy of using this for cervical cancer screening. Methods: Six hundred and seventy-one residual cytological specimens were obtained from outpatients aged 25 to 64 years. The specimens were evaluated by the Digene Hybrid Capture 2 (HC2) HPV DNA test and fluorescence in situ hybridization (FISH) with a chromosome probe to TERC (3q26). Colposcopic examination and histological evaluation were performed where indicated. Results: The TERC positive rate was higher in the CIN2+ (CIN2, CIN3 and SCC) group than in the normal and CIN 1 groups (90.0% vs. 10.4%, p < 0.01). In comparison with the HC2 HPV DNA test, the TERC amplification test had lower sensitivity but higher specificity (90.0% vs. 100.0%, 89.6% vs. 44.0%, respectively). TERC amplification test used in conjunction with the HC2 HPV DNA test showed a combination of 90.0% sensitivity and 92.2% specificity. Conclusion: The TERC amplification test can be used to diagnose cervical precancerous lesions. TERC and HPV DNA co-testing shows an optimal combination of sensitivity and specificity for cervical cancer screening.

응급실에서 시행한 소변 독성 검사의 유용성: 마약을 중심으로 (Availability of urine toxicologic screening tests in the emergency department: focused on illegal drugs)

  • Purpose: In Korea, it is predicted that the proportion of drug abusers among patients visiting the emergency room will soon increase. Several emergency medical institutions in Korea are conducting field urine screening tests for poisoning. In this study, we investigated the characteristics and usefulness of urine toxicology screening tests. Methods: The medical records of patients with positive results for tetrahydrocannabinol and methamphetamine from urine toxicology screening tests at a tertiary university hospital from August 2016 to August 2019 were reviewed retrospectively. The subjects were classified into positive and false-positive groups, and their clinical characteristics were compared and analyzed. Results: Of the 2,026 patients surveyed, 823 patients (40.6%) tested positive for one or more drugs. Among them, 12 cases (0.6%) were positive for methamphetamine and 40 cases (2.0%) were positive for tetrahydrocannabinol. The positive and the false-positive rates for methamphetamine were 66.7% and 33.3%, respectively. The positive and the false-positive rates for tetrahydrocannabinol were 2.5% and 97.5%, respectively. Conclusion: Methamphetamine showed a relatively low false-positive rate in our study. Therefore, this test seemed to assist in diagnosing methamphetamine poisoning when considered together with the present illness and physical examination results. On the other hand, the high false-positive rate for tetrahydrocannabinol tests indicates that this test was unlikely to assist in diagnosing tetrahydrocannabinol poisoning. However, considering the growing trend of illegal drug abusers in Korea, it may still be useful as a diagnostic tool for identifying drug users.
