This study investigates the fundamental properties of high strength concrete made with various blame values of cement, manufactured by the particle screening method in a pulverizing process. Test showed that concrete using lower blame of cement, such as large particle (L) and both ordinary and large particle (OL), increased the fluidity of fresh concrete. As tine progressive, it was noticeable that the specimens using ordinary cement (OPC) gradually decreased the fluidity, but the other specimens showed the sudden decline until 30 minutes, which is followed by a gradual decrease after 60 minutes. For the setting time, higher blaine of cement accelerated the initial and final setting time, especially concrete using minute size of cement (M) was 10 hours faster than OPC. Compressive strength of L exhibited similar value at 1 days as to that of strength in OPC at 3 days. Importantly, the specimens using M also revealed the similar strength value, regardless of curing temperature between $-5^{\circ}C$ and $20^{\circ}C$, which means that using this minute particle of cement in concrete can secure the strength development even in the lower temperature circumstance. Therefore it is clear that using OPC+M simultaneously at cold weather concreting can resist the early frost and develop the early strength of concrete.
Purpose: This study was conducted to investigate relationship between delirium, risk factors on delirium, and patient prognosis based on Donabedian's structure-process-outcome model. Methods: This study utilized a path analysis design. We extracted data from the electronic medical records containing delirium screening data. Each five hundred data in a delirium and a non-delirium group were randomly selected from electronic medical records of medical and surgical intensive care patients. Data were analyzed using SPSS 20 and AMOS 24. Results: In the final model, admission via emergency department (Β=.06, p=.019), age over 65 years (Β=.11, p=.001), unconsciousness (Β=.18, p=.001), dependent activities (Β=.12, p=.001), abnormal vital signs (Β=.12, p=.001), pressure ulcer risk (Β=.12, p=.001), enteral nutrition (Β=.12, p=.001), and use of restraint (Β=.30, p=.001) directly affecting delirium accounted for 56.0% of delirium cases. Delirium had a direct effect on hospital mortality (Β=.06, p=.038), hospital length of stay (Β=5.06, p=.010), and discharge to another facility (not home) (Β=.12, p=.001), also risk factors on delirium indirectly affected patient prognosis through delirium. Conclusion: The use of interventions to reduce delirium may improve patient prognosis. To improve the dependency activities and risk of pressure ulcers that directly affect delirium, early ambulation is encouraged, and treatment and nursing interventions to remove the ventilator and drainage tube quickly must be provided to minimize the application of restraint. Further, delirium can be prevented and patient prognosis improved through continuous intervention to stimulate cognitive awareness and monitoring of the onset of delirium. This study also discussed the effects of delirium intervention on the prognosis of patients with delirium and future research in this area.
The molecular modification of antiepileptic drugs and direct synthesis of new drugs with the predetermined antiepileptic properties are perspective. New neurochemical attacking to solve the problem including prevention and inhibition of seizures seems to be related to ginsenosides and ginseng polypeptides. The main study based on the severity of febrile convulsions of rat pups has been done from the earlier investigations of antiepileptical action of ginsenosides between KGTRI and MSU (Chepurnov, Park et al., 1995) with different kinds of experimental models of epilepsy. From the cultured cell line DAN25 of ginseng root, the extracts of ginsenosides made in "BIOKHIMMASH" were studied by the project of preclinical anticonvulsant screening (Stables, Kupferberg, 1997). The inhibition of severity of convulsions, decrease of seizures threshold, decrease of audiogenic seizures in rats of different strains and normalization of cerebral blood flow (measured by hydrogen test) were demonstrated in rats after i.c.v., intraperitoneally and orally administration, respectively. The antiepileptical effects by the combination of compounds from ginseng; were compared with the iuluence of Rg1, Rb1, Rc and with the well known antiepileptical drugs such as carbamazepine, valproic acid. The base for the research is obtained by using the WAG/Rij strain (Luijtelaar, Coenen, Kuznetcova), an excellent genetic model for human generalized absence epilepsy. The improving action of gensinosides was effectively demonstrated on the model of electrical kindling of amygdala of WAG/Rij rats with genetically determined absences, and the influences of ginsenosides on the slow wave discharges have also been being investigated. The different characteristics of a kindling process exerted in the sex-different region of the amygdala and demonstrated that the level of sex steroids and content of neurosteroids in amygdaloid tissue can modify the development of seizures. The chemical structures of ginsenosides not only have some principal differences from well-known antiepileptical drugs but the Plant Pharmacology gives us unique possibility to develop new class of antiepileptic drugs and to improve its biological activity.
Purpose: The purpose of this study was to develop evidence-based guidelines for infection control in nursing homes in Korea (ENIK). Methods: Three steps were planned for the development which were developing a draft and testing the content validity. First, the draft was based on evidence and developed through focus group interviews with nurses in nursing homes, a comprehensive review of international guidelines and literature, and systematic reviews of interventions for infection control and outbreaks in long-term care facilities. Clinical applicability was established through reviews of nursing records and job assignments in one nursing home. The final step consisted of experts evaluating the content validity. The ENIK was revised to fit Korean nursing homes. Results: The ENIK consisted of recommendations in 9 compositions and a one-page practical algorithm. The principles of infection control were presented by statements and specific strategies were recommended in resident care programs. The infection control practical algorithm was organized into 3 steps: screening at admission, prevention, and control at the early stage. The practice to control infection was composed of a 5-step process. Conclusion: The ENIK will contribute to improving the competency of infection control practice because it provides standardized practice and is tailored to Korean nursing homes.
Objectives : The aim of this study is to review the topical anesthetic effect of herbal medicine. Methods : Using domestic database(OASIS, RISS) and foreign database(Pubmed, Embase, Cochrane library, CNKI), clinical studies about efficacy of herbal medicine as topical anesthetics were searched. Only randomized controlled trials(RCT) were selected and analyzed. Results : Total 647 studies were searched. After screening process, 5 articles were selected. 2 were from Chinese database and 3 were from English database. Articles were about clinical use of herbal medicine as topical anesthetics and compare their efficacy with conventional topical anesthetics. Out of 5 articles, 4 articles used pain rating scales recorded by subjects as outcome measurement. All studies showed lower pain score in herbal medicine using groups. 4 studies showed that there was no statistically significant difference in pain score between herbal medicine using group and conventional topical anesthetics using group. Conclusions : The results suggest that topical anesthetics using herbal medicine is as effective as or maybe even more effective than conventional topical anesthetics. However, experimental design and composition of intervention were all different, which provides inaccuracy when comparing the experimental results. Also, sample sizes were small and the number of RCTs were insufficient to identify full list of herbs that have potentials to be used topical anesthetics. Further studies need to be conducted to find out potentials of herbal medicine as topical anesthetics.
Leachate from municipal solid waste (MSW) landfill, effluent from leachate treatment plant, and ground water sample from a monitoring well near landfill site were tested for an acute toxicity. Microtox toxicity test was used for testing the acute toxicity of leachate and other samples. EC$_{50}$ values which a concentration of pollutant for reducing 50% light output from luminescent bacteria, Photobacterium phosphoreum were determined to assess the toxicity of pollutants as well as the relative toxicity. In addition, characteristics of leachate were studied and compared to those of phenol and pentachlorophenol (PCP) which are typical aquatic toxic pollutants. For leachate, EC$_{50}$ for 30 min incubation was 10.8%, while for phenol and PCP, 46 ppm and 1.2 ppm, respectively. the relative toxicity of treated leachate by in situ aeration with activated sludge was reduced to more than 75% of toxicity of the untreated leachate. Microtox toxicity test was failed to figure out EC$_{50}$ values for groundwater from a monitoring well since the relative toxicity of the unconcentrated sample was too low to estimate EC$_{50}$. Addition of activated carbon to leachate was reduced the relative toxicity. The reduction Pattern of the relative toxicity of leachate by mechanical aeration was similar to that of PCP, but different from that of phenol. These findings suggest that the toxicity of leachate may come from PCP-like toxic compounds rather than phenol-like one. In conclusion, the process of aeration with activated sludge might be very important to reduce the environmental toxicity of leachate. And Microtox test could be a reasonable bioassay for screening and monitoring the environmental toxicity of leachate from municipal solid waste landfill as well as for determining the reduction efficiency of the leachate toxicity by various treatment processes in leachate treatment plant.
This study set out to improve the physical and pharmaceutical characteristics of the present formulation using an appropriate experimental design. The work described here concerns the formulation of the dispersible tablet applying direct compression method containing roxithromycin in the form of coated granules. In this study $2^3$ factorial design was used as screening test model and Central Composite Design (CCC) associated with response surface methodology was used as optimization study model to develop and to optimize the proper formulation of roxithromycin dispersible tablet. The three independent variables investigated were functional excipients like binder (X1), disintegrant (X2) and lubricant (X3). The effects of these variables were investigated on the following responses: hardness (Y1), friability (Y2) and disintegration time (Y3) of tablet. Three replicates at the center levels of the each design were used to independently calculate the experimental error and to detect any curvature in the response surface. This enabled the best formulations to be selected objectively. The effect order of each term to all response variable was X3> X2> Xl> X1*X2> X2*X2> X2*X3> X3*X3> Xl*X3> Xl*Xl and model equations on each response variables were generated. Optimized compositions of formula were accordingly computed using those model equations and confirmed by following demonstration study. As a result, this study has demonstrated the efficiency and effectiveness of using a systematic formulation optimization process to develop the tablet formulation of roxithromycin dispersible tablet with limited experiment.
Objectives : This study aimed to investigate the effects of Angelicae sinensis Radix on longitudinal bone growth rate in rats. We have screened traditional medicinal herbs to develop the longitudinal bone growth stimulator by well-established rat model. A. sinensis was identified as one of the effective herbs in the screening process. Methods : Adolescent female rats were administered A. sinensis at doses of 30 mg/kg and 300 mg/kg for 10 consecutive days. To observe the rate of longitudinal bone growth, tetracycline was injected intraperitoneally on day 8 to stain a fluorescent band on the anew formed bone. To elucidate the mode of action, we observed insulin-like growth factor-1 (IGF-1) and bone morphogenetic protein-2 (BMP-2) expression after A. sinensis administration in growth plate. Results : In the 300 mg/kg A. sinensis group, the length between the proximal endpoint of the tetracycline label and the division line between growth plate and bone was significantly increased compared with vehicle-treated control group. Height of the proximal tibial growth plate was higher in the A. sinensis group compared with control group. A. sinensis also upregulated the expressions of IGF-1 and BMP-2 in the proliferative zone and hypertrophic zone of the proximal tibial growth plate. Conclusions : A. sinensis increases longitudinal bone growth rate in rats. According to immunohistochemistry, A. sinensis increases local IGF-1 and BMP-2 expressions in the growth plate which can be considered as direct stimulation of GH on the local growth plate.
