Purpose: The aim of this study was to retrospectively evaluate the clinical and radiological results of the first metatarsophalangeal joint arthrodesis with two crossed screws fixation. Materials and Methods: We treated 23 patients (24 cases) with arthrodesis of the first metatarsophalangeal joint using two crossed screws fixation between December 2000 and May 2005. There were 3 male patients and 20 female patients. Ages ranged from 28 to 74 years (mean, 50 years). Follow-up ranged from 4.1 to 8.2 years (mean, 6.5 years). The American Orthopaedic Foot and Ankle Society (AOFAS) score and their satisfaction was evaluated clinically, foot anteroposterior and lateral radiograph, radiologically. Results: Of the 24 cases, 6 had surgery for dorsal plate and screws fixation because of failure to acquire firm fixation with two crossed screws fixation. All 6 cases acquired bony union. Fusion of the hallux first metatarsophalangeal joint occurred in 16/18 cases (89%). Nonunion occurred in 2 cases (11%) and was asymptomatic. At last follow-up, hallux valgus angle ranged from 11 to 25 degrees(mean, 17.7 degrees), dorsiflexion ranged from 15 to 25 degrees (mean, 22 degrees).The mean preoperative AOFAS score of 37 points(range, 28 to 45 points) improved to a mean of 77 points (range, 65~90 points) postoperatively. The result of the procedure as rated subjectively by the patient was excellent for 5 cases, good for 11 cases and fair for 2. Conclusion: Comparatively, the arthrodesis of the first metatarsophalangeal joint with crossed screws fixation showed a satisfactory clinical results, we thought that require technical attention for firm fixation in operation.
Objective : To evaluate the surgical outcomes of ventral interbody grafting and anterior or posterior spinal instrumentation for the treatment of advanced spondylodiscitis with patients who had failed medical management. Methods : A total of 28 patients were evaluated for associated medical illness, detected pathogen, level of involved spine, and perioperative complications. Radiological evaluation including the rate of bony union, segmental Cobb angle, graft- and instrumentation-related complications, and clinical outcomes by mean Frankel scale and VAS score were performed. Results : There are 14 pyogenic spondylodiscitis, 6 postoperative spondylodiscitis, and 8 tuberculous spondylodiscitis. There were 21 males and 7 females. Mean age was 51 years, with a range from 18 to 77. Mean follow-up period was 10.9 months. Associated medical illnesses were 6 diabetes, 3 pulmonary tuberculosis, and 4 chronic liver diseases. Staphylococcus was the most common pathogen isolated (25%), and Mycobacterium tuberculosis was found in 18% of the patients. Operative approaches, either anterior or posterior spinal instrumentation, were done simultaneously or delayed after anterior aggressive debridement, neural decompression, and structural interbody bone grafting. All patients with neurological deficits improved after operation, except only one who died from aggravation as military tuberculosis. Mean Frankel scale was changed from $3.78{\pm}0.78$ preoperatively to $4.78{\pm}0.35$ at final follow up and mean VAS score was improved from $7.43{\pm}0.54$ to $2.07{\pm}1.12$. Solid bone fusion was obtained in all patients except only one patient who died. There was no need for prolongation of duration of antibiotics and no evidence of secondary infection owing to spinal instrumentations. Conclusion : According to these results, debridement and anterior column reconstruction with ventral interbody grafting and instrumentation is effective and safe in patients who had failed medical management and neurological deficits in advanced spondylodiscitis.
Objective : To analyze the clinical outcomes of computed tomography (CT) fluoroscopy-guided selective neve root block (SNRB) for severe arm pain caused by acute cervical disc herniation. Methods : The authors analyzed the data obtained from 25 consecutive patients who underwent CT fluoroscopy-guided SNRB for severe arm pain, i.e., a visual analogue scale (VAS) score of 8 points or more, caused by acute soft cervical disc herniation. Patients with chronic arm pain, motor weakness, and/or hard disc herniation were excluded. Results : The series comprised 19 men and 6 women whose mean age was 48.1 years (range 35-72 years). The mean symptom duration was 17.5 days (range 4-56 days) and the treated level was at C5-6 in 13 patients, C6-7 in 9, and both C5-6 and C6-7 in 3. Twenty-three patients underwent SNRB in 1 session and 2 underwent the procedure in 2 sessions. No complications related to the procedures occurred. At a mean follow-up duration of 11.5 months (range 6-22 months), the mean VAS score and NDI significantly improved from 9 and 58.2 to 3.4 and 28.1, respectively. Eighteen out of 25 patients (72%) showed successful clinical results. Seven patients (28%) did not improve after the procedure, and 5 of these 7 underwent subsequent anterior cervical discectomy and fusion. Conclusion : CT fluoroscopy-guided SNRB may play a role as a primary conservative treatment for severe arm pain caused by acute cervical disc herniation.

Objective : The purpose of this study is to evaluate the efficacy and necessity of combined anterior approach [discectomy and fusion] and posterior approach[open-door laminoplasty] in the treatment of cervical spondylotic myelopathy. Methods : The authors reviewed 14 cases in whom combined anterior and posterior approach performed for cervical myelopathy due to simultaneous anterior and posterior pathology such as huge central disc herniation with narrow spinal canal between January 2000 and December 2002. Clinical symptoms were evaluated by Japanese Orthopaedic Association [JOA] score and then the cervical curvature, change of spinal canal to vertebral body[SC/VB] ratio and canal widening were measured and compared to the clinical symptoms. Results : The mean JOA score increased from $10.4{\pm}3.1$ preoperatively to $14.8{\pm}1.2$ at the final follow up with a mean recovery rate 66.4%. In all cases, there were not neurologic deterioration. Mild postoperative complications developed in two cases. One patient had a limitation of range of neck motion and the other one showed kyphotic change. Postoperative radiography showed an improvement of body to canal ratios [average $0.70{\pm}0.08$ before surgery to $1.05{\pm}0.12$ after surgery and mainte nance or recovery of cervical lordosis. Canal widening of antero-posterior diameter and dimension after operation is 6.8mm. $116.61mm^2$. Conclusion : Combined anterior and posterior procedure could be helpful in decompression of the spinal cord and good functional recovery in spondylotic myelopathy patients with combined anterior and posterior pathology such as huge disc herniation accompanying narrow spinal canal.

The O2O store model dissolves the boundaries between online and offline platforms, providing significant convenience to customers. To effectively operate such platforms, small business owners must provide necessary information in digital format. Specifically, the process of digitizing Korean menus manually can lead to multiple issues, and the use of OCR technology often results in high error rates due to the low accuracy in recognizing Korean. In response, this paper proposes an enhanced OCR model based on the popular EasyOCR framework, aimed at improving the recognition accuracy of Korean. The proposed model integrates the structural advantages of VGG and ResNet, and incorporates an attention mechanism to significantly improve the recognition performance of Korean. Moreover, experimental results indicate that the proposed model achieved approximately a 3.5% improvement in accuracy and around a 1% improvement in both confidence score and normalized edit distance compared to EasyOCR. Therefore, this demonstrates that the proposed method effectively addresses the existing challenges.
Objective : The authors performed a retrospective study to assess the clinical and radiological outcome in symptomatic lumbar spondylolysis patients who underwent a direct pars repair surgery using two different surgical methods; pedicle screw with universal hook system (PSUH) and direct pars screw fixation (DPSF), and compared the results between two different treated groups. Methods : Forty-seven consecutive patients (PSUH; 23, DPSF; 15) with symptomatic lumbar spondylolysis who underwent a direct pars repair surgery were included. The average follow-up period was 37 months in the PSUH group, and 28 months in the DPSF group. The clinical outcome was measured using visual analogue pain scale (VAS) and Oswestry disability index (ODI). The length of operation time, the amount of blood loss, the duration of hospital stay, surgical complications, and fusion status were also assessed. Results : When compared to the DPSF group, the average preoperative VAS and ODI score of the PSUH group were less decreased at the last follow-up; (the PSUH group; back VAS : 4.9 vs. 3.0, leg VAS : 6.8 vs. 2.2, ODI : 50.6% vs. 24.6%, the DPSF group; back VAS : 5.7 vs. 1.1, leg VAS : 6.1 vs. 1.2, ODI : 57.4% vs. 18.2%). The average operation time was 174.9 minutes in the PSUH group, and 141.7 minutes in the DPSF group. The average blood loss during operation was 468.8 cc in the PSUH group, and 298.8 cc in the DPSF group. The average hospital stay after operation was 8.9 days in the PSUH group, and 7 days in the DPSF group. In the PSUH group, there was one case of a screw misplacement requiring revision surgery. In the DPSF group, one patient suffered from transient leg pain. The successful bone fusion rate was 78.3% in the PSUH group, and 93.3% in the DPSF group. Conclusion : The present study suggests that the technique using direct pars screw would be more effective than the method using pedicle screw with lamina hook system, in terms of decreased operation time, amount of blood loss, hospital stay, and increased fusion success rate, as well as better clinical outcome.

Building change monitoring based on building detection is one of the most important fields in terms of monitoring artificial structures using high-resolution multi-temporal images such as CAS500-1 and 2, which are scheduled to be launched. However, not only the various shapes and sizes of buildings located on the surface of the Earth, but also the shadows or trees around them make it difficult to detect the buildings accurately. Also, a large number of misdetection are caused by relief displacement according to the azimuth and elevation angles of the platform. In this study, object-based building detection was performed using the azimuth angle of the Sun and the corresponding main direction of shadows to improve the results of building change detection. After that, the platform's azimuth and elevation angles were used to detect changed buildings. The object-based segmentation was performed on a high-resolution imagery, and then shadow objects were classified through the shadow intensity, and feature information such as rectangular fit, Gray-Level Co-occurrence Matrix (GLCM) homogeneity and area of each object were calculated for building candidate detection. Then, the final buildings were detected using the direction and distance relationship between the center of building candidate object and its shadow according to the azimuth angle of the Sun. A total of three methods were proposed for the building change detection between building objects detected in each image: simple overlay between objects, comparison of the object sizes according to the elevation angle of the platform, and consideration of direction between objects according to the azimuth angle of the platform. In this study, residential area was selected as study area using high-resolution imagery acquired from KOMPSAT-3 and Unmanned Aerial Vehicle (UAV). Experimental results have shown that F1-scores of building detection results detected using feature information were 0.488 and 0.696 respectively in KOMPSAT-3 image and UAV image, whereas F1-scores of building detection results considering shadows were 0.876 and 0.867, respectively, indicating that the accuracy of building detection method considering shadows is higher. Also among the three proposed building change detection methods, the F1-score of the consideration of direction between objects according to the azimuth angles was the highest at 0.891.

Face recognition refers to the process of identifying individuals based on their facial features. It has recently become one of the most popular research areas in the fields of computer vision, machine learning, and pattern recognition because it spans numerous consumer applications, such as access control, surveillance, security, credit-card verification, and criminal identification. However, illumination variation on face generally cause performance degradation of face recognition systems under practical environments. Thus, this paper proposes an novel face recognition system using a fusion approach based on local binary pattern and two-dimensional principal component analysis. To minimize illumination effects, the face image undergoes the local binary pattern operation, and the resultant image are divided into two sub-images. Then, two-dimensional principal component analysis algorithm is separately applied to each sub-images. The individual scores obtained from two sub-images are integrated using a weighted-summation rule, and the fused-score is utilized to classify the unknown user. The performance evaluation of the proposed system was performed using the Yale B database and CMU-PIE database, and the proposed method shows the better recognition results in comparison with existing face recognition techniques.
Molecular docking is a critical event which mostly forms Van der waals complex in molecular recognition. Since the majority of developed drugs are small molecules, docking them into proteins has been a prime concern in drug discovery community. Since the binding pose space is too vast to cover completely, many search algorithms such as genetic algorithm, Monte Carlo, simulated annealing, distance geometry have been developed. Proper evaluation of the quality of binding is an essential problem. Scoring functions derived from force fields handle the ligand binding prediction with the use of potential energies and sometimes in combination with solvation and entropy contributions. Knowledge-based scoring functions are based on atom pair potentials derived from structural databases. Forces and potentials are collected from known protein-ligand complexes to get a score for their binding affinities (e.g. PME). Empirical scoring functions are derived from training sets of protein-ligand complexes with determined affinity data. Because non of any single scoring function performs generally better than others, some other approaches have been tried. Although numerous scoring functions have been developed to locate the correct binding poses, it still remains a major hurdle to derive an accurate scoring function for general targets. Recently, consensus scoring functions and target specific scoring functions have been studied to overcome the current limitations.
To compare heart rate, $O_2$ uptake, $Vo_2$ ($O_2$ consumption), blood pressure (systolic, diastolic), reaction time, stability, flicker fusion value during 4 load levels with Rs (self-paced respiration) and Rp (paced respiration), 4 subjects participated in this experiment 1 hour/day, 6 days/week for 9 weeks. The cycle of Rp is 6 sec. (inspiration: 3 sec. & expiration: 3 sec.) Implications of the results are discussed in terms of the change in the physiological responses and human performance by the respiratory pattern. The results are as follows, 1. The changing magnitude of heart rate with Rp was larger than with Rs and the variance during load level 4 was significant. 2. The $Vo_2$ with Rp was smaller than with Rs and maximal $O_2$ uptake given load levels with Rp occurred and for two subjects, it significantly moved from low load level to high load level. 3. The changing magnitude of blood pressure was not consistent but the systolic pressure with Rp was smaller at rest than with Rs. 4. The score of reaction time test and stability test with Rp was better than with Rs.
