수지뜸이 여성의 월경통 완화에 미치는 영향 (The Effect of Hand Moxibustion Therapy on Pain during Menstruation)

  • Purpose: The purpose of this study was to identify the effect of hand moxibustion for reduction of pain during menstruation. Method: Fifteen women were selected as subjects and data collection was conducted from September to December 2003. The Visual Analogue Scale (VAS) and Digital Infrared Thermotraphic Image (DITI) were used to measure the degree of pain and body heat. Result: The results showed that there was a significant reduction of pain during menstruation when hand moxibustion was performed. Also, there were significant differences in the change of body heat. These were left hands (Z=-2.805, p=.005), right hands (Z=-2.805, p=.005) and lower abdomen (Z=-2.803, p=.005). Conclusion: It was found that hand moxibustion decreased pain during menstruation and increased body heat. Therefore, hand moxibustion may be a good method for intervention to decrease pain during menstruation.

숙련성에 따른 중환자실 간호사들의 업무수행 유형 (Characteristics of Nursing Performance based on the Expertise among ICU Nurses)

  • Purpose: The purpose of this research was to describe how nurses in intensive care units (ICU) work. Method: A total of 18 ICU nurses participated in the research. The data was collected through individual in-depth interviews and analyzed by grounded theory method using NUDIST 4.0 software program. Results: Three different patterns regarding nursing performance among ICU nurses were identified. These are 1) nursing performance of nurses who perform excellently, 2) nursing performance of nurses who do not perform well because of their lack of experience, and 3) nursing performance of nurses who do not perform well in spite of their good years of experience. These three different nursing performances were described in terms of seven different categories; 1) assessing and monitoring nursing problems, 2) clinical decision making, 3) interpersonal relationships, 4) holistic care, 5) technical skills, 6) problem solving, 7) working independently and creatively. This study also identified two intervening factors that influenced the advancement of their expertise. Conclusion: The results of this study might help nurse managers in developing continuing educational programs for inexperienced nurses or those nurses not performing well to become experts by a deeper understanding of the nature of nursing performance and the factors that influence nursing performance in ICU settings.

  • Purpose: Deep Vein Thrombosis (DVT) is the cause of fatal diseases such as pulmonary embolism, due to a prolonged immobility, surgery, paralysis, and injuries. This study aimed to develop an evidence-based practice guideline for DVT prevention and apply it to patients with surgery in diverse nursing sites in South Korea. Methods: A 24-staged processes of adaptation was carried out on the basis of "adaptation of nursing practical guidelines" developed by Gu et al., in 2012. Results: Developed nursing guidelines of DVT prevention are composed of 79 recommendations in 8 domains. The extent and ratings of each recommendation with its evidence were addressed along with the background information. Conclusion: The developed DVT prevention guideline is necessary to be added to the evidence-based practice guidelines for the fundamentals of nursing practice. The developed guideline is needed to be disseminated to diverse nursing clinical settings in order to prevent DVT and enhance the quality nursing care.

국내 시설 거주 미혼모의 양육역량 향상을 위한 중재의 통합적 고찰 (An Integrative Review of Interventions to Improve Parenting Competencies of Unmarried Mothers Living in Residential Facilities in Korea)

  • Purpose: To understand the core elements and the effects of interventions on the improvement of parenting capabilities of unmarried mothers living in residential facilities, this study reviewed the literatures related to this concept. Methods: Five electronic databases (KISS, KMbase, KoreaMed, NDSL, and RISS) were searched, and eight studies were ultimately selected for the integrative review. Results: The interventions were categorized into two types: psychological intervention and sociocultural intervention. The core elements of the psychological interventions included cognitive, emotional, behavioral, and relational contents. Sociocultural aspects included raising personal skills for finding social support resources. Various studies showed the effects of intervention on efficacy, attachment, or parenting behavior. Conclusion: Based on the results of this study, health promotion programs to improve maternal parenting competency for unmarried mothers living in residential facilities need to be developed and applied.

한국판 청소년용 음식중독도구의 타당도와 신뢰도 (Validity and Reliability of a Korean Version of Yale Food Addiction Scale for Children (YFAS-C))

  • Purpose: This study evaluated the psychometric properties of the Korean version of Yale Food Addiction Scale for Children (YFAS-C). Methods: Participants were 419 young adolescent students (11~15 years old). The content validity of the expert group was calculated as the content validity index (CVI) after the translation and reverse translation process of the 25 items of the YFAS-C. The multitrait-multimethod matrix (MTMM) method was used to verify the construct validity; the generalized linear model (GLM) was used to evaluate the concurrent and incremental validity. Reliability was calculated as Kuder-Richardson-20 (KR-20) and Spearman-Brown coefficients. Results: The CVI of the 25 items was greater than the item-level CVI .80 and the scale-level CVI .90. The Korean version of YFAS-C had verified convergent validity in emotional eating and external eating and discriminant validity in restrained eating. In addition, it had verified concurrent validity in emotional eating and external eating. Finally the incremental validity of the Korean version of YFAS-C was statistically significant on BMI. Reliability was KR-20 ${\alpha}=.69$ and the Spearman-Brown coefficient was .64. Conclusion: The Korean version of YFAS-C is a valid and reliable scale for measuring the severity of food addiction; it can be a useful scale for preventing obesity by predicting food addiction early.

간호대학생이 인지하는 유전학에 대한 지식과 태도 및 유전간호 역량 (Undergraduate Nursing Students' Perceived Knowledge and Attitudes Toward Genetics and Nursing Competencies for Genetics)

  • Purpose: The purpose of this descriptive, cross-sectional study was to examine Korean undergraduate nursing students' perceived knowledge and attitudes toward genetics. Nursing competencies for genetics among nursing students were also assessed. Methods: Two hundred eighty-six Korean junior and senior undergraduate nursing students who went through their clinical practicum completed a packet of self-report questionnaires exploring opinions about the integration of genetics into the nursing curriculum, as well as their perceived knowledge, attitudes, and nursing competencies toward genetics. Collected data were analyzed using descriptive statistics, t-test, one-way ANOVA. Results: Overall, Korean undergraduate nursing students reported limited genetic knowledge, but most of them were comfortable with drawing pedigree. In general, nursing students were competent in providing genetic nursing. Their genetic nursing competencies were significantly different depending on the level of general understanding of genetics. There was no significant difference between juniors and seniors regarding knowledge, attitudes, and competencies toward genetics. More than 60% of nursing students reported the need for genetic nursing, and the majority considered lectures to be the most effective method to learn genetic knowledge. Conclusion: It is imperative to integrate genetics into nursing curriculum in order to improve nursing students' genetic nursing competencies as well as genetic knowledge.

A Comparison of NANDA and CCC used in Hospital-based Home Health Care

  • Background: Recent changes in the medical environment have increased the need for the home health care nursing in Korea. Even though the number of home health care patients is increasing, the major nursing problems have not been identified due to lack of a standardized nursing diagnosis. Aim: An investigative study was conducted to determine the frequency and appropriateness of nursing problems in hospital-based home health care patients in Korea using two internationally standardized nursing diagnosis classification systems. Methods: Nursing records of 249 hospital-based home health care patients were reviewed and nursing problems were identified using the North American Nursing Diagnosis Association Nursing Diagnosis Taxonomy I (NANDA) and the Clinical Care Classification of Nursing Diagnoses (CCC). Findings: Out of 463 nursing problems. 403 nursing problems were described using the NANDA whereas 427 nursing problems were described using the CCC. Nursing diagnoses not captured by the NANDA classification include nausea/vomiting, anorexia, risk for nutrition deficit, decreased blood pressure, dying process, blood sugar impairment. infection unspecified, and disuse syndrome. Nursing diagnoses not captured by the CCC include nausea/vomiting and anorexia. Conclusions: In describing nursing problems of home health care patients, it was found that the CCC was able to represent more diagnoses than the NANDA.

간호대학 실습생들의 성격/행동유형과 임상실습 스트레스 양상 및 대처 방식간의 관계 (Levels of Clinical Practice Stress and Coping Strategies according to Nursing Students' Personality/Behavior Types)

  • Purpose: This study was performed to identify levels of clinical practice stress and coping strategies according to the personality/behavior types of nursing students. Methods: The subjects of this study were 143 third- and fourth-grade nursing students in 14 nursing colleges who had experienced clinical practice. The questionnaire consisted of instruments to identify Dominance, Influence, Steadiness, and Conscientiousness (DISC) personality/behavior types, clinical stress, and stress coping strategies. The data were analyzed using Cronbach's ${\alpha}$, descriptive statistics, and ANOVA, using the SPSS 24.0 program. Results: The DISC pattern showed high priority in Steadiness (48.3%), followed by Conscientiousness (21.0%), Influence (16.1%), and Dominance (14.7%), in that order. The steadiness type showed the lowest level of stress among four patterns. Conclusion: This study found that the personality/behavior types of nursing students differ. Additionally, different personality/behavior types can be attributed to stress levels and stress coping behaviors. Based on these results, different personality and behavior types of nursing students should be considered to manage their stress levels related to the clinical practicum experience.

만성 심부전 환자의 약물복용 이행 영향요인 (Factors Affecting Medication Adherence in Patients with Chronic Heart Failure)

  • Purpose: The purpose of this study was to explore socio-demographic, clinical and psychological factors associated with medication adherence among patients with chronic heart failure (CHF). Methods: In a cross-sectional survey, 133 patients with CHF who had visited a clinic located in Seoul participated. Medication adherence (MA) was measured using a single item visual analogue rating scale (VAS). Barriers and attitudes toward adherence, and depression were measured using a structured questionnaire. Medical records were reviewed to identify their clinical characteristics. Multivariate logistic regression was performed identify factors affecting medication adherence. Results: About one third of participants (31.1%) reported their MA as 100%. Overall attitudes on MA were highly positive and 49 (36.8%) of participants were depressive. MA was associated with significantly barriers (p<.001), attitudes (p<.001), age (p<.001), and duration of disease (p<.001). The high adherent group had significantly less barriers than other groups (OR=.389, p=0.02). Conclusion: This study indicates that barriers and attitudes toward MA were the most important factors affecting their adherence to HF medication. To improve MA for patient with CHF, the nursing strategies which can lead poor adherent patients to their optimal level should be developed.

심폐소생술금지 결정에 대한 간호활동의 변화 (Changes of Nursing Activities after DNR(do-not-resuscitate) Decision)

  • Purpose: The purpose of this study was to examine hospital nurses' nursing activities changes after DNR(do-not-resuscitate) decision. Methods: The subjects were 120 registered nurses working in two university hospitals. The data were collected from September 1 to September 15, 2015 using self-report questionnaires. The data were analyzed using SPSS 20.0 program. Results: DNR awareness of the educational needs was very high(91.7%). But DNR educational experience was low(59.2%). Nursing activity change was classified as the physical, emotional, spiritual, and social areas. There were significant difference among religion(F=3.459, p=.010), working unit(F=3.410, p=.036), DNR awareness of the educational needs(t=5.048, p=.027), DNR educational experience(t=-2.816, p=.006) and nursing activities changes. Conclusion: Nurses are needed DNR educational programs to take care of DNR patients. And the criteria for nursing activities related to DNR is required.
