A Case Report of Improvement of Docetaxel-induced Fluid Retention in a Patient with Advanced Gastric Cancer using Herbal Decoction, Modified Bangkihwangki-tang

  • Objectives: A 54-year-old advanced gastric cancer patient developed severe fluid retention after docetaxel chemotherapy, at cumulative dose of $930mg/m^2$, with no improvement using diuretics or prophylactic glucocorticoids. Consequently, he changed chemotherapy regimen from docetaxel to paclitaxel. After he treated with herbal decoction Bangkihwangki-tang (BHT), which is widely used to treat several symptoms including edema in traditional Korean medicine, docetaxel-induced fluid retention (DIFR) was significantly improved along with associated symptoms, such as peripheral edema, pleural and pericardial effusion, pain, scleroderma, and hypoesthesia. As the symptoms were relieved, his performance status and ambulatory ability were improved. During herbal treatment, he continued chemotherapy without any problems and didn't show any adverse events related to herbal medicine. Although there is possibility of natural improvement progress after withdrawal of docetaxel, he showed shorter recovery period and significant improvement despite of severity of initial symptoms.

Predictors of Cognitive Improvement during 12 Weeks of Antidepressant Treatment in Patients with Major Depressive Disorder

  • Objective: Cognitive disturbance is one of the major symptoms of depression and may be improved by treatment with antidepressants. This study aimed to investigate the predictors of cognitive improvement in patients with major depressive disorder (MDD) who were taking antidepressants. Methods: This study included 86 patients with MDD who completed 12 weeks of antidepressant monotherapy. Cognitive symptoms were assessed using the Perceived Deficits Questionnaire-Korean version (PDQ-K), which addresses four domains of cognitive functioning (attention/concentration, retrospective memory, prospective memory, and organization/planning) and was administered at study entry and at the 12-week end point. A variety of demographic, clinical, and treatment-related variables were evaluated as predictors of changes in total and domain scores. Results: All PDQ-K domains showed significant improvement after 12 weeks of antidepressant treatment. More severe initial depressive symptoms, fewer sick-leave days at study entry, and reduced use of concomitant anxiolytics/hypnotics during treatment were significantly associated with greater cognitive improvement. Conclusion: Cognitive symptoms are more responsive to antidepressant treatment in patients with severe MDD. Reduced use of anxiolytics and hypnotics could improve the cognitive functioning of patients with MDD taking antidepressants.

  • The 7th National Curriculum requires the part of a school library for supporting porformance evaluation, optional actives, and extracurricular activities. The school library which is very important part of school education needs support of necessary resources and adequate facilities for performing its functions effectively and efficiently. This reserch is based on the school library for present education to need. And, the objective of this reserch is to grasp the present condition of school library and to show improvement device for planning efficiently the time when school library is designed in future.

The Improvement and Completion of Outcome index: A new assessment system for quality of orthodontic treatment

  • Objective: Given the considerable disagreement between the Peer Assessment Rating (PAR) index and the American Board of Orthodontics Cast-Radiograph Evaluation, we aimed to develop a novel assessment system-the Improvement and Completion of Outcome (ICO) index-to evaluate the outcome of orthodontic treatment. Methods: Sixteen criteria from 4 major categories were established to represent the pretreatment malocclusion status, as well as the degree of improvement and level of completion of outcome during/after treatment: dental relationship (arch length discrepancy, irregularity, U1-SN, and IMPA); anteroposterior relationship (overjet, right and left molar position, ANB); vertical relationship (anterior overbite, anterior open-bite, lateral open-bite, SN-MP); and transverse relationship (dental midline discrepancy, chin point deviation, posterior cross-bite, occlusal plane cant). The score for each criterion was defined from 0 or -1 (worst) to 5 (ideal value or normal occlusion) in gradations of 1. The sum of the scores in each category indicates the area and extent of the problems. Improvement and completion percentages were estimated based on the pre- and post-treatment total scores and the maximum total score. If the completion percentage exceeded 80%, treatment outcome was considered successful. Results: Two cases, Class I malocclusion and skeletal Class III malocclusion, are presented to represent the assessment procedure using the ICO index. The difference in the level of improvement and completion of treatment outcome can be clearly explained by using 2 percentage values. Conclusions: Thus, the ICO index enables the evaluation of the quality of orthodontic treatment objectively and consecutively throughout the entire treatment process.

Effects of Forensic Science Program on Scientific Creative Problem-Solving Abilities of Gifted Students in Elementary School (과학수사 프로그램이 초등 영재의 과학 창의적 문제해결력에 미치는 효과)

  • The purpose of this study was to develop forensic science program for the improvement of scientific creative problem-solving abilities in gifted elementary-school students. A program that consists of six sessions (18 hours) is developed in accordance with the CPS model, which has been already proven effective for the improvement of creative problem-solving abilities. This program was applied to sixth-grade 18 gifted students in an elementary school in Gyeonggi province. Examinations of scientific creative problem-solving abilities were performed before and after applying the program in order to determine its effect on gifted elementary students. A qualitative analysis of students' activity sheets, peer assessment and teacher's class journal was made in order to examine the process of improvement of students' scientific creative problem-solving abilities. The results of this study are as follows: First, forensic science program to enhance the scientific creative problem-solving abilities of gifted students was developed. Second, forensic science program is significantly effective in the improvement of scientific creative problem-solving abilities of gifted children of elementary school (p<.05). Third, in early stage of the class, a student, who showed the highest range of change in pre and post tests, revealed the trend of responding in a short answer type. In the late stage of the class, he revealed the capability of producing various creative ideas promptly. On the other hand, students belonging to the upper group of both pre and post test revealed the improvement of divergent thinking skills such as fluency, flexibility, and originality. Fourth, after class, the students responded that the forensic science program developed in this study intrigued the interests and curiosities, and helped them break away from fixed ideas.

Korean Red Ginseng Powder in the Treatment of Melasma: An Uncontrolled Observational Study

  • Exposure to the sun, ultraviolet radiation, and oxidative stress are the chief etiologic factors responsible for melasma. The ingredients of Korean red ginseng powder, which include ginsenoside and phenolic compounds, have antioxidative effects and reduce ultraviolet B-induced pigmentation. This study was designed to evaluate the effectiveness and safety of Korean red ginseng powder in patients with melasma. In 25 female patients, 3 g of Korean red ginseng powder was orally administered for a 24 week period. The level of pigmentation and erythema were determined and clinical improvement was evaluated by the melasma area and severity index (MASI), melasma quality of life scale (MELASQoL), and patient- and investigator-rated global improvement scale. After 24 weeks, the MASI score decreased from 8.8 to 5.6, and MELASQoL showed improvement in 91% of patients (p<0.05). The mean level of pigmentation decreased from 184.3 to 159.7 and erythema levels decreased from 253.6 to 216.4 (p<0.05). Additionally, 74% of the patients showed some improvement in both patient- and investigator-rated global improvement scales at week 24. Korean red ginseng powder was well tolerated by most of the patients. In conclusion, Korean red ginseng powder showed good tolerability and beneficial effects in patients with melasma. The use of Korean red ginseng would be counted as a useful adjunctive therapy for patients with melasma.

  • As a part of school safety network construction project, a system of school marshal in Seoul-si and a project of academy safety guard in other regions are on the way of improvement. But, the crimes in and around the school, occurring frequently, are not solved easily, as this study points out. The effective improvement way of school safety network construction project is as followings. First, to enforce assistant-class school safety teacher system in order to nurture and educate experts in school safety, as a systematic support for strengthening occupational professionalism. Second, to enforce school safety network construction projects for construct circumstances through legal fix of school safety CPTED to make students free from crimes. This study looks into the real status and problem of school safety networks, and suggests system of school safety teacher and a legal enactment of CPTED, as systematic and a planed school safety countermeasure, not temporary.

  • Personnel engaged in the medical field have implemented continual improvement by team activities in an effort to construct a system that reduces the risks involved in medical care. Knowledge in total quality management (TQM), especially statistical quality control (SQC) developed for industry, seems to be applicable to medical care. This paper describes the application of SQC to continual improvement in medical care.
