산업재해 입원환자를 위한 사례관리실천모형의 효과성에 관한 연구 (A Study on Efficiency of Case Management Practical Model for Industrial Injury Inpatients)

  • This study is to observe the effectiveness of the applied model and to present the improvement plan and directions for development for the case management practical model suitable for the actual condition of Korea Labor Welfare Co. and needs of the industrial injury patients. The concrete purpose of this study is: First, observe the difference of stressor experience and experience degree between the experimental group and the comparative group. Second, observe the difference of stress of the experimental group and the comparative group. Third, find out how the stress affects the support degree and satisfaction degree. Fourth, present the improvement plan of case management model, which can promote the psychosocial rehabilitation of the industrial injury patient based on the research results. The outline of the main research results identified in this study is as follows. The stressors the industrial injury patients perceived are health problems, family matters, the problems concerning hospital recuperation (hospital staff and environmental problems), economical problems, problems of coming back to society, problems with companies, problems with Korea labour Welfare Co. and other problems. And the experience of stressor was prominently lower in experimental group than comparative group in the whole problem, health problem, problems with Korea Labour-Welfare Co. and other problems. The stressor experience degree was conspicuously lower in experimental group in the whole problem experience degree, health problem experience degree, problem with Korea Labour Co. experience degree and other problem experience degree. Besides whether or not the case management is applied is having a prominent affect on the primary factor affecting the stressor experience degree, therefore the patients applied with case management has less stressor than the patient who didn't. The difference of degree of tension experienced by the stressor in both groups, the degree of stress, was not conspicuous in statistics so it shows that the application of case management in this research has not affected the degree of tension. The field which had been the most help was emotional support in help level the experimental group perceived through applying case management about industrial accident patients and recuperation, compensation problem, medical treatment problem, family matters has been helpful in this order. The help level of the whole problem was in higher level than the middle value. The stress factor which affects the case management problem settlement is the whole body of stress. The satisfaction level of help through applying case management was highest in emotional support and family matters, recuperation problem, company problem, compensation problem, and medical treatment problem was the next highest. The satisfaction level of the whole problem was higher than the middle value. The stress factor affecting the satisfaction level of help is the whole body of stress. Therefore to reduce the stress level of industrial accident patients and for them to come back to local societies, we need to reinforce the continuance and responsibility of case management model, increase staff, reinforce the role of counsel and medical treatment, intervene in the patient's plan of leaving the hospital, develop social support system and the need to establish After Care Center.

농촌지역 보건소 한방진료실 확대설치 요구도 및 관련요인 - 일개 군 보건소 한방진료실 내소자를 대상으로 - (Need Assessment for Enlargement of Oriental Medical Care Service Room in Rural Community Health Center)

  • 본 연구는 2002년 5월부터 9월까지 5개월간 경상남도 일개 군 보건소 한방진료실에 내소한 재진 환자를 대상으로 한방진료실 직원이 직접 설문지를 통한 일대일 면접 조사를 실시하였고, 면접 조사를 꺼리는 대상자에 대해서는 자기기입식 방법으로 자료를 수집하여 한방의료 이용 실태와 주민들의 이해도와 만족도를 파악하고 한방진료실을 한방의료서비스 부족지역으로 확대 설치하는 것에 대한 의견을 수렴하는데 그 목적을 두었다. 총 조사 대상자는 163명이었으며 본 연구의 결과를 요약하면 다음과 같다. 첫째, 보건소 한방진료실을 이용하는 주된 대상은 61세 이상이 전체의 65.0%로 과반수 이상을 차지하고 있으며 대상자의 13.5%만이 자신의 건강상태가 좋다고 인지하고 있었다. 둘째, 보건소 한방진료실의 치료법에 대한 사전인식 분석 결과 보건소 한방진료실에서 시행하는 치료법에 대해 올바르게 인식하는 비율은 54.0%로 나타났으며, 특히 침 시술의 경우에는 조사 대상자 모두가 보건소 한방진료실에서 시술한다는 것으로 인식하고 있었다. 셋째, 보건소 한방진료실 이용행태에 관한 분석 결과로는 한의학적인 치료를 받고 싶어서 한방진료실을 찾은 경우가 66.9%로 가장 많았으며 집에서 보건소까지의 소요시간이 1시간에서 2시간 사이까지 많은 시간이 걸린다고 응답한 사람도 9.2%나 되었다. 넷째, 보건소 한방진료실 이용자의 한방보건의료에 대한 요구도를 조사한 결과 이동진료, 금연프로그램, 거동불능자 방문 프로그램 순으로 한방지역보건사업에 대한 요구률이 높았으며 다른 면지역으로의 한방진료실 확대 설치에 대한 요구도 조사에서는 조사 대상자의 73.7%가 한방진료실 확대 설치가 필요하다고 응답하였다. 다섯째, 한방진료실 확대설치 요구도는 학력이 높은 집단, 집과 보건소와의 거리가 멀다고 생각하는 대상자, 치료비에 대한 만족도가 높은 집단에서 높은 것으로 나타났다. 이상의 결과를 볼 때 농촌지역의 한방진료서비스에 대한 이용도가 높은 것으로 나타났으며 도시지역에 비하여 한방의료자원의 분포가 낙후되어 있는 농촌지역 실정으로 보건소에 한방진료실을 설치하여 시행한 한방진료서비스에 대한 이용자들의 만족도 또한 매우 높다는 것을 파악할 수 있었다. 이러한 주민들의 높은 한방진료서비스에 대한 요구에 부응하기 위해 한방지역보건사업을 합리적으로 실시하고 한방진료실이 설치되어 있지 않은 면지역 보건지소로의 적극적인 확대설치가 적극 검토되어야 할 것이다.

환자안전에 대한 환자의 이해: 초점집단토의 결과를 중심으로 (Patient Understanding of Patient Safety: Based on Results from Focus Group Discussion)

  • Purpose: To examine the degree of recognition regarding the concept of patient safety, as perceived by the patient, using a focus group discussion. Methods: A focus group discussion was conducted with a patient group comprising seven patients. Results: When the participants heard the term "patient safety" they seemed to understand it to be related to the hospital environment or satisfaction with the overall hospitalization experience. The participants emphasized communication between the medical staff and the patients in relation to the explanation of treatments, as well as the provision of information regarding prevention, experience, and the treatment of incidents with patient safety. They agreed on the need for indicators reported by patients. However, they emphasized that additional items and a questionnaire method that considers the patients' point of view are needed. Conclusion: It is necessary to establish and implement various strategies that can raise the awareness of patient safety using patient safety indicators and increase participation in patient safety activities.

Investigating Sexual Function and Affecting Factors in Women with Breast Cancer in Iran

  • Background: Since the breast is strongly relevant to sexual desire, and physical and sexual attractiveness, the high prevalence of breast cancer (BC) in Iran and long-term survival of patients experiencing side effects means that measures to identify associated sexual problems are necessary. Therefore, this study was conducted to assess sexual function and affecting factors in women with BC. Materials and Methods: This cross-sectional study was performed on 94 women with BC, referred to Imam Reza (AS) Hospital, Mashhad, Iran, in 2014. The data were collected through demographic and clinical questionnaires and also a sexual function questionnaire and analyzed using SPSS version 16. Results: The total score of women's sexual function was about $24.3{\pm}4.41$. Of the total, 63 (71.3%) reported sexual dysfunction, for example reduced satisfaction or more pain. Age was the only significantly related factor. Conclusions: Breast cancer can adversely affect women's sexual function and decrease quality of life. Thus, taking measures to overcome women's sexual problems are necessary.

동북아시아 4개국의 양.한방 의료협진체계 비교 (A comparative Study on the Combined Oriental and Western Medicine(COWM) in Four Northeast Countries)

  • Since 1990s, the use of Complementary and Alternative Medicine(CAM) has been rising rapidly all of the world. In 1983, WHO recommended that the traditional medicine actively be utilized. At the end of 20th century, as chronic and intractable diseases increased in western countries, traditional medicine has attracted considerable attention. COWM shows possibilities of new approaches for these intractable diseases. Thus, we try to show our proper approach of COWM through the international comparative study. In order to fulfill the objectives, we applied the following methodology: 1) Literature review on previous study, 2) Local survey using self-administered questionnaire, and 3) FGI(Focus Group Interview) with local experts. The results were as follows : Three Asian countries, China, Korea and Taiwan, are very active in implementing COWM policy. Japan, however, has independent system of unified medicine. In regards to the combined care policy and system, China has the most advanced COWM system among four countries. In respect to combined care education, it is needed to increase the COWM education contents and the amount of cross educational curriculum. Based on the current COWM system, Chinese, Japanese and Taiwanese doctors can prescribe both oriental and western drugs. But, Korean medical law prohibits western doctors and oriental doctors from prescribing the counterpart´s medicine. So, the revision of current medical law is urgent for COWM in Korea. And when it comes to patient satisfaction, more than fifty percent responded positively in China, Korea and Taiwan. To achieve the goal of COWM ; 1) mutual understanding and recognition of COWM is essential. 2) institutional and legal support system for COWM is desperately urgent. 3) possible international collaboration and cooperation should be sought to untangle these complex cultural dilemmas.

상급 종합병원 내 의사, 간호사 간 의사소통 인식에 대한 조사연구 (A Study on the Perception of Communication Between Doctors and Nurses in Advanced General Hospital)

  • 본 연구는 전문 의료인인 의사와 간호사간 의사소통 인식 정도와 의사소통에 대한 세부 영역과 만족도를 파악하기 위한 서술적 조사 연구이다. 자료수집은 C시의 종합병원 의사, 간호사 372명을 대상으로 2021년 3월부터 5월까지 수집하였다. 수집된 자료는 SPSS/WIN 20.0 프로그램을 이용하여 기술통계, 평균과 표준편차, t-검정, ANOVA, Scheffe test, 상관관계 분석을 이용하여 분석하였다. 연구 결과, 의사와 간호사 간 의사소통 인식의 차이가 있었으며, 의사가 인식한 의사소통과 간호사가 인식한 의사소통 세부영역 중 개방성(t=9.91), 직종 간 상호이해(t=5.25), 만족도(t=8.13)에서 통계적으로 유의한 차이를 보였다. 의사소통 세부영역에서 간호사와 의사 그룹 모두 의사소통 세부영역과 만족도는 양의 상관관계가 있었다. 간호사는 개방성(r=.72, p<.001), 직종 간 상호이해(r=.71, p<.001)가 의사소통 만족도와 높은 양의 상관관계가 있었고, 의사는 직종 간 상호이해(r=.79, p<.001), 적시성(r=.73, p<.001)이 의사소통 만족도와 높은 양의 상관관계가 있는 것으로 나타났다. 따라서 환자안전을 보장하고 양질의 의료를 제공할 수 있도록 간호사, 의사 직종 간 상호이해, 개방성 등 취약부분을 효과적으로 개선할 수 있는 의사소통 프로그램 개발이 필요하다.

개심술 환자를 위한 조기 운동프로그램의 개발 및 적용에 관한 연구 (Development and Application of an Early Exercise Program for Open Heart Surgery Patients)

  • Purpose: This study was to develop and evaluate stability and effects of an early exercise program for patients with open heart surgery. Methods: The subjects of this study were 30 patients who had either a coronary bypass surgery or a valvular heart surgery at a tertiary hospital in Seoul. The data was collected by observation and measurement from October 1, 2004 to November 15, 2004. Results: The early exercise program developed for this study consisted of range of motion exercise and walking. Intensity of walking was 1~3 METs and increased progressively to daily target distance. During exercise, the subjects were monitored heart rate, blood pressure and RPE (Rating of Perceived Exertion). The mean FIM (Functional Independent Measurements) score of subjects was significantly improved after the early exercise program. However, several complaints such as dizziness or pain were also reported. Most complaints were associated with chest tube and RPE. Conclusion: The early exercise program can help to recover patients' physical activities after surgery, and can be applied to most patients. Patients' RPE, dizziness and pain was possible limitations, therefore, active pain control and prevention of accidents for patients would be needed.

긍정적 탐구 활동이 신규간호사의 긍정심리자본과 조직몰입에 미치는 효과 (The Effect of Appreciative Inquiry on Positive Psychological Capital and Organizational Commitment of New Nurses)

  • Purpose : The purpose of this study was to determine whether appreciative inquiry (AI) is an effective intervention for increasing the positive psychological capital and organizational commitment of new nurses. Method : The study used a nonequivalent control group pretest-posttest design. The participants were 60 new nurses in a tertiary hospital in Seoul. The experimental group received 2 classes of AI education and in-unit AI activities. The control group received the existing education program. Results : There was no statistically significant difference in the positive psychological capital and organizational commitment between the experimental group and the control group over time. Satisfaction with the AI education scored 3.69, which was higher than the average. The reason why the experimental group members were satisfied with the program was that AI education helped them to adapt and the in-unit AI activities made staff more cooperative and the atmosphere of the unit more positive. Conclusion : When applying AI activities to new nurses to promote positive psychological capital and organizational commitment, it is necessary to provide a workshop in which the participants can fully concentrate on education and to extend the period of use to one year in order to maintain the effect of AI activities.

감각통합치료 세팅에서 일하는 작업치료사의 직무만족도에 관한 연구 (Job Satisfaction of Occupational Therapists Working in Sensory Integration Treatment Setting)

  • 목적 : 본 연구는 감각통합치료 세팅에서 일하는 작업치료사의 직무만족도와 이와 관련된 요소들을 알아보고자 하였다. 연구방법 : 현재 감각통합치료 세팅에서 근무하는 대한감각통합치료학회 회원 중 62명을 대상으로 2009년 12월 ~ 2010년 3월까지 설문조사를 실시하였다. 결과 : 일반적 특성에 따른 직무의 전체만족도는 35세 이상 그룹이 가장 높았고, 감독만족도는 감각통합치료학회 치료과정을 이수한 그룹이 가장 높았다. 임금만족도는 35세 이상, 임상경력 12개월 미만 그룹이 가장 높았고, 승진 만족도는 25세 미만 그룹이 가장 높았고, 동료만족도는 여성이 남성보다 높았다. 업무환경에 따른 직무의 전체만족도는 치료사 수가 5~10명 미만, 사설기관 그룹이 가장 높았다. 감독만족도는 일주일 치료 평균시간이 45시간 이상, 사설기관 그룹이 가장 높았고, 임금만족도는 치료사수가 15명 이상 그룹에서 가장 높았고, 100~150만원 미만 그룹이 150~200만원 미만그룹보다 통계학적으로 유의하게 높았다. 치료사 수가 5~10명 미만인 그룹이 승진만족도와 기관 만족도가 가장 높았고, 이직 의도는 장애전담어린이집 그룹이 가장 높았다. 결론 : 본 연구 결과를 통해 조직규모와 근무기관의 형태는 직무만족도에 크게 영향을 미치는 요소임을 알 수 있었다. 직무만족도 향상을 위한 직무환경 개선은 조직규모에 따라 다른 관점의 접근이 필요하며, 근무기관의 형태에 따른 작업치료사의 전문성과 역할 정립을 통해 직무만족도를 향상시킬 수 있을 것이다.

백내장 수술환자 진료결과들간의 관계 - 사전 연구를 중심으로 - (Relationship among Patient Outcomes in Cataract Surgical Patient - Pilot study -)

  • Background : This study was done to assess the relationship among multiple patient outcomes of cataract surgery perioperatively, 3-4 months and 12 months after surgery. The patient outcomes include changes in visual acuity(operated eye, better eye), visual function(VF-14), patient satisfaction, subjective satisfaction with vision, and subjective overall health status. Methods : For the assessment of relationship, prospective study was performed with 92 patients who had undergone either one or both eye cataract surgery by 3 ophthalmologists practicing at a university hospital. Patients were interviewed. and clinical data were obtained. Doctors were questioned with self-entered questionnaire forms. Medical record was examined to understand surgery process. The survey was conducted at 4 stages : preoperatively, perioperatively, postoperative 3-4 months, and postoperative 12 months. Results : The correlations within patient outcomes at 4 stages - the visual acuity of operated eye and that of better eye, patient satisfaction and VF-14, subjective overall health status and relative health status as against others - were found to be positively correlated. The change in the visual acuity of operated eye and better eye was correlated with VF-14 as well as with patient satisfaction. The change was also correlated with overall health status. However, the correlations between variables were decreased as the postoperative period got longer. Conclusion : As for the postoperative clinical patient outcomes, VF-14 is acted to linker between visual acuity - clinical outcomes and overall health status - endpoint outcomes. Therefore. VF-14 is the index of patient-sided and disease-specific outcome for cataract surgery.
